| 1 | Current Tracking on Effectiveness and Mechanisms of Acupuncture Therapy: A Literature Review of High-Quality Studies显示文摘The scientific evidenee of acupuncture studies has been improved in recent years,and one of the important manifestations is that more and more acupuncture clinical trials and mechanism researches have been published in the source periodicals of Science Citation Index(SCI).This study summarized the dominant diseases of acupuncture focusing on of acupuncture efficacy and mechanisms,and discussed the existing problems,highlight!ng the directi on of future developme nts.Most clinical studies were published in journals with journal impact factor(JIF)score of 10 or above,and majority of the basic researches had JIF scores of 5 to 10.The above literature were further divided according to the International Classification of Diseases(ICD).The most concerned diseases in these articles were neurological diseases,musculoskeletal system and connective tissue diseases,tumor and digestive system diseases.The therapeutic effect and mechanism of acupuncture on each kind of disease were summarized.The results showed that the therapeutic effect of acupuncture on nerve injury focused on the anti-oxidation pathway,neuroprotective and anti-inflammatory processes.The antiinflammatory effect also played an important role in the treatme nt of musculoskeletal diseases.The an algesic effect was underlined in most of these studies.Clinical trials were well carried out on acupuncture curative effect of tumor complications and side effects of chemo-radiotherapy,but the potential mechanisms have not been clarified.Somato-visceral reflex was suggested to be strongly associated with the effects of acupuncture changing the motor activity of the gastrointestinal tract.Functional magnetic resonance imaging studies indicated that non-specific effects play important roles in acupuncture analgesia.Lines of evidenee have pointed out that the regulation of neuro-endocrine-immune networks may be a common switch of acupuncture on different nerve system diseases. | YANG Fu-ming YAO Lin WANG Shen-jun GUO Yi XU Zhi-fang ZHANG Chien-Hung ZHANG Kuo FANG Yu-xin LIU Yang-yang | 2020 | Chinese Journal of Integrative Medicine2020，26，4： | 17 |

| 4 | CDH13 Genetic Polymorphisms, Adiponectin and Ischemic Stroke: a Chinese Family-based Sib-pair Study显示文摘Objective To understand the relationships between CDH13(T-cadherin) genetic polymorphisms, adiponectin levels and ischemic stroke, and possible interactions between CDH13 polymorphisms and other risk factors. Methods We recruited 342 Chinese ischemic stroke sib pairs. We genotyped rs4783244 and rs7193788 on CDH13 using time-of-flight mass spectrometry genotyping technology and measured total and high-molecular weight(HMW) adiponectin levels. We investigated associations between SNPs and ischemic stroke, and interactions between SNPs and other risk factors using multi-level mixed-effects regression model. Results In individuals without ischemic stroke, CDH13 rs4783244 was associated with total adiponectin levels(per T: Coef =-0.257, P = 0.001). CDH13 rs7193788 was associated with total adiponectin levels(per A: Coef =-0.221, P = 0.001) and HMW adiponectin levels(per A: Coef =-0.163, P = 0.003). rs7193788 was significantly associated with ischemic stroke(GA/AA vs. GG: OR = 1.55, 95% CI: 1.07 to 2.24, P = 0.020) after Bonferroni correction(α = 0.025). There was an interaction between rs7193788 and diabetes(P = 0.036). Compared to diabetes-free individuals with rs7193788 GG genotype, diabetes patients with rs7193788 GA/AA genotypes had higher risks for ischemic stroke(OR = 2.64, 95% CI: 1.58-4.40, P < 0.001). Conclusion CDH13 genetic polymorphisms are associated with adiponectin levels and ischemic stroke. | 8 | Initiation of Acupoint Molecular Mechanisms for Manual Acupuncture Analgesia—Nuclear FactorκB Signaling Pathway显示文摘Objective To identify the prominent molecular signaling in acupoints and explore their roles in initiating the analgesia effect of manual acupuncture(MA).Method A three-step study was conducted,the experiment 1 was a genome-wide analysis of the tissue at acupoint Zusanli(ST 36),including 12 Wistar rats which were divided into control,control+MA1,and control+MA7 groups.In the experiment 