Transcatheter embolization therapy in liver cancer:an update of clinical evidences显示文摘Transarterial chemoembolization(TACE) is a form of intra-arterial catheter-based chemotherapy that selectively delivers high doses of cytotoxic drug to the tumor bed combining with the effect of ischemic necrosis induced by arterial embolization.Chemoembolization and radioembolization are at the core of the treatment of liver hepatocellular carcinoma(HCC) patients who cannot receive potentially curative therapies such as transplantation, resection or percutaneous ablation.TACE for liver cancer has been proven to be useful in local tumor control,to prevent tumor progression, prolong patients' life and control patient symptoms.Recent evidence showed in patients with single-nodule HCC of 3 cm or smaller without vascular invasion,the5-year overall survival(OS) with TACE was similar to that with hepatic resection and radiofrequency ablation.Although being used for decades,Lipiodol~(Lipiodol~ Ultra Fluid~,Guerbet,France) remains important as a tumor-seeking and radio-opaque drug delivery vector in interventional oncology. There have been efforts to improve the delivery of chemotherapeutic agents to tumors.Drug-eluting bead(DEB) is a relatively novel drug delivery embolization system which allows for fixed dosing and the ability to release the anticancer agents in a sustained manner.Three DEBs are available,i.e.,Tandem~(CeloNova Biosciences Inc.,USA),DC-Beads~(BTG,UK) and HepaSphere~(BioSphere Medical,Inc.,USA).Transarterial radioembolization(TARE) technique has been developed, and proven to be efficient and safe in advanced liver cancers and those with vascular complications. Two types of radioembolization microspheres are available i.e.,SIR-Spheres~(Sirtex Medical Limited,Australia) and TheraSphere~(BTG,UK).This review describes the basic procedure of TACE,properties and efficacy of some chemoembolization systems and radioembolization agents which are commercially available and/or currendy under clinical evaluation. The key clinical trials of transcatheter arterial therapy for liver cancer are summarized. | Yi-Xiang J.Wang Thierry De Baere Jean-Marc Idee Sebastien Ballet | 2015 | Chinese Journal of Cancer Research2015,27,2: | 72
Comparison between endoscopic sclerotherapy and band ligation for hemostasis of acute variceal bleeding显示文摘AIM:To compare band ligation(BL) with endoscopic sclerotherapy(SCL) in patients admitted to an emergency unit for esophageal variceal rupture. METHODS:A prospective,randomized,single-center study without crossover was conducted. After endoscopic diagnosis of esophageal variceal rupture, patients were randomized into groups for SCL or BL treatment.Sclerotherapy was performed by ethanolamine oleate intravascular injection both above and below the rupture point, with a maximum volume of 20 mL.For BL patients, banding at the rupture point was attempted, followed by ligation of all variceal tissue of the distal esophagus.Primary outcomes for both groups were initial failure of bleeding control(5 d) ,early re-bleeding(5 d to 6 wk),and complications, including mortality. From May 2005 to May 2007,100 patients with variceal bleeding were enrolled in thestudy:50 SCL and 50 BL patients. No differences between groups were observed across gender, age,ChildPugh status, presence of shock at admission, mean hemoglobin levels, and variceal size. RESULTS:No differences were found between groups for bleeding control, early re-bleeding rates, complications,or mortality. After 6 wk,36(80%) SCL and 33(77 %) EBL patients were alive and free of bleeding.A statistically significant association between Child-Pugh status and mortality was found, with 16%mortality in Child A and B patients and 84%mortality in Child C patients(P<0.001) . CONCLUSION:Despite the limited number of patients included, our results suggest that SCL and BL are equally efficient for the control of acute variceal bleeding. | Gustavo Oliveira Luz Fauze Maluf-Filho Sérgio Eiji Matuguma Fábio Yuji Hondo Edson Ide Jeane Martins Melo Spencer Cheng Paulo Sakai | 2011 | World Journal of Gastrointestinal Endoscopy2011,3,5: | 20
