1帕博利珠单抗单用或与放疗联用治疗转移性非小细胞肺癌:两个随机试验的汇总分析显示文摘背景放疗可以提高整个机体对免疫治疗的应答。在Ⅱ期PEMBRO-RT研究和Ⅰ/Ⅱ期MDACC研究中,患有转移性非小细胞肺癌(NSCLC)的患者被随机分配入组,接受免疫治疗(帕博利珠单抗)+放疗联合疗法,或免疫治疗单一疗法。当上述2个研究单独分析时,联合疗法组显示出潜在获益。由于每个研究的样本量较小,缓解率和结局并未显示出统计学意义,然而却有显著的临床获益。因此,本研究进行汇总分析,来判断放疗是否会改善转移性NSCLC患者的免疫治疗应答。方法PEMBRO-RT和MDACC研究纳入标准:患者年龄≥18岁,患有转移性NSCLC,且有≥1处未经放疗照射的病灶,以便进行射野外应答监测。PEMBRO-RT研究纳入曾接受过化疗患者,MDACC研究纳入曾接受过治疗或新诊断患者。2个研究中的患者均未接受过免疫治疗。在PEMBRO-RT研究中患者被等比例随机分配入组,并根据吸烟状态进行分层(分为<10年组和≥10年组)。MDACC研究的患者根据放疗计划可行性被等比例随机分配入2个受试组。由于联合治疗组的干预本质,每个研究中的放疗均不适用盲法。在2个研究中,不论是否进行放疗,均静脉滴入帕博利珠单抗(每3周200 mg)。在PEMBRO-RT研究中,在放疗(24 Gy 3次分割照射)结束后1周给予第1剂帕博利珠单抗。在MDACC研究中,在第1次放疗(50 Gy 4次分割照射或45 Gy 15次分割照射)同时给予帕博利珠单抗。仅检测未经照射病灶的应答。本研究的终点为最佳射野外(远隔)应答率(ARR)、最佳射野外疾病控制率(ACR)、12周时ARR、12周时ACR、无进展生存期(PFS)和总生存期(OS)。2个研究的意向治疗(ITT)人群均纳入分析。PEMBRO-RT研究(NCT02492568)和MDACC研究(NCT02444741)均在ClinicalTrials.gov上注册。发现纳入148例患者,76例接受帕博利珠单抗治疗,72例接受帕博利珠单抗+放疗治疗。所有患者随访时间中位数为33个月[四分位距(IQR):32.4~33.6]。148例患者中124例(84%)组织学特征为非鳞癌,111例(75%)患者曾经接受过化疗。组间没有基线特征差异,包括PD-L1表达状态和转移灶体积。最常见的照射部位为肺转移灶(39%,28/72)、胸腔内淋巴结(21%,15/72)和非原发灶(17%,12/72)。帕博利珠单抗组和联合治疗组的最佳ARR分别为19.7%(15/76)和41.7%(30/72),OR=2.96,95%CI:1.42~6.20,P=0.0039;最佳ACR分别为43.4%(33/76)和65.3%(47/72),OR=2.51,95%CI:1.28~4.91,P=0.0071;PFS中位数分别为4.4(IQR:2.9~5.9)和9.0个月(IQR:6.8~11.2),HR=0.67,95%CI:0.45~0.99,P=0.045;OS中位数分别为8.7(IQR:6.4~11.0)和19.2个月(IQR:14.6~23.8),OR=0.67,95%CI:0.54~0.84,P=0.0004。在汇总分析中没有发现新的安全问题。解读帕博利珠单抗免疫疗法+放疗显著提高转移性NSCLC患者的应答和改善治疗结局。这些结果需要在三期临床试验中进行验证。陈大卫(翻译) 于金明(校对) Willemijn S M E Theelen Vivek Verma Brian P Hobbs Heike M U Peulen Joachim G J V Aerts Idris Bahce Anna Larissa N Niemeijer Joe Y Chang Patricia M de Groot Quynh-Nhu Nguyen Nathan I Comeaux George R Simon Ferdinandos Skoulidis Steven H Lin Kewen He Roshal Patel John Heymach Paul Baas James W Welsh 2021中华肿瘤防治杂志2021,28,24:49
2RESEARCH ON WEIGHTED PRIORITY OF EXEMPLAR-BASED IMAGE INPAINTING显示文摘The priority of the filled patch play a key role in the exemplar-based image inpainting, and it should be determined firstly to optimize the process of image inpainting. A modified image inpainting algorithm is proposed by weighted-priority based on the Criminisi algorithm. The improved algorithm demonstrates better relationship between the data term and the confidence term for the optimization of the priority than the classical Criminisi algorithm. 3The FTO/miR-181b-3p/ARL5B signaling pathway regulates cell migration and invasion in breast cancer显示文摘Background:N6-methyladenosine(m6A)RNA modification has been demonstrated to be a significant regulatory process in the progression of various tumors,including breast cancer.Fat mass and obesity-associated(FTO)enzyme,initially known as the obesity-related protein,is the first identified m6A demethylase.However,the relationship between FTO and breast cancer remains controversial.In this study,we aimed to elucidate the role and clinical significance of FTO in breast cancer and to explore the underlying mechanism.Methods:We first investigated the expression of FTO in breast cancer cell lines and tissues by quantitative reverse transcription-PCR(qRT-PCR),Western blotting,and immunohistochemistry.Wound healing assay and Transwell assay were performed to determine the migration and invasion abilities of SKBR3 and MDAMB453 cells with either knockdown or overexpression of FTO.RNA sequencing(RNA-seq)was conducted to decipher the downstream targets of FTO.qRT-PCR,luciferase reporter assay,and Western blotting were employed to confirm the existence of the FTO/miR-181b-3p/ARL5B axis.The biological function of ADP ribosylation factor like GTPase 5B(ARL5B)in breast cancer cells was evaluated by wound healing assay and Transwell invasion assay.Results:High FTO expression was observed in human epidermal growth factor receptor 2(HER2)-positive breast cancer,predicting advanced progression(tumor size[P<0.001],nuclear grade[P=0.001],peritumoral lymphovascular invasion[P<0.001),lymph node metastasis[P=0.002],and TNM stage[P=0.001])and poor prognosis.Moreover,FTO promoted cell invasion and migration in vitro.Mechanistically,RNA-seq and further confirmation studies suggested that FTO up-regulated ARL5B by inhibiting miR-181b-3p.We further verified that ARL5B also displayed carcinogenic activity in breast cancer cells.Conclusion:Our work demonstrated the carcinogenic activity of FTO in promoting the invasion and migration of breast cancer cells via the FTO/miR-181b-3p/ARL5B signaling pathway.Yuanyuan Xu Shuang Ye Nan Zhang Shuhui Zheng Huatao Liu Kewen Zhou Ling Wang Yue Cao Peng Sun Tinghuai Wang 2020Cancer Communications2020,40,10:24
