| 3 | Comparison of the diagnostic yield and outcomes between standard 8 h capsule endoscopy and the new 12 h capsule endoscopy for investigating small bowel pathology显示文摘AIM: To evaluate the completion rate and diagnostic yield of the Pill Cam SB2-ex in comparison to the Pill Cam SB2.METHODS:Two hundred cases using the 8-h Pill Cam SB2 were retrospectively compared to 200 cases using the 12 h Pill Cam SB2-ex at a tertiary academic center.Endoscopically placed capsules were excluded from the study.Demographic information,indications for capsule endoscopy,capsule type,study length,completion of exam,clinically significant findings,timestamp of most distant finding,and significant findings beyond 8 h were recorded.RESULTS:The 8 and 12 h capsule groups were well matched respectively for both age(70.90±14.19vs 71.93±13.80,P=0.46)and gender(45.5%vs48%male,P=0.69).The most common indications for the procedure in both groups were anemia and obscure gastrointestinal bleeding.Pill Cam SB2-ex had a significantly higher completion rate than Pill Cam SB2(88%vs 79.5%,P=0.03).Overall,the diagnostic yield was greater for the 8 h capsule(48.5%for SB2vs 35%for SB2-ex,P=0.01).In 4/70(5.7%)of abnormal SB2-ex exams the clinically significant findingwas noted in the small bowel beyond the 8 h mark.CONCLUSION:In our study,we found the Pill Cam SB2-ex to have a significantly increased completion rate,though without any improvement in diagnostic yield compared to the Pill Cam SB2. | Merajur Rahman Stuart Akerman Bethany De Vito Larry Miller Meredith Akerman Keith Sultan | 2015 | World Journal of Gastroenterology2015,21,18: | 12 |
| 4 | Interferon-a plus lamivudine vslamivudine reduces breakthroughs, but does not affect sustained response in HBeAg negative chronic hepatitis B显示文摘AIM: To investigate the efficacy of combination treatment of IFN-α and lamivudine compared to lamivudine monotherapy, after 24 mo of administration in HBeAg-negative hepatitis B patients.METHODS: Fifty consecutive patients were randomly assigned to receive IFN-α-2b (5 MU thrice per week, n = 24) plus lamivudine (100 mg daily) or lamivudine only (n = 26) for 24 mo. Patients were followed up for further 6 mo. The primary outcome was the proportion with sustained virological response (undetectable serum HBV DNA concentrations) and or sustained biochemical response (transaminase levels within normal range) at 30 mo (6 mo after the end of therapy). Secondary end-points were timed from initial virological (biochemical) response to VBR (BBR, respectively) and the emergence of YMDD mutants across the two arms.RESULTS: Five of twenty-four (21%) patients in the combination arm vs 3/26 (12%) in the lamivudine arm had sustained response (i.e., normal serum transaminase levels and undetectable HBV DNA by PCR assay) 6 mo after treatment discontinuation. A reduction in the emergence of YMDD mutants and in the development of virological breakthroughs was observed in patients receiving combination treatment (10% vs46%, P= 0.01 and 14% vs46%, P= 0.03,respectively).Time from initial virologic response to virologic breakthrough (VBR) was greater among initial responders receiving combination treatment compared to those receiving lamivudine (22.9 mo vs 15.9 mo, respectively; P = 0.005).CONCLUSION: Our results demonstrate that IFN-α plus lamivudine combination therapy does not increase the sustained response, compared to lamivudine. | 11 | Remote ischaemic conditioning for stroke: unanswered questions and future directions显示文摘Remote ischaemic conditioning(RIC)refers to a process whereby periods of intermittent ischaemia,typically via the cyclical application of a blood pressure cuff to a limb at above systolic pressure,confers systemic protection against ischaemia in spatially distinct vascular territories.The mechanisms underlying this have not been characterised fully but have been shown to involve neural,hormonal and systemic inflammatory signalling cascades.Preclinical and early clinical studies have been promising and suggest beneficial effects of RIC in acute ischaemic stroke,symptomatic intracranial stenosis and vascular cognitive impairment.Through systematic searches of several clinical trials databases we identified 48 active clinical trials of RIC in ischaemic stroke,intracerebral haemorrhage and subarachnoid haemorrhage.We summarise the different RIC protocols and outcome measures studied in ongoing clinical trials and highlight which studies are most likely to elucidate the underlying biological mechanisms of RIC and characterise its efficacy in the near future.We discuss the uncertainties of RIC including the optimal frequency and duration of therapy,target patient groups,cost-effectiveness,the confounding impact of medications and the absence of a clinically meaningful biomarker of the conditioning response.With several large clinical trials of RIC expected to report their outcomes within the next 2 years,this review aims to highlight the most important studies and unanswered questions that will need to be addressed before this potentially widely accessible and low-cost intervention can be used in clinical practice. | Sheharyar Baig Bethany Moyle Krishnan Padmakumari Sivaraman Nair Jessica Redgrave Arshad Majid Ali Ali | 2021 | Stroke & Vascular Neurology2021,6,2: | 2 |
