| 1 | Prognostic value of inflammation-based markers in patients with pancreatic cancer administered gemcitabine and erlotinib显示文摘AIM: To evaluate the value of systemic inflammationbased markers as prognostic factors for advanced pancreatic cancer(PC). METHODS: Data from 82 patients who underwent combination chemotherapy with gemcitabine and erlotinib for PC from 2011 to 2014 were collected retrospectively. Data that included the neutrophil-to-lymphocyte ratio(NLR), the platelet-to-lymphocyte ratio, and the C-reactive protein(CRP)-to-albumin(CRP/Alb) ratio were analyzed. Kaplan-Meier curves, and univariate and multivariate Cox proportional hazards regression analyses were used to identify the prognostic factors associated with progression-free survival(PFS) and overall survival(OS). RESULTS: The univariate analysis demonstrated the prognostic value of the NLR(P = 0.049) and the CRP/Alb ratio(P = 0.047) in relation to PFS, and a positiverelationship between an increase in inflammation-based markers and a poor prognosis in relation to OS. The multivariate analysis determined that an increased NLR(hazard ratio = 2.76, 95%CI: 1.33-5.75, P = 0.007) is an independent prognostic factor for poor OS. There was no association between the PLR and the patients' prognoses in those who had received chemotherapy that comprised gemcitabine and erlotinib in combination. | 2 | Effect of metronome rates on the quality of bag-mask ventilation during metronome-guided 30:2 cardiopulmonary resuscitation:A randomized simulation study显示文摘BACKGROUND: Metronome guidance is a feasible and effective feedback technique to improve the quality of cardiopulmonary resuscitation(CPR). The rate of the metronome should be set between 100 to 120 ticks/minute and the speed of ventilation may have crucial effect on the quality of ventilation. We compared three different metronome rates(100, 110, 120 ticks/minute) to investigate its effect on the quality of ventilation during metronome-guided 30:2 CPR.METHODS: This is a prospective, randomized, crossover observational study using a Respi Trainer r. To simulate 30 chest compressions, one investigator counted from 1 to 30 in cadence with the metronome rate(1 count for every 1 tick), and the participant performed 2 consecutive ventilations immediately following the counting of 30. Thirty physicians performed 5 sets of 2 consecutive(total 10) bag-mask ventilations for each metronome rate. Participants were instructed to squeeze the bag over 2 ticks(1.0 to 1.2 seconds depending on the rate of metronome) and defl ate the bag over 2 ticks. The sequence of three different metronome rates was randomized.RESULTS: Mean tidal volume significantly decreased as the metronome rate was increased from 110 ticks/minute to 120 ticks/minute(343±84 m L vs. 294±90 m L, P=0.004). Peak airway pressure significantly increased as metronome rate increased from 100 ticks/minute to 110 ticks/minute(18.7 vs. 21.6 mm Hg, P=0.006).CONCLUSION: In metronome-guided 30:2 CPR, a higher metronome rate may adversely affect the quality of bag-mask ventilations. | 3 | Endoscopic submucosal tunnel dissection salvage technique for ulcerative early gastric cancer显示文摘Endoscopic submucosal dissection is an effective treatment modality for early gastric cancer(EGC), though the submucosal fibrosis found in ulcerative EGC is an obstacle for successful treatment. This report presents two cases of ulcerative EGC in two males, 73- and 80-year-old, with severe fibrosis. As endoscopic ultrasonography suggested that the EGCs had invaded the submucosal layer, the endoscopic submucosal tunnel dissection salvage technique was utilized for complete resection of the lesions. Although surgical gastrectomy was originally scheduled, the two patients had severe coronary heart disease, and surgeries were refused because of the risks associated with their heart conditions. The endoscopic submucosal tunnel dissection salvage technique procedures described in these cases were performed under conscious sedation, and were completed within 30 min. The complete en bloc resection of EGC using endoscopic submucosal tunnel dissection salvage technique was possible with a free resection margin, and no other complications were noted during the procedure. This is the first known report concerning the use of the endoscopic submucosal tunnel dissection salvage technique salvage technique for treatment of ulcerative EGC. | 7 | Hwangryunhaedoktang exerts anti-inflammation on LPS-induced NO production by suppressing MAPK and NF-κB activation in RAW264.7 macrophages显示文摘OBJECTIVE: This study aimed to evaluate whether Hwangryunhaedoktang(HHT), a herbal compound, has an inhibitory effect on lipopolysaccharide(LPS)-induced inflammation in RAW264.7 macrophages.METHODS: The effects of HHT were evaluated by confirming nitric oxide(NO) production and expression of inducible NO synthase(iN OS) and mitogen-activated protein