| 2 | Adjuvant transcatheter arterial chemoembolization after curative resection for hepatocellular carcinoma patients with solitary tumor and microvascular invasion: a randomized clinical trial of efficacy and safety显示文摘Background:The optimal strategy for adjuvant therapy after curative resection for hepatocellular carcinoma(HCC)patients with solitary tumor and microvascular invasion(MVI)is controversial.This trial evaluated the efficacy and safety of adjuvant transcatheter arterial chemoembolization(TACE)after hepatectomy versus hepatectomy alone in HCC patients with a solitary tumor≥5 cm and MVI.Methods:In this randomized,open-labeled,phase III trial,HCC patients with a solitary tumor≥5 cm and MVI were randomly assigned(1:1)to receive either 1-2 cycles of adjuvant TACE after hepatectomy(Hepatectomy-TACE)or hepatectomy alone(Hepatectomy Alone).The primary endpoint was disease-free survival(DFS);the secondary end-points included overall survival(OS)and adverse events.Results:Between June 1,2009,and December 31,2012,250 patients were enrolled and randomly assigned to the Hepatectomy-TACE group(n=125)or the Hepatectomy Alone group(n=125).Clinicopathological characteristics were balanced between the two groups.The median follow-up time from randomization was 37.5 months[interquartile range 18.3-48.2 months].The median DFS was significantly longer in the Hepatectomy-TACE group than in the Hepatectomy Alone group[17.45 months(95%confidence interval[CI]11.99-29.14)vs.9.27 months(95%CI 6.05-13.70),hazard ratio[HR]=0.70(95%CI 0.52-0.95),P=0.020],respectively.The median OS was also significantly longer in the Hepatectomy-TACE group than in the Hepatectomy Alone group[44.29 months(95%CI 25.99-62.58)vs.22.37 months(95%CI 10.84-33.91),HR=0.68(95%CI 0.48-0.97),P=0.029].Treatment-related adverse events were more frequently observed in the Hepatectomy-TACE group,although these were generally mild and manageable.The most common grade 3 or 4 adverse events in both groups were neutropenia and liver dysfunction.Conclusion:Hepatectomy followed by adjuvant TACE is an appropriate option after radical resection in HCC patients with solitary tumor≥5 cm and MVI,with acceptable toxicity. | Wei Wei Pei-En Jian Shao-Hua Li Zhi-Xing Guo Yong-Fa Zhang Yi-Hong Ling Xiao-Jun Lin Li Xu Ming Shi Lie Zheng Min-Shan Chen Rong-Ping Guo | 2018 | Cancer Communications2018,38,1: | 62 |

| 7 | Changes of gut bacteria and immune parameters in liver transplant recipients显示文摘BACKGROUND: Liver transplantation is one of the most effective therapeutic options for patients with end-stage liver diseases, and gut microbiota is actively involved in potential infections in pretransplant and posttransplant patients. However, the diversity of gut microbiota and its relationship with the immune parameter of liver transplantation recipients are not well understood. METHODS: We collected fresh feces and blood samples from 190 participants in China from November 2004 to May 2008, including 28 healthy volunteers, 51 cirrhotic patients and 111 liver-transplanted patients. Six interesting gut bacteria, plasma endotoxin, serum cytokines (i.e., tumor necrosis factor alpha and interleukin-6) and fecal secretory IgA (SIgA) were investigated by real-time quantitative PCR, chromogenic limulus amoebocyte assay, sandwich-type enzyme-linked immunosorbent assay and radioimmunoassay, respectively. RESULTS: All Eubacteria, Bifidobacterium spp., Faecalibacterium prausnitzii and Lactobacillus spp. were significantly lower in the liver transplantation recipients while Enterobacteriaceae and Enterococcus spp. were significantly higher (P<0.05). Except for Enterococcus spp., other bacteria showed a tendency to restore to normal level along with the time after liver transplantation. Plasma endotoxin, interleukin-6 and fecal SIgA in cirrhotic patients increased significantly, but not in liver transplantation recipients. Plasma endotoxin and interleukin-6 were negatively correlated with all Eubacteria and the Bacteroides-Prevotella group, while tumor necrosis factor alpha was not significantly correlated with these six gut bacteria in cirrhotic patients.CONCLUSIONS: Our study demonstrates that abundant gut bacteria were altered significantly in both cirrhotic and liver transplantation patients, while plasma endotoxin and interleukin-6 increased remarkably in cirrhotic patients, showing significant correlations with gut microbiota. | 9 | Effects of Long-Term Winter Planted Green Manure on Physical Properties of Reddish Paddy Soil Under a Double-Rice Cropping System显示文摘Soil physical properties are important indicators of the