1Probiotic monotherapy and Helicobacter pylori eradication: A systematic review with pooled-data analysis显示文摘AIM To define probiotic monotherapy effect on Helicobacter pylori(H. pylori) status by performing a systematic review.METHODS Methods of analysis and inclusion criteria were based on PRISMA recommendations. Relevant publications were identified by searching Pub Med, MEDLINE, Science Direct, and EMBASE. The end-point was to estimate eradication rate and urea breath test delta value before and after probiotic monotherapy across all studies and, overall, with a pooled data analysis. Adverse events of probiotic therapy were evaluated. The data were expressed as proportions/percentages, and 95%CIs were calculated. For continuous variables, we evaluated the weighted mean difference. Odd ratios(ORs) were calculated according to the Peto method for the comparison of eradication rates between probiotics and placebo.RESULTS Eleven studies were selected. Probiotics eradicated H. pylori in 50 out of 403 cases. The mean weighted eradication rate was 14%(95%CI: 2%-25%, P =0.02). Lactobacilli eradicated the bacterium in 30 out of 235 patients, with a mean weighted rate of 16%(95%CI: 1%-31%). Saccharomyces boulardii achieved eradication in 6 out of 63 patients, with a pooled eradication rate of 12%(95%CI: 0%-29%). Multistrain combinations were effective in 14 out of 105 patients, with a pooled eradication rate of 14%(95%CI: 0%-43%). In the comparison of probiotics vs placebo, we found an OR of 7.91 in favor of probiotics(95%CI: 2.97-21.05, P < 0.001). Probiotics induced a mean reduction in delta values higher than placebo(8.61% with a 95%CI: 5.88-11.34, vs 0.19% for placebo, P < 0.001). 2Hydroxytryptamine transporter gene-linked polymorphic region(5HTTLPR)is associated with delusions in Alzheimer’s disease显示文摘Background:Serotoninergic pathways underlying delusion symptoms in Alzheimer’s disease(AD)have not been fully clarified.5-Hydroxytryptamine transporter gene-linked polymorphic region(5-HTTLPR)is a variable number tandem repeats in the promoter region of serotonin transporter encoding-gene affecting transcription.Methods:We investigated the association of 5-HTTLPR with delusions in a total of 257 consecutive patients clinically diagnosed as AD according to the National Institute on Aging-Alzheimer’s Association criteria.All participants underwent a comprehensive evaluation with a standardized comprehensive geriatric assessment and Neuropsychiatric Inventory.Results:Delusion symptoms were observed in 171 patients(66.54%).In respect to AD patients without delusions,AD patients with delusions showed a low prevalence of S-plus carriers(5-HTTLPR-L/S+5-HTTLPR-S/S genotypes)[p<0.001;odds ratio(OR)=0.240,95% confidence interval(CI)=0.121–0.471].Logistic regression analysis adjusted for the apolipoprotein E polymorphism showed that in AD patients with delusions the presence of an 5-HTTLPR-S allele may reduce disease duration(p=0.005;OR=0.680,95% CI=0.522–0.886)and increase aberrant motor activity(p=0.013;OR=2.257,95% CI=1.195–4.260).The present findings suggested that 5-HTTLPR might be associated with delusions in AD.S-plus carriers might be associated with protective effect against delusions in AD.Conclusions:More studies on wider samples of high selected demented patients are needed to confirm our results.However,the present findings suggested that a genetic factor related to serotonin metabolism might exert a protective role on the clinical expression of neuropsychiatric clusters in AD with important implications regarding mechanisms underlying delusions and their possible treatment across the AD and dementia spectrum.Grazia D’Onofrio Francesco Panza Daniele Sancarlo Michele Lauriola Mariangela P.Dagostino Giulia Paroni Madia Lozupone Antonio Mangiacotti Paola Bisceglia Carolina Gravina Maria Urbano Filomena Addante Francesco Paris Leandro Cascavilla Antonio Greco Davide Seripa 2019Translational Neurodegeneration2019,8,1:2

5Proximal sensor-enhanced soil mapping in complex soil-landscape areas of Brazil显示文摘Portable X-ray fluorescence(pXRF) spectrometry and magnetic susceptibility(MS) via magnetometer have been increasingly used with terrain variables for digital soil mapping. However, this methodology is still emerging in many countries with tropical soils. The objective of this study was to use proximal soil sensor data associated with terrain variables at varying spatial resolutions to predict soil classes using the Random Forest(RF) algorithm. The study was conducted on a 316-ha area featuring highly variable soil classes and complex soil-landscape relationships in Minas Gerais State, Brazil. The overall accuracy and Kappa index were evaluated using soils that were classified at 118 sites, with 90 being used for modeling and 28 for validation. Digital elevation models(DEMs) were created at 5-, 10-, 20-, and 30-m resolutions using contour lines from two sources. The resulting DEMs were processed to generate 12 terrain variables. Total Fe, Ti, and SiO_(2) contents were obtained using pXRF, with MS determined via a magnetometer. Soil class prediction was performed using the RF algorithm. The quality of the soil maps improved when using only the five most important covariates and combining proximal sensor data with terrain variables at different spatial resolutions. The finest spatial resolution did not always provide the most accurate maps. The high soil complexity in the area prevented highly accurate predictions. The most important variables influencing the soil mapping were MS, Fe, and Ti. 7Risk factors and prediction of acute kidney injury after liver transplantation:Logistic regression and artificial neural network approaches显示文摘BACKGROUND Acute kidney injury(AKI)has serious consequences on the prognosis of patients undergoing liver transplantation.Recently,artificial neural network(ANN)was reported to have better predictive ability than the classical logistic regression(LR)for this postoperative outcome.AIM To identify the risk factors of AKI after deceased-donor liver transplantation(DDLT)and compare the prediction performance of ANN with that of LR for this complication.METHODS Adult patients with no evidence of end-stage kidney dysfunction(KD)who underwent the first DDLT according to model for end-stage liver disease(MELD)score allocation system was evaluated.AKI was defined according to the International Club of Ascites criteria,and potential predictors of postoperative AKI were identified by LR.The prediction performance of both ANN and LR was tested.RESULTS The incidence of AKI was 60.6%(n=88/145)and the following predictors were identified by LR:MELD score>25(odds ratio[OR]=1.999),preoperative kidney dysfunction(OR=1.279),extended criteria donors(OR=1.191),intraoperative arterial hypotension(OR=1.935),intraoperative massive blood transfusion(MBT)(OR=1.830),and postoperative serum lactate(SL)(OR=2.001).The area under the receiver-operating characteristic curve was best for ANN(0.81,95%confidence interval[CI]:0.75-0.83)than for LR(0.71,95%CI:0.67-0.76).The root-mean-square error and mean absolute error in the ANN model were 0.47 and 0.38,respectively.CONCLUSION The severity of liver disease,pre-existing kidney dysfunction,marginal grafts,hemodynamic instability,MBT,and SL are predictors of postoperative AKI,and ANN has better prediction performance than LR in this scenario.Luis Cesar Bredt Luis Alberto Batista Peres Michel Risso Leandro Cavalcanti de Albuquerque Leite Barros 2022World Journal of Hepatology2022,14,3:1
