1Correlation analysis of dual-energy CT iodine maps with quantitative pulmonary perfusion MRI显示文摘AIM:To correlate dual-energy computed tomography(DECT) pulmonary angiography derived iodine maps with parameter maps of quantitative pulmonary perfusion magnetic resonance imaging(MRI).METHODS:Eighteen patients with pulmonary perfusion defects detected on DECT derived iodine maps were included in this prospective study and additionally underwent time-resolved contrast-enhanced pulmonary MRI [dynamic contrast enhanced(DCE)-MRI].DCE-MRI data were quantitatively analyzed using a pixel-by-pixel deconvolution analysis calculating regional pulmonary blood flow(PBF),pulmonary blood volume(PBV) and mean transit time(MTT) in visually normal lung parenchyma and perfusion defects.Perfusion parameterswere correlated to mean attenuation values of normal lung and perfusion defects on DECT iodine maps.Two readers rated the concordance of perfusion defects in a visual analysis using a 5-point Likert-scale(1 = no correlation,5 = excellent correlation).RESULTS:In visually normal pulmonary tissue mean DECT and MRI values were:22.6 ± 8.3 Hounsfield units(HU);PBF:58.8 ± 36.0 mL/100 mL per minute;PBV:16.6 ± 8.5 mL;MTT:17.1 ± 10.3 s.In areas with restricted perfusion mean DECT and MRI values were:4.0 ± 3.9 HU;PBF:10.3 ± 5.5 mL/100 mL per minute,PBV:5 ± 4 mL,MTT:21.6 ± 14.0 s.The differences between visually normal parenchyma and areas of restricted perfusion were statistically significant for PBF,PBV and DECT(P < 0.0001).No linear correlation was found between MRI perfusion parameters and attenuation values of DECT iodine maps(PBF:r = 0.35,P = 0.15;PBV:r = 0.34,P = 0.16;MTT:r = 0.41,P = 0.08).Visual analysis revealed a moderate correlation between perfusion defects on DECT iodine maps and the parameter maps of DCE-MRI(mean score 3.6,k 0.45).CONCLUSION:There is a moderate visual but not statistically significant correlation between DECT iodine maps and perfusion parameter maps of DCE-MRI.Jan Hansmann Paul Apfaltrer Frank G Zoellner Thomas Henzler Mathias Meyer Gerald Weisser Stefan O Schoenberg Ulrike I Attenberger 2013World Journal of Radiology2013,5,5:6

3儿童扩张型心肌病的发病率、病因及预后显示文摘背景:扩张型心肌病(dilated cardiomyopathy,DCM)是儿童心肌病最常见的类型,也是小儿心脏移植最常见的原因。但是,目前尚不清楚儿童DCM的流行病学特点和临床过程。 目的:详细描述儿童DCM的发病率、病因、预后及相关危险因素。 设计及地点:本项以人群为基础的前瞻性纵向队列研究入选1996年1月至2003年2月在89个儿科心脏中心诊断为DCM的儿童以及1990年1月至1995年12月在北美大型三级治疗中心诊断的患儿。 参试者:共入选1426例来自美国和加拿大的18岁以下DCM患儿。通过严格的超声心动图和/或病理学标准来诊断原发性DCM。因内分泌、免疫、药物中毒及其他原因患病者除外。 主要观测指标:每年每10万儿童的发病率、死亡率和心脏移植率。 结果:18岁以下儿童DCM每年的总发病率为0.57/10万,男孩的年发病率高于女孩(分别为0.66/10万和0.47/10万;P〈0.001),黑人的年发病率高于白人(分别为0.98/10万和0.46/10万;P〈0.001),婴儿(〈1岁)的年发病率高于儿童(分别为4.40/10万和0.34/10万;P〈0.001)。绝大部分属特发性,最常见的病因为心肌炎(46%)和神经肌肉性疾病(26%)。1年和5年死亡/移植率分别为31%和46%。死亡/移植的独立危险因素包括患儿诊断DCM时年龄较大、伴有充血性心力衰竭、左室收缩期Z评分较低以及DCM的病因(P均〈0.001)。 结论:儿童DCM有其多样性。患儿预后主要取决于病因、年龄和心力衰竭状况。人种、性别和年龄对发病率有一定影响。大部分患儿无明确病因,限制了特异性治疗。Jeffrey A. 4Th1 cytokines promote T-cell binding to antigen-presenting cells via enhanced hyaluronan production and accumulation at the immune synapse显示文摘Hyaluronan(HA)production by dendritic cells(DCs)is known to promote antigen presentation and to augment T-cell activation and proliferation.We hypothesized that pericellular HA can function as intercellular‘glue’directly mediating T cell–DC binding.Using primary human cells,we observed HA-dependent binding between T cells and DCs,which was abrogated upon pre-treatment of the DCs with 4-methylumbelliferone(4-MU),an agent which blocks HA synthesis.Furthermore,T cells regulate HA production by DCs via T cell-derived cytokines in a T helper(Th)subset-specific manner,as demonstrated by the observation that cell-culture supernatants from Th1 but not Th2 clones promote HA production.Similar effects were seen upon the addition of exogenous Th1 cytokines,IL-2,interferon c(IFN-c)and tumor necrosis factor a(TNF-a).The critical factors which determined the extent of DC–T cell binding in this system were the nature of the pre-treatment the DCs received and their capacity to synthesize HA,as T-cell clones which were pre-treated with monensin,added to block cytokine secretion,bound equivalently irrespective of their Th subset.These data support the existence of a feedforward loop wherein T-cell cytokines influence DC production of HA,which in turn affects the extent of DC–T cell binding.We also document the presence of focal deposits of HA at the immune synapse between T-cells and APC and on dendritic processes thought to be important in antigen presentation.These data point to a pivotal role for HA in DC–T cell interactions at the IS.Paul L Bollyky Stephen P Evanko Rebecca P Wu Susan Potter-Perigo S Alice Long Brian Kinsella Helena Reijonen Kelly Guebtner Brandon Teng Christina K Chan Kathy R Braun John A Gebe Gerald T Nepom Thomas N Wight 2010Cellular & Molecular Immunology2010,7,3:3
