| 1 | Increased susceptibility for intrahepatic cholestasis of pregnancy and contraceptive-induced cholestasis in carriers of the 1331T>C polymorphism in the bile salt export pump显示文摘AIM: To study the association of three common ABCB11 and ABCC2 polymorphisms (ABCB11: 1331T>C V444A; ABCC2: 3563T>A V1188E and 4544G>A C1515Y) with intrahepatic cholestasis of pregnancy (ICP) and contraceptive-induced cholestasis (CIC). METHODS: ABCB11 and ABCC2 genotyping data were available from four CIC patients and from 42 and 33 ICP patients, respectively. Allele-frequencies of the studied polymorphisms were compared with those in healthy pregnant controls and Caucasian individuals. Furthermore, serum bile acid levels were correlated with the presence or absence of the 1331 C allele.RESULTS: The ABCB11 1331T>C polymorphism was significantly more frequent in cholestatic patients than in pregnant controls: C allele 76.2% (CI, 58.0-94.4) vs 51.3% (CI 35.8-66.7), respectively (P = 0.0007); and CC allele 57.1% (CI 36.0-78.3) vs 20% (CI 7.6-32.4), respectively (P = 0.0065). All four CIC patients were homozygous carriers of the C allele. In contrast, none of the studied ABCC2 polymorphism was overrepresented in ICP or CIC patients. Higher serum bile acid levels were found in carriers of the 1331CC genotype compared to carriers of the TT genotype.CONCLUSION: Our data support a role for the ABCB11 1331T>C polymorphism as a susceptibility factor for the ←←← development of estrogen-induced cholestasis, whereas no such association was found for ABCC2. | 2 | Exogenous sphingomyelinase causes impaired intestinal epithelial barrier function显示文摘AIM: To test the hypothesis that hydrolysis of sphingomyelin to ceramide changes the composition of tight junctions (TJs) with increasing permeability of the intestinal epithelium. METHODS: Monolayers of Caco-2 cells were used as an in vitro model for the intestinal barrier. Permeability was determined by quantifi cation of transepithelialflux and transepithelial resistance. Sphingolipid-rich membrane microdomains were isolated by a discontinuous sucrose gradient and characterized by Western-blot. Lipid content of microdomains was analysed by tandem mass spectrometry. Ceramide was subcellularly localized by immunofluorescent staining.RESULTS: Exogenous sphingomyelinase increased transepithelial permeability and decreased transepithelial resistance at concentrations as low as 0.01 U/mL. Lipid analysis showed rapid accumulation of ceramide in the membrane fractions containing occludin and claudin-4, representing TJs. In these fractions we observed a concomitant decrease of sphingomyelin and cholesterol with increasing concentrations of ceramide. Immunofluorescent staining confirmed clustering of ceramide at the sites of cell-cell contacts. Neutralization of surface ceramide prevented the permeability-increase induced by platelet activating factor. CONCLUSION: Our findings indicate that changes in lipid composition of TJs impair epithelial barrier functions. | 3 | 多种癌-睾丸抗原基因在术前化疗非小细胞肺癌中的表达显示文摘目的 研究癌 睾丸抗原 (CT)抗原在经术前化疗的非小细胞肺癌 (NSCLC)中的表达谱 ,以确定NSCLC经化疗、手术后可能的辅助免疫治疗疫苗治疗组成成分。方法 用逆转录 聚合酶链反应 (RT PCR)检测了 5 1例经化疗后手术的原发性NSCLC病人肿瘤组织中 11种CT抗原基因的表达情况。结果 所检测的 11种CT基因在经化疗后的NSCLC表达最高的为MAGE 3,38% ,其次分别为NY ESO 12 1% ,CT10 17% ,LAGE 115 % ,MAGE 4 13% ,SCP 1、SSX1、SSX4均为 12 % ,MAGE 110 % ,SSX2 6 % ,CT72 %。在这些病人中 ,有 34例 (6 7% )病人表达至少 1种CT基因。在不表达所检测的任何CT基因的 17例病人中 ,13例 (76 % )为Ⅰ期病人。MAGE 3和CT10在NSCLC中表现出了随疾病进展而增加的现象 ;反之 ,SCP 1的表达主要在Ⅰ期和Ⅱ期。将NSCLC经病理分类分为鳞癌和腺癌 ,发现鳞癌中至少表达 1种CT基因 (75 % )和共表达CT基因的频率 (5 5 % )均比腺癌 (5 3%和 2 4 % )高。结论 (1)包括MAGE 3、NY ESO 1、LAGE 1等抗原成分的CT抗原疫苗可应用于术前化疗的NSCLC病人的治疗 ;(2 )对不同NSCLC肿瘤分期的病人可采取不同CT抗原成分疫苗 ;(3)对晚期手术和化疗疗效不好的病人和鳞癌病人 ,CT抗原疫苗治疗是一个值得研究的重要方向。 | 王歈 梁震 袁艳华 韩燕 刘月香 刘年 Yao-Tseng Chen Gerd Ritter Lloyd J Old 张力建 | 2004 | 中华医学杂志2004，84，6： | 4 |

| 8 | Glucocorticoid receptor gene haplotype structure and steroid therapy outcome in IBD patients显示文摘AIM: To study whether the glucocorticoid receptor (GR/ NR3C1) gene haplotypes influence the steroid therapy outcome in inflammatory bowel disease (IBD). METHODS: We sequenced all coding exons and flanking intronic sequences of the NR3C1 gene in 181 IBD patients, determined the single nucleotide polymorphisms, and predicted the NR3C1 haplotypes. Furthermore, we investigated whether certain NR3C1 haplotypes are significantly associated with steroid therapy outcomes. RESULTS: We detected 13 NR3C1 variants, which led to the formation of 17 different haplotypes with a certainty of > 95% in 173 individuals. The three most commonly occurring haplotypes were included in the association analysis of the influence of haplotype on steroid therapy outcome or IBD activity. None of the NR3C1 haplotypes showed statistically signifi cant association with glucocorticoid therapy success.
