1Petrogenesis of the Neopro-terozoic bimodal volcanic rocks along the western margin of the Yangtze Block:New constraints from Hf isotopes and Fe/Mn ratios显示文摘High-precision major element and Hf isotope data are reported for the Neoproterozoic Suxiong volcanic rocks along the western margin of the Yangtze Block. These volcanic rocks have variable εHf(T) values and Fe/Mn ratios. The relatively primitive basalts have high Fe/Mn ratios and high Hf-Nd isotopic compositions, indicating that they were generated by partial melting of garnet clinopyroxene in mantle plume at high pressure. Thus, the Suxiong basalts are genetically related to the proposed Neoproterozoic super-plume. On the contrary, a few differentiated basalts have low Fe/Mn ratios and low Hf-Nd isotopic compositions. They are likely to experience assimilation-fractional crystallization process. The Suxiong rhyolites have consistent Hf and Nd model ages of 1.3―1.4 Ga. 6Effects of different resuscitation fluid on severe acute pancreatitis显示文摘AIM: To compare effects of different resuscitation fluid on microcirculation, inflammation, intestinal barrier and clinical results in severe acute pancreatitis (SAP). METHODS: One hundred and twenty patients with SAP were enrolled at the Pancreatic Disease Institute between January 2007 and March 2010. The patients were randomly treated with normal saline (NS group), combination of normal saline and hydroxyethyl starch (HES) (SH group), combination of normal saline, hydroxyethyl starch and glutamine (SHG group) in resuscitation. The ratio of normal saline to HES in the SH and SHG groups was 3:1. The glutamine (20% glutamine dipeptide, 100 mL/d) was supplemented into the resuscitation liquid in the SHG group. Complications and outcomes including respiratory and abdominal infection, sepsis, abdominal hemorrhage, intra-abdominal hypertension, abdominal compartment syndrome (ACS), renal failure, acute respiratory distress syndrome (ARDS), multiple organ dysfunction syndrome (MODS), operation intervention, length of intensive care unit stay, length of hospital stay, and mortality at 60 d were compared. Moreover, blood oxygen saturation (SpO 2 ), gastric intramucosal pH value (pHi), intra-abdominal pressure (IAP), inflammation cytokines, urine lactulose/mannitol (L/M) ratio, and serum endotoxin were investigated to evaluate the inflammatory reaction and gut barrier. RESULTS: Compared to the NS group, patients in the SH and SHG groups accessed the endpoint more quickly (3.9 ± 0.23 d and 4.1 ± 0.21 d vs 5.8 ± 0.25 d, P < 0.05) with less fluid volume (67.26 ± 28.53 mL/kg/d, 61.79 ± 27.61 mL/kg per day vs 85.23 ± 21.27 mL/kg per day, P < 0.05). Compared to the NS group, incidence of renal dysfunction, ARDS, MODS and ACS in the SH and SHG groups was obviously lower. Furthermore, incidence of respiratory and abdominal infection was significantly decreased in the SH and SHG groups, while no significant difference in sepsis was seen. Moreover, less operation time was needed in the SH and SHG group than the NS group, but the difference was not significant. The mortality did not differ significantly among these groups. Blood SpO 2 and gastric mucosal pHi in the SH and SHG groups increased more quickly than in the NS group, while IAP was significantly decreased in the SH and SHG group. Moreover, the serum tumor necrosis factor-α, interleukin-8 and C-reactive protein levels in the SH and SHG groups were obviously lower than in the NS group at each time point. 92018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 13Epalrestat protects against diabetic peripheral neuropathy by alleviating oxidative stress and inhibiting polyol pathway显示文摘Epalrestat is a noncompetitive and reversible aldose reductase inhibitor used for the treatment of diabetic neuropathy. This study assumed that epalrestat had a protective effect on diabetic peripheral nerve injury by suppressing the expression of aldose reductase in peripheral nerves of diabetes mellitus rats. The high-fat and high-carbohydrate model rats were established by intraperitoneal injection of streptozotocin. Peripheral neuropathy occurred in these rats after sustaining high blood glucose for 8 weeks. At 12 weeks after streptozotocin injection, rats were intragastrically administered epalrestat 100 mg/kg daily for 6 weeks. Transmission electron microscope revealed that the injuries to myelinated nerve fibers, non-myelinated nerve fibers and Schwann cells of rat sciatic nerves had reduced compared to rats without epalrestat administuation. Western blot assay and immunohistochemical results demonstrated that after intervention with epalrestat, the activities of antioxidant enzymes such as superoxide dismutase, catalase and glutathione peroxidase gradually increased, but aldose reductase protein expression gradually diminished. 