1Ultrasound: A promising tool for contemporary airway management显示文摘Airway evaluation and its management remainsan ever emerging clinical science. Present airway management tools are static and do not provide dynamic airway management option. Visualized procedures like ultrasound(US) provide point of care real time dynamic views of the airway in perioperative, emergency and critical care settings. US can provide dynamic anatomical assessment which is not possible by clinical examination alone. US aids in detecting gastric contents and the nature of gastric contents(clear fluid, thick turbid or solid) as well. US can help in predicting endotracheal tube size by measuring subglottic diameter and diameter of left main stem bronchus. US was found to be a sensitive in detecting rotational malposition of LMA in children. Also, US is the fastest and highly sensitive tool to rule out a suspected intraoperative pneumothorax. In intensive care units, US helps torule out causes of inadequate ventilation, determine the tracheal width and distance from the skin to predict tracheotomy tube size and shape and assist with percutaneous dilatational tracheostomy. US can help in confirming the correct tracheal tube placement by dynamic visualisation of the endotracheal tube insertion, widening of vocal cords(children), and bilateral lung-sliding and diaphragmatic movement. Thus, ultrasonography has brought a paradigm shift in the practise of airway management. With increasing awareness, portability, accessibility and further sophistication in technology, it is likely to find a place in routine airway management. 2Smart polymers for the controlled delivery of drugs–a concise overview显示文摘Smart polymers have enormous potential in various applications.In particular,smart polymeric drug delivery systems have been explored as“intelligent”delivery systems able to release,at the appropriate time and site of action,entrapped drugs in response to specific physiological triggers.These polymers exhibit a non-linear response to a small stimulus leading to a macroscopic alteration in their structure/properties.The responses vary widely from swelling/contraction to disintegration.Synthesis of new polymers and crosslinkers with greater biocompatibility and better biodegradability would increase and enhance current applications.The most fascinating features of the smart polymers arise from their versatility and tunable sensitivity.The most significant weakness of all these external stimuli-sensitive polymers is slow response time.The versatility of polymer sources and their combinatorial synthesis make it possible to tune polymer sensitivity to a given stimulus within a narrow range.Development of smart polymer systems may lead to more accurate and programmable drug delivery.In this review,we discuss various mechanisms by which polymer systems are assembled in situ to form implanted devices for sustained release of therapeutic macromolecules,and we highlight various applications in the field of advanced drug delivery.Honey Priya James Rijo John Anju Alex Anoop K.R. 2014Acta Pharmaceutica Sinica B2014,4,2:5
3右美托咪定复合罗哌卡因椎旁阻滞在经皮肾镜碎石术中的应用显示文摘目的:探讨右美托咪定复合罗哌卡因在超声引导椎旁阻滞对经皮肾镜碎石术患者阻滞完善时间、镇痛持续时间以及心率、血压的影响。方法:将择期行经皮肾镜碎石术患者86例纳入研究,按照随机数字表法分成实验组(43例)和对照组(43例),均采用超声引导椎旁阻滞,实验组注入0.5μg/kg右美托咪定+0.375%罗哌卡因共21mL,而对照组给予等容量单纯0.375%罗哌卡因。观察术中患者阻滞完善时间、术后镇痛持续时间及心动过缓、高血压、低血压等不良反应。结果:实验组阻滞完善时间明显短于对照组,差异具有统计学意义(P=0.011)。两组术后3h、6h静息视觉模拟疼痛(VAS)评分无统计学差异(P>0.05);实验组术后12h静息VAS评分和3、6、12h运动VAS评分较对照组低,差异有统计学意义(P<0.05)。两组术中心动过缓、高血压、低血压、气胸、局麻药中毒等不良反应发生率无统计学差异(P>0.05)。结论:右美托咪定复合罗哌卡因在超声引导椎旁阻滞经皮肾镜碎石术患者可缩短阻滞完善时间,增强镇痛效果,延长术后镇痛持续时间,且不增加其心动过缓、高血压、低血压的发生率。Anju 夏瑞 徐伟 毛庆军 刘伟伟 朱琼 2018长江大学学报(自然科学版)2018,15,24:4
4Hydrogen cyanide production by soil bacteria: Biological control of pests and promotion of plant growth in sustainable agriculture显示文摘Currently, plant diseases and insect infestations are mainly controlled by the extraneous application of pesticides. Unfortunately, the indiscriminate use of such agrochemicals can cause ecological and environmental problems, as well as human health hazards. To obviate the potential pollution arising from the application of agrochemicals, biological control of soilborne pathogens or insect pests using antagonistic microorganisms may be employed. Certain soil bacteria, algae, fungi, plants and insects possess the unique ability to produce hydrogen cyanide(HCN), which