1Coronary physiology assessment in the catheterization laboratory显示文摘Physicians cannot rely solely on the angiographic appearance of epicardial coronary artery stenosis when evaluating patients with myocardial ischemia. Instead, sound knowledge of coronary vascular physiology and of the methods currently available for its characterization can improve the diagnostic and prognostic accuracy of invasive assessment of the coronary circulation, and help improve clinical decision-making. 7Digital blinding of radiographs to mask allocation in a randomized control trial显示文摘AIM To demonstrate the effectiveness of a digital radiographic altering technique in concealing treatment allocation to blind outcome assessment of distal femur fracture fixation.METHODS Digital postoperative anteroposterior and lateral radiographs from a sample of 33 randomly-selected patients with extra-articular distal femur fractures treated by surgical fixation at a Level 1 trauma center were included. Using commercially available digital altering software, we devised a technique to blind the radiographs by overlaying black boxes over the implant hardware while preserving an exposed fracture site for assessment of fracture healing. Three fellowship-trained surgeons evaluated a set of blinded radiographs twice and a control set of unblinded radiographs once. Each set of radiographs were reviewed independently and in a randomly-assigned order. The degrees of agreement and disagreement among evaluators in identifying implant type while reviewing both blinded and unblinded radiographs were assessed using the Bang Blinding Index and James Blinding Index. The degree of agreement in fracture union was assessed using kappa statistics.RESULTS The assessment of blinded radiographs with both the Bang Blinding Index(BBI) and James Blinding Index(JBI) demonstrated a low degree of evaluator success at identifying implant type(Mean BBI, far cortical locking:-0.03, SD: 0.04; Mean BBI, standard screw: 0, SD: 0; JBI: 0.98, SD: 0), suggesting near perfect blinding. The assessment of unblinded radiographs with both blinding indices demonstrated a high degree of evaluator success at identifying implant type(Mean BBI, far cortical locking: 0.89, SD: 0.19; Mean BBI, standard screw: 0.87, SD: 0.04; JBI: 0.26, SD: 0.12), as expected. There was moderate agreement with regard to assessment of fracture union among the evaluators in both the blinded(Kappa: 0.38, 95%CI: 0.25-0.52) and unblinded(Kappa: 0.35, 95%CI: 0.25-0.45) arms of the study. 15Using computed tomography scans to develop an ex-vivo gastric model显示文摘The objective of this research was to use abdominal computed tomography (CT) scans to non-invasively quantify anthropometrical data of the human stomach and to concomitantly create an anatomically correct and distensible ex-vivo gastric model. Thirty-three abdominal CT scans of human subjects were obtained and were imported into reconstruction software to generate 3D models of the stomachs. Anthropometrical data such as gastric wall thickness, gastric surface area and gastric volume were subsequently quantified. A representative 3D computer model was exported into a selective laser sintering (SLS) rapid prototyping machine to create an anatomically correct solid gastric model. Subsequently, a replica wax template of the SLS model was created. A negative mould was offset around the wax template such that the offset distance was equivalent to that of the gastric wall thickness. A silicone with similar mechanical properties to the human stomach was poured into the offset. The lost wax manufacturing technique was employed to create a hollow distensible stomach model. 3D computer gastric models were generated from the CT scans. A hollow distensible silicone ex-vivo gastric model with similar compliance to that of the human stomach was created. The anthropometrical data indicated that there is no significant relationship between BMI and gastric surface area or gastric volume. There were inter- and intra-group differences between groups with respect to gastric wall thickness. 16Revision total hip arthroplasty: An analysis of the quality and readability of information on the internet显示文摘The demand for revision total hip arthroplasty(THA)is increasing.Information quality on the internet has been extensively analysed in relation to primary THA but no such analysis has ever been performed for revision THA.Our aim was to assess the quality and readability of this information.Three major internet search engines were searched for information on revision THA.All websites were assessed for quality of information using the DISCERN score,the Journal of the American Medical Association benchmark criteria and a novel scoring system specific to revision THA[Vancouver Revision Arthroplasty Information(VRAI)score].Website readability was assessed,as was presence of the Health On the Net Foundation(HON)seal.The majority of websites(52%)were academic with a post-graduate reading level.Only 6.5%of websites had the HON seal.Twentyeight percent of websites had a‘good’DISCERN score and only 28%had a‘good’score with the novel VRAI scoring system.Health information websites had significantly higher rates of‘good’VRAI scores(P=0.008).Websites with the HON seal had significantly higher DISCERN scores(P=0.01).All governmental websites were at a reading level suitable for patient review.Information on the internet relating to revision THA is of low quality,much lower than the quality of information on primary THA.We recommend governmental websites for their readability and health information websites for their quality of information specific to revision THA.Websites with the HON seal provide higher quality information and should be recommended to patients as reading material regarding revision THA.Gerard Anthony Sheridan Carl O’Brien Bassam A Masri Clive P Duncan Donald S Garbuz 2020World Journal of Orthopedics2020,11,2:0
