| 1 | Cell Death Mechanisms Induced by Cytotoxic Lymphocytes显示文摘One of the functions of the immune system is to recognize and destroy abnormal or infected cells to maintain homeostasis. This is accomplished by cytotoxic lymphocytes. Cytotoxicity is a highly organized multifactor process. Here,we reviewed the apoptosis pathways induced by the two main cytotoxic lymphocyte subsets,natural killer(NK) cells and CD8+ T cells. In base to recent experimental evidence,we reviewed NK receptors involved in recognition of target-cell,as well as lytic molecules such as perforin,granzymes-A and-B,and granulysin. In addition,we reviewed the Fas-FasL intercellular linkage mediated pathway,and briefly the cross-linking of tumor necrosis factor(TNF) and TNF receptor pathway. | 2 | Tenofovir vs lamivudine plus adefovir in chronic hepatitis B:TENOSIMP-B study显示文摘AIM To demonstrate the non-inferiority(15% non-inferiority limit) of monotherapy with tenofovir disoproxil fumarate(TDF) vs the combination of lamivudine(LAM) plus adefovir dipivoxil(ADV) in the maintenance of virologic response in patients with chronic hepatitis B(CHB) and prior failure with LAM.METHODS This study was a Phase IV prospective, randomized, open, controlled study with 2 parallel groups(TDF and LAM+ADV) of adult patients with hepatitis B e antigen(HBe Ag)-negative CHB, prior failure with LAM, on treatment with LAM+ADV for at least 6 mo, without prior resistance to ADV and with an undetectable viral load at the start of the study, in 14 Spanish hospitals. The follow-up time for each patient was 48 wk after randomization, with quarterly visits in which the viral load, biochemical and serological parameters, adverse effects, adherence to treatment and consumption of hospital resources were analysed.RESULTS Forty-six patients were evaluated [median age: 55.4 years(30.2-75.2); 84.8% male], including 22 patients with TDF and 24 with LAM+ADV. During study development, hepatitis B virus DNA(HBV-DNA) remained undetectable, all patients remained HBe Ag negative, and hepatitis B surface antigen(HBs Ag) positive. Alanine aminotransferase(ALT) values at the end of the study were similar in the 2 groups(25.1± 7.65, TDF vs 24.22 ± 8.38, LAM+ADV, P = 0.646). No significant changes were observed in creatinine or serum phosphorus values in either group. No significant differences between the 2 groups were noted in the identification of adverse effects(AEs)(53.8%, TDF vs 37.5%, LAM+ADV, P = 0.170), and none of the AEs which occurred were serious. Treatment adherence was 95.5% and 83.3% in the TDF and the LAM+ADV groups, respectively(P = 0.488). The costs associated with hospital resource consumption were significantly lower with the TDF treatment than the LAM+ADV treatment(€4943 ± 1059 vs €5811 ± 1538, respectively, P < 0.001).CONCLUSION TDF monotherapy proved to be safe and not inferior to the LAM+ADV combination therapy in maintaining virologic response in patients with CHB and previous LAM failure. | 3 | Expression of triggering receptor on myeloid cell 1 and histocompatibility complex molecules in sepsis and major abdominal surgery显示文摘AIM: To evaluate the surface expression of triggering receptor on myeloid cell 1 (TREM-1), class Ⅱ major histocompatibility complex molecules (HLA-DR), andthe expression of the splicing variant (svTREM-1) ofTREM-1 in septic patients and those subjected to major abdominal surgery.METHODS: Using flow cytometry, we examined the surface expression of TREM-1 and HLA-DR in peripheral blood monocytes from 11 septic patients, 7 elective gastrointestinal surgical patients, and 10 healthy volunteers. svTREM-1 levels were analyzed by RT-PCR. RESULTS: Basal expression of TREM-1 and HLA-DR in healthy volunteers was 35.91±14.75 MFI and75.8±18.3%, respectively. In septic patients, TREM-1 expression was 59.9±23.9 MFI and HLA-DR expression was 44.39±20.25%, with a significant differencebetween healthy and septic groups (P<0.05) for bothmolecules. In the surgical patients, TREM-1 and HLA-DR expressions were 56.8±20.85 MFI and 71±13.8% before surgery and 72.65±29.92 MlFI and 72.82±22.55% after surgery. TREM-1 expression was significantly different(P = 0.0087) between the samples before and aftersurgery and svTREM-1 expression was 0.8590±0.1451 MF1, 0.8820±0.1460 MF1, and 2.210±0.7873MF1 in the healthy, surgical (after surgery) and septic groups, respectively. There was a significant difference (P = 0.048) in svTREM-1 expression between the healthy and surgical groups and the septic group.CONCLUSION: TREM-1 expression is increased during systemic inflammatory conditions