| 3 | Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China显示文摘Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. | 4 | Wall shear stress in portal vein of cirrhotic patients with portal hypertension显示文摘AIM To investigate wall shear stress(WSS) magnitude and distribution in cirrhotic patients with portal hypertension using computational fluid dynamics. METHODS Idealized portal vein(PV) system models were reconstructed with different angles of the PV-splenic vein(SV) and superior mesenteric vein(SMV)-SV. Patient-specific models were created according to enhanced computed tomography images. WSS was simulated by using a finite-element analyzer, regarding the blood as a Newtonian fluid and the vessel as a rigid wall. Analysis was carried out to compare the WSSin the portal hypertension group with that in healthy controls.RESULTS For the idealized models, WSS in the portal hypertension group(0-10 dyn/cm2) was significantly lower than that in the healthy controls(10-20 dyn/cm2), and low WSS area(0-1 dyn/cm2) only occurred in the left wall of the PV in the portal hypertension group. Different angles of PV-SV and SMV-SV had different effects on the magnitude and distribution of WSS, and low WSS area often occurred in smaller PV-SV angle and larger SMV-SV angle. In the patient-specific models, WSS in the cirrhotic patients with portal hypertension(10.13 ± 1.34 dyn/cm2) was also significantly lower than that in the healthy controls(P < 0.05). Low WSS area often occurred in the junction area of SV and SMV into the PV, in the area of the division of PV into left and right PV, and in the outer wall of the curving SV in the control group. | 8 | A fiber optoacoustic guide with augmented reality for precision breastconserving surgery显示文摘Lumpectomy,also called breast-conserving surgery,has become the standard surgical treatment for early-stage breast cancer.However,accurately locating the tumor during a lumpectomy,especially when the lesion is small and nonpalpable,is a challenge.Such difficulty can lead to either incomplete tumor removal or prolonged surgical time,which result in high re-operation rates(~25%)and increased surgical costs.Here,we report a fiber optoacoustic guide(FOG)with augmented reality(AR)for sub-millimeter tumor localization and intuitive surgical guidance with minimal interference.The FOG is preoperatively implanted in the tumor.Under external pulsed light excitation,the FOG omnidirectionally broadcasts acoustic waves through the optoacoustic effect by a specially designed nano-composite layer at its tip.By capturing the acoustic wave,three ultrasound sensors on the breast skin triangulate the FOG tip’s position with 0.25-mm accuracy.An AR system with a tablet measures the coordinates of the ultrasound sensors and transforms the FOG tip’s position into visual feedback with<1-mm accuracy,thus aiding surgeons in directly visualizing the tumor location and performing fast and accurate tumor removal.We further show the use of a head-mounted display to visualize the same information in the surgeons’first-person view and achieve hands-free guidance.Towards clinical application,a surgeon successfully deployed the FOG to excise a“pseudo tumor”in a female human cadaver.With the high-accuracy tumor localization by FOG and the intuitive surgical guidance by AR,the surgeon performed accurate and fast tumor removal,which will significantly reduce re-operation rates and shorten the surgery time. | Lu Lan Yan Xia Rui Li Kaiming Liu Jieying Mai Jennifer Anne Medley Samilia Obeng-Gyasi Linda K.Han Pu Wang Ji-Xin Cheng | 2018 | Light(Science & Applications)2018,7,1: | 4 |

| 10 | Increased circulating level of interleukin-6 and CD8+T cell exhaustion are associated with progression of COVID-19显示文摘Background: Coronavirus disease 2019(COVID-19)is pandemic.It is critical to identify COVID-19 patients who are most likely to develop a severe disease.This study was designed to determine the clinical and epidemiological features of COVID-19 patients associated with the development of pneumonia and factors associated with disease progression.Methods:: Seventy consecutive patients with etiologically confirmed COVID-19 admitted to PLA General Hospital in Beijing,China from December 27,2019 to March 12,2020 were enrolled in this study and followed-up to March 16,2020.Differences in clinical