plays an important role in the biotic interactions of those organisms. In particular, cyanogenic bacteria have been found to inhibit the growth of various pathogenic fungi, weeds, insects, termites and nematodes. Thus, the use of HCN-producing bacteria as biopesticides offers an ecofriendly approach for sustainable agriculture. The enzyme, HCN synthase,involved in the synthesis of HCN, is encoded by the hcnABC gene cluster. The biosynthetic regulation of HCN, antibiotics and fluorescent insecticidal toxins through the conserved global regulatory GacS/GacA system is elaborated in this review, including approaches that may optimize cyanogenesis for enhanced pest control. In addition, the effects of bacterially synthesized HCN on the production of indole acetic acid, antibiotics and fluorescent insecticidal toxins, 1-aminocyclopropane-1-carboxylate deaminase utilization and phosphate solubilization may result in the stimulation of plant growth. 5Efficient Microorganism Compost Benefits Plant Growth and Improves Soil Health in Calendula and Marigold显示文摘Organic farming needs to be promoted as a means of sustaining soil health and reducing the cost of farming, especially for small farmers.The present study deals with the effect of organic compost, prepared using Efficient Microorganism(EM) consortium and applied along with full or half of the recommended dose of chemical fertilizers, on the growth of Calendula and Marigold plants, soil physico-chemical parameters and soil enzyme activities. Soil enzyme activities were improved with the increase in the rate of EM compost application in both Calendula and Marigold. Carotenoid pigment increased by 46.11% and 12.19% with application of EM compost over the control in Calendula and Marigold flowers respectively. Soil humus, available nitrogen and organic carbon content also increased due to the supplementation of EM compost resulting in better soil fertility. For Calendula, treatment T5(Half dose NPK + EM compost 20 000 kg·hm^(-2)) was found to be the most promising in terms of acid phosphatase(82.63 μg p-Nitrophenyl Phosphate·g^(-1)·h^(-1)), dehydrogenase(10.46 μg Triphenyl Formazan·g^(-1)·d^(-1)) and β-glucosidase(0.30 IU·g^(-1)) activities. 6Review of efficacy and safety of laxatives use in geriatrics显示文摘AIM:To study the efficacy and safety of pharmacolo-gical treatment of constipation in geriatrics.METHODS:Pub Med,MEDLINE,google scholar,and Ovid were searched to identify human studies performed on the use of laxatives in elderly with constipation,which were conducted between January1990 and January 2013 using the specified keywords.Controlled studies that enrolled geriatric patients with a diagnosis of constipation and addressed the efficacy and/or the safety of pharmacological treatments were included.Studies were excluded from this review if they were non-controlled trials,case series,or case reports.RESULTS:Out of twenty three studies we initially retrieved in our search,only nine studies met the eligibility criteria of being controlled trials within geriatrics.The laxatives examined in the nine studies were senna,lactulose,sorbital,polyethylene glycol(PEG),lubiprostone,linaclotide,and prucalopride.In those studies,senna combinations had a higher efficacy than sorbitol or lactulose as well as,a very good adverse effect profile.PEG was also shown to be safe and effective in geriatric population.Furthermore,it has been shown that PEG is as safe in geriatrics as in general population.New agents like lubiprostone and prucalopride show promising results but the data about these agents in geriatrics are still limited which warrant further investigation.CONCLUSION:Senna combinations and PEG appear to have a more favorable profile over the other traditionally used laxatives in elderly patients with constipation.Manhal Izzy Anju Malieckal Erin Little Sury Anand 2016World Journal of Gastrointestinal Pharmacology and Therapeutics2016,7,2:3