14Programmable time-domain digital-coding metasurface for non-linear harmonic manipulation and new wireless communication systems显示文摘Optical non-linear phenomena are typically observed in natural materials interacting with light at high intensities, and they benefit a diverse range of applications from communication to sensing. However,controlling harmonic conversion with high efficiency and flexibility remains a major issue in modern optical and radio-frequency systems. Here, we introduce a dynamic time-domain digital-coding metasurface that enables efficient manipulation of spectral harmonic distribution. By dynamically modulating the local phase of the surface reflectivity, we achieve accurate control of different harmonics in a highly programmable and dynamic fashion, enabling unusual responses, such as velocity illusion. As a relevant application, we propose and realize a novel architecture for wireless communication systems based on the time-domain digital-coding metasurface, which largely simplifies the architecture of modern communication systems, at the same time yielding excellent performance for real-time signal transmission. 17Insomnia is associated with increased mortality in patients with first-ever stroke:a 6-year follow-up in a Chinese cohort study显示文摘Objective Insomnia is a highly prevalent disorder among patients suffering from stroke.The association between insomnia and stroke mortality is less studied,particularly using the latest diagnostic criteria.The current study examined the relationship between insomnia and mortality among patients with first-evonal hazard models were used to calculate HRs for stroke er stroke in China.Methods Patients with acute cerebrovascular diseases(stroke)were recruited from 56 hospitals in China's Mainland.Insomnia was defined as difficulty falling asleep,or difficulty staying asleep or waking up early,for at least two consecutive visits.Demographic data,medical history and clinical data were collected.Four follow-up visits occurred within the first year after stroke,and the last follow-up call was conducted 6 years later.Cox proportional hazard models were used to calculate HRs for stroke mortality.results Insomnia was reported by 38.4%(489/1273)of patients at baseline.During the 6 years of follow-up,after adjusting for all confounders,insomnia was found to be associated with increased mortality(HR=1.66,95%CI 1.10 to 2.48).Old age(HR=1.08,95%CI 1.06 to 1.10),stroke recurrence in the first year of follow-up(HR=2.53,95%CI 1.48 to 4.31)and stroke survivors with hypertension(HR=1.62,95%CI 1.04 to 2.53)had substantially higher risk of mortality.Conclusions Besides old age,stroke recurrence in the first year of follow-up and hypertension,insomnia is associated with increased risk of mortality in patients with first-ever stroke in China.More studies about prompt and efficient interventions for insomnia are expected in the future.Trial registration number rctn62169508.Li-Jun Li Yang Yang Bo-Yuan Guan Qi Chen An-Xin Wang Yong-Jun Wang Ning Zhang Chun-Xue Wang 2018Stroke & Vascular Neurology2018,3,4:38
18China National Medical Products Administration approval summary:anlotinib for the treatment of advanced non-small cell lung cancer after two lines of chemotherapy显示文摘Background:On May 8,2018,the China National Medical Products Administration(NMPA)approved anlotinib,an orally administered anti-angiogenesis inhibitor,for the treatment of patients with advanced non-small cell lung can-cer(NSCLC)who have progressed after treatment with two or more lines of prior systemic chemotherapy.Main body of the abstract:China NMPA reviewed and inspected a regional double-blinded,placebo-controlled,Phase III trial comparing the overall survival(OS)of NSCLC patients between the anlotinib and placebo arms.A total of 437 patients were randomized(2:1)to receive either anlotinib(n=294)or placebo(n=143)once daily on a 2-week on and 1-week off schedule.Patients with epidermal growth factor receptor(EGFR)or activating anaplastic lymphoma kinase(ALK)genomic tumor aberrations should have disease progression on NMPA-approved therapy.Anlotinib is the first NMPA-approved drug for patients with advanced NSCLC who have progressed on at least two lines of prior systemic chemotherapies in China.The approval was based on a statistically and clinically