kinases(MAPKs) in LPS-stimulated RAW264.7 macrophages via the Griess assay, Western blotting, and real-time reverse transcription quantitative polymerase chain reaction. Western blot analyses and luciferase assays were used to evaluate whether HHT has an effect on the phosphorylation and translocation of nuclear factor-κB(NF-κB). The secretion and expression of tumor necrosis factor-α(TNF-α) and interleukin-6(IL-6) were determined via enzyme-linked immunosorbent assay and Western blot analyses.RESULTS: HHT suppressed LPS-induced NO production and expression of iN OS in a dose-dependent manner. Additionally, MAPKs activation was also attenuated via inhibition of phosphorylation of extracellular signal-regulated kinases 1/2, c-Jun N-terminal kinase and p38 which were related to inflammatory pathway. Furthermore, HHT also effectively attenuated NF-κB activation and its translocation to the nucleus, a process that is closely linked to inflammation. LPS normally induced the expression of inflammatory cytokines such as TNF-α and IL-6, but the secretion and expression of TNF-α and IL-6 were significantly attenuated by pretreating the cells with HHT.CONCLUSION: HHT suppressed LPS-induced NO production by blocking the activation of NF-κB and MAPK signaling pathways in RAW264.7 macrophages. Furthermore, HHT may have an anti-inflammatory effect by suppressing the LPS-induced secretion of TNF-α and IL-6. A 72-year-old woman presented with a 2-mo history of right upper abdominal pain unrelated to diet and indigestion. Fourteen years earlier, she had been diagnosed with NF-1, which manifested as café au lait spots and multiple nodules on the skin. Computed tomography(CT) revealed a multilocular low-density mass with septation, and mural nodules in the right hepatic lobe, as well as a 1.7-cm-sized well-demarcated enhancing mass in the third portion of the duodenum. The patient subsequently underwent right hepatectomy and duodenal wedge resection. We present here the first report of a case involving a synchronous IPNB and GIST in a patient with NF-1. | 9 | Involvement of heme oxygenase-1 induction in anti-vascular inflammation effects of Xanthoceras sorbifolia in human umbilical vein endothelial cells显示文摘OBJECTIVE: To define the effects of Xanthoceras sorbifolia(EXS) on vascular inflammation and the mechanisms in endothelial cells.METHODS: Vascular protective effects of an ethanol extract of seeds from EXS(1-50 μg/mL) against tumor necrosis factor-α(TNF-α)-induced vascularinflammation were examined in human umbilical vein endothelial cells(HUVECs).RESULTS: EXS significantly decreased TNF-α-induced expression of cell adhesion molecules, such as intracellular adhesion molecule-1, vascular cell adhesion molecule-1, and endothelial cell selectin,in a dose-dependent manner. Pre-treatment with EXS significantly inhibited translocation and transcriptional activity of nuclear factor-κB(NF-κB) increased by TNF-α. EXS also significantly inhibited formation of intracellular reactive oxygen species(ROS). Moreover, the vascular protective effects of EXS were linked to up-regulation of heme oxygenase-1(HO-1) and nuclear factor E2-related factor-2(Nrf-2) expression. EXS-induced HO-1 expression was significantly decreased in SnPP(HO-1 inhibitor)-and HO-1 siRNA-treated cells, whereas an increase was found in cobalt protoporphyrin IX(CoPP)(HO-1 inducer)-treated cells. In addition, pretreatment with EXS increased HO-1 and Nrf-2 expression under TNF-α stimulation with or without N-acetyl-L-cysteine. | 11 | Differential diagnoses of magnetic resonance imaging for suspected acute appendicitis in pregnant patients显示文摘BACKGROUND: Accurate and timely diagnosis of acute surgical disease in pregnant patient is chal enging. Although magnetic resonance imaging(MRI) is the most accurate modality to diagnose acute appendicitis in pregnant patients, it is often used as a last resort because of high cost and long scan time. We performed this study to analyze differential diagnoses of appendix MRI and to investigate if there are any blood tests that can predict surgical condition in pregnant patients.METHODS: A retrospective, cross-sectional study was conducted on 46 pregnant patients who underwent non-enhanced appendix MRI in suspicion of acute appendicitis from 2010 to 2016. Differential diagnoses of appendix MRI were analyzed and blood tests were compared between those who had surgical and non-surgical disease.RESULTS: Appendix MRI differentiated two surgical disease; acute appendicitis and ovarian torsion; and various non-surgical conditions such as uterine myoma, hydronephrosis, ureterolithiasis and diverticulitis among clinically suspected acute appendicitis in pregnancy. The diagnostic accuracy of MRI for acute appendicitis in this study was 93.5%. Patients who had surgical disease showed significantly higher WBC count(≥11,000/mm^3), proportion of neutrophils in the WBC(≥79.9%), neutrophil-to-lymphocyte ratio(NLR≥6.4), levels of C-reactive protein(CRP≥1.82 mg/dL) and bilirubin(≥0.66 mg/dL) than those who had non-surgical disease.CONCLUSION: MRI can reliably differentiate surgical conditions and several blood tests(WBC, proportion of neutrophils in the WBC, NLR, CRP, bilirubin) can help anticipate acute surgical condition among pregnant patients suspected to have acute appendicitis. | Ji Yong Jung Ji Ung Na Sang Kuk Han Pil Cho Choi Jang Hee LEE Dong Hyuk Shin | 2018 | World Journal of Emergency Medicine2018,9,1: | 4 |