such as sepsis and the postoperative phase. Simultaneous low expression of HLA-DR molecules correlates with the severity of illness and increases susceptibility to infection. Additionally, TREM-1 expression is distinctly different in surgical patients at different stages of the inflammatory response before and after surgery. Thus, surface TREM-1 appears to be an endogenous signal during the course of the inflammatory response. svTREM-1 expression is significantly increased during sepsis, appearing to be an indicator of severity of illness. Together, these data indicate that TREM-1 may play an important role in establishing and amplifying the systemic inflammatory response. | 4 | Drawing the Yongquan protocol into the different stages of the cardiopulmonary resuscitation sequence显示文摘AIM: To introduce new applications into the ILCORcardiopulmonary resuscitation(CPR) 'chain' sequence.METHODS: Stages of the CPR sequence('chain'): prior to the application of chest massage: assess the victim's state of consciousness and lung-heart failure;seek help(call 911), or in situations in which it is impossible to start the ILCOR protocol:(1) if the victim is trapped in car crash, overturned car, landslide, massive number of victims or catastrophe; or(2) delayed CPR.During chest compression: Yongquan is simultaneously stimulated by a third rescuer. During defibrillator application: activate K-1 Yongquan through needles before defibrillation. Unsuccessful CPR: 'gold standard'for legal clinical death.RESULTS: Implies comparing two hypotheses: Ho(null hypothesis) demonstrates no association between the two variables studied; Ha(alternative hypothesis) implies some degree of relation between them. Difference between the two treatments is observed. If it is greater than the standard error multiplied by a coefficient of security, the difference is significant: Ha will be accepted and Ho rejected. First we will compare CPR without defibrillator(method 'A') and K-1 Yongquan method(method 'B'), using percentages of representative samples(treatment 'A': 6.4% response, treatment 'B': 85% response). If │PA- PB│ is greater than the product of 1.96 times the standard error, the difference is significant. Because │PA- PB│ = 0.786 is greater than 0.098, the difference between 0.064 and 0.85 is statistically significant. Thus, we reject Ho and accept Ha as correct. Thus, it is improbable that chance was responsible for this association. This analysis shows that K-1 Yongquan method has a 'quality guarantee'. Second, we compare defibrillators('A') with K-1 Yongquan method('B')(treatment 'A': 48%, treatment 'B': 84%, │PA- PB│= 0.36; │PA- PB│ = 0.36 is greater than SE × 1.96 = 0.0148 and also statistically significant, demonstrating again the comparative value of the Yongquan method. | 5 | Biometric and ICL-related risk factors associated to sub-optimal vaults in eyes implanted with implantable collamer lenses显示文摘Background:To identify biometric and implantable collamer lens(ICL)-related risk factors associated with suboptimal postoperative vault in eyes implanted with phakic ICL.Methods:This study reports a retrospective case series of the first operated eye in 360 patients implanted with myopic spherical or toric ICL.Preoperatively,white-to-white(WTW),central keratometry(Kc)and central corneal thickness(CCT)were measured using the Pentacam.Anterior-segment optical coherence tomography(AS-OCT,Visante)was applied preoperatively for measuring the horizontal anterior-chamber angle-to-angle distance(ATA),internal anterior chamber depth(ACD),crystalline lens rise(CLR),anterior-chamber angle(ACA)and postoperatively the vault.Eyes were divided into three vault groups:low(LVG:≤250μm),optimal(OVG:>250 and<1000μm)and high(HVG:≥1000μm).Multinomial logistic regression(MLR)was used to find the sub-optimal vault predictors.Results:MLR showed that CLR,ICL size minus the ATA(ICL size-ATA),age,ICL spherical equivalent(ICLSE)and ICL size as contributing factors for sub-optimal vaults(pseudo-R2=0.40).Increased CLR(OR:1.01,CI:1.00-1.01)and less myopic ICLSE(OR:1.22,CI:1.07-1.40)were risk factors for low vaults.Larger ICL size-ATA(OR:41.29,CI:10.57-161.22)and the 13.7mm ICL(OR:7.08,CI:3.16-15.89)were risk factors for high vaults,whereas less myopic ICLSE(OR:0.85,CI:0.76-0.95)and older age(OR:0.92,CI:0.88-0.98)were protective factors.Conclusion:High CLR and low ICLSE were the major risk factors in eyes presenting low vaults.In the opposite direction,ICL size-ATA was the major contributor for high vaults.This relationship was more critical in higher myopic ICLSE,younger eyes and when 13.7mm ICL were used.The findings show that factors influencing the vault have differentiated weight of influence depending on the type of vault(low,optimal or high). | Santiago Cerpa Manito Angel Sánchez Trancón Oscar Torrado Sierra António Manuel Baptista Pedro Miguel Serra | 2021 | Eye and Vision2021,8,1: | 5 |