potential for agricultural production.Our objective was to evaluate the effects of long-term inputs of green manures on physical properties of a reddish paddy soil(Fe-Typic Hapli-Stagnic Anthrosols) under a double cropping system.The common cropping pattern before the study was early-late rice-fallow(winter).The field treatments included rice-rice-fallow(R-R-WF),rice-rice-rape(R-R-RP),rice-rice-Chinese milk vetch(RR-MV),and rice-rice-ryegrass(R-R-RG).The rape,Chinese milk vetch and ryegrass were all incorporated as green manures 15 d before early rice transplanting during the following year.The soil bulk density in all green manure treatments was significantly reduced compared with the winter fallow treatment.Soil porosity with green manure applications was significantly higher than that under the winter fallow.The green manure treatments had higher 0.25-5 mm water stable aggregates and aggregates stabilities in the plow layer(0-15 cm depth) compared with the fallow treatment.The mean weight diameter(MWD) and normalized mean weight diameter(NMWD) of aggregates in the green manure treatment were larger than that with the winter fallow.Soil given green manure retained both a higher water holding capacity in the plow layer soil,and a larger volume of moisture at all matric potentials(-10,-33 and-100 kPa).We conclude that the management of double-rice fields in southern central China should be encouraged to use green manures along with chemical fertilizers to increase SOC content,improve soil physical properties and soil fertility. | YANG Zeng-ping XU Ming-gang ZHENG Sheng-xian NIE Jun GAO Ju-sheng LIAO Yu-lin XIE Jian | 2012 | Journal of Integrative Agriculture2012,11,4: | 40 |
| 10 | Application of serum protein fingerprinting coupled with artificial neural network model in diagnosis of hepatocellular carcinoma显示文摘Background Hepatocellular carcinoma tends to present at a late clinical stage with poor prognosis. Therefore, it is urgent to explore and develop a simple, rapid diagnostic method, which has high sensitivity and specificity for hepatocellular carcinoma at an early stage. In this study, the serum proteins in patients with hepatocellular carcinoma or liver cirrhosis and in normal controls were analysed. Surface enhanced laser desorption/ionization time-of-flight mass (SELDI-TOF-MS) spectrometry was used to fingerprint serum protein using the protein chip technique and explore the value of the fingerprint, coupled with artificial neural network, to diagnose hepatocellular carcinoma.Methods Of the 106 serum samples obtained, 52 were from patients with hepatocellular carcinoma, 22 from patients with liver cirrhosis and 32 from healthy volunteers. The samples were randomly assigned into a training group ( n =70, 35 patients with hepatocellular carcinoma, 14 with liver cirrhosis, and 21 normal controls) and a testing group ( n =36, 17 patients with hepatocellular carcinoma, 8 with liver cirrhosis, and 11 normal controls). An artificial neural network was trained on data from 70 individuals in the training group to develop an artificial neural network diagnostic model and this model was tested. The 36 sera in the testing group were analysed with blind prediction by using the same flowchart and procedure of data collection. The 36 serum protein spectra were clustered with the preset clustering method and the same mass/charge ( M/Z ) peak values as those in the training group. Matrix transfer was performed after data were output. Then the data were input into the previously built artificial neural network model to get the prediction value. The M/Z peaks of the samples with more than 2000 M/Z were normalized with biomarker wizard of ProteinChip Software version 3.1 for noise filtering. The first threshold for noise filtering was set at 5, and the second was set at 2. The 10% was the minimum threshold for clustering. The statistical analysis of the data of serum protein mass spectrum was performed in the groups (normal vs. hepatocellular carcinoma, and liver cirrhosis vs. hepatocellular carcinoma) with the t test. Results Comparison between the groups of hepatocellular carcinoma and normal control: The mass spectra from 56 samples (hepatocellular carcinoma and normal controls) in the training group were analysed and 241 peaks were obtained. In addition, 21 peaks from them were used for comparison between the groups of hepatocellular carcinoma and normal controls ( P <0.01). Only 2 peaks at 3015 M/Z and 5900 M/Z were selected with significant difference ( P <10 -9). A model was developed based on these two proteins with different M/Z . It was confirmed