| 1 | Postoperative adjuvant chemoradiotherapy in D2-dissected gastric cancer: Is radiotherapy necessary after D2-dissection?显示文摘Studies from the Far East have demonstrated that D2-dissection is superior to D0/1-dissection. The effect of postoperative chemoradiotherapy(CRT) after D2-dissection has not been accepted due to the lack of D2-dissection in Western countries, as well as the potential harmful effect of radiotherapy. In the current NCCN guideline, adjuvant chemotherapy alone is recommended in D2-dissected patients. However, three recent prospective randomized controlled trials in South Korea and China(ARTIST, NCC and Multicenter IMRT Trials) demonstrated that adjuvant CRT can be safely administered to D2-dissected patients with notable benefits. To identify the role of radiotherapy(RT) in the D2-dissected postoperative setting, clinical research attempts should include(1) identification of high-risk patients for loco-regional recurrence who might benefit from CRT;(2) modification of RT target volume based on the findings that failure patterns should be different after D1- and D2-dissection; and(3) integration of new RT techniques to decrease treatment-related toxicity. The present paper is a review of recent studies addressing these fields. | 3 | Saeng-Kankunbi-Tang(生肝健脾汤) Protects Liver against Oxidative Damage through Activation of ERK/Nrf2 Pathway显示文摘Objective: To investigate the cytoprotective effects of Saeng-kankunbi-tang(生肝健脾汤, SKT), a herbal prescription consisting of Artemisia capillaris and Alisma canaliculatum, and its underlying mechanism involved. Methods: In mice, blood biochemistry and histopathology were assessed in carbon tetrachloride(CCl4)-induced oxidative hepatic injury in vivo. The animal groups included vehicle-treated control, CCl4, SKT 500 mg/(kg·day) CCl4+SKT 200 or 500 mg/(kg·day). In Hep G2 cell, tert-butyl hydroperoxide(t BHP) induced severe oxidative stress and mitochondrial dysfunction in vitro. The cyto-protective effects of SKT were determined by 3-(4,5-dimethylthiazol)-2,5-diphenyltetrazolium bromide(MTT) assay, fluorescence activated cell sorting analysis and western blotting. Results: The administration of SKT prevented liver damage induced by CCl4 in mice, by inhibition of hepatocyte degeneration and inflammatory cell infiltration as well as plasma parameters such as alanine aminotransferase(P<0.01). Moreover, treatment with t BHP induced hepatocyte death and cellular reactive oxygen species production in hepatocyte cell line. However, SKT pretreatment(30–300 μg/m L) reduced this cell death and oxidative stress(P<0.01). More importantly, SKT inhibited the ability of t BHP to induce changes in mitochondrial membrane transition in cell stained with rhodamine 123(P<0.01). Furthermore, treatment with SKT induced extracellular signal-regulated kinases-mediated nuclear factor erythroid-2-related factor 2(Nrf2) activation as well as the expressions of heme oxygenase 1 and glutamate-cystein ligase catalytic, Nrf2 target genes. | 17 | Predictors of the patency of self-expandable metallic stents in malignant gastroduodenal obstruction显示文摘AIM:To investigate the predictive factors of selfexpandable metallic stent patency after stent placement in patients with inoperable malignant gastroduodenal obstruction. METHODS:A total of 116 patients underwent stent placements for inoperable malignant gastroduodenal obstruction at a tertiary academic center. Clinical success was defined as acceptable decompression of the obstructive lesion within the malignant gastroduodenal neoplasm. We evaluated patient comorbidities and clinical statuses using the World Health Organization's scoring system and categorized patient responses to chemotherapy using the Response Evaluation Criteria in Solid Tumors criteria. We analyzed the relationships between possible predictive factors and stent patency. RESULTS:Self-expandable metallic stent placement was technically successful in all patients(100%),and the clinical success rate was 84.2%. In a multivariate Cox proportional hazards model,carcinoembryonic antigen(CEA) levels were correlated with a reduction in stent patency [P = 0.006; adjusted hazard ratio(a HR)= 2.92,95%CI:1.36-6.25]. | 18 | Rare case of pancreatic cancer with leptomeningeal carcinomatosis显示文摘Leptomeningeal carcinomatosis occurs very rarely in patients with pancreatic cancer.Leptomeningealcarcinomatosis is characterized by multifocal seeding of the leptomeninges by malignant cells that originate from a solid tumor.To the best of our knowledge,brain metastasis from pancreatic cancer is extremely rare.Leptomeningeal carcinomatosis is estimated to occur in 3% to 8% of cases of solid tumors.The clinical manifestation usually involves neurological symptoms,including dizziness,headache,vomiting,nausea,and hemiparesis,symptoms similar to those of meningitis or brain tumors.Diagnostic methods for leptomeningeal carcinomatosis include brain magnetic resonance imaging and cerebrospinal fluid examination.Here,we describe a case of leptomeningeal carcinomatosis in which the primary tumor was later determined to be pancreatic cancer.Brain magnetic resonance imaging findings showed mild enhancement of the leptomeninges,and cerebrospinal fluid cytology was negative at first.However,after repeated spinal taps,atypical cells were observed on cerebrospinal fluid analysis and levels of tumor markers such as carbohydrate antigen 19-9 in cerebrospinal fluid were elevated.Abdominal computed tomography,performed to determine the presence of extracerebral tumors,revealed pancreatic cancer.Pancreatic cancer was confirmed histopathologically on examination of an endoscopic ultrasound-guided fine needle aspiration specimen. | In Kyung Yoo Hong Sik Lee Chang Duk Kim Hoon Jai Chun Yoon Tae Jeen Bora Keum Eun Sun Kim Hyuk Soon Choi Jae Min Lee Seung Han Kim Seung Joo Nam Jong Jin Hyun | 2015 | World Journal of Gastroenterology2015，21，3： | 0 |