| 6 | Superoxide dismutase prevents development of adenocarcinoma in a rat model of Barrett's esophagus显示文摘AIM: To test whether antioxidant treatment could prevent the progression of Barrett's esophagus to adenocarcinoma.METHODS: In a rat model of gastroduodenoesophageal reflux by esophagojejunal anastomosis with gastric preservation, groups of 6-10 rats were randomized to receive treatment with superoxide dismutase (SOD) or vehicle and followed up for 4 mo. Rat's esophagus was assessed by histological analysis, superoxide anion and peroxinitrite generation, SOD levels and DNA oxidative damage.RESULTS: All rats undergoing esophagojejunostomy developed extensive esophageal mucosal ulceration and inflammation by mo 4. The process was associated with a progressive presence of intestinal metaplasia beyondthe anastomotic area (9% 1st mo and 50% 4th mo) (94% at the anastomotic level) and adenocarcinoma(11% 1st mo and 60% 4th mo). These changes were associated with superoxide anion and peroxinitrite mucosal generation, an early and significant increase of DNA oxidative damage and a significant decrease in SOD levels (P<0.05). | 10 | Hepatic flow is an intraoperative predictor of early allograft dysfunction in whole-graft deceased donor liver transplantation: An observational cohort study显示文摘BACKGROUND Early allograft dysfunction(EAD)after liver transplantation(LT)is an important cause of morbidity and mortality.To ensure adequate graft function,a critical hepatocellular mass is required in addition to an appropriate blood supply.We hypothesized that intraoperative measurement of portal venous and hepatic arterial flow may serve as a predictor in the diagnosis of EAD.AIM To study whether hepatic flow is an independent predictor of EAD following LT.METHODS This is an observational cohort study in a single institution.Hepatic arterial blood flow and portal venous blood flow were measured intraoperatively by transit flow.EAD was defined using the Olthoff criteria.Univariate and multivariate analyses were used to determine the intraoperative predictors of EAD.Survival analysis and prognostic factor analysis were performed using the Kaplan-Meier and Cox regression models.RESULTS A total of 195 liver transplant procedures were performed between January 2008 and December 2014 in 188 patients.A total of 54(27.7%)patients developed EAD.The median follow-up was 39 mo.Portal venous flow,hepatic arterial flow(HAF)and total hepatic arterial flow were associated with EAD in both the univariate and multivariate analyses.HAF is an independent prognostic factor for 30-d patient mortality.CONCLUSION Intraoperative measurement of blood flow after reperfusion appears to be a predictor of EAD;Moreover,HAF should be considered a predictor of 30-d patient mortality. | Pablo Lozano Lominchar Maitane Igone Orue-Echebarria Lorena Martín Cristina Julia Lisbona María Magdalena Salcedo Luis Olmedilla Hemant Sharma Jose Manuel Asencio JoséAngel López-Baena | 2019 | World Journal of Hepatology2019,11,9: | 4 |

| 13 | Outcomes of critically ill cancer patients with Acinetobacter baumannii infection显示文摘AIM: To describe the intensive care unit(ICU) outcomes of critically ill cancer patients with Acinetobacter baumannii(AB) infection.METHODS: This was an observational study that included 23 consecutive cancer patients who acquired AB infections during their stay at ICU of the National Cancer Institute of Mexico(INCan), located in Mexico City. Data collection took place between January 2011, and December 2012. Patients who had AB infections before ICU admission, and infections that occurred during the first 2 d of ICU stay were excluded. Data were obtained by reviewing the electronic health record of each patient. This investigation was approved by the Scientific and Ethics Committees at INCan. Because of its observational nature, informed consent of the patients was not required.RESULTS: Throughout the study period, a total of 494 critically ill patients with cancer were admitted to the ICU of the INCan, 23(4.6%) of whom developed AB infections. Sixteen(60.9%) of these patients had hematologic malignancies. Most frequent reasons for ICU admission were severe sepsis or septic shock(56.2%) and postoperative care(21.7%). The respiratory tract was