| 1 | Probiotics and prebiotics in chronic inflammatory bowel diseases显示文摘The prokaryotic and eukaryotic cells of the colon exist in a highly complex, but harmonious relationship. Disturbances in this remarkable symbiosis can result in the development of inflammatory bowel diseases (IBD). Although the etiology of IBD is not entirely understood, it is known that the chronic inflammation of Crohn’s disease, ulcerative colitis and chronic pouchitis are a result of an overly aggressive immune response to the commensal intestinal flora in genetically susceptible hosts. Recent studies have enhanced our ability to understand the interaction between the host and its intestinal microflora and the role the microflora plays in maintaining intestinal homeostasis. As we begin to understand the benefi ts conferred to the intestine by the microflora, the notion of modifying the composition of the bacterial load to improve human health has arisen. A signifi cant body of research now exists investigating the role of probiotics and prebiotics in ameliorating chronic intestinal inflammation. This article will begin with an overview of the role of the commensal microflora in maintaining mucosal immune homeostasis, and how a dysregulated immune response to the intestinal microflora results in IBD. In particular, the interaction(crosstalk) between cartilage and subchondral bone is thought to be a central feature of this process. The interface between articular cartilage and bone of articulating long bones is a unique zone, which comprises articular cartilage,below which is the calcified cartilage sitting on and intercalated into the subchondral bone plate. Below the subchondral plate is the trabecular bone at the end of the respective long bones. In OA, there are welldescribed progressive destructive changes in the articular cartilage, which parallel characteristic changes in the underlying bone. 目的:在超重但不肥胖(体重指数为25—30)的人群中研究限制热量6个月伴/不伴运动对其产生的影响。
设计、地点及参试者:于2002年3月至2004年8月在位于路易斯安娜州首府巴顿鲁治的研究中心对48名不好动的健康人进行随机对照研究。
干预:参试者在6个月内随机分为4个组:对照组(饮食可维持体重);限制热量组(限制基线所需能量的25%);限制热量+运动组(限制12.5%的热量+运动增加12.5%的能耗);极低热量饮食组(每日摄入890keal,直至体重减少15%,随后采用维持体重的饮食)。
主要观测指标:机体组成;硫酸脱氢表雄酮(dehydroepiandrostemne sulfate,DHEAS)、葡萄糖及胰岛素水平;蛋白羰基化合物;DNA损伤;24小时能耗;核心体温。
结果:6个月时各组体重变化的均值(标准误)为:对照组-1.0%(1.1%);限制热量组-10.4%(0.9%);限制热量+运动组-10.0%(0.8%);极低热量饮食组-13.9%(0.7%)。6个月时,各干预组空腹血糖水平较基线明显降低(P均〈0.01),而DHEAS和葡萄糖水平没有改变。限制热量组及限制热量+运动组核心体温有所下降(P均〈0.05)。对机体组成进行校正后发现,对照组24小时静息能耗无变化,但限制热量组(-135kcal/d[42kcal/d])、限制热量+运动组(-117kcal/d[52kcal/d])和极低热量饮食组(-125kcal/d[35kcal/d])24小时静息能耗有所下降(P均〈0.008)。上述“代谢调节”(超出预计值的-6%)与对照组相比存在显著差异(P〈0.05)。6个月时,各组蛋白羰基化合物的浓度较基线时无变化,而各干预组DNA损伤有所减少(P〈0.005)。

| 13 | Pre-diagnostic levels of adiponectin and soluble vascular cell adhesion molecule-1 are associated with colorectal cancer risk显示文摘AIM: To examine the relationships between pre-diagnostic biomarkers and colorectal cancer risk and assess their relevance in predictive models.METHODS: A nested case-control study was designed to include all first primary incident colorectal cancer cases diagnosed between inclusion in the SUpplémentation en VItamines et Minéraux AntioXydants cohort in 1994 and the end of follow-up in 2007. Cases (n = 50) were matched with two randomly selected controls (n = 100). Conditional logistic regression models were used to investigate the associations between pre-diagnostic levels of hs-CRP, adiponectin, leptin, soluble vascular cell adhesion molecule-1 (sVCAM-1), soluble intercellular adhesion molecule-1, E-selectin, monocyte chemoattractant protein-1 and colorectal cancer risk. Area under the receiver operating curves (AUC) and relative integrated discrimination improvement (RIDI) statistics were used to assess the discriminatory potential of the models. RESULTS: Plasma adiponectin level was associated with decreased colorectal cancer risk (P for linear trend = 0.03). Quartiles of sVCAM-1 were associated with increased colorectal cancer risk (P for linear trend = 0.02). No association was observed with any of the other biomarkers. Compared to standard models with known risk factors, those including both adiponectin and sVCAM-1 had substantially improved performance for colorectal cancer risk prediction (P for AUC improvement = 0.01, RIDI = 26.5%). CONCLUSION: These results suggest that pre-diagnostic plasma adiponectin and sVCAM-1 levels are associated with decreased and increased colorectal cancer risk, respectively. Here we used the pollen records from three lakes in this region to reconstruct the evolution of local vegetation and climate since 8200 cal. yr BP. The history of temperature and precipitation was reconstructed