8Late Cenozoic seismic stratigraphy of the Andaman Forearc Basin,Indian Ocean显示文摘Interpretation of new multichannel seismic reflection data from the Andaman Forearc Basin(AFB) in the northern Indian Ocean is presented here. The highquality multichannel seismic data from the Andaman Forearc region enable us to examine the seismic characters and to demarcate seismic sequences bounded by distinct unconformities. Ages of marked seismic horizons have been calibrated with available litholog data from nearby industry boreholes. Seismic interpretation of new data shows that the AFB is filled with * 4.5-s-two way travel time(TWT) thick Neogene to Recent sediments. The entire basin assemblage exhibits two distinct major sequences pertaining to the Neogene and Quaternary times. A large part of the basin is filled with intermittent mass transport deposits(MTD). We infer that the episodic uplift of the Invisible Bank, protuberance of the outerarc and regular deformation through reactivation of preexisting normal faults since the Pleistocene could be attributed as causal mechanisms for the MTDs. Strong bottom simulating reflectors are identified in the Late Miocene and younger sediments of the outerarc and AFB at a depth of * 0.6 s TWT and correspond to the presence of gas hydrates in this region. 10Actinobacteria-enhanced plant growth, nutrient acquisition, and crop protection:Advances in soil, plant, and microbial multifactorial interactions显示文摘Agricultural areas of land are deteriorating every day owing to population increase, rapid urbanization, and industrialization. To feed today’s huge populations, increased crop production is required from smaller areas, which warrants the continuous application of high doses of inorganic fertilizers to agricultural land. These cause damage to soil health and, therefore, nutrient imbalance conditions in arable soils. Under these conditions, the benefits of microbial inoculants (such as Actinobacteria) as replacements for harmful chemicals and promoting ecofriendly sustainable farming practices have been made clear through recent technological advances. There are multifunctional traits involved in the production of different types of bioactive compounds responsible for plant growth promotion, and the biocontrol of phytopathogens has reduced the use of chemical fertilizers and pesticides. 12Evaluation of functional outcome and stability of sutureless scleral tunnel fixated IOLs in children with ectopia lentis显示文摘AIM:To evaluate functional outcome of sutureless scleral tunnel intraocular lens(SSTIOL)in children with crystalline lens subluxation of more than 7 clock hours.METHODS:A prospective interventional study was conducted consisting of 45 eyes of 44 children in age group 6-18 y having>7 clock hours of lens subluxation who underwent lensectomy-vitrectomy followed by SSTIOL implantation.Primary outcome was improvement in best corrected visual acuity(BCVA)and secondary outcomes were assessment of intraocular lens(IOL)tilt using ultrasound biomicroscopy(UBM),mean change in astigmatism at last follow-up of 1 y and associated complications.RESULTS:The mean preoperative and postoperative BCVA was 1.05±0.28 and 0.64±0.45(log MAR)respectively(P=0.001)at last follow-up.The mean astigmatism preoperatively and postoperatively was-4.17±2.69 D and-1.86±1.25 D respectively(P=0.011).Significant IOL tilt(>5 degrees)was present in 5 cases.The mean percentage endothelial loss was 3.65%±1.92%.The most serious complication encountered was retinal detachment seen in 2 cases.CONCLUSION:SSTIOL implantation provides efficient visual rehabilitation in children provided there is stringent case selection.We recommend caution in children having white-to-white distance>12 mm and presence of peripheral retinal degenerations.Anju Rastogi Prateek Kumar Shweta Dhiman Manisha Mishra Kamlesh AnAND Ankita Bhardwaj 2020International Journal of Ophthalmology(English edition)2020,13,1:2

14Basement tectonics and flexural subsidence along western continental margin of India显示文摘The Paleocene-recent post-rift subsidence history recorded in the Mumbai Offshore Basin off western continental margin of India is examined. Results obtained through 2-D flexural backstripping modelling of new seismic data reveal considerable thermo-tectonic subsidence over last ca. 56 Myr. Reverse postrift subsidence modelling with variable β stretching factor predicts residual topography of ca. 2000 m to the west of Shelf Margin Basin and fails to restore late Paleocene horizon and the underlying igneous basement to the sea level. This potentially implies that:(1) either the igneous basement formed during the late Cretaceous was emplaced under open marine environs; or(2) a laterally varying cumulative subsidence occurred within Mumbai Offshore Basin(MOB) during ca. 68 to ca. 56 Ma. Pre-depositional