Deep sedation during gastrointestinal endoscopy: Propofol-fentanyl and midazolam-fentanyl regimens显示文摘AIM: To compare deep sedation with propofol-fentanyl and midazolam-fentanyl regimens during upper gastrointestinal endoscopy. METHODS: After obtaining approval of the research ethics committee and informed consent, 200 patients were evaluated and referred for upper gastrointestinal endoscopy. Patients were randomized to receive propofol-fentanyl or midazolam-fentanyl (n = 100/group).We assessed the level of sedation using the observer's assessment of alertness/sedation (OAA/S) score and bispectral index (BIS). We evaluated patient and physician satisfaction, as well as the recovery time and complication rates. The statistical analysis was performed using SPSS statistical software and included the MannWhitney test, χ 2 test, measurement of analysis of variance, and the κ statistic. RESULTS: The times to induction of sedation, recovery, and discharge were shorter in the propofolfentanyl group than the midazolam-fentanyl group. According to the OAA/S score, deep sedation events occurred in 25% of the propofol-fentanyl group and 11% of the midazolam-fentanyl group (P = 0.014). Additionally, deep sedation events occurred in 19% of the propofol-fentanyl group and 7% of the midazolamfentanyl group according to the BIS scale (P = 0.039). There was good concordance between the OAA/S score and BIS for both groups (κ = 0.71 and κ = 0.63, respectively). Oxygen supplementation was required in 42% of the propofol-fentanyl group and 26% of the midazolam-fentanyl group (P = 0.025). The mean time to recovery was 28.82 and 44.13 min in the propofolfentanyl and midazolam-fentanyl groups, respectively (P < 0.001). There were no severe complications in either group. Although patients were equally satisfied with both drug combinations, physicians were more satisfied with the propofol-fentanyl combination. CONCLUSION: Deep sedation occurred with propofolfentanyl and midazolam-fentanyl, but was more frequent in the former. Recovery was faster in the propofol-fentanyl group. | Marcos Eduardo Lera dos Santos Fauze Maluf-Filho Dalton Marques Chaves Sergio Eiji Matuguma Edson Ide Gustavo de Oliveira Luz Thiago Ferreira de Souza Fernanda C Simoes Pessorrusso Eduardo Guimares Hourneaux de Moura Paulo Sakai | 2013 | World Journal of Gastroenterology2013,19,22: | 16
Narrow-band imaging without magnification for detecting early esophageal squamous cell carcinoma显示文摘AIM:To compare narrow-band imaging(NBI)without image magnification, and chromoendoscopy with Lugol's solution for detecting high-grade dysplasia and intramu-cosal esophageal squamous cell carcinoma(SCC)in patients with head and neck cancer.METHODS:This was a prospective observational study of 129 patients with primary head and neck tumors consecutively referred to the Gastrointestinal Endoscopy Unit of Hospital das Clínicas,S?o Paulo University Medical School,Brazil, between August 2006 and Feb-ruary 2007.Conventional examinations with NBI and Lugol chromoendoscopy were consecutively performed, and the discovered lesions were mapped, recorded and sent for biopsy. The results of the three methods were compared regarding sensitivity, specificity, accuracy, positive predictive value, negative predictive value, positive likelihood value and negative likelihood value.RESULTS:Of the 129 patients,nine(7%)were diagnosed with SCC,5 of which were in situ and 4 which were intramucosal. All carcinomas were detected through NBI and Lugol chromoendoscopy.Only 4 lesions were diagnosed through conventional examination, all of which were larger than 10 mm.CONCLUSION:NBI technology with optical filters has high sensitivity and high negative predictive value for detecting superficial esophageal SCC,and produces results comparable to those obtained with 2.5%Lugol chromoendoscopy. | Edson Ide Fauze Maluf-Filho Dalton Marques Chaves Sergio Eiji Matuguma Paulo Sakai | 2011 | World Journal of Gastroenterology2011,17,39: | 15
Cell transplantation for the treatment of spinal cord injury–bone marrow stromal cells and choroid plexus epithelial cells显示文摘Transplantation of bone marrow stromal cells(BMSCs) enhanced the outgrowth of regenerating axons and promoted locomotor improvements of rats with spinal cord injury(SCI).BMSCs did not survive long-term,disappearing from the spinal cord within 2–3 weeks after transplantation.Astrocyte-devoid areas,in which no astrocytes or oligodendrocytes were found, formed at the epicenter of the lesion. It was remarkable that numerous regenerating axons extended through such astrocyte-devoid areas.Regenerating axons were associated with Schwann cells embedded in extracellular matrices.Transplantation of choroid plexus epithelial cells(CPECs) also enhanced axonal regeneration and locomotor improvements in rats with SCI.Although CPECs disappeared from the spinal cord shortly after transplantation,an extensive outgrowth of regenerating axons occurred through astrocyte-devoid areas,as in the case of BMSC transplantation. These findings suggest that BMSCs and CPECs secret neurotrophic factors that promote tissue repair of the spinal cord, including axonal regeneration and reduced cavity formation. This means that transplantation of BMSCs and CPECs promotes 'intrinsic' ability of the spinal cord to regenerate. The treatment to stimulate the intrinsic regeneration ability of the spinal cord is the safest method of clinical application for SCI.It should be emphasized that the generally anticipated long-term survival, proliferation and differentiation of transplanted cells are not necessarily desirable from the clinical point of view of safety. | Chizuka Ide Norihiko Nakano Kenji Kanekiyo | 2016 | Neural Regeneration Research2016,11,9: | 8