quantitatively with multi-bioclimatic indexes and a transfer function from pollen records. Results show that the steppe/meadow dominated during the period of 8200―6500 cal. yr BP, especially 8200―7200 cal. yr BP, indicating the central Tibetan Plateau was controlled by strong monsoon. The steppe dominated during the periods of 6000―4900, 4400―3900, and 2800―2400 cal. yr BP. The steppe decreased gradually and the meadow expanded during the period of 4900―4400 cal. yr BP. Three century-scale drought events occurred during 5800―4900, 4400―3900 and 2800 cal. yr BP, respectively. The first time when the regional climate shifted to the present level was at 6500 cal. yr BP in the central Plateau. Since 3000 cal. yr BP, the temperature and precipitation have decreased gradually to the present level. | 17 | What is the quantitative risk of gastric cancer in the first-degree relatives of patients? A meta-analysis显示文摘AIM To quantify the risk of gastric cancer in first-degree relatives of patients with the cancer. METHODS A comprehensive literature search was performed. Case-control trials comparing the frequency of a positive family history of gastric cancer in patients with gastric cancer, vs non-gastric cancer controls were retrieved. Studies with missed or non-extractable data, studies in children, abstracts, and duplicate publications were excluded. A meta-analysis of pooled odd ratios was performed using Review Manager 5.0.25. We performed subgroup analysis on Asian studies and a sensitivity analysis based on the quality of the studies, type of the outcome, sample size, and whether studies considered only first-degree relatives. RESULTS Thirty-two relevant studies out of 612 potential abstracts(n = 80690 individuals) were included. 19.0% of the patients and 10.9% of the controls had at least one relative with gastric cancer(P < 0.00001). The pooled relative risk for the development of gastric cancer in association with a positive family history was 2.35(95%CI: 1.96-2.81). The Cochran Q test for heterogeneity was positive(P < 0.00001, I 2 = 92%). After excluding the three outlier studies with the highest relative risks, heterogeneity remained significant(P < 0.00001, I 2 = 90%). The result was not different among Asian studies as compared to others and remained robust in several sensitivity analyses. In the 26 studies which exclusively analysed the history of gastric cancer in first-degree relatives, the relative risk was 2.71(95%CI: 2.08-3.53; P < 0.00001). CONCLUSION Individuals with a first-degree relative affected with gastric cancer have a risk of about 2.5-fold for the development of gastric cancer. This could be due to genetic or environmental factors. | 19 | 青藏高原东南缘贡嘎山地区大气总汞时间序列分析及其影响因子显示文摘使用Tekran 2537A大气汞自动分析仪对中国科学院贡嘎山高山生态系统观测试验站磨西基地站(102°72''E 29°92''N)进行了为期1a的大气总汞(TGM)高时间分辨率观测.研究区域的平均TGM浓度为(4±1.38) ng m-3(N=57310),高于全球大气总汞背景值1.5~2.0 ng m-3.不同季节表现出相似的日变化模式,即白天相对夜晚具有较高的TGM浓度,最大TGM浓度出现在中午,最小值出现在日出前,春季和夏季日变化高峰值出现时间比秋季和冬季早1~2h.以冬季TGM浓度最高,为(6.13±1.78) ng m-3;夏季最低,为(3.17±0.67) ng m-3.观测期间不同风向间TGM浓度无显著差异.相关分析表明,TGM浓度与温度、饱和水汽压、降水量、紫外辐射、大气压有显著相关性,这种相关性随季节而变化.贡嘎山地区大气汞浓度主要受局地源的影响和调节. | 朱万泽 付学吾 冯新斌 Julia Y. | 20 | Multiparametric analysis of colorectal cancer immune responses显示文摘Colorectal cancer(CRC) is a heterogeneous disease, with a diverse and plastic immune cell infiltrate. These immune cells play an important role in regulating tumour growth-progression or elimination. Some populations of cells have a strong correlation with disease-free survival, making them useful prognostic markers. In particular, the infiltrate of CD3^+ and CD8^+ T cells into CRC tumours has been validated worldwide as a valuable indicator of patient prognosis. However, the heterogeneity of the immune response, both between patients with tumours of different molecular subtypes, and within the tumour itself, necessitates the use of multiparametric analysis in the investigation of tumour-specific immune responses. This review will outline the multiparametric analysis techniques that have been developed and applied to studying the role of immune cells in the tumour, with a focus on colorectal cancer. Because much of the data in this disease relates to T cell subsets and heterogeneity, we have used T cell populations as examples throughout. Flow and mass cytometry give a detailed representation of the cells within the tumour in a single-cell suspension on a per-cell basis. Imaging technologies, such as imaging mass cytometry, are used to investigate increasing numbers of markers whilst retaining the spatial and structural information of the tumour section and the infiltrating immune cells.