4Does conflict matter in the success of mass collaboration? Investigating antecedents and consequence of conflict in Wikipedia显示文摘Purpose: Web2.0 has facilitated the emergence of mass collaboration. Being a new model of virtual collaboration, mass collaboration has distinct characteristics such as self-organized voluntary participation and many-to-many communications mechanism. Behind the sparking achievements, mass collaboration systems are suffering from high coordination cost, reflecting intense conflict among the participants. This study explains how conflict interacts with its antecedents(participation and task complexity) and decedent(group performance) related to the mass collaboration process.Design/methodology/approach: Theoretical model is built upon literatures. Twenty-six measurable items, including social network indices used to reflect conflicts(e.g., density and degree), are extracted from Wikipedia database. The structural equation modeling is used to validate the proposed model.Findings: Participation increases task complexity, conflict and performance; task complexity and conflict increase group performance, but task complexity decreases conflict.Research limitations: This study is limited to one mass collaboration platform, other contexts can cause variance in the result. More characteristics of conflict network(e.g., self-loop)should be taken into consideration in future studies.Practical implications: Conflict management is critical to mass collaboration, and functions that increase the awareness of conflict may motivate users to participant, resulting better group performance.Originality/value: Few studies have investigated the consequences of conflict in mass collaboration context. Meanwhile, limited number of social network studies build network based on negative link. Moreover, most of extant studies on Wiki are descriptive researches without theoretical model. 5Association between AKT1 gene polymorphisms and depressive symptoms in the Chinese Han population with major depressive disorder显示文摘For this study, 461 Chinese Han patients with depressive disorder were recruited. The AKT1 genotype and allele distribution were determined by PCR amplification and direct sequencing. UNPHASED software was used to analyze associations between the 17-item Hamilton Depression Rating Scale, total score, four factors and the AKT1 rs2494746 and rs3001371 polymorphisms. The results indicate that there is a significant association between suicidal ideation and anxiety symptoms in depressed patients and the rs2494746 polymorphism. The other AKT1 polymorphism, rs3001371, was significantly associated with work and activities. Patients with the rs3001371-A allele had a significantly more severe illness compared to patients with the rs3001371-G allele. 9Sodium Alginate/Carboxyl-Functionalized Graphene Composite Hydrogel Via Neodymium Ions Coordination显示文摘A facile method for the preparation of sodium alginate(SA)/carboxyl-functionalized graphene(G-COOH)composite hydrogel was developed. Based on the coordination ability of lanthanide ions to the carboxyl groups, a series of hydrogel derived from different ratios of SA and G-COOH was fabricated by neodymium(Nd^(3+)) ions coordination. A relatively uniform layered structure was recorded by SEM at the interior of SA/G-COOH hydrogel. Several parameters such as water content, swelling ratio(SR), tensile test and solvent resistance were also investigated. The SA/G-COOH composite hydrogel showed excellent mechanical strength, and the tensile strength of SA/G-COOH composite hydrogel reaches 53.72 MPa at high water content. 12Output-Feedback Based Simplified Optimized Backstepping Control for Strict-Feedback Systems with Input and State Constraints显示文摘In this paper,an adaptive neural-network(NN)output feedback optimal control problem is studied for a class of strict-feedback nonlinear systems with unknown internal dynamics,input saturation and state constraints.Neural networks are used to approximate unknown internal dynamics and an adaptive NN state observer is developed to estimate immeasurable states.Under the framework of the backstepping design,by employing the actor-critic architecture and constructing the tan-type Barrier Lyapunov function(BLF),the virtual and actual optimal controllers are developed.In order to accomplish optimal control effectively,a simplified reinforcement learning(RL)algorithm is designed by deriving the updating laws from the negative gradient of a simple positive function,instead of employing existing optimal control methods.In addition,to ensure that all the signals in the closed-loop system are bounded and the output can follow the reference signal within a bounded error,all state variables are confined within their compact sets all times.Finally,a simulation example is given to illustrate the effectiveness of the proposed control strategy.Jiaxin Zhang Kewen Li Yongming Li 2021IEEE/CAA Journal of Automatica Sinica2021,8,6:2