significant improvement in median OS with anlotinib(9.46 months)compared with placebo[6.37 months;hazard ratio(HR])=0.70,95%confidence interval(CI)=0.55-0.89;two-sided log-rank P=0.002].The confirmed objective response rate(ORR)was 9.2%in the anlotinib arm and 0.7%in the placebo arm.The median duration of response(DoR)was 4.83 months,with a 95%CI of 3.31-6.97 months.The toxicity profile of anlotinib was consistent with that of known anti-angiogenesis inhibitors.Common adverse drug reactions(ADRs)in anlotinib-treated patients included hypertension(67.4%),hand-foot syndrome(43.9%),hemoptysis(14.0%),thyroid stimulating hormone(TSH)elevation(46.6%),and corrected QT interval(QTc)prolongation(26.2%).Short conclusion:Anlotinib demonstrated a clinically significant OS prolongation as a novel therapeutic option for advanced or metastatic NSCLC following at least two lines of chemotherapy.Ming Zhou Xiaoyuan Chen Hong Zhang Lin Xia Xin Tong Limin Zou Ruimin Hao Jianhong Pan Xiao Zhao Dongmei Chen Yuanyuan Song Yueli Qi Ling Tang Zhifang Liu Rong Gao Yuankai Shi Zhimin Yang 2019Cancer Communications2019,39,1:35
19Effects of resource additions on species richness and ANPP in an alpine meadow community显示文摘Aims Theories based on resource additions indicate that plant species richness is mainly determined by the number of limiting resources.However,the individual effects of various limiting resources on species richness and aboveground net primary productivity(ANPP)are less well understood.Here,we analyzed potential linkages between additions of limiting resources,species loss and ANPP increase and further explored the underlying mechanisms.Methods Resources(N,P,K and water)were added in a completely randomized block design to alpine meadow plots in the Qinghai-Tibetan Plateau.Plant aboveground biomass,species composition,mean plant height and light availability were measured in each plot.Regression and analysis of variance were used to analyze the responses of these measures to the different resource-addition treatments.Important Findings Species richness decreased with increasing number of added limiting resources,suggesting that plant diversity was apparently determined by the number of limiting resources.Nitrogen was the most important limiting resource affecting species richness,whereas Pand K alone had negligible effects.The largest reduction in species richness occurred when all three elements were added in combination.Water played a different role compared with the other limiting resources.Species richness increased when water was added to the treatments with N and P or with N,P and K.The decreases in species richness after resource additions were paralleled by increases in ANPP and decreases in light penetration into the plant canopy,suggesting that increased light competitionwas responsible for the negative effects of resource additions on plant species richness.Zhengwei Ren Qi Li Chengjin Chu Luqiang Zhao Jieqi Zhang Dexiecuo Ai Yingbo Yang Gang Wang 2010Journal of Plant Ecology2010,3,1:40
20Pathological changes at early stage of multiple organ injury in a rat model of severe acute pancreatitis显示文摘BACKGROUND:Severe acute pancreatitis (SAP) is a commonly seen acute abdominal syndrome characterized by sudden onset,rapid progression and high mortality rate.The damage in peripheral organs may be more severe than that in the pancreas,and can even lead to multiple organ dysfunction.It is critical to recognize early pathological changes in multiple organs.This study aimed to assess the early pathological features of damaged organs in a rat model of SAP.METHODS:Thirty clean grade healthy male Sprague-Dawley rats weighing 250-300 g were randomly divided into a model control group (n=15) and a sham-operated group (n=15).The SAP rat model was induced by sodium taurocholate.Samples of blood and from multiple organs were collected 3 hours after operation.We assessed the levels of IL-6,TNF-α,PLA2,NO,ET-1,MDA,amylases and endotoxin in blood and observed the early pathological changes in multiple damaged organs.RESULTS:Levels of IL-6,TNF-α,PLA2,NO,ET-1 and MDA in serum and of amylase and endotoxin in plasma of the model control group rats were significantly higher than those of the sham-operated group (P<0.01).Different degrees of pathological change were observed in multiple damaged organs.CONCLUSION:Multiple organ injury may occur at the early stage of SAP in rats.Zhang, Xi-Ping Zhang, Jie Ma, Mei-Li Cai, Yang Xu, Ru-Jun Xie, Qi Jiang, Xin-Ge Ye, Qian 2010Hepatobiliary & Pancreatic Diseases International2010,9,1:33
