| 2 | Management of diabolical diabetes mellitus and periodontitis nexus: Are we doing enough?显示文摘Periodontitis is the commonest oral disease affecting population worldwide.This disease is notorious for the devastation of tooth supporting structures,ensuing in the loss of dentition.The etiology for this disease is bacterial biofilm,which accumulates on the teeth as dental plaque.In addition to the biofilm microorganisms,other factors such as environmental,systemic and genetic are also responsible in progression of periodontitis.Diabetes mellitus(DM) is metabolic disorder which has an impact on the global health.DM plays a crucial role in the pathogenesis of periodontitis.Periodontitis is declared as the 'sixth' major complication of DM.Evidence based literature has depicted an enhanced incidence and severity of periodontitis in subjects with DM.A 'two way' relationship has been purported between periodontitis and DM.Mutual management of both conditions is necessary.Periodontal therapy(PT) may assist to diminish the progression of DM and improve glycemic control.Various advanced technological facilities may be utilized for the purpose of patient education and disease management.The present paper clarifies the etio-pathogenesis of periodontitis,establishing it as a complication of DM and elaborating the various mechanisms involved in the pathogenesis.The role of PT in amelioration of DM and application of digital communication will be discussed.Overall,it is judicious to create an increased patient cognizance of the periodontitis-DM relationship.Conjunctive efforts must be undertaken by the medical and oral health care professionals for the management of periodontitis affected DM patients. | Abhijit N Gurav | 2016 | World Journal of Diabetes2016,7,4: | 22 |

| 4 | Over expression of resistin in adipose tissue of the obese induces insulin resistance显示文摘AIM: To compare resistin mRNA expression in subcutaneous adipose tissue (SAT) and its correlation with insulin resistance (IR) in postmenopausal obese women. METHODS: A total of 68 postmenopausal women (non obese = 34 and obese = 34) were enrolled for the study. The women of the two groups were age matched (49-70 years). Fasting blood samples were collected at admission and abdominal SAT was obtained during surgery for gall bladder stones or hysterectomy. Physical parameters [age, height, weight, body mass index (BMI)] were measured. Biochemical (plasma insulin and plasma glucose) parameters were estimated by enzymatic methods. RNA was isolated by the Trizol method.SAT resistin mRNA expression was done by real time- reverse transcription polymerase chain reaction (RT-PCR) by using Quanti Tect SYBR Green RT-PCR master mix. Data was analyzed using independent Student's t test, correlation and simple linear regression analysis. RESULTS: The mean weight (52.81 ± 8.04 kg vs 79.56 ± 9.91 kg; P < 0.001), BMI (20.23 ± 3.05 kg/m 2 vs 32.19 ± 4.86 kg/m 2 ; P < 0.001), insulin (8.47 ± 3.24 U/mL vs 14.67 ± 2.18 U/mL; P < 0.001), glucose (97.44 ± 11.31 mg/dL vs 109.67 ± 8.02 mg/dL; P < 0.001) and homeostasis model assessment index (2.01 ± 0.73 vs 3.96 ± 0.61; P < 0.001) were significantly higher in postmenopausal obese women compared to postmenopausal non obese women. The mean serum resistin level was also significantly higher in postmeno-pausal obese women compared to postmenopausal non obese women (9.05 ± 5.15 vs 13.92 ± 6.32, P < 0.001). Furthermore, the mean SAT resistin mRNA expression was also significantly (0.023 ± 0.008 vs 0.036 ± 0.009; P < 0.001) higher and over expressed 1.62 fold (upregulated) in postmenopausal obese women compared to postmenopausal non obese women. In postmeno-pausal obese women, the relative SAT resistin mRNA expression showed positive (direct) and significant correlation with BMI (r = 0.78, P < 0.001) and serum resistin (r = 0.76, P < 0.001). Furthermore, the SAT resistin mRNA expression in postmenopausal obese women also showed significant and direct association (r = 0.45, P < 0.01) with IR, while in postmenopausal non obese women it did not show any association (r = -0.04, P > 0.05). | 6 | Blumgart's technique of pancreaticojejunostomy: Analysis of safety and outcomes显示文摘Background: Blumgart's pancreaticojejunostomy(PJ) has been described with low pancreatic leak rates. This study aimed to evaluate our experience with this technique regarding the pancreatic leak and other perioperative outcomes. Methods: We performed a single-center retrospective analysis of a cohort of 81 patients who underwent pancreaticoduodenectomy in our department from January 2011 to February 2018. The primary endpoint was the occurrence of a clinically relevant postoperative pancreatic fistula(CR-POPF) and analysis of its risk factors. Results: The CR-POPF rate was 12.3%. Fistula risk score(FRS) was the only significant risk factor for the occurrence of overall POPF in multivariate