| 1 | 高PM2.5地区呼吸系统疾病发病率流行病学调查显示文摘目的探讨高PM2.5地区居民呼吸系统疾病发病率情况。方法采取随机抽样方法,廊坊市区、呼和浩特市区常住居民共3564例填写呼吸系统流行病学调查表,统计并比较两地市区居民2008年1月1日至2016年12月30日呼吸系统疾病发病率、症状发生率。在廊坊市区被调查者中随机抽取200例填写2012—2016年每年度呼吸系统流行病学调查表,并统计年均PM2.5浓度,探讨呼吸系统疾病和PM2.5浓度的相关性。结果廊坊市年均PM2.5浓度、空气质量指数明显高于呼和浩特市区(P值均〈0.05)。廊坊市区居民鼻炎、鼻敏感、慢性咽炎、上呼吸道感染、鼻窦炎、支气管炎、支气管哮喘、慢性阻塞性肺疾病(COPD)、肺炎、呼吸系统肿瘤、间质性肺病发病率均高于呼和浩特市区居民(P值均〈0.05);廊坊市区居民咳嗽、咳痰、喘息气促、咽部不适、鼻塞、喉咙痛、喘鸣、呼吸困难发生率高于呼和浩特市区居民(P值均〈0.05)。PM2.5浓度与鼻炎、慢性咽炎、上呼吸道感染、支气管炎、支气管哮喘呈正相关(P值均≤0.05)。结论高PM2.5地区鼻炎、鼻敏感、慢性咽炎、上呼吸道感染、鼻窦炎、支气管炎、支气管哮喘、COPD、肺炎、呼吸系统肿瘤、间质性肺病的发病率较高;高PM2.5地区居民更易发生咳嗽、咳痰、喘息气促、咽部不适、鼻塞、喉咙痛、喘鸣、呼吸困难。 | 高菲 边毓尧 郭文有 修明文 刘永纯 陈光远 Nishant Raj Pandey | 2018 | 国际呼吸杂志2018,38,18: | 13 |
| 2 | Significance of blood pressure variability in patients with sepsis显示文摘BACKGROUND: This study was undertaken to observe the characteristics of blood pressure variability(BPV) and sepsis and to investigate changes in blood pressure and its value on the severity of illness in patients with sepsis.METHODS: Blood parameters, APACHE II score, and 24-hour ambulatory BP were analyzed in 89 patients with sepsis.RESULTS: In patients with APACHE II score>19, the values of systolic blood pressure(SBPV), diasystolic blood pressure(DBPV), non-dipper percentage, cortisol(COR), lactate(LAC), platelet count(PLT) and glucose(GLU) were significantly higher than in those with APACHE II score ≤19(P<0.05), whereas the values of procalcitonin(PCT), white blood cell(WBC), creatinine(Cr), PaO2, C-reactive protein(CRP), adrenocorticotropic hormone(ACTH) and tumor necrosis factor α(TNF-α) were not statistically significant(P>0.05). Correlation analysis showed that APACHE II scores correlated significantly with SBPV and DBPV(P<0.01, r=0.732 and P<0.01, r=0.762). SBPV and DBPV were correlated with COR(P=0.018 and r=0.318; P=0.008 and r=0.353 respectively). However, SBPV and DBPV were not correlated with TNF-α, IL-10, and PCT(P>0.05). Logistic regression analysis of SBPV, DBPV, APACHE II score, and LAC was used to predict prognosis in terms of survival and non-survival rates. Receiver operating characteristics curve(ROC) showed that DBPV was a better predictor of survival rate with an AUC value of 0.890. However, AUC of SBPV, APACHE II score, and LAC was 0.746, 0.831 and 0.915, respectively.CONCLUSIONS: The values of SBPV, DBPV and non-dipper percentage are higher in patients with sepsis. | 3 | Dihydromyricetin and Salvianolic acid B inhibit alpha-synuclein aggregation and enhance chaperone-mediated autophagy显示文摘Background:Progressive accumulation ofα-synuclein is a key step in the pathological development of Parkinson’s disease.Impaired protein degradation and increased levels ofα-synuclein may trigger a pathological aggregation in vitro and in vivo.The chaperone-mediated autophagy(CMA)pathway is involved in the intracellular degradation processes ofα-synuclein.Dysfunction of the CMA pathway impairsα-synuclein degradation and causes cytotoxicity.Results:In the present study,we investigated the effects on the CMA pathway andα-synuclein aggregation using bioactive ingredients(Dihydromyricetin(DHM)and Salvianolic acid B(Sal B))extracted from natural medicinal plants.In both cell-free and cellular models ofα-synuclein aggregation,after administration of DHM and Sal B,we observed significant inhibition ofα-synuclein accumulation and aggregation.Cells were co-transfected with a Cterminal modifiedα-synuclein(SynT)and synphilin-1,and then treated with DHM(10μM)and Sal B(50μM)16 hours after transfection;levels ofα-synuclein aggregation decreased significantly(68%for DHM and 75%for Sal B).Concomitantly,we detected increased levels of LAMP-1(a marker of lysosomal homeostasis)and LAMP-2A(a key marker of CMA).Immunofluorescence analyses showed increased colocalization between LAMP-1 and LAMP-2A withα-synuclein inclusions after treatment with DHM and Sal B.We also found increased levels of LAMP-1 and LAMP-2A both in vitro and in vivo,along with decreased levels ofα-synuclein.Moreover,DHM and Sal B treatments exhibited anti-inflammatory activities,preventing astroglia-and microglia-mediated neuroinflammation in BAC-α-syn-GFP transgenic mice.Conclusions:Our data indicate that DHM and Sal B are effective in modulatingα-synuclein accumulation and aggregate formation and augmenting activation of CMA,holding potential for the treatment of Parkinson’s disease. | Jia-Zhen Wu Mustafa Ardah Caroline Haikal Alexander Svanbergsson Meike Diepenbroek Nishant N.Vaikath Wen Li Zhan-You Wang Tiago F.Outeiro Omar M.El-Agnaf Jia-Yi Li | 2019 | Translational Neurodegeneration2019,8,1: | 11 |

