1Expression of HER2 and bradykinin B_1 receptors in precursor lesions of gallbladder carcinoma显示文摘AIM:To determine the expression of HER2 and bradykinin B 1 receptors(B 1 R) in the two pathogenic models of gallbladder cancer:the metaplasia-dysplasia-carcinoma and the adenoma-carcinoma pathways.METHODS:Receptor proteins were visualized by immunohistochemistry on 5-μm sections of paraffin-embedded tissue.Expression of both receptors was studied in biopsy samples from 92 patients(6 males and 86 females;age ranging from 28 to 86 years,mean 56 years).High HER2 expression in specimens was additionally investigated by fluorescence in situ hybridiza-tion.Cell proliferation in each sample was assessed by using the Ki-67 proliferation marker.RESULTS:HER2 receptor protein was absent in adenomas and in normal gallbladder epithelium.On the contrary,there was intense staining for HER2 on the basolateral membrane of epithelial cells of intestinal metaplasia(22/24;91.7%) and carcinoma in situ(9/10;90%),the lesions that displayed a significantly high proliferation index.Protein up-regulation of HER2 in the epithelium with metaplasia or carcinoma in situ was not accompanied by HER2 gene amplification.A similar result was observed in invasive carcinomas(0/12).The B 1 R distribution pattern mirrored that of HER2 except that B 1 R was additionally observed in the adenomas.The B 1 R appeared either as cytoplasmic dots or labeling on the apical cell membrane of the cells composing the epithelia with intestinal metaplasia(24/24;100%) and carcinoma in situ(10/10;100%) and in the epithelial cells of adenomas.In contrast,both HER2(4/12;33%) and B 1 R(1/12;8.3%) showed a low expression in invasive gallbladder carcinomas.CONCLUSION:The up-regulation of HER2 and B1R in precursor lesions of gallbladder carcinoma suggests cross-talk between these two receptors that may be of importance in the modulation of cell proliferation in gallbladder carcinogenesis.Cesar Toledo Carola E Matus Ximena Barraza Pamela Arroyo Pamela Ehrenfeld Carlos D Figueroa Kanti D Bhoola Maeva del Pozo Maria T Poblete 2012World Journal of Gastroenterology2012,18,11:6

3Coronary artery calcium score on low-dose computed tomography for lung cancer screening显示文摘AIM: To evaluate the feasibility of coronary artery calcium score(CACS) on low-dose non-gated chest CT(ngCCT).METHODS: Sixty consecutive individuals(30 males; 73 ± 7 years) scheduled for risk stratification by means of unenhanced ECG-triggered cardiac computed to-mography(gCCT) underwent additional unenhanced ngCCT. All CT scans were performed on a 64-slice CT scanner(Somatom Sensation 64 Cardiac, Siemens, Germany). CACS was calculated using conventional methods/scores(Volume, Mass, Agatston) as previ-ously described in literature. The CACS value obtained were compared. The Mayo Clinic classification was used to stratify cardiovascular risk based on Agatston CACS. Differences and correlations between the two methods were compared. A P-value < 0.05 was considered sig-nificant.RESULTS: Mean CACS values were significantly higher for gCCT as compared to ngCCT(Volume: 418 ± 747 vs 332 ± 597; Mass: 89 ± 151 vs 78 ± 141; Agatston: 481 ± 854 vs 428 ± 776; P < 0.05). The correlation between the two values was always very high(Volume: r = 0.95; Mass: r = 0.97; Agatston: r = 0.98). Of the 6 patients with 0 Agatston score on gCCT, 2(33%) showed an Agatston score > 0 in the ngCCT. Of the 3 patients with 1-10 Agatston score on gCCT, 1(33%) showed an Agatston score of 0 in the ngCCT. Overall, 23(38%) patients were reclassified in a different car-diovascular risk category, mostly(18/23; 78%) shifting to a lower risk in the ngCCT. 4Association of insertion-deletions polymorphisms with colorectal cancer risk and clinical features显示文摘AIM To investigate the association between 16 insertiondeletions(INDEL) polymorphisms, colorectal cancer(CRC) risk and clinical features in an admixed population.METHODS O n e h u n d re d a n d fo r ty p a t i e n t s w i t h C R C a n d 140 cancer-free subjects were examined. Genomic DNA was extracted from peripheral blood samples. Polymorphisms and genomic ancestry distribution were assayed by Multiplex-PCR reaction, separated by capillary electrophoresis on the ABI 3130 Genetic Analyzer instrument and analyzed on Gene Mapper ID v3.2. Clinicopathological data were obtained by consulting the patients' clinical charts, intra-operative documentation, and pathology scoring.RESULTS Logistic regression analysis showed that polymorphism variations in IL4 gene was associated with increased CRC risk, while TYMS and UCP2 genes were associated with decreased risk. Reference to anatomical localization of tumor Del allele of NFKB1 and CASP8 were associated with more colon related incidents than rectosigmoid. In relation to the INDEL association with tumor node metastasis(TNM) stage risk, the Ins alleles of ACE, HLAG and TP53(6 bp INDEL) were associated with higher TNM stage. Furthermore, regarding INDEL association with relapse risk, the Ins alleles of ACE, HLAG, and UGT1A1 were associated with early relapse risk, as well as the Del allele of TYMS. Regarding INDEL association with death risk before 10 years, the Ins allele of SGSM3 and UGT1A1 were associated with death risk.CONCLUSION The INDEL variations in ACE, UCP2, TYMS, IL4, NFKB1, CASP8, TP53, HLAG, UGT1A1, and SGSM3 were associated with CRC risk and clinical features in an admixed population. 8Hypofractionated and Low Dose Radiotherapy Combined with Temozolomide in Naive Unresectable Glioblastoma:A Pilot Study显示文摘Background/Aim: To assess safety of hypo-fractionated radiotherapy( HyRT) followed by low dose radiation therapy( LD-FRT) plus Temozolomide( TMZ) in naive unresectable Glioblastoma. Methods: Patients( ECOG < 2,age > 18) undergone to biopsy or with gross residual tumor after surgery were enrolled. HyRT( 30 Gy in ten fraction) plus TMZ was administered. From the second adjuvant cycle of TMZ,patients received LD-FRT( 0. 40 Gy twice daily over 5 days,every 28 days) for two cycles. The primary endpoints were safety and toxicity. Moreover we analyzed response,overall survival( OS) and progression-free survival( PFS). Results: Twenty patients were enrolled. Median dose of LD-FRT was 12 Gy. All toxicities were reversible. Four out of 20 patients had Partial Response( PR)and 6 experienced a stable disease( SD). Median OS and PFS were 14 and 11 months,respectively.Conclusions: HyRT followed by LD-FRT plus TMZ is safe and shows good clinical outcomes.
