| 1 | Outcome of transarterial chemoembolization-based multi-modal treatment in patients with unresectable hepatocellular carcinoma显示文摘AIM:To investigate the efficacy and safety of transarterial chemoembolization(TACE)-based multimodal treatment in patients with large hepatocellular carcinoma(HCC).METHODS:A total of 146 consecutive patients were included in the analysis,and their medical records and radiological data were reviewed retrospectively.RESULTS:In total,119 patients received TACE-based multi-modal treatments,and the remaining 27 received conservative management.Overall survival(P<0.001)and objective tumor response(P=0.003)were significantly better in the treatment group than in the conservative group.After subgroup analysis,survival benefits were observed not only in the multi-modal treatment group compared with the TACE-only group(P=0.002)but also in the surgical treatment group compared with the loco-regional treatment-only group(P<0.001).Multivariate analysis identified tumor stage(P<0.001)and tumor type(P=0.009)as two independent pre-treatment factors for survival.After adjusting for significant pre-treatment prognostic factors,objective response(P<0.001),surgical treatment(P=0.009),and multi-modal treatment(P=0.002)were identified as independent post-treatment prognostic factors.CONCLUSION:TACE-based multi-modal treatments were safe and more beneficial than conservative management.Salvage surgery after successful downstaging resultedin long-term survival in patients with large,unresectable HCC. | Do Seon Song Soon Woo Nam Si Hyun Bae Jin Dong Kim Jeong Won Jang Myeong Jun Song Sung Won Lee Hee Yeon Kim Young Joon Lee Ho Jong Chun Young Kyoung You Jong Young Choi Seung Kew Yoon | 2015 | World Journal of Gastroenterology2015，21，8： | 7 |
| 2 | Computed tomography and magnetic resonance imaging findings of metastatic rectal linitis plastica from prostate cancer:A case report and review of literature显示文摘Linitis plastica is a rare condition showing circumferentially infiltrating intramural anaplastic carcinoma in a hollow viscus, resulting in a tissue thickening of the involved organ as constricted, inelastic, and rigid. While most secondary rectal linitis plastica(RLP) is caused by metastasis from stomach, breast, gallbladder, or bladder cancer, we report an extremely rare and unique case of secondary RLP due to prostate cancer with computed tomography(CT) and magnetic resonance imaging(MRI) findings, including diffusion weighted imaging(DWI). A 78-year-old man presented with approximately a 2-mo history of constipation and without cancer history. On sigmoidoscopy, there was a luminal narrowing and thickening of rectum with mucosa being grossly normal in its appearance. On contrast-enhanced CT,marked contrast enhancement with wall thickening of rectum was noted. On pelvic MRI, rectal wall thickening showed a target sign on both T2-weighted imaging and DWI. A diffuse infiltrative lesion was suspected in the prostate gland based on low signal intensity on T2-weighted imaging and restricted diffusion. A transanal full-thickness excisional biopsy revealed metastasis from a prostate adenocarcinoma invading the submucosa to the muscularis propria consistent with metastatic RLP. | 3 | Scalp electroacupuncture at the Baihui acupoint (DU 20) improves functional recovery in rats with cerebral ischemia Association with increased expression of vascular endothelial growth factors显示文摘In this study, we induced cerebral infarction in rats by occluding the right middle cerebral artery, and tested the effects of electroacupuncture at the Baihui acupoint (DU 20). Motor and sensory function was tested using Garcia’s scale and motor weakness grading, and the expression of vascular endothelial growth factor in the brain was quantified using immunoblotting and immunohistochemistry. We found that scalp electroacupuncture at DU 20 significantly improved motor performance and sensory function in rats with stroke, and this was accompanied by an increased expression of vascular endothelial growth factor in the ischemic brain tissue and peri-ischemic area. | 4 | Effects of exogenous recombinant human bone morphogenic protein-7 on the corneal epithelial mesenchymal transition and fibrosis显示文摘AIM:To evaluate the effect of exogenous recombinant human bone morphogenic protein-7(rhBMP-7)on transforming growth factor-β(TGF-β)-induced epithelial mesenchymal cell transition(EMT)and assessed its antifibrotic effect via topical application.METHODS:The cytotoxic effect of rhBMP-7 was evaluated and the EMT of human corneal epithelial cells(HECEs)was induced by TGF-β. HECEs were then cultured in the presence of rhBMP-7 and/or hyaluronic acid(HA). EMT markers,fibronectin,E-cadherin,α-smooth muscle actin(α-SMA),and matrix metaloproteinase-9(MMP-9),were evaluated. The level of corneal fibrosis and the reepithelization rate were evaluated using a rabbit keratectomy model. Expression of α-SMA in keratocytes were quantified following treatment with different concentrations of rhBMP-7.RESULTS:Treatment with rhBMP-7 attenuated TGF-β-induced EMT in HECEs. It significantly attenuated fibronectin secretion(31.6%; P<0.05),the α-SMA protein level(72.2%; P<0.01),and MMP-9 expression(23.6%,P<0.05)in HECEs compared with cells grown in the presence of TGF-β alone. E-cadherin expression was significantly enhanced(289.7%; P<0.01)in the presence of rhBMP-7. Topical application of rhBMP-7 combined with 0.1% HA significantly reduced the amount of α-SMA^+ cells by 43.18%(P<0.05)at a concentration of 2.5 μg/mL and by 47.73%(P<0.05)at 25 μg/mL,compared with the control group,without disturbing corneal reepithelization.CONCLUSION:rhBMP-7 attenuates TGF-β-induced EMT in vitro,and topical application of rhBMP-7 reduces keratocyte myodifferentiation during the early wound healing stages in vivo without hindering reepithelization.
