| 1 | A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. | 7 | Clinical characteristics of fatal and recovered cases of coronavirus disease 2019 in Wuhan,China:a retrospective study显示文摘Background:The 2019 novel coronavirus has caused the outbreak of the acute respiratory disease in Wuhan,Hubei Province of China since December 2019.This study was performed to analyze the clinical characteristics of patients who succumbed to and who recovered from 2019 novel coronavirus disease(COVID-19).Methods:Clinical data were collected from two tertiary hospitals in Wuhan.A retrospective investigation was conducted to analyze the clinical characteristics of fatal cases of COVID-19(death group)and we compare them with recovered patients(recovered group).Continuous variables were analyzed using the Mann-WhitneyU test.Categorical variables were analyzed byχ2 test or Fisher exact test as appropriate.Results:Our study enrolled 109 COVID-19 patients who died during hospitalization and 116 recovered patients.The median age of the death group was older than the recovered group(69[62,74]vs.40[33,57]years,Z=9.738,P<0.001).More patients in the death group had underlying diseases(72.5%vs.41.4%,χ2=22.105,P<0.001).Patients in the death group had a significantly longer time of illness onset to hospitalization(10.0[6.5,12.0]vs.7.0[5.0,10.0]days,Z=3.216,P=0.001).On admission,the proportions of patients with symptoms of dyspnea(70.6%vs.19.0%,χ2=60.905,P<0.001)and expectoration(32.1%vs.12.1%,χ2=13.250,P<0.001)were significantly higher in the death group.The blood oxygen saturation was significantly lower in the death group(85[77,91]%vs.97[95,98]%,Z=10.625,P<0.001).The white blood cell(WBC)in death group was significantly higher on admission(7.23[4.87,11.17]vs.4.52[3.62,5.88]×109/L,Z=7.618,P<0.001).Patients in the death group exhibited significantly lower lymphocyte count(0.63[0.40,0.79]vs.1.00[0.72,1.27]×109/L,Z=8.037,P<0.001)and lymphocyte percentage(7.10[4.45,12.73]%vs.23.50[15.27,31.25]%,Z=10.315,P<0.001)on admission,and the lymphocyte percentage continued to decrease during hospitalization(7.10[4.45,12.73]%vs.2.91[1.79,6.13]%,Z=5.242,P<0.001).Alanine transaminase(22.00[15.00,34.00]vs.18.70[13.00,30.38]U/L,Z=2.592,P=0.010),aspartate transaminase(34.00[27.00,47.00]vs.22.00[17.65,31.75]U/L,Z=7.308,P<0.001),and creatinine levels(89.00[72.00,133.50]vs.65.00[54.60,78.75]μmol/L,Z=6.478,P<0.001)were significantly higher in the death group than those in the recovered group.C-reactive protein(CRP)levels were also significantly higher in the death group on admission(109.25[35.00,170.28]vs.3.22[1.04,21.80]mg/L,Z=10.206,P<0.001)and showed no significant improvement after treatment(109.25[35.00,170.28]vs.81.60[27.23,179.08]mg/L,Z=1.219,P=0.233).The patients in the death group had more complications such as acute respiratory distress syndrome(ARDS)(89.9%vs.8.6%,χ2=148.105,P<0.001),acute cardiac injury(59.6%vs.0.9%,χ2=93.222,P<0.001),acute kidney injury(18.3%vs.0%,χ2=23.257,P<0.001),shock(11.9%vs.0%,χ2=14.618,P<0.001),and disseminated intravascular coagulation(DIC)(6.4%vs.0%,χ2=7.655,P=0.006).Conclusions:Compared to the recovered group,more patients in the death group exhibited characteristics of advanced age,pre-existing comorbidities,dyspnea,oxygen saturation decrease,increased WBC count,decreased lymphocytes,and elevated CRP levels.More patients in the death group had complications such as ARDS,acute cardiac injury,acute kidney injury,shock,and DIC. | Yan Deng Wei Liu Kui Liu Yuan-Yuan Fang Jin Shang Ling Zhou Ke Wang Fan Leng Shuang Wei Lei Chen Hui-Guo Liu | 2020 | Chinese Medical Journal2020,,11: | 29 |

| 9 | Endoscopic patterns of gastric mucosa and its clinicopathological significance显示文摘AIM: To explore the correlation of magnifying endoscopic patterns and histopathology, Helicobacter pylori (H pylori) infection of the gastric mucosa.METHODS: Gastric mucosal patterns in 140 patients with chronic gastritis were studied using Olympus GIF-Q240Z magnifying endoscope. Histopathological examination, rapid urease test and Warrthin-Starry staining were taken with biopsy samples from the magnified sites of stomach. The magnifying endoscopic patterns were compared with histopathological results and H pylori detection.RESULTS: The pit patterns of gastric mucosa were classified as types A (round spot), B (short rod), C (branched), D (reticular) and E (villus). The detection rate of chronic atrophic gastritis (CAG) by magnifying endoscopy was 94.3 % (33/35),which was significantly higher than that by routine endoscopy (22.9 %, 8/35) (P<0.01). The pit patterns of 31 cases of intestinal metaplasia (IM) appeared