| 5 | Effects of polydispersity on the micro–macro behavior of granular assemblies under different deformation paths显示文摘The micromechanical and macromechanical behavior of idealized granular assemblies, made by linearly elastic, frictionless, polydisperse spheres, are studied in a periodic, triaxial box geometry, using the discrete element method. Emphasis is put on the effect of polydispersity under purely isotropic loading and unloading, deviatoric(volume conserving), and uniaxial compression paths.We show that scaled pressure, coordination number and fraction of rattlers behave in a very similar fashion as functions of volume fraction, irrespective of the deformation path applied. Interestingly, they show a systematic dependence on the deformation mode and polydispersity via the respective jamming volume fraction. This confirms that the concept of a single jamming point has to be rephrased to a range of variable jamming points, dependent on microstructure and history of the sample, making the jamming volume fraction a state-variable.This behavior is confirmed when a simplified constitutive model involving structural anisotropy is calibrated using the purely isotropic and deviatoric simulations. The basic model parameters are found to depend on the polydispersity of the sample through the different jamming volume fractions. The predictive power of the calibrated model is checked by comparison with an independent test, namely uniaxial compression. | 10 | Economical effect of lumen apposing metal stents for treating benign foregut strictures显示文摘AIM To evaluate the clinical and economical efficacy of lumen apposing metal stent(LAMS) in the treatment of benign foregut strictures.METHODS A single center retrospective database of patients who underwent endoscopic treatment of benign foregut strictures between January 2014 and May 2017 was analyzed. A control group of non-stented patients who underwent three endoscopic dilations was compared to patients who underwent LAMS placement. Statistical tests performed included independent t-tests and fiveparameter regression analysis RESULTS Nine hundred and ninety-eight foregut endoscopic dilations were performed between January 2014 and May 2017. 15 patients underwent endoscopic LAMS placement for treatment of benign foregut stricture. Thirty-six patients with recurrent benign foregut strictures underwent three or more endoscopic dilations without stent placement. The cost ratio of endoscopic dilation to LAMS(stent, placement and retrieval) is 5.77. Cost effective analysis demonstrated LAMS to be economical after three endoscopic dilation overall.LAMS was cost effective after two dilations in the Postsurgical stricture subgroup. CONCLUSION Endoscopists should consider LAMS for the treatment of benign foregut strictures if symptoms persist past three endoscopic dilations. Post-surgical strictures may benefit from LAMS if symptoms persist after two dilations in a post-surgical. | 12 | Novel biomarkers and the future of targeted therapies in cholangiocarcinoma:a narrative review显示文摘Background and Objectives:Cholangiocarcinoma is a highly aggressive and heterogenous group of biliary malignancies arising from any site in the biliary tree,comprising 15%of all primary liver cancers.The nature of the disease and nonspecific presentation leads to late diagnosis and ultimately poor outcomes for patients.Combination gemcitabine and cisplatin has been the standard of care for cholangiocarcinoma(CCA)since 2010,with a median overall survival of 11.7 months.The five-year survival for CCA remains 5-10%,revealing a clear need for improved treatment options.Methods:This targeted review highlights the role of next generation sequencing in CCA and the clinically relevant tumor biomarkers that have become the focus of therapeutic development.Key Content and Findings:These tumor biomarkers or actionable mutations hold the potential to enable earlier diagnosis,provide prognostic information,and guide treatment decisions for patients with CCA.Specifically,the FGFR2 fusion and IDH1 mutation have shown considerable promise in development of targeted therapies.Clinical trials with inhibitors targeting FGFR2 fusion and IDH1 mutation have created expectations that these drugs will soon enter clinical practice.Other biomarkers including KRAS and B-raf protooncogenes,Her2/neu genes,and BRCA1 and 2 tumor-suppressor genes have also been touted as potential targets for future therapies,with early data showing promise for new drug development.Conclusions:The discovery of these actionable mutations and identification of targeted therapies have challenged the notion of a“one-size fits all”for treatment of CCA,and generated optimism that these novel treatments will soon be available for patients with CCA. | Nishant Munugala Shishir K.Maithel Rachna T.Shroff | 2022 | Hepatobiliary Surgery and Nutrition2022,11,2: | 2 |