2,the paw withdrawal latency(PWL),immunohistochemistry and Western blot analysis of phospho-nuclear factor kappa B(NFκB)p65(p-p65),phospho-NFκB p50(p-p50)at ST 36 were performed on rats of saline,saline+MA,and complete Freund’s adjuvant(CFA)+MA groups(n=6).In experiment 3,24 rats were divided into saline+DMSO,CFA+DMSO,CFA+DMSO+MA,and CFA+BAY 11–7082+MA groups,the PWL and immunofluorescence assay of NFκB p65 at ST 36 was conducted.Result(1)The gene:inhibitor of NFκB(Nfkbia),interleukin-1β(Il1b),interleukin-6(Il6),chemokine c-x-c motif ligand 1(Cxcl1),monocyte chemoattractant protein-1(MCP-1/Ccl2)expressions in the control+MA7 group were significantly increased(P<0.05 or P<0.01),and the expression of NFκB p65(Rela),NFκB p50(Nfkb1)were increased in the control+MA7 group(P<0.05).(2)CFA+MA groups showed increased PWL from day 1 to 7(P<0.01 vs.CFA),and the Western blot results were consistent with immunohistochemistry,the expression of NFκB p-p65 and NFκB p-p50 were significantly increased in the MA-related groups compared with control and CFA groups(P<0.05).(3)Compared with the CFA+DMSO+MA group,the PWL of the CFA+BAY 11–7082+MA group decreased significantly and continued until day 5 and 7(P<0.05 and P<0.01,respectively),and the NFκB p65 expression of CFA+BAY 11–7082+MA was significantly reduced compared with CFA+DMSO+MA(P<0.01).Conclusion Local NFκB signaling cascade in acupoint caused by MA is an important step in initiating the analgesic effect,which would provide new evidence for the initiation of MA-effect and improve the understanding of the scientific basis of acupuncture analgesia. | ZHANG Kuo ZHAO Xue YANG Tao ZHANG Yan-fang YAN Ya-wen XU Zhi-fang GONG Yi-nan LIU Yang-yang GUO Yong-ming GUO Yi | 2021 | Chinese Journal of Integrative Medicine2021，27，12： | 3 |
| 9 | Determinants of delayed diagnosis and treatment of tuberculosis in Cambodia:a mixed-methods study显示文摘Background:Cambodia is among the 30 countries in the world with the highest burden of tuberculosis(TB),and it is estimated that 40%of people with TB remain undiagnosed.In this study,we aimed to investigate the determinants of delayed diagnosis and treatment of TB in Cambodia.Methods:This mixed-method explanatory sequential study was conducted between February and September 2019 in 12 operational districts in Cambodia.It comprised of a retrospective cohort study of 721 people with TB,followed by a series of in-depth interviews.We assessed factors associated with time to TB diagnosis and treatment initiation using Cox proportional hazards model.Subsequently,we conducted in-depth interviews with 31 people with TB purposively selected based on the time taken to reach TB diagnosis,sex,and residence.Transcripts were coded,and thematic analyses were performed.Results:The median time from the onset of symptoms to TB diagnosis was 49 days(Interquartile range[IQR]:21–112).We found that longer time to diagnosis was significantly associated with living in rural area(Adjusted hazards ratio[aHR]=1.25;95%confidence interval[CI]:1.06–1.48);TB symptoms-cough(aHR:1.52;95%CI:1.18–1.94),hemoptysis(aHR 1.32;95%CI:1.07–1.63),and night sweats(aHR:1.24;95%CI:1.05–1.46);seeking private health care/self-medication(aHR:1.23;95%CI:1.04–1.45);and higher self-stigma(aHR:1.02;95%CI:1.01–1.03).Participants who received education level above the primary level were inversely associated with longer time to diagnosis(aHR:0.78;95%CI:0.62–0.97).The median time from TB diagnosis to the initiation of treatment was two days(IQR:1–3).The use of smear microscopy for TB diagnosis(aHR:1.50;95%CI:1.16–1.95)was associated with longer time to treatment initiation.Seeking private health care and self-medication before TB diagnosis,lack of perceived risk,threat,susceptibility,and stigma derived qualitatively further explained the quantitative findings.Conclusions:TB diagnostic delay was substantial.Increasing public awareness about TB and consciousness regarding stigma,engaging the private healthcare providers,and tailoring approaches targeting the rural areas could further improve early detection of TB and narrowing the gap of missing cases in Cambodia. | Alvin Kuo Jing Teo Chetra Ork Sothearith Eng Ngovlyly Sok Sovannary Tuot Li Yang Hsu Siyan Yi | 2020 | Infectious Diseases of Poverty2020，9，3： | 2 |