Carcinosarcoma of the liver with mesenchymal differentiation显示文摘We report an extremely rare case where a mesenchymal differentiation, especially embryonal sarcoma, was demonstrated in cholangiocarcinoma. At autopsy, a yellowish-white tumor (15 cm x 12 cm) was found in the right hepatic lobe, and there were several daughter nodules in both hepatic lobes. Histologically, most of the main tumor and all of the daughter nodules examined showed sarcomatous changes (spindle cells, pleomorphic cells and hyalization). Histologic examination of a part of the main tumor disclosed a focus of adenocarcinoma within the tumor. The frequent transitions between the adenocarcinomatous areas and the sarcomatous areas suggested that sarcomatous transformation occurred in the cholangiocarcinoma and then spread rapidly. Immunohistochemically, the adenocarcinomatous elements were positive for cytokeratin, carcinoembryonic antigen (CEA) and epithelial membrane antigen, and negative in the sarcomatous cells. Vimentin was positive only in the sarcomatous elements. The findings of the present case support the view that carcinosarcomas represent carcinomas that develop sarcomatous elements via metaplasia of the epithelial element. | Shinichi Sumiyoshi Masataka Kikuyama Yuji Matsubayashi Fujito Kageyama Yoshihiro Ide Yoshimasa Kobayashi Hirotoshi Nakamura | 2007 | World Journal of Gastroenterology2007,13,5: | 6
Enzyme-digested Colla Corii Asini(E’jiao) prevents hydrogen peroxide-induced cell death and accelerates amyloid beta clearance in neuronal-like PC12 cells显示文摘As an aging-associated degenerative disease,Alzheimer’s disease is characterized by the deposition of amyloid beta(Aβ),oxidative stress, inflammation, dysfunction and loss of cholinergic neurons.Colla Corii Asini(CCA)is a traditional Chinese medicine which has been used for feebleness-related diseases and anti-aging.CCA might delay aging-induced degenerative changes in neurons. In the present study,we evaluated antioxidant activity, cytoprotective effects, and Aβremovability of enzyme-digested Colla Corii Asini(CCAD).Oxygen radical absorbance capacity(ORAC)activity assay showed that,as compared to gelatins from the skin of porcine, bovine and cold water fish,CCA exhibited the highest ORAC activity. The ORAC activity of CCA and CCAD was increased gradually by the length of time in storage.Ultrastructure analysis by scanning electron microscopy showed that among CCA manufactured in 2008,2013,2017 and gelatin from cold water fish skin,CCA manufactured in 2008 presented the smoothest surface structure. We further tested the protective effects of CCAD(manufactured in 2008)and enzyme-digested gelatin from cold water fish skin(FGD)on hydrogen peroxide(H2O2)-induced cell death in nerve growth factor-differentiated neuronal-like PC12 cells.Presto blue assay showed that both FGD and CCAD at 0.5 mg/m L increased cell viability in H2O2-treated neuronal-like PC12 cells. The protection of CCAD was significantly superior to that of FGD.Acetylcholinesterase(Ach E)assay showed that both FGD and CCAD inhibited Ach E activity in nerve growth factor-differentiated neuronal-like PC12 cells to 89.1%and 74.5%of that in non-treated cells, respectively. The data suggest that CCAD might be able to increase the neurotransmitter acetylcholine.Although CCAD inhibited Ach E activity in neuronal-like PC12 cells,CCAD prevented H2O2-induced abnormal deterioration of Ach E.ELISA and neprilysin activity assay results indicated that CCAD reduced amyloid beta accumulation and increased neprilysin activity in Aβ1–42-treated neuronal-like PC12 cells, suggesting that CCAD can enhance Aβclearance. Our results suggest that CCA might be useful for preventing and treating Alzheimer’s disease. | Li Xiao Feng Liao Ryoji Ide Tetsuro Horie Yumei Fan Chikako Saiki Nobuhiko Miwa | 2020 | Neural Regeneration Research2020,15,12: | 6