the most frequent site of AB infection(91.3%). The most common organ dysfunction observed in our group of patients were the respiratory(100%), cardiovascular(100%), hepatic(73.9%) and renal dysfunction(65.2%). The ICU mortality of patients with 3 or less organ system dysfunctions was 11.7%(2/17) compared with 66.6%(4/6) for the group of patients with 4 or more organ system dysfunctions(P = 0.021). Multivariate analysis identified blood lactate levels(BLL) as the only variable independently associated with inICU death(OR = 2.59, 95%CI: 1.04-6.43, P = 0.040). ICU and hospital mortality rates were 26.1% and 43.5%, respectively.CONCLUSION: The mortality rate in critically ill patients with both HM, and AB infections who are admitted to the ICU is high. | 15 | Prognosis and follow-up of 135 patients with ischemic colitis over a five-year period显示文摘AIM:To study the prognosis(recurrence and mortality)of patients with ischemic colitis(IC).METHODS:This study was conducted in four Spanish hospitals,participants in the Ischemic Colitis in Spain study We analyzed prospectively 135 consecutive patients who met criteria for definitive or probable IC according to Brandt criteria,and follow up these patients during the next five years,retrospectively.Long-term results(recurrence and mortality)were evaluated retrospectively after a median interval of 62 mo(range54-75 mo).RESULTS:Estimated IC recurrence rates were 2.9%,5.1%,8.1%and 9.7%at years 1,2,3 and 5 years,respectively.Five-year survival was 69%(93 of 135)and 24%(10 of 42 patients)died for causes related to the IC.Among these 10 patients,8 died in their first episode at hospital(4 had gangrenous colitis and 4 fulminant colitis)and 2 due to recurrence.CONCLUSION:The five-year recurrence rate of IC was low.On the other hand,mortality during follow-up was high and was not associated with ischemic colitis. | Angel Cosme Miguel Montoro Santos Santolaria Ana B Sanchez-Puertolas Marta Ponce Margarita Durán Jose Luis Cabriada Nerea Borda Cristina Sarasqueta Luis Bujanda | 2013 | World Journal of Gastroenterology2013,19,44: | 2 |

Its model to protect small sites for endemic and endangered plants has been exported to several countries around the globe. This paper highlights 1) the role of PMRs to complement the protection provided by large protected areas, 2) how the establishment of PMRs fosters the increase of floristic knowledge, and 3)the fact that continuous monitoring of PMRs also yields new records of endangered species found within the same PMRs. | 18 | Adipokines,cytokines and body fat stores in hepatitis Cvirus liver steatosis显示文摘AIM: To identify patients with or without liver steatosis and its severity in treatment-na?ve patients affected by hepatitis C virus(HCV) infection.METHODS: We included 56 HCV infected patients, and assessed the amount of liver fat by histomorphometry, and its relationships with fat and lean mass at different parts of the body(by densitometry), hormones [insulin, homeostatic model assessment(HOMA)], adipokines(resistin, adiponectin, leptin), and cytokines(tumor necrosis factor α, interleukin-6).RESULTS: Although the intensity of liver steatosis is related to trunk fat mass and HOMA, 33% of patients showed no liver steatosis, and this finding was not related to body mass index or genotype. Besides trunkfat mass, no other factor was related to the presence or not of liver steatosis, or to the intensity of it, by multivariate analysis. Lean mass was not related to liver steatosis. Adiponectin levels were lower among patients. | 20 | Polyubiquitination inhibition of estrogen receptor alpha and its implications in breast cancer显示文摘Estrogen receptor alpha(ERα) is detected in more than 70% of the cases of breast cancer. Nuclear activity of ERα, a transcriptional regulator, is linked to the development of mammary tumors, whereas the extranuclear activity of ERα is related to endocrine therapy resistance. ERα polyubiquitination is induced by the estradiol hormone, and also by selective estrogen receptor degraders, resulting in ERα degradation via the ubiquitin proteasome system. Moreover, polyubiquitination is related to the ERα transcription cycle, and some E3-ubiquitin ligases also function as coactivators for ERα. Several studies have demonstrated that ERα polyubiquitination is inhibited by multiple mechanisms that include posttranslational modifica-tions, intera-ctions with coregula-tors, a-nd forma-tion of specific protein complexes with ERα. These events are responsible for an increase in ERα protein levels and deregulation of its signaling in breast cancers.
