| 1 | Celiac disease serology in patients with different pretest probabilities: Is biopsy avoidable?显示文摘AIM: To establish the diagnostic performance of sev-eral serological tests, individually and in combination, for diagnosing celiac disease (CD) in patients with different pretest probabilities, and to explore potential se- rological algorithms to reduce the necessity for biopsy. METHODS: We prospectively performed duodenal biopsy and serology in 679 adults who had either high risk (n = 161) or low risk (n = 518) for CD. Blood samples were tested using six assays (enzyme-linked immunosorbent assay) that detected antibodies to tissue transglutaminase (tTG) and deamidated gliadin peptide (DGP). RESULTS: CD prevalence was 39.1% in the high-risk population and 3.3% in the low-risk group. In high-risk patients, all individual assays had a high diagnostic efficacy [area under receiving operator characteristic curves (AU ROC): 0.968 to 0.999]. In contrast, assays had a lower diagnostic efficacy (AU ROC: 0.835 to 0.972) in the low-risk group. Using assay combinations, it would be possible to reach or rule out diagnosis of CD without biopsy in 92% of cases in both pretest populations. We observed that the new DGP/tTG Screen assay resulted in a surplus compared to more conventional assays in any clinical situation. CONCLUSION: The DGP/tTG Screen assay could be considered as the best initial test for CD. | 2 | Safety of endovascular therapy for symptomatic intracranial artery stenosis:a national prospective registry显示文摘Introduction The safety outcomes of endovascular therapy for intracranial artery stenosis in a real-world stetting are largely unknown.The Clinical Registration Trial of Intracranial Stenting for Patients with Symptomatic Intracranial Artery Stenosis(CRTICAS)was a prospective,multicentre,real-world registry designed to assess these outcomes and the impact of centre experience.Methods 1140 severe,symptomatic intracranial arterial stenosis(ICAS)patients treated with endovascular therapy were included from 26 centres,further divided into three groups according to the annual centre volume of intracranial angioplasty and stent placement procedures over 2 years:(1)high volume for≥25 cases/year;(2)moderate volume for 10–25 cases/year and(3)low volume for<10 cases/year.Results The rate of 30-day stroke,transient ischaemic attack or death was 9.7%(111),with 5.4%,21.1%and 9.7%in high-volume,moderate-volume and low-volume centres,respectively(p<0.05).Multivariable logistic regression confirmed high-volume centres had a significantly lower primary endpoint compared with moderate-volume centres(OR=0.187,95%CI:0.056 to 0.627;p≤0.0001),while moderate-volume and low-volume centres showed no significant difference(p=0.8456).Conclusion Compared with the preceding randomised controlled trials,this real-world,prospective,multicentre registry shows a lower complication rate of endovascular treatment for symptomatic ICAS.Non-uniform utilisation in endovascular technology,institutional experience and patient selection in different volumes of centres may have an impact on overall safety of this treatment. | Yabing Wang Tao Wang Adam Andrew Dmytriw Kun Yang Liqun Jiao Huaizhang Shi Jie Lu Tianxiao Li Yujie Huang Zhenwei Zhao Wei Wu Jieqing Wan Qinjian Sun Bo Hong Yongli Li Liyong Zhang Jianfeng Chu Qiong Cheng Yiling Cai Pengfei Wang Qi Luo Hua Yang Baijing Dong Yang Zhang Jun Zhao Zuoquan Chen Wei Li Xiaoxin Bai Weiwen He Xueli Cai Maimai Ti Osama O Zaidat | 2022 | Stroke & Vascular Neurology2022，7，2： | 3 |

| 8 | Persistence of tumor-infiltrating CD8 T cells is tumor-dependent but antigen-independent显示文摘How tumor-infiltrating lymphocytes(TILs)that are tumor-specific but functionally tolerant persist in the antigen-expressing tumor tissue is largely unknown.We have previously developed a modified TRansgenic Adenocarcinoma of the Mouse Prostate(TRAMP)model where prostate cancer cells express the T-cell epitope SIYRYYGL(SIY)recognized by CD8 T cells expressing the 2C T-cell receptor(TCR)(referred to as TRP-SIY mice).In TRP-SIY mice,activated 2C T cells rapidly become tolerant following infiltration into the prostate tumor.In this study,we show that tolerant 2C T cells persist in the prostate tumor of TRP-SIY mice by proliferating slowly in a tumor-dependent,but antigen-,interleukin(IL)-7-and IL-15-independent manner.We also show that disappearance of 2C T cells from the lymphoid organs of TRP-SIY mice are due to antigen-induced T-cell contraction rather than altered trafficking or generalized T-cell depletion in the mice.Finally,we show that clonal T cells unreactive to SIY are equally capable of persisting in the prostate tumor.These findings suggest that while functional tolerance of TILs is induced by antigen,persistence of tolerant TILs in the tumor tissue is mediated by a novel mechanism:slow proliferation independent of antigen and homeostatic cytokines.These results also allow CD8 T-cell survival in the tumor environment to be compared with T-cell survival in chronic infection. | Mobolaji O Olurinde Ching-Hung Shen Adam Drake Ailin Bai Jianzhu Chen | 2011 | Cellular & Molecular Immunology2011，8，5： | 1 |