Points regarding cell transplantation for the treatment of spinal cord injury显示文摘Transplantation of somatic cells, including bone marrow stromal cells(BMSCs),bone marrow mononuclear cells(BMNCs),and choroid plexus epithelial cells(CPECs),enhances the outgrowth of regenerating axons and promotes locomotor improvements. They are not integrated into the host spinal cord, but disappear within 2-3 weeks after transplantation.Regenerating axons extend at the spinal cord lesion through the astrocyte-devoid area that is filled with connective tissue matrices.Regenerating axons have characteristics of peripheral nerves:they are associated with Schwann cells, and embedded in connective tissue matrices. It has been suggested that neurotrophic factors secreted from BMSCs and CPECs promote 'intrinsic' ability of the spinal cord to regenerate.Transplanted Schwann cells survive long-term,and are integrated into the host spinal cord, serving as an effective scaffold for the outgrowth of regenerating axons in the spinal cord. The disadvantage that axons are blocked to extend through the glial scar at the border of the lesion is overcome.Schwann cells have been approved for clinical applications.Neural stem/progenitor cells(NSPCs) survive long-term,proliferate,and differentiate into glial cells and/or neurons after transplantation. No method is available at present to manipulate and control the behaviors of NPSCs to allow them to appropriately integrate into the host spinal cord.NPSP transplantation is not necessarily effective for locomotor improvement. | Chizuka Ide Kenji Kanekiyo | 2016 | Neural Regeneration Research2016,11,7: | 5

Biological, clinical and epidemiological features of COVID-19, SARS and MERS and AutoDock simulation of ACE2显示文摘Background:The outbreak of coronavirus disease 2019(COVID-19)has caused a public catastrophe and global concern. The main symptoms of COVID-19 are fever, cough, myalgia, fatigue and lower respiratory tract infection signs.Almost all populations are susceptible to the virus, and thebasic reproduction number(R0)is 2.8-3.9.The fight against COVID-19 should have two aspects:one is the treatment of infected patients, and the other is the mobilization of the society to avoid the spread of the virus. The treatment of patients includes supportive treatment, antiviral treatment, and oxygen therapy. For patients with severe acute respiratory distress syndrome(ARDS),extracorporeal membrane oxygenation(ECMO)and circulatory support are recommended.Plasma therapy and traditional Chinese medicine have also achieved good outcomes. This review is intended to summarize the research on this new coronavirus,to analyze the similarities and differences between COVID-19 and previous outbreaks of severe acute respiratory syndrome(SARS)and Middle East respiratory syndrome(MERS)and to provide guidance regarding new methods of prevention, diagnosis and clinical treatment based on autodock simulations. Methods:This review compares the multifaceted characteristics of the three coronaviruses including COVID-19,SARS and MERS.Our researchers take the COVID-19,SARS,and MERS as key words and search literatures in the Pubmed database. We compare them horizontally and vertically which respectively means concluding the individual characteristics of each coronavirus and comparing the similarities and differences between the three coronaviruses. Results:We searched for studies on each outbreak and their solutions and found that the main biological differences among SARS-CoV-2,SARS-CoV and MERS-CoV are in ORF1a and the sequence of gene spike coding protein-S.We also found that the types and severity of clinical symptoms vary, which means that the diagnosis and nursing measures also require differentiation. In addition to the common route of transmission including airborne transmission, these three viruses have their own unique routes of transmission such as fecal-oral route of transmission COVID-19.Conclusions:In evolutionary history, these three coronaviruses have some similar biological features as well as some different mutational characteristics.Their receptors and routes of transmission are not all the same, which makes them different in clinical features and treatments. We discovered through the autodock simulations that Met124 plays a key role in the efficiency of drugs targeting ACE2,such as remdesivir, chloroquine, ciclesonide and niclosamide, and may be a potential target in COVID-19. | Xue-Yan Zhang Hao-Jie Huang Dong-Lin Zhuang Moussa Ide Nasser Ming-Hua Yang Ping Zhu Ming-Yi Zhao | 2020 | Infectious Diseases of Poverty2020,9,4: | 4