| 15 | Radial growth response of Pinus densiflora and Quercus spp.to topographic and climatic factors in South Korea显示文摘Aims This study aimed to develop radial growth models and to predict the potential spatial distribution of Pinus densiflora(Japanese red pine)and Quercus spp.(Oaks)in South Korea,considering topographic and climatic factors.Methods We used a dataset of diameter at breast height and radial growth estimates of individual trees,topographic and climatic factors in systematic sample plots distributed over the whole of South Korea.On the basis that radial growth is attributed primarily to tree age,we developed a radial growth model employing tree age as an explanatory variable.We estimated standard growth(SG),defined as radial growth of the tree at age 30,to eliminate the influence of tree age on radial growth.In addition,SG estimates including the Topographic Wetness Index,temperature and precipitation were calculated by the Generalized Additive Model.Important Findings As a result of variogram analysis of SG,we found spatial autocorrelation between SG,topographic and climatic factors.Incremental temperature had negative impacts on radial growth of P.densiflora and positive impacts on that of Quercus spp.Precipitation was associated with positive effects on both tree species.Based on the model,we found that radial growth of P.densiflora would be more vulnerable than that of Quercus spp.to climatic factors.Through simulation with the radial growth model,it was predicted that P.densiflora stands would be gradually replaced with Quercus spp.stands in eastern coastal and southern regions of South Korea in the future.The models developed in this study will be helpful for understanding the impact of climatic factors on tree growth and for predicting changes in distribution of P.densiflora and Quercus spp.due to climate change in South Korea. | Jae Gyun Byun Woo Kyun Lee Moonil Kim Doo Ahn Kwak Hanbin Kwak Taejin Park Woo Hyuk Byun Yowhan Son Jung Kee Choi Young Jin Lee Joachim Saborowski Dong Jun Chung Jin Hyun Jung | 2013 | Journal of Plant Ecology2013,6,5: | 3 |
| 16 | Endoscopic en bloc resection of an exophytic gastrointestinal stromal tumor with suction excavation technique显示文摘Here, we report the first successful endoscopic resection of an exophytic gastrointestinal stromal tumor(GIST) using a novel perforation-free suction excavation technique. A 49-year-old woman presented for further management of a gastric subepithelial tumor on the lesser curvature of the lower body, originally detected via routine upper gastrointestinal endoscopy. Abdominal computed tomography and endoscopic ultrasound showed a 4-cm extraluminally protruding mass originating from the muscularis propria layer. The patient firmly refused surgical resection owing to potential cardiac problems, and informed consent was obtained for endoscopic removal. Careful dissection and suction of the tumor was repeated until successful extraction was achieved without serosal injury. We named this procedure the suction excavation technique. The tumor's dimensions were 3.5 cm × 2.8 cm × 2.5 cm. The tumor was positive for C-KIT and CD34 by immunohistochemical staining. The mitotic count was 6/50 high-power fields. The patient was followed for 5 years without tumor recurrence. | 17 | Bevacizumab as a second- or later-line of treatment for metastatic colorectal cancer显示文摘AIM: To determine the efficacy of bevacizumab in patients with metastatic colorectal cancer (MCRC) who have failed prior chemotherapy without bevacizumab. METHODS: Between March 2002 and June 2010, 40 patients in South Korea with MCRC who were treated with bevacizumab plus chemotherapy as a second or later-line treatment were analyzed retrospectively for their overall response rate (ORR), overall survival (OS), and progression-free survival (PFS). The tumor responses were assessed using the RECIST (Response Evaluation Criteria in Solid Tumors) guidelines. RESULTS: All of the patients had progressed under prior chemotherapy without bevacizumab. Three patients (7.5%) exhibited an ORR, twenty one patients (52.5%) exhibited stable disease (SD), and fifteen patients (37.5%) exhibited disease progression. The median duration of the OS and PFS were 14.0 and 6.13 mo respectively. The median OSs were 16.60, 14.07 and 13.00 mo for second-line, third-line and fourth- or later-line