that this artificial neural network model can be used for comparison between the groups of hepatocellular carcinoma and normal controls. The sensitivity was 100% (17/17), and the specificity was 100% (11/11). Comparison between the groups of hepatocellular carcinoma and liver cirrhosis: The mass spectra from 49 samples in the training group (including patients with hepatocellular carcinoma and liver cirrhosis) were analysed and 208 peaks were obtained. In addition, 21 peaks from them were used for comparison between the groups of hepatocellular carcinoma and liver cirrhosis ( P <0.01). Only 2 peaks at 7759 M/Z , 13134 M/Z were selected with significant difference ( P <10 -9). A model was developed based on these two proteins with dfferent M/Z . It was confirmed that this artificial neural network model can be used for comparison between the groups of hepatocellular carcinoma and liver cirrhosis. | 11 | Effects of Long-Term Winter Planted Green Manure on Distribution and Storage of Organic Carbon and Nitrogen in Water-Stable Aggregates of Reddish Paddy Soil Under a Double-Rice Cropping System显示文摘In agricultural systems, maintenance of soil organic matter has long been recognized as a strategy to reduce soil degradation. Manure amendments and green manures are management practices that can increase some nutrient contents and improve soil aggregation. We investigated the effects of 28 yr of winter planted green manure on soil aggregate-size distribution and aggregateassociated carbon(C) and nitrogen(N). The study was a randomized completed block design with three replicates. The treatments included rice-rice-fallow, rice-rice-rape, rice-rice-Chinese milk vetch and rice-rice-ryegrass. The experiment was established in 1982 on a silty light clayey paddy soil derived from Quaternary red clay(classified as Fe-Accumuli-Stagnic Anthrosols) with continuous early and late rice. In 2009, soil samples were collected(0-15 cm depth) from the field treatment plots and separated into water-stable aggregates of different sizes(i.e., >5, 2-5, 1-2, 0.5-1, 0.25-0.5 and <0.25 mm) by wet sieving. The long-term winter planted green manure significantly increased total C and N, and the formation of the 2-5-mm water-stable aggregate fraction. Compared with rice-rice-rape, rice-rice-Chinese milk vetch and rice-rice-ryegrass, the rice-rice-fallow significantly reduced 2-5-mm water-stable aggregates, with a significant redistribution of aggregates into micro-aggregates. Long-term winter planted green manure obviously improved C/N ratio and macro-aggregate-associated C and N. | 13 | Sorafenib in treatment of patients with advanced hepatocellular carcinoma:a systematic review显示文摘BACKGROUND: Sorafenib has become the standard first-line treatment for patients with advanced hepatocellular carcinoma (HCC). This study aimed to assess the efficacy and safety of sorafenib in advanced HCC patients and explore its true value for specific subgroups. DATA SOURCES: A computer-based systematic search from January 2005 to June 2011 with 'sorafenib' and 'advanced hepatocellular carcinoma' as search terms was performed for possible clinical trials. Hazard ratios (HR) and their 95% confidence intervals (CI) for overall survival (OS) and time to progression (TTP), rates of partial response (PR), rates of toxicity effects, and details of subgroup analysis were extracted. Meta-analyses were done using the software Review Manager (version 5.0). RESULTS: Six trials with 1164 patients were included. Based on three randomized controlled trials, the pooled HR (sorafenib/ placebo) was 0.66 for OS (95% CI: 0.56-0.78; P<0.00001) and 0.57 for TTP (95% CI: 0.47-0.68; P<0.00001). The pooled odds ratio (OR) for PR was 2.96 (95% CI: 0.96-9.15; P=0.06). For three single-arm trials, the pooled HR was 0.69 for OS (95% CI: 0.56-0.84; P=0.0002) and 0.64 for TTP (95% CI: 0.52-0.78; P<0.00001). The pooled OR for PR in three single-arm trials was 3.56 (95% CI: 1.22-10.39; P=0.02). Subgroup analysis indicated that sorafenib was less effective in patients with extrahepatic spread (with: P=0.13 vs without: P<0.0001), with normal alpha-fetoprotein level (AFP) (P=0.15 vs elevated: P=0.0006), and with elevated level of serum bilirubin (P=0.06 vs normal: P=0.0009). Sorafenib-based therapy significantly increased the risk of grade 3/4 hand-foot skin reaction, diarrhea, fatigue, and rash/desquamation.CONCLUSIONS: Sorafenib-based therapy benefits advanced HCC patients. | 14 | Long-Term Effect of Fertilizer and Rice Straw on Mineral Composition and Potassium Adsorption in a Reddish Paddy Soil显示文摘Increasing K+ adsorption can be