as type E in 18 cases (58.1%), type D in 8 cases (25.8 %) and type C in 5 cases (16.1%). Fourteen out of 18 patients (77.8 %) with complete type (type Ⅰ) of IM appeared as type E of pit patterns, whereas only 4 of 13 (30.8 %) patients with incomplete type (types Ⅱ and Ⅲ) of IM appeared as type E (P<0.05). Collecting venules in the anterior of lower part of gastric corpus were subgrouped into types R (regular), I (irregular) and D (disappeared).H pyloriinfection was found in 12.2 %(9/74),60 %(9/15) and 84.3 %(43/51) cases in these types respectively. | 10 | Compound Danshen injection improves endotoxin-induced microcirculatory disturbance in rat mesentery显示文摘AIM: To investigate the effect of compound Danshen injection on lipopolysaccharide (LPS)-induced rat mesenteric microcirculatory dysfunctions and the underlying possible mechanism by an inverted intravital microscope and high-speed video camera system. METHODS: LPS was continuously infused through the jugular artery of male Wistar rats at the dose of 2 mg/kg per hour. Changes in mesenteric microcirculation,such as diameters of arterioles and venules,velocity of RBCs in venules,leukocyte rolling,adhesion and emigration,free radicals released from post-capillary venules,FITC- albumin leakage and mast cell degranulation,were observed through an inverted intravital microscope assisted with CCD camera and SIT camera. Meanwhile,the expression of adhesion molecules CD11b/CD18 and the production of free radical in neutrophils,and the expression of intercellular adhesion molecule 1 (ICAM-1) in human umbilical vein endothelial cells (HUVECs) were quantified by flow cytometry (FACS) in vitro. RESULTS: The continuous infusion with LPS resulted in a number of responses in microcirculation,including a significant increase in the positive region of venulestained with Monastral blue B,rolling and adhesion of leukocytes,production of oxygen radical in venular wall,albumin efflux and enhanced mast cell degranulation in vivo,all of which,except for the leukocyte rolling,were attenuated by the treatment with compound Danshen injection. Experiments performed in vitro further revealed that the expression of CD11b/CD18 and the production of oxygen free radical in neutrophils,and the expression of ICAM-1 in HUVECs were increased by exposure to LPS,and they were attenuated by compound Danshen injection. | 11 | Blocking NF-kB nuclear translocation leads to p53-related autophagy activation and cell apoptosis显示文摘AIM: To investigate the anti-tumor effects of nuclear factor-κB (NF-κB) inhibitor SN50 and related mechanisms of SGC7901 human gastric carcinoma cells. METHODS: MTT assay was used to determine the cytotoxic effects of SN50 in gastric cancer cell line SGC7901. Hoechst 33258 staining was used to detect apoptosis morphological changes after SN50 treatment. Activation of autophagy was monitored with monodansylcadaverine (MDC) staining after SN50 treatment.Immunofluorescence staining was used to detect the expression of light chain 3 (LC3). Mitochondrial membrane potential was measured using the fluorescent probe JC-1. Western blotting analysis were used to determine the expression of proteins involved in apoptosis and autophagy including p53, p53 upregulated modulator of apoptosis (PUMA), damage-regulated autophagy modulator (DRAM), LC3 and Beclin 1. We detected the effects of p53-mediated autophagy activation on the apoptosis of SGC7901 cells with the p53 inhibitor pifithrin-α. RESULTS: The viability of SGC7901 cells was inhibited after SN50 treatment. Inductions in the expression of apoptotic protein p53 and PUMA as well as autophagic protein DRAM, LC3 and Beclin 1 were detected with Western blotting analysis. SN50-treated cells exhibited punctuate microtubule-associated protein 1 LC3 in immunoreactivity and MDC-labeled vesicles increased after treatment of SN50 by MDC staining. Collapse of mitochondrial membrane potential Δψ were detected for 6 to 24 h after SN50 treatment. SN50-induced increases in PUMA, DRAM, LC3 and Beclin 1 and cell death were blocked by the p53 specific inhibitor pifithrin-α. | 15 | Tectonic dynamics and correlation of major earthquake sequences of the Xiaojiang and Qujiang-Shiping fault systems, Yunnan, China显示文摘The N-S trending Xiaojiang fault zone and the NW-SE trending Qujiang-Shiping fault zone are adjacent active fault systems and seismogenic zones associated with strong and major earthquakes in Yunnan, China. To understand the interaction of the two fault systems, and its probable influence on earthquake occurrences, this paper conducts a synthetic study based on data of active tectonics, historical