analysis. However, none of the other factors including FRS was found to be significantly associated with CR-POPF risk. A strong positive correlation was found between the CR-POPF and the incidence of delayed gastric emptying, post-pancreatectomy hemorrhage and increased length of hospital stay. Conclusion: Blumgart's technique is a safe technique of pancreatico-enteric anastomosis with low rates of CR-POPF. CR-POPF with this technique is independent of most of the preoperative and intraoperative factors. | 7 | A review of UHMWPE wear-induced osteolysis:the role for early detection of the immune response显示文摘In a world where increasing joint arthroplasties are being performed on increasingly younger patients,osteolysis as the leading cause of failure after total joint arthroplasty(TJA) has gained considerable attention.Ultra-high molecular weight polyethylene wear-induced osteolysis is the process by which prosthetic debris mechanically released from the surface of prosthetic joints induces an immune response that favors bone catabolism, resulting in loosening of prostheses with eventual failure or fracture. The immune response initiated is innate in that it is nonspecific and self-propagating, with monocytic cells and osteoclasts being the main effectors. To date, detecting disease early enough to implement effective intervention without unwanted systemic side effects has been a major barrier. These barriers can be overcome using newer in vivo imaging techniques and modules linked with fluorescence and/or chemotherapies. We discuss the pathogenesis of osteolysis, and provide discussion of the challenges with imaging and therapeutics. | 8 | Colorectal cancers in ulcerative colitis from a low-prevalence area for colon cancer显示文摘AIM: To determine the incidence and risk factors for colorectal cancer(CRC) in patients with ulcerative colitis from a low prevalence region for CRC.METHODS: Our prospective database yielded a cohort of 430 patients [age: 44 ± 14.6 years; 248 men(57.7%)] with ulcerative colitis(median disease duration 6, range: 1-39 years) for analysis. Of these, 131(30.5%) had left-sided colitis and 159(37%) extensive colitis. Patients with histologically confirmed CRC within the segment with colitis were compared with those without CRC, to determine the risk factors for the development of CRC.RESULTS: Twelve patients(2.8%) developed CRC. The overall incidence density was 3.56/1000 patient-years of disease- 3/1000 in the first 10 years, 3.3/1000 at 10 to 20 years, and 7/1000 at > 20 years. Three of our 12 patients developed CRC within 8 years of disease onset. On univariate analysis, extensive colitis, longer duration of disease, and poor control of disease were associated with development of CRC. | 9 | A high-resolution seismic catalog for the 2021 M_(S)6.4/M_(W)6.1 Yangbi earthquake sequence, Yunnan, China: Application of AI picker and matched filter显示文摘We present a high-resolution seismic catalog for the 2021 M_(S)6.4/M_(W)6.1 Yangbi sequence.The catalog has a time range of 2021-05-01 to 2021-05-28,and contains~8,000 well located events.It captures the features of the whole foreshock sequence and the early aftershocks.We designed a detection strategy incorporating both an artificial intelligent(AI)picker and a matched filter algorithm.Here,we adopt a hybrid AI method incorporating convolutional and recurrent neural network(CNN&RNN)for event detection and phase picking respectively(i.e.CERP),a light-weight AI picker that can be trained with small volume of data.CERP is first trained with detections from a STA/LTA and Kurtosis-based method called PAL,and then construct a rather complete template set of~4,000 events.Finally,the matched filter algorithm MESS augments the initial detections and measures differential travel times with cross-correlation,which finally results in precise relocation.This process gives 9,026 detections,among which 7,943 events can be well relocated.The catalog shows as expected power-law distribution of frequency magnitude and reveals detailed pattern of seismicity evolution.The main features are:(1)the foreshock sequence images simple fault geometry with consistent strike,but also show a variable event depth along strike;(2)the mainshock ruptures the same fault of the foreshock sequence and activate conjugate faults further to the southeast;(3)complex seismicity are developed in the post-seismic period,indicating complex triggering mechanisms.Thus,our catalog provides a reliable basis for further investigations,such as b-value studies,rupture process,and triggering relations. | Yijian Zhou Abhijit Ghosh Lihua Fang Han Yue Shiyong Zhou Youjin Su | 2021 | Earthquake Science2021,34,5: | 6 |