topographic variations at ca. 56 Ma across the basin could be attributed to the extensional processes such as varied lower crustal underplating along Western Continental Margin of India(WCMI). Investigations about basement tectonics after unroofing of sediments since late Paleocene from this region support a transitional and heavily stretched nature of crust with high to very high β factors. Computations of past sediment accumulation rates show that the basin sedimentation peaked during late Miocene concurrently with uplift of Himalayan-Tibetan Plateau and intensification of Indian monsoon system. Results from basin subsidence modelling presented here may have significant implications for further studies attempting to explore tectono-climatic interactions in Asia.D.K.Pandey Nisha Nair Anju Pandey G. 15Latent autoimmune diabetes in adults:A distinct but heterogeneous clinical entity显示文摘Latent autoimmune diabetes in adults(LADA) accounts for 2-12 of all cases of diabetes.Patients are typically diagnosed after 35 years of age and are often misdiagnosed as type Ⅱ Diabetes Mellitus(DM).Glycemic control is initially achieved with sulfonylureas but patients eventually become insulin dependent more rapidly than with type Ⅱ DM patients.Although they have a type Ⅱ DM phenotype,patients have circulating beta(β) cell autoantibodies,a hallmark of type Ⅰ DM.Alternative terms that have been used to describe this condition include type 1.5 diabetes,latent type Ⅰ diabetes,slowly progressive Insulin Dependent Diabetes Mellitus,or youth onset diabetes of maturity.With regards to its autoimmune basis and rapid requirement for insulin,it has been suggested that LADA is a slowly progressive form of type Ⅰ DM.However,recent work has revealed genetic and immunological differences between LADA and type Ⅰ DM.The heterogeneity of LADA has also led to the proposal of criteria for its diagnosis by the Immunology of Diabetes Society.Although many workers have advocated a clinically oriented approach for screening of LADA,there are no universally accepted criteria for autoantibody testing in adult onset diabetes.Following recent advances in immunomodulatory therapies in type Ⅰ DM,the same strategy is being explored in LADA.This review deals with the contribution of the genetic,immunological and metabolic components involved in the pathophysiology of LADA and recent approaches in screening of this distinct but heterogeneous clinical entity.Bimota Nambam Shakti Aggarwal Anju Jain 2010World Journal of Diabetes2010,1,4:2

17Mechanism of crustal extension in the Laxmi Basin,Arabian Sea显示文摘Continental rifting and magmatism has been extensively studied worldwide as it is believed that continental rifting, break up of continents and associated magmatism lead to genesis of new oceanic crust. However, various regions of the world show that these processes may lead to genesis of other types of crust than the oceanic crust. Laxmi Basin in the western continental margin of the India is one such region with an enigmatic crust.Due to its extreme strategic significance for the palaeogeographic reconstruction of continents during Cretaceous continental breakup of India, this basin has attracted various workers for more than two decades. However, still the issue of nature of crust in the basin remains controversial. In this contribution, in order to identify nature of crust, mechanism of continental extension in the Laxmi Basin has been studied for the first time through newly acquired seismic data from the basin. Here, we propose a plausible mechanism of crustal extension in the Laxmi Basin which eventually constrains the nature of crust of the Laxmi Basin. We have demonstrated that the crust in the Laxmi Basin can be categorised in two zones of stretched and transitional crust. In the stretched zone several fault bounded horst and graben structures are identified which preserve syn- and post-rift sediments along with different periods of hiatus in sedimentations as unconformities. These faults are identified as listric faults in the upper crust which sole out in the detachment faults.Detachment faults decouples the upper brittle and lower ductile crust. The transitional crust is identified as heavily intruded by sills and basaltic volcanic which were emplaced due to melting of subcontinental mantle(SCM) after hyper-stretching of crust and serpentinisation of the SCM.