13Equilibrium of liquid-liquid extraction of 2-phenylbutyric acid enantiomers: Experiment and model显示文摘The utilization of liquid–liquid extraction for the separation of 2-phenylbutyric acid(2-PBA) enantiomers was proposed. Factors affecting the extract process were investigated, including organic solvents, β-cyclodextrin derivatives, cyclodextrin concentration, p H and temperature. A model was proposed to describe the separation process based on the homogeneous phase reaction mechanism. Important parameters of this model were determined experimentally. The physical distribution coefficients for molecular and ionic 2-PBA were0.129 and 7.455, respectively. The equilibrium constants of the complexation reactions were 89.36 and36.78 L·mol^(-1) for(+)-and(-)-2-PBA, respectively. The model was verified by experiments and proved to be an excellent means to optimize the separation system. Through modeling prediction and experiment, the best conditions(e.g., pH value of 3.00, extractant concentration of 0.1 mol·L^(-1), temperature of 5.0 °C) were acquired. 14Modeling multiple chemical equilibrium for reactive extraction of naproxen enantiomers with HP-β-CD as hydrophilic selector显示文摘Reactive extraction is an emerging technology for large-scale continuous resolution of drug enantiomers.The enantioselective extraction of R,S-naproxen by hydrophilic HP-β-CD in 1,2-dichloroethane was studied at 5 °C.The experimental data were described by a reactive extraction model with a homogeneous aqueous phase reaction of R,S-naproxen with HP-β-CD which couples a complete description of chemical equilibria in aqueous phase with the overall phase equilibria of the system.Important parameters of this model were determined experimentally.The physical distribution coefficients for molecular and ionic NAP were 0.041 and 1.730,respectively.Here we show that the efficiency of extraction depends strongly on two process variables including pH and HP-β-CD concentration.The model predictions are compared graphically with the results of previous experiments and there is a good agreement between each other.By the use of modeling and experiment,an optimized extraction condition with pH of 2.5 and HP-β-CD concentration of 0.1 mol/L was obtained with high enantioselectivity(α) of 1.59 and performance factor(pf) of 0.049.The model gives a good means of predicting enantiomers partitioning over a range of experimental conditions.TANG KeWen ZHANG PanLiang PAN ChunYue LI HongJian 2011Science China Chemistry2011,54,7:2
15Generalized Switched-Capacitor Step-up Multilevel Inverter Employing Single DC Source显示文摘In this paper,a new generalized step-up multilevel DC-AC converter is proposed,which is suitable for applications with low-voltage input sources,such as photovoltaic power generation and electric vehicles.This inverter can achieve a high voltage gain by controlling the series-parallel conversion of the DC power supply and capacitors.Only one DC voltage source and a few power devices are employed.The maximum output voltage and the number of output levels can be further increased through the switched-capacitor unit’s extension and the submodule cascaded extension.Moreover,the capacitor voltages are self-balanced without complicated voltage control circuits.The complementary operating mechanism between each pair of switches simplifies the modulation algorithm.The inductiveload ability is fully taken into account in the proposed inverter.Additionally,a remarkable characteristic of the inverter is that the charging and discharging states among different capacitors are synchronous,which reduces the voltage ripple of the frontend capacitors.The circuit structure,the working principle,the modulation strategy,the capacitors and losses analysis are presented in detail.Afterwards,the advantages of the proposed inverter are analyzed by comparing with other recently proposed inverters.Finally,the steady-state and dynamic performance of the proposed inverter is verified and validated by simulation and experiment.Yaoqiang Wang Kaige Wang Gen Li Fengjiang Wu Kewen Wang Jun Liang 2022CSEE Journal of Power and Energy Systems2022,8,2:2