and laboratory findings between COVID-19 patients with pneumonia and those without were determined by theχ2 test or the Fisher exact test(categorical variables)and independent group t test or Mann–Whitney U test(continuous variables).The Cox proportional hazard model and Generalized Estimating Equations were applied to evaluate factors that predicted the progression of COVID-19.Results: The mean incubation was 8.67(95%confidence interval,6.78–10.56)days.Mean duration from the first test severe acute respiratory syndrome coronavirus 2-positive to conversion was 11.38(9.86–12.90)days.Compared to pneumonia-free patients,pneumonia patients were 16.5 years older and had higher frequencies of having hypertension,fever,and cough and higher circulating levels of neutrophil proportion,interleukin-6,low count(<190/µl)of CD8+T cells,and neutrophil/lymphocyte ratio.Thirteen patients deteriorated during hospitalization.Cox regression analysis indicated that older age and higher serum levels of interleukin-6,C-reactive protein,procalcitonin,and lactate at admission significantly predicted the progression of COVID-19.During hospitalization,circulating counts of T lymphocytes,CD4+T cells,and CD8+T cells were lower,whereas neutrophil proportion,neutrophil/lymphocyte ratio,and the circulating levels of interleukin-6,C-reactive protein,and procalcitonin were higher,in pneumonia patients than in pneumonia-free patients.CD8+lymphocyte count in pneumonia patients did not recover when discharged.Conclusions: Older age and higher levels of C-reactive protein,procalcitionin,interleukin-6,and lactate might predict COVID-19 progression.T lymphocyte,especially CD8+cell-mediated immunity is critical in recovery of COVID-19.This study may help in predicting disease progression and designing immunotherapy for COVID-19. | Peng-Hui Yang Yi-Bo Ding Zhe Xu Rui Pu Ping U Jin Yan Ji-Luo Liu Fan-Ping Meng Lei Huang Lei Shi Tian-Jun Jiang En-Qiang Qin Min Zhao Da-Wei Zhang Peng Zhao Ling-Xiang Yu Zhao-Hai Wang Zhi-Xian Hong Zhao-Hui Xiao Qing Xi De-Xi Zhao Peng Yu Cai-Zhong Zhu Zhu Chen Shao-Geng Zhang Jun-Sheng Ji Fu-Sheng Wang Guang-Wen Cao | 2020 | Infectious Diseases of Poverty2020,9,6: | 3 |
| 11 | Study on dynamic characteristics of fission products in 2 MW molten salt reactor显示文摘In this study,a numerical flow model of the fission products(FPs)in the primary loop system of a molten salt reactor(MSR)was established and solved using Mathematica 7.0.The simulation results were compared with those of the ORIGEN-S program in the static burnup mode,and the deviation was found to be less than 10%,which indicates that the results are in good agreement.Furthermore,the FPs distribution in the primary loop system under normal operating conditions of the 2 MW MSR was quantitatively analyzed.In addition,the distribution phenomenon of the FPs under different flow rate conditions was studied.At the end of life,the FPs activity in the core region(including active region,and upper and lower plenum regions)accounted for 77.3%,and that in the hot leg #1,main pump,hot leg #2,heat exchanger,and cold leg region accounted for 1.2%,16.15%,0.99%,2.5%,and 1.9%,respectively,of the total FPs in the primary loop under normal operating conditions.The proportion of FPs in the core decreased with the increase in flow rate in the range of 2.24-22,400 cm^3 s^-1.The established analytical method and conclusions of this study can provide an important basis for radiation safety design of the primary loop,radioactive source management design,thermal-hydraulic safety analysis,and radiochemical analysis of FPs of 2 MW MSRs. | Bo Zhou Xiao-Han Yu Yang Zou Pu Yang Shi-He Yu Ya-Fen Liu Xu-Zhong Kang Gui-Feng Zhu Rui Yan | 2020 | Nuclear Science and Techniques2020,31,2: | 3 |