treatments, respectively. The median PFSs were 7.23, 7.30 and 3.87 mo for the second-line, third-line and fourth- or later-line treatments, respectively. | 18 | Cytomegalovirus ileo-pancolitis presenting as toxic megacolon in an immunocompetent patient: A case report显示文摘BACKGROUND Cytomegalovirus(CMV) enterocolitis presenting in the form of pancolitis or involving the small and large intestines in an immunocompetent patient is rarely encountered, and CMV enterocolitis presenting with a serious complication, such as toxic megacolon, in an immunocompetent adult has only been reported on a few occasions.CASE SUMMARY We describe the case of a 70-year-old male with no history of inflammatory bowel disease or immunodeficiency who presented with toxic megacolon and subsequently developed massive hemorrhage as a complication of CMV ileopancolitis. The patient was referred to our institute for abdominal pain and distension. Abdominal X-ray showed marked dilatation of ileum and whole colon without air-fluid level, and sigmoidoscopy with biopsy failed to reveal any specific finding. After 7 d of conservative treatment, massive hematochezia developed, and he was diagnosed to have CMV enterocolitis by colonoscopy with biopsy. Although the diagnosis of CMV enterocolitis was delayed, the patient was treated successfully by repeat colonoscopic decompression and antiviral therapy with intravenous ganciclovir.CONCLUSION This report cautions that CMV-induced colitis should be considered as a possible differential diagnosis in a patient with intractable symptoms of enterocolitis or megacolon of unknown cause, even when the patient is nonimmunocompromised. | Joon Hyun Cho Joon Hyuk Choi | 2020 | World Journal of Clinical Cases2020,8,3: | 3 |
| 19 | Comparison of a novel bedside portable endoscopy device with nasogastric aspiration for identifying upper gastrointestinal bleeding显示文摘AIM: To compare outcomes using the novel portable endoscopy with that of nasogastric(NG) aspiration in patients with gastrointestinal bleeding.METHODS: Patients who underwent NG aspiration for the evaluation of upper gastrointestinal(UGI) bleeding were eligible for the study. After NG aspiration, we performed the portable endoscopy to identify bleeding evidence in the UGI tract. Then, all patients underwent conventional esophagogastroduodenoscopy as the gold-standard test. The sensitivity, specificity, and accuracy of the portable endoscopy for confirming UGI bleeding were compared with those of NG aspiration.RESULTS: In total, 129 patients who had GI bleedingsigns or symptoms were included in the study(age 64.46 ± 13.79, 91 males). The UGI tract(esophagus, stomach, and duodenum) was the most common site of bleeding(81, 62.8%) and the cause of bleeding was not identified in 12 patients(9.3%). Specificity for identifying UGI bleeding was higher with the portable endoscopy than NG aspiration(85.4% vs 68.8%, P = 0.008) while accuracy was comparable. The accuracy of the portable endoscopy was significantly higher than that of NG in the subgroup analysis of patients with esophageal bleeding(88.2% vs 75%, P = 0.004). Food material could be detected more readily by the portable endoscopy than NG tube aspiration(20.9% vs 9.3%, P = 0.014). No serious adverse effect was observed during the portable endoscopy.CONCLUSION: The portable endoscopy was not superior to NG aspiration for confirming UGI bleeding site. | 20 | Increasing sulfur utilization in lithium-sulfur batteries by a Co-MOF-74@MWCNT interlayer显示文摘To improve lithium-sulfur battery performance,Co-MOF-74 has been applied for the first time as an interlayer with multiwalled carbon nanotubes(MWCNTs).Co-MOF-74@MWCNT was synthesized using a solvothermal method.The fabrication of Co-MOF-74@MWCNT was confirmed by scanning electron microscopy,X-ray diffraction,thermogravimetric analysis,and Brunauer-Emmett-Teller testing.The interlayer was fabricated using a filtration method.Assembled batteries were prepared using a CoMOF-74@MWCNT interlayer and an MWCNT interlayer and subsequently investigated via cyclic voltammetry tests.Co-MOF-74 promotes a redox reaction and shows a small peak at 1.85 V.A symmetric and full cell test revealed that the Co-MOF-74@MWCNT cell enables a faster redox reaction and higher capacity than that of the MWCNT cell.After 15 cycles,the Co-MOF-74@MWCNT cell achieved a value of 1112 mAh g^(-1),which is 26% greater than that of the MWCNT cell(880 mAh g^(-1)) at 0.2 C.Voltage profile testing showed that the reason for the higher capacity of the Co-MOF-74@MWCNT cell is that it promotes the conversion of Li_(2)S_(2) to Li_(2)S.Various electrochemical analyses confirmed that the Co-MOF-74@MWCNT interlayer acts not only as a physical and chemical barrier but also promotes the transformation of Li_(2)S_(2) to Li_(2)S. | SiHyeon Sung Byung Hyuk Kim SeungTaek Lee Sanghyeon Choi Woo Young Yoon | 2021 | Journal of Energy Chemistry2021,30,9: | 3 |