an effective alternative in building an available K pool in soils to optimize crop recovery and minimize losses into the environment. We hypothesized that long-term fertilization might change K+ adsorption because of changes in the chemical and mineralogical properties of a rice (Oryza sativa L.). The aims of this study were (i) to determine clay minerals in paddy soil clay size fractions using X-ray diffraction methods and a numerical diagram-decomposition method; (ii) to measure K+ adsorption isotherms before and after H 2 O 2 oxidation of organic matter, and (iii) to investigate whether K+ adsorption is correlated with changes in soil chemical and mineral properties. The 30-yr long-term fertilization treatments caused little change in soil organic C (SOC) but a large variation in soil mineral composition. The whole-clay fraction (<5 μm) corresponded more to the fertilization treatment than the fine-clay fraction (<1 μm) in terms of percentage of illite peak area. The total percentage of vermiculite-chlorite peak area was significantly negatively correlated with the total percentage of illite peak area in the <5 μm soil particles (R=-0.946, P<0.0006). Different fertilization treatments gave significantly different results in K+ adsorption. The SOC oxidation test showed positive effects of SOC on K+ adsorption at lower K+ concentration ( 120 mg L-1) and negative effects at higher K+ concentration (240 mg L-1). The K+ adsorption by soil clay minerals after SOC oxidization accounted for 60-158% of that by unoxidized soils, suggesting a more important role of soil minerals than SOC on K+ adsorption. The K+ adsorption potential was significantly correlated to the amount of poorly crystallized illite present (R=0.879, P=0.012). Rhei Radix et Rhizoma (rhubarb roots and rhizomes) is one of the most popular Chinese materia medica and has been widely used for strong laxative function. About 200 compounds with six different types of skeletons (anthraquinone, anthrone, stilbene, flavonoids, acylglucoside, and pyrone) have so far been isolated from eighteen species of the genus Rheum L. These constituents showed extensive pharmacological activities including cathartic, diuretic, anticancer, hepatoprotective, anti-inflammatory, and analgesic effects, as well as toxicological effects. Chemical fingerprint, LC-MS, and other analytical techniques have been used for the quality control of rhubarb. This comprehensive review summarizes the researches into the isolation, pharmacological activities, and phytochemical analysis reported since investigations began in the late 1940s. | 20 | A national report from China Liver Transplant Registry: steroid avoidance after liver transplantation for hepatocellular carcinoma显示文摘Objective:We aimed to evaluate the efficacy and safety of steroid-free immunosuppression after liver transplantation(LT)for hepatocellular carcinoma(HCC).Methods:We retrospectively analyzed HCC recipients without steroids after LT(SF group,n=368)based on the China Liver Transplant Registry(CLTR)database.These recipients were matched 1:2 with patients using steroids(S group,n=736)for the same period after LT for HCC,according to propensity scores.Results:Multivariate analysis indicates that recipients with younger age[odds ratio(OR),1.053;P=0.011],preoperative hepatitis B virus(HBV)DNA≥1,000 copies/mL(OR,2.597;P=0.004)and beyond Milan criteria(OR,4.255;P<0.001)were identified as the risk factors associated with tumor recurrence in steroid avoidance recipients after LT.The patients fulfilling the Milan criteria in the SF group presented higher overall and tumor-free survival rates than those in the S group(P<0.05).Multivariate analysis revealed that recipient beyond Milan criteria was an independent prognostic factor for overall survival(OR,1.690;P<0.001)and tumor-free survival(OR,2.066;P<0.001).The incidences of new-onset diabetes mellitus(21.20% vs.33.29%,P<0.001),new-onset hypertension(10.05%vs.18.61%,P<0.001)and hyperlipidemia(4.08% vs.7.20%,P=0.042)were significantly lower in the SF group.Conclusions:Steroid-free immunosuppression could be safe and feasible for HBV-related HCC patients in LT.Age,HBV DNA level and Milan criteria maybe risk factors associated with tumor recurrence in steroid avoidance recipients.Recipient beyond Milan criteria was an independent prognostic factor and recipient fulfilling Milan criteria can benefit the most from steroid-free immunosuppression. | Qiang Wei Feng Gao Runzhou Zhuang Qi Ling Qinghong Ke Jian Wu Tian Shen Mangli Zhang Min Zhang Xiao Xu Shusen Zheng | 2017 | Chinese Journal of Cancer Research2017,29,5: | 25 |