| 13 | Micro RNA-548a-5p promotes proliferation and inhibits apoptosis in hepatocellular carcinoma cells by targeting Tg737显示文摘AIM: To investigate whether Tg737 is regulated by micro RNA-548a-5p(mi R-548a-5p), and correlates with hepatocellular carcinoma(HCC) cell proliferation and apoptosis.METHODS: Assays of loss of function of Tg737 were performed by the colony formation assay, CCK assay and cell cycle assay in HCC cell lines. The interaction between mi R-548a-5p and its downstream target, Tg737, was evaluated by a dual-luciferase reporter assay and quantitative real-time polymerase chain reaction. Tg737 was then up-regulated in HCC cells to evaluate its effect on mi R-548a-5p regulation. Hep G2 cells stably overexpressing mi R-548a-5p or mi R-control were also subcutaneously inoculated into nude mice to evaluate the effect of mi R-548a-5p up-regulation on in vivo tumor growth. As the final step, the effect of mi R-548a-5p on the apoptosis induced by cisplatin was evaluated by flow cytometry.RESULTS: Down-regulation of Tg737, which is a target gene of mi R-548a-5p, accelerated HCC cell proliferation, and mi R-548a-5p promoted HCC cell proliferation in vitro and in vivo. Like the downregulation of Tg737, overexpression of mi R-548a-5p in HCC cell lines promoted cell proliferation, increased colony forming ability and hampered cell apoptosis. In addition, mi R-548a-5p overexpression increased HCC cell growth in vivo. Mi R-548a-5p downregulated Tg737 expression through direct contact with its 3' untranslated region(UTR), and mi R-548a-5p expression was negatively correlated with Tg737 levels in HCC specimens. | 14 | IL-37b suppresses epithelial mesenchymal transition in hepatocellular carcinoma by inhibiting IL-6/STAT3 signaling显示文摘Background: Interleukin-37 b(IL-37 b), a vital negative regulator of the innate immune system, has been reported to be a tumor inhibitor in different type of cancers. However, little is known about the relationship between IL-37 b and hepatocellular carcinoma(HCC). The present study aimed to investigate the potential roles of IL-37 b in HCC progression. Methods: Subjects( n = 237) were recruited, and serum IL-37 b was measured using ELISA. The tumorsuppressive capacity and underlying mechanisms of IL-37 b in HCC were investigated in vitro and in vivo. Results: Compared to healthy controls, serum IL-37 b levels were elevated in chronic hepatitis B(CHB) patients but decreased significantly in HBV-HCC patients, especially for those with portal venous tumor thrombus. Low level serum IL-37 b in HBV-HCC patients correlated with high HCC stage and poor overall survival and disease-free survival. In vitro and in vivo, recombinant human IL-37 b inhibited proliferation and metastasis in HCC cells. Furthermore, IL-37 b inhibited epithelial mesenchymal transition in HCC cells in vitro by downregulating IL-6, pSTAT3(Y705), N-cadherin, and vimentin expression and by upregulating E-cadherin expression. These effects were partially reversed by transfection of adenovirus encoding human IL-6. Conclusions: IL-37 b inhibits HCC growth, metastasis and epithelial mesenchymal transition by regulating IL-6/STAT3 signaling. Syngenetic brine sedimentation, hot submarine spring eruption—accumulation, syngenetic sedimentation—vadose brine replacement had been supposed as the genetic models before. Under these models, syngenetic mineralization should be dominant in ore formation. However, a series of geological and geochemical properties investigated by authors of this paper are contrary to these models. | 17 | Study on neutronics design of ordered-pebble-bed fluoride-salt- cooled high-temperature experimental reactor显示文摘This paper presents a neutronics design of a 10 MW ordered-pebble-bed fluoride-salt-cooled high-temperature experimental reactor. Through delicate layout, a core with ordered arranged pebble bed can be formed,which can keep core stability and meet the space requirements for thermal hydraulics and neutronics measurements.Overall, objectives of the core include inherent safety and sufficient excess reactivity providing 120 effective full power days for experiments. Considering the requirements above, the reactive control system is designed to consist of 16 control rods distributed in the graphite reflector. Combining the large control rods worth about 18000–20000 pcm, molten salt drain supplementary means(-6980 to -3651 pcm) and negative temperature coefficient(-6.32 to -3.80 pcm/K) feedback of the whole core, the reactor can realize sufficient shutdown margin and safety under steady state. Besides, some main physical properties, such as reactivity control, neutron spectrum and flux, power density distribution, and reactivity coefficient,have been calculated and analyzed in this study. | 19 | Determinants of self-management behaviors among pulmonary tuberculosis patients: a path analysis显示文摘Background Tuberculosis(TB)is one of