Stem cell therapy for central nerve system injuries: glial cells hold the key显示文摘Mammalian adult central nerve system(CNS) injuries are devastating because of the intrinsic difficulties for effective neuronal regeneration. The greatest problem to be overcome for CNS recovery is the poor regeneration of neurons and myelin-forming cells, oligodendrocytes. Endogenous neural progenitors and transplanted exogenous neuronal stem cells can be the source for neuronal regeneration. However, because of the harsh local microenvironment, they usually have very low efficacy for functional neural regeneration which cannot compensate for the loss of neurons and oligodendrocytes. Glial cells(including astrocytes, microglia, oligodendrocytes and NG2 glia) are the majority of cells in CNS that provide support and protection for neurons. Inside the local microenvironment, glial cells largely influence local and transplanted neural stem cells survival and fates. This review critically analyzes current finding of the roles of glial cells in CNS regeneration, and highlights strategies for regulating glial cells' behavior to create a permissive microenvironment for neuronal stem cells. | Li Xiao Chikako Saiki Ryoji Ide | 2014 | Neural Regeneration Research2014,9,13: | 3

Occult hepatitis B virus infection and surgical outcomes in non-B, non-C patients with curative resection for hepatocellular carcinoma显示文摘AIM To investigate the prevalence, clinicopathological characteristics and surgical outcomes of occult hepatitis B virus(HBV) infection(OBI) in patients with non-B, non-C(NBNC) hepatocellular carcinoma(HCC).METHODS This study retrospectively examined the cases of 78 NBNC patients with curative resection for HCC for whom DNA could be extracted from formalin-fixed paraffin-embedded tissue. OBI was determined by the HBV-DNA amplification of at least two different sets of primers by TaqM an realtime polymerase chain reaction. Possibly carcinogenetic factors such as alcohol abuse, diabetes mellitus, obesity and non-alcoholic steatohepatitis(NASH) were examined. Surgical outcomes were evaluated according to diseasefree survival(DFS), overall survival(OS) and diseasespecific survival(DSS).RESULTS OBI was found in 27/78 patients(34.6%) with NBNC HCC. The OBI patients were significantly younger than the non-OBI cases at the time of surgery(average age 63.0 vs 68.1, P = 0.0334) and the OBI cases overlapped with other etiologies significantly more frequently compared to the non-OBI cases(P = 0.0057). OBI had no impact on the DFS, OS or DSS. Only tumorrelated factors affected these surgical outcomes.CONCLUSION Our findings indicate that OBI had no impact on surgical outcomes. The surgical outcomes of NBNC HCC depend on early tumor detection; this reconfirms the importance of a periodic medical examination for individuals who have NBNC HCC risk factors. | Hiroki Koga Keita Kai Shinichi Aishima Atsushi Kawaguchi Koutaro Yamaji Takao Ide Junji Ueda Hirokazu Noshiro | 2017 | World Journal of Hepatology2017,9,35: | 2
Primary biliary cholangitis metachronously complicated with combined hepatocellular carcinoma-cholangiocellular carcinoma and hepatocellular carcinoma显示文摘Primary biliary cholangitis(PBC) is a progressive cholestatic liver disease characterized by the presence of highly specific antimitochondrial antibodies, portal inflammation and lymphocyte-dominated destruction of the intrahepatic bile ducts, which leads to cirrhosis. While its pathogenesis remains unclear, PBC that shows histological progression to fibrosis carries a high risk of carcinogenesis; the same is true of viral liver diseases. In patients with PBC, the development of hepatocellular carcinoma(HCC) is rare; the development of combined hepatocellular carcinoma and cholangiocellular carcinoma(c HCC-CCC) is extraordinary. Herein, we report a rare case of PBC metachronously complicated by c HCC-CCC and HCC, which, to the best of our knowledge, has never been reported. We present a case report of a 74-year-old Japanese woman who was diagnosed as PBC in her 40's by using blood tests and was admitted to our department for further management of an asymptomatic liver mass. She had a tumor of 15 mm in size in segment 8 of the liver and underwent a partial resection of the liver. Subsequent pathological findings resulted in the diagnosis of c HCC-CCC, arising from stage 3 PBC. One year after the initial hepatectomy, a second tumor of 10 mm in diameter was found in segment 5 of the liver; a partial resection of the liver was performed. Subsequent pathological findings led to HCC diagnosis. The component of HCC in the initial tumor displayed a trabecular growth pattern while the second HCC showed a pseudoglandular growth pattern, suggesting that metachronous tumors that arise from PBC are multicentric. | Ryuta Ide Akihiko Oshita Takashi Nishisaka Hideki Nakahara Shiomi Aimitsu Toshiyuki Itamoto | 2017 | World Journal of Hepatology2017,9,36: | 2
