| 1 | Management of entecavir-resistant chronic hepatitis B with adefovir-based combination therapies显示文摘AIM: To evaluate the long-term efficacy adefovir(ADV)-based combination therapies in entecavir(ETV)-resistant chronic hepatitis B(CHB) patients. METHODS: F i fty CHB pat ient s wi t h genotypic resistance to ETV at 13 medical centers in South Korea were included for the analysis. All the patients received rescue therapy with the combination of ADV plus ETV(ADV/ETV,n = 23) or ADV plus lamivudine(LMV)(ADV/LMV,n = 27) for more than 12 mo. Patients were monitored at least every 3-4 mo during ADV-based combination therapy by clinical examination as well as biochemical and virological assessments. Hepatitis B virus(HBV) DNA levels were measured by realtime PCR and logarithmically transformed for analysis. Cumulative rates of virologic response(VR; HBV DNA < 20 IU/m L) were calculated using the Kaplan-Meier method,and the difference was determined by a logrank test. Multivariate logistic regression and Cox proportional hazards models were used to identify independent risk factors significantly associated with short-term and long-term VR,respectively.RESULTS: Baseline median HBV DNA levels were 5.53(2.81-7.63) log10 IU/m L. The most commonly observed ETV genotypic mutation sites were rt184 and rt202. Patients were treated for a median of 27(12-45) mo. Overall,cumulative VR rates at 6,12,24,and 36 mo were 26%,36%,45%,and 68%,respectively. Patients treated with the ADV/ETV combination showed higher cumulative VR rates(35%,43%,65%,and 76%,respectively) than those with the ADV/LAM combination(18%,30%,30%,and 62%,respectively; P = 0.048). In the multivariate analysis,low baseline HBV DNA levels(< 5.2 log10 IU/m L) and initial virologic response at 3 mo(IVR-3; HBV DNA < 3.3 log10 IU/m L after 3 mo) were independent predictive factors for VR. Patients with favorable predictors achieved cumulative VR rates up to 90% at 36 mo. | 7 | Recurrent intramuscular lipoma at extensor pollicis brevis:A case report显示文摘BACKGROUND This report describes and discusses recurrent intramuscular lipoma(IML)of the extensor pollicis brevis(EPB).An IML usually occurs in a large muscle of the limb or torso.Recurrence of IML is rare.Recurrent IMLs,especially those with unclear boundaries,necessitate complete excision.Several cases of IML in the hand have been reported.However,recurrent IML appearing along the muscle and tendon of EPB on wrist and forearm has not been reported yet.CASE SUMMARY In this report,the authors describe clinical and histopathological features of recurrent IML at EPB.A 42-year-old Asian woman presented with a slowgrowing lump in her right forearm and wrist area six months ago.The patient had a history of surgery for a lipoma of the right forearm one year ago with a scar of 6cm on the right forearm.magnetic resonance imaging confirmed that the lipomatous mass,which had attenuation similar to subcutaneous fat,had invaded the muscle layer of EPB.Excision and biopsy were performed under general anesthesia.On histological examination,it was identified as an IML showing mature adipocytes and skeletal muscle fibers.Therefore,surgery was terminated without further resection.No recurrence occurred during a follow-up of five years after surgery.CONCLUSION Recurrent IML in the wrist must be examined to differentiate it from sarcoma.Damage to surrounding tissues should be minimized during excision. | Je Yeon Byeon Yong Seon Hwang Ji Hye Lee Hwan Jun Choi | 2023 | World Journal of Clinical Cases2023,11,3: | 0 |
| 8 | Finger compartment syndrome due to a high-pressure washer injury:A case report显示文摘BACKGROUND Although the finger compartment syndrome is not common,it compresses the neurovascular bundles in a limited space and blocks blood flow to the fingers,causing necrosis of the fingertips.Finger fasciotomy through unilateral or bilateral midline release of the finger can achieve decompression of the finger compartment.Herein,we report a case of the compartment syndrome in a finger injury caused by a high-pressure water flow which is commonly used in car washing stations.CASE SUMMARY A 60-year-old man injured his right middle finger while using a high-pressure washer at a car washing station.The patient complained of severe pain in his middle finger and a 0.2 cm punctured open wound on the volar side of the distal phalangeal joint of the middle finger.The fingertip was pale,numb,and characterized by severe swelling and a limited range of motion.Finger radiography showed that there was no fracture in the finger.Digital decompression was performed through finger fasciotomy by bilateral midline incision.On the second day after surgery,the color of the fingertip returned to pink,swelling was resolved,and the range of motion returned to normal.The sensation of the fingertip was completely restored,and the capillary refill test and pinprick test were positive.CONCLUSION The fingertip compartment syndrome can be caused by a high-pressure water flow damage to the fingers when using high-pressure washers at a car washing station.To avoid finger necrosis,rapid diagnosis of the finger compartment syndrome and appropriate digital decompression are essential to better outcome. | Jun Ho Choi Seung Yeon Choi Jae Ha Hwang Kwang Seog Kim Sam Yong Lee | 2023 | World Journal of Clinical Cases2023,11,13: | 0 |

| 10 | Does the advantage of transcutaneous oximetry measurements in diabetic foot ulcer apply equally to free flap reconstruction?显示文摘BACKGROUND Transcutaneous oxygen pressure(TcpO2)is a precise method for determining oxygen perfusion in wounded tissues.The device uses either electrochemical or optical sensors.AIM To evaluate the usefulness of TcpO2 measurements on free flaps(FFs)in diabetic foot ulcers(DFUs).METHODS TcpO2 was measured in 17 patients with DFUs who underwent anterolateral thigh(ALT)-FF surgery and compared with 30 patients with DFU without FF surgery.RESULTS Significant differences were observed in the ankle-brachial index;duration of diabetes;and haemoglobin,creatinine,and C-reactive protein levels between the two groups.TcpO2 values were similar between two groups except on postoperative days 30 and 60 when the values in the ALT-FF group remained<30 mmHg and did not increase>50 mmHg.CONCLUSION Even if the flap is clinically stable,sympathectomy due to adventitia stripping during anastomosis and arteriovenous shunt progression due to diabetic polyneuropathy could lead to low TcpO2 values in the ALT-FF owing to its thick fat tissues,which is supported by the slow recovery of the sympathetic tone following FF.Therefore,TcpO2 measurements in patients with DFU who underwent FF reconstruction may be less accurate than in those who did not. | Da Woon Lee Yong Seon Hwang Je Yeon Byeon Jun Hyuk Kim Hwan Jun Choi | 2023 | World Journal of Clinical Cases2023,11,31: | 0 |
