13Experience of living donor liver transplantation for hepatocellular carcinoma in the University of Hong Kong Hospital显示文摘Aim:To describe the current practise of living donor liver transplantation(LDLT)for hepatocellular carcinoma(HCC),including the patient selection criteria,surgical techniques,management of small-for-size syndrome,postoperative complications,and the results of our units,in the Liver Transplant Centre of Queen Mary Hospital,Hong Kong,one of the high-volume centres for LDLT in Asia.Methods:Our centre practises careful selection for HCC patients using the University of California,San Francisco(UCSF)criteria,supplemented by alpha-fetoprotein level and the model for end-stage liver disease score.Slight flexibility is offered to enthusiastic donors and recipients in LDLT while balancing the risks and benefits.We pioneered in using the extended right lobe graft and the novel hepatic venoplasty technique,which lessen the risk of hyperperfusion and small-for-size syndrome with improved overall recipient survival.Data were collected prospectively and presented as the mean values and ranges,or the number of patients in proportion of total patient population.Results:Of our patients,74.9% met the UCSF criteria,and 64.5% met the Milan criteria.A 5-year overall and disease-free survival rate of 78.9% and 76.3% were achieved.Conclusion:LDLT is an ideal treatment for HCC in Hong Kong with regard to the critical organ shortage and high demand for transplantation.The current surgical techniques and post-transplant surveillance contribute to the positive outcome.Janice Hoi Man Mok Ka Wing Ma Kenneth Siu Ho Chok 2022Hepatoma Research2022,8,1:0
14Long-Term and Immediate Impacts of Health Emergency and Disaster Risk Management(Health-EDRM) Education Interventions in a Rural Chinese Earthquake-Prone Transitional Village显示文摘Ma'an Qiao Village, a Dai and Yi ethnic minority-based community in Sichuan Province, China sustained complete infrastructure devastation during the 2008 Panzhihua earthquake. Health emergency and disaster risk management(Health-EDRM) education intervention programs were implemented in 2010 and 2011. This serial cross-sectional survey study aimed to examine the immediate and long-term impacts of the Health-EDRM interventions in this remote rural community. The findings demonstrate knowledge improvement in areas of water and sanitation, food and nutrition, and disaster preparedness immediately after the Health-EDRM education interventions. Temporal stability of knowledge retention was observed in household hygiene and waste management and smoking beliefs in 2018, 7 years after the interventions.Other important findings include knowledge uptake pattern differences of oral rehydration solution(ORS) between earthquake-prone and flood-prone communities. Usage of Internet and mobile technology for accessing disaster-related information was found to be independent of gender and income. Overall, this study demonstrated the knowledge improvement through Health-EDRM education interventions in a remote rural community. Promoting behavioral changes through interventions to raise awareness has the potential to reduce health risks in transitional post-disaster settings. 15Impact of climate change on dengue fever epidemics in South and Southeast Asian settings:A modelling study显示文摘The potential for dengue fever epidemic due to climate change remains uncertain in tropical areas.This study aims to assess the impact of climate change on dengue fever transmission in four South and Southeast Asian settings.We collected weekly data of dengue fever incidence,daily mean temperature and rainfall from 2012 to 2020 in Singapore,Colombo,Selangor,and Chiang Mai.Projections for temperature and rainfall were drawn for three Shared Socioeconomic Pathways(SSP126,SSP245,and SSP585)scenarios.Using a disease transmission model,we projected the dengue fever epidemics until 2090s and determined the changes in annual peak incidence,peak time,epidemic size,and outbreak duration.A total of 684,639 dengue fever cases were reported in the four locations between 2012 and 2020.The projected change in dengue fever transmission would be most significant under the SSP585 scenario.In comparison to the 2030s,the peak incidence would rise by 1.29 times in Singapore,2.25 times in Colombo,1.36 times in Selangor,and>10 times in Chiang Mai in the 2090s under SSP585.Additionally,the peak time was projected to be earlier in Singapore,Colombo,and Selangor,but be later in Chiang Mai under the SSP585 scenario.Even in a milder emission scenario of SSP126,the epidemic size was projected to increase by 5.94%,10.81%,12.95%,and 69.60%from the 2030se2090s in Singapore,Colombo,Selangor,and Chiang Mai,respectively.The outbreak durations in the four settings were projected to be prolonged over this century under SSP126 and SSP245,while a slight decrease is expected in 2090s under SSP585.The results indicate that climate change is expected to increase the risk of dengue fever transmission in tropical areas of South and Southeast Asia.Limiting greenhouse gas emissions could be crucial in reducing the transmission of dengue fever in the future.Yawen Wang Shi Zhao Yuchen Wei Kehang Li Xiaoting Jiang Conglu Li Chao Ren Shi Yin Janice Ho Jinjun Ran Lefei Han Benny Chung-ying Zee Ka Chun Chong 2023Infectious Disease Modelling2023,8,3:0