earthquakes, relocated small earthquakes, GPS station velocities and focal mechanism resolutions. The study makes several conclusions. (1) The active southward motion of the western side of the Xiaojiang fault zone (i.e. the side of the Sichuan-Yunnan block) has a persistent and intensive effect on the Qujiang-Shiping fault zone. The later fault zone has absorbed and transformed the southward motion of the western side of the former fault zone through dextral strike-slip/shearing as well as transverse shortening/thrusting. (2) Along the Xiaojiang fault zone, the present sinistral strike-slip/shearing rate decreases from 10 and 8 mm/a on the northern, central and central-southern segments to 4 mm/a on the southern segment. The decreased rate has been adjusted in the area along and surrounding the Qujiang-Shiping fault zone through reverse-dextral faulting and distributed shearing and shortening. (3) The tectonic-dynamic relation between the Xiaojiang fault zone and the Qujiang-Shiping fault zone is also manifested by a close correlation of earthquake occurrences on the two fault zones. From 1500 to 1850 a sequence of strong and major earthquakes occurred along the Xiaojiang fault zone and its northern neighbor, the Zemuhe fault zone, which was characterized by gradually accelerating strain release, gradually shortening intervals between M≥7 events, and major releases occurring in the mid to later stages of the sequence. As a response to this sequence, after an 88-year delay, another sequence of 383 years (from 1588 to 1970) of strong and major earthquakes occurred on the Qujiang-Shiping fault zone, and had the same features in accelerating strain release and its temporal course. | 17 | circRNA_0046366 inhibits hepatocellular steatosis by normalization of PPAR signaling显示文摘AIM To investigate micro(mi)R-34 a-antagonizing circular(circ)RNA that underlies hepatocellular steatosis.METHODS The effect of circ RNA on mi R-34 a was recognized by the mi RNA response element(MRE), and validated by the dual-luciferase reporter assay. Its association with hepatocellular steatosis was investigated in Hep G2-based hepatocellular steatosis induced by free fatty acids(FFAs; 2:1 oleate:palmitate) stimulation. After normalization of the steatosis-related circRNA by expression vector, analysis of mi R-34 a activity,peroxisome proliferator-activated receptor(PPAR)α level, and expression of downstream genes were carried out so as to reveal its impact on the mi R-34 a/PPARα regulatory system. Both triglyceride(TG) assessment and cytopathological manifestations uncovered the role of circRNA in miR-34 a-dependent hepatosteatogenesis.RESULTS Bioinformatic and functional analysis verified circRNA_0046366 to antagonize the activity of mi R-34 a via MRE-based complementation. In contrast to its lowered level during FFA-induced hepatocellular steatosis, circ RNA_0046366 up-regulation abolished the mi R-34 a-dependent inhibition of PPARα that played a critical role in metabolic signaling pathways. PPARα restoration exerted transcriptional improvement to multiple genes responsible for lipid metabolism. TGspecific lipolytic genes [carnitine palmitoyltransferase 1 A(CPT1 A) and solute-carrier family 27 A(SLC27 A)] among these showed significant increase in their expression levels. | 20 | A Review of Seasonal Climate Prediction Research in China显示文摘The ultimate goal of climate research is to produce climate predictions on various time scales. In China, efforts to predict the climate started in the 1930 s. Experimental operational climate forecasts have been performed since the late 1950 s,based on historical analog circulation patterns. However, due to the inherent complexity of climate variability, the forecasts produced at that time were fairly inaccurate. Only from the late 1980 s has seasonal climate prediction experienced substantial progress, when the Tropical Ocean and Global Atmosphere project of the World Climate Research program(WCRP) was launched. This paper, following a brief description of the history of seasonal climate prediction research, provides an overview of these studies in China. Processes and factors associated with the climate variability and predictability are discussed based on the literature published by Chinese scientists. These studies in China mirror aspects of the climate research effort made in other parts of the world over the past several decades, and are particularly associated with monsoon research in East Asia. As the climate warms, climate extremes, their frequency, and intensity are projected to change, with a large possibility that they will increase.
