| 1 | Efficacy and safety of metformin and sitagliptin based triple antihyperglycemic therapy(STRATEGY):a multicenter,randomized,controlled,non-inferiority clinical trial显示文摘Despite the current guideline's recommendation of a timely stepwise intensification therapy,the 'clinical inertia',termed as the delayed treatment intensification,commonly exists in the real world,which may be partly due to the relatively little substantial evidence and no clear consensus regarding the efficacy and safety of triple oral agents in patients inadequately controlled with dual therapy.In this clinical trial performed in 237 centers in China,5,535 type 2 diabetic patients inadequately controlled by previous therapies were treated with a stable metformin/sitagliptin dual therapy for 20 weeks.The patients who did not reach the glycated hemoglobin A1c(HbA1c) goal were then further randomized into glimepiride,gliclazide,repaglinide,or acarbose group for an additional 24-week triple therapy.A mean HbAlc reduction of 0.85%was observed when sitagliptin was added to the patients inadequately controlled with metformin in 16 weeks.Further HbAlc reductions in the 24-week triple therapy stage were 0.65%in glimepiride group,0.70%in gliclazide group,0.61%in repaglinide group,and 0.45%in acarbose group.The non-inferiority criterion for primary hypotheses was met for gliclazide and repaglinide,but not for acarbose,compared with glimepiride,when added to metformin/sitagliptin dual therapy.The incidences of adverse events(AEs) were 29.2%in the dual therapy stage and30.3%in the triple therapy stage.Metformin/sitagliptin as baseline therapy,with the addition of a third oral antihyperglycemic agent,including glimepiride,gliclazide,repaglinide,or acarbose,was effective,safe and well-tolerated for achieving an HbAlc<7.0%goal in type 2 diabetic patients inadequately controlled with previous therapies.The timely augmentation of up to three oral antihyperglycemic agents is valid and of important clinical benefit to prevent patients from exposure to unnecessarily prolonged hyperglycemia. | Wen Xu Yiming Mu Jiajun Zhao Dalong Zhu Qiuhe Ji Zhiguang Zhou Bin Yao Anhua Mao Samuel S.Engel Bin Zhao Yan Bi Longyi Zeng Xingwu Ran Juming Lu Linong Ji Wenying Yang Weiping Jia Jianping Weng | 2017 | Science China(Life Sciences)2017,60,3: | 19 |
| 2 | Pollution of organic compounds and heavy metals in a coal gangue dump of the Gequan Coal Mine, China显示文摘Samples around a coal gangue dump of the Gequan Coal Mine were collected in April 2009. GC (gas chromatography) and GC/MS (gas chromatography/mass spectrometry) were employed to analyze the composition of organic matter in the samples. ICP-MS (inductively coupled plasma mass spectrometry) was used to determine the concentrations of heavy metals. The contents of organic extracts are within the range of 140-750 mg/kg. Alkand aro-ratios are relatively high. Compared to those of the background sample (GQ13 ), the contents of saturated hydrocarbon compounds in all the samples are relatively high. The contents of polycyclic aromatic hydrocarbons (PAHs) are relatively high with the distance getting closer to the coal gangue dump. These indicate that organic matter in the samples is from coal particles of the coal gangue dump. | 4 | Types of Organic Fades and Source Rock Assessment of the Coal-Measure Mudstone in the Turpan-Hami Basin显示文摘This paper discusses the enviromental characteristics of carbonaceous mudstone and mudstone (coal-measure mudstone in short) of the Early and Middle Jurassic in the Turpan-Hami basin, which were formed in swamps. Through the organic facies study of the coal-measure mudstone in this area, the authors clarify that the flowing-water swamp is the most advanced organic facies belt. | 5 | Characterization of coal burning-derived individual particles emitted from an experimental domestic stove显示文摘Coal combustion in the domestic stoves, which is common in most parts of the Chinese countryside, can release harmful substances into the air and cause health issues. In this study, particles emitted from laboratory stove combustion of the raw powder coals were analyzed for morphologies and chemical compositions by using transmission electron microscopy(TEM) coupled with energy-dispersive X-ray spectrometry(EDX). The coal burning-derived individual particles were classified into two groups: carbonaceous particles(including soot aggregates and organic particles) and non-carbonaceous particles(including sulfate, mineral and metal particles). The non-carbonaceous particles, which constituted a majority of the coal burning-derived emissions, were subdivided into Si-rich, S-rich, K-rich,Ca-rich, and Fe-rich particles according to the elemental compositions. The Si-rich, S-rich and K-rich particles are commonly observed in the coal burning emission. The proportions for particles of different types exhibit obvious coal-issue dependence. Burning of coal with high ash yield could emit more non-carbonaceous particles, and burning of coal with high sulfur content can emit more S-rich particles. | 10 | Oxidative damages to DNA by indoor PM_(10)s: Their relationships with trace element compositions显示文摘Plasmid DNA assay and ICP-MS analysis were conducted in order to investigate the bioreactivity of inhalable particles (PM_ 10) and the relationship between bioreactivity and trace element compositions of PM_ 10 in Beijing air. A total of four PM_ 10 samples were carefully selected to represent the indoor and corresponding outdoor environments: one from urban smoker’s home, two from non-smoker’s homes, and the other from the outdoor. In general, the oxidative damage by indoor PM_ 10 was slightly higher than that of outdoor. Among the four sets of samples, the PM_ 10 from the smoker’s home had a lowest TD50 (toxic dose of PM_ 10 causing 50% DNA to be damaged), being 100 μg·mL -1, suggesting the highest bioreactivity. The heavy metals are believed to be the main reason for oxidative damage to plasmid DNA. The ICP-MS analysis combined with the DNA assay showed that the water-soluble zinc levels had better relationship with TD50 values than other elements, implying that water-soluble zinc might play an important role in the damage of DNA. It is concluded that the PM_ 10 in smoker’s home had the highest level of water-soluble zinc as well as the lowest TD50 (highest bioreactivity). Iron is considered to be one of the most bioreactive elements, but it will cause little damage to plasmid DNA, probably because iron is mainly in water-insoluble state in Beijing PM_ 10. | SHAO Longyi ZHAO Houyin T.