Patients often present with a relatively high acuity and, depending on surgical approach, morbidity and mortality can be comparatively high. Among the most prevalent and most frequently fatalitybound perioperative complications are those affecting the pulmonary system; evidence of clinical or subclinical lung injury triggered by spine surgical procedures is emerging. Increasing burden of comorbidity among the patient population further increases the likelihood of adverse outcome. This review is intended to give an overview over some of the most important causes of pulmonary complications after spine surgery, their pathophysiology and possible ways to reduce harm associated with those conditions. | 14 | Access related complications during anterior exposure of the lumbar spine显示文摘The new millennium has witnessed the emergence of minimally invasive, non-posterior based surgery of the lumbar spine, in particular via lateral based methodologies to discectomy and fusion. In contrast, and perhaps for a variety of reasons, anterior motion preservation(non-fusion) technologies are playing a comparatively lesser, though incompletely defined, role at present. Lateral based motion preservation technologies await definition of their eventual role in the armamentarium of minimally invasive surgical therapies of the lumbar spine. While injury to the major vascular structures remains the most serious and feared complication of the anterior approach, this occurrence has been nearly eliminated by the use of lateral based approaches for discectomy and fusion cephalad to L5-S1. Whether anterior or lateral based, non-posterior approaches to the lumbar spine share certain access related pitfalls and complications, including damage to the urologic and neurologic structures, as well as gastrointestinal and abdominal wall issues. | 18 | Systematic Assessment of Synthesized Tri-magnesium Phosphate Powders(Amorphous,Semi-crystalline and Crystalline) and Cements for Ceramic Bone Cement Applications显示文摘Magnesium phosphate cements have come under investigation in recent years for use as an alternative to calcium phosphate cements for bone void repair applications.Evidence indicates that magnesium phosphate cements obtain higher initial strengths after cement reaction and resorption in more clinically appropriate time frames than commercially available calcium phosphate cements.In this study,amorphous,partially amorphous and crystalline tri-magnesium phosphate powders were synthesized via an aqueous precipitation reaction with subsequent thermal treatment,and characterized using techniques such as X-ray diffraction and Fourier transform infrared spectroscopy.These materials were assessed for their functionality in cementing reaction with a 3.0 mol/L,pH 7.0 ammonium phosphate solution,including setting time and pH evolution in phosphate buffered saline solution.Results indicated that the amorphous and semi-crystalline tri-magnesium phosphate powders were highly reactive with the setting solution but resulted in mechanically weak cements,while the crystalline tri-magnesium phosphate powder reacted efficiently with the cement solution and were mechanically strong following the cement reaction.X-ray diffraction and scanning electron microscopy analyses indicated significant changes in the phase composition and morphology of the cements following incubation in phosphate buffered saline.These were perceived to be detrimental to the integrity of the amorphous and semi-crystalline tri-magnesium phosphate derived cements but not to those created with fully crystalline tri-magnesium phosphate.The crystalline tri-magnesium phosphate material resulted in the most functional cement as this embodiment displayed the most clinically relevant setting time as well as the highest mechanical resilience and neutral pH during incubation in saline solution rendering them potentially viable as bone void fillers. | Nicole Ostrowski Vidisha Sharma Abhijit Roy Prashant N.Kumta | 2015 | Journal of Materials Science & Technology2015,31,5: | 3 |

They representing a large share of the cell’s transcriptional output, demonstrate functional attributes viz. tissue-specific expression, determination of cell fate, controlled expression, RNA processing and editing, dosage compensation, genomic imprinting, conserved evolutionary traits etc. These long non coding variants are well associated with pathogenicity of various diseases including the neurological disorders like Alzheimer’s disease, schizophrenia, Huntington’s disease, Parkinson’s disease etc. Neurological disorders are widespread and there knowing the underlying mechanisms become crucial. The lncRNAs take part in the pathogenesis by a plethora of mechanisms like decoy, scaffold, mi-RNA sequestrator, histone modifiers and in transcriptional interference. Detailed knowledge of the role of lncRNAs can help to use them further as novel biomarkers for therapeutic aspects. Here, in this review we discuss regulation and functional roles of lncRNAs in eight neurological diseases and psychiatric disorders, and the mechanisms by which they act.
