| 5 | DMP1 prevents osteocyte alterations,FGF23 elevation and left ventricular hypertrophy in mice with chronic kidney disease显示文摘During chronic kidney disease (CKD),alterations in bone and mineral metabolism include increased production of the hormone fibroblast growth factor 23 (FGF23) that may contribute to cardiovascular mortality.The osteocyte protein dentin matrix protein 1 (DMP1) reduces FGF23 and enhances bone mineralization,but its effects in CKD are unknown.We tested the hypothesis that DMP1 supplementation in CKD would improve bone health,prevent FGF23 elevations and minimize consequent adverse cardiovascular outcomes.We investigated DMP1 regulation and effects in wild-type (WT) mice and the Col4a3^-/- mouse model of CKD.Col4a3^-/- mice demonstrated impaired kidney function,reduced bone DMP1 expression,reduced bone mass,altered osteocyte morphology and connectivity,increased osteocyte apoptosis,increased serum FGF23,hyperphosphatemia,left ventricular hypertrophy (LVH),and reduced survival.Genetic or pharmacological supplementation of DMP1 in Col4a3^-/- mice prevented osteocyte apoptosis,preserved osteocyte networks,corrected bone mass,partially lowered FGF23 levels by attenuating NFAT-induced FGF23 transcription,and further increased serum phosphate.Despite impaired kidney function and worsened hyperphosphatemia,DMP1 prevented development of LVH and improved Col4a3^-/- survival.Our data suggest that CKD reduces DMP1 expression,whereas its restoration represents a potential therapeutic approach to lower FGF23 and improve bone and cardiac health in CKD. | Corey Dussold Claire Gerber Samantha White Xueyan Wang Lixin Qi Connor Francis Maralee Capella Guillaume Courbon Jingya Wang Chaoyuan Li Jian Q. | 7 | Scientific evidence is just the starting point: A generalizable process for developing sports injury prevention interventions显示文摘Background: The 2 most cited sports injury prevention research frameworks incorporate intervention development, yet little guidance is available in the sports science literature on how to undertake this complex process. This paper presents a generalizable process for developing implementable sports injury prevention interventions, including a case study applying the process to develop a lower limb injury prevention exercise training program(Footy First) for community Australian football.Methods: The intervention development process is underpinned by 2 complementary premises:(1) that evidence-based practice integrates the best available scientific evidence with practitioner expertise and end user values and(2) that research evidence alone is insufficient to develop implementable interventions.Results: The generalizable 6-step intervention development process involves(1) compiling research evidence, clinical experience, and knowledge of the implementation context;(2) consulting with experts;(3) engaging with end users;(4) testing the intervention;(5) using theory; and(6)obtaining feedback from early implementers. Following each step, intervention content and presentation should be revised to ensure that the final intervention includes evidence-informed content that is likely to be adopted, properly implemented, and sustained over time by the targeted intervention deliverers. For Footy First, this process involved establishing a multidisciplinary intervention development group, conducting 2targeted literature reviews, undertaking an online expert consensus process, conducting focus groups with program end users, testing the program multiple times in different contexts, and obtaining feedback from early implementers of the program.Conclusion: This systematic yet pragmatic and iterative intervention development process is potentially applicable to any injury prevention topic across all sports settings and levels. Saltwater intrusion and water management in coastal wetlands显示文摘Coastal wetlands perform a unique set of physical,chemical,and biological functions,which provide billions of dollars of ecosystem services annually.These wetlands also face myriad environmental and anthropogenic pressures,which threaten their ecological condition and undermine their capacity to provide these services.Coastal wetlands have adapted to a dynamic range of natural disturbances over recent millennia,but face growing pressures from human population growth and coastal development.These anthropogenic pressures are driving saltwater intrusion(SWI)in many coastal systems.The position of coastal wetlands at the terrestrial-marine interface also makes them vulnerable to increasing rates of sealevel rise and changing climate.Critically,anthropogenic and natural stressors to coastal wetlands can act synergistically to create negative,and sometimes catastrophic,consequences for both human and natural systems.This review focused on the drivers and impacts of SWI in coastal wetlands and has two goals:(1)to synthesize understanding of coastal wetland change driven by SWI and(2)to review approaches for improved water management to mitigate SWI in impacted systems.While we frame this review as a choice between restoration and retreat,we acknowledge that choices about coastal wetland management are context-specific and may be confounded by competing management goals.In this setting,the choice between restoration and retreat can be prioritized by identifying where the greatest return in ecosystem services can be achieved relative to restoration dollars invested.We conclude that restoration and proactive water management is feasible in many impacted systems. | Elliott White,Jr. David Kaplan | 2017 | Ecosystem Health and Sustainability2017,3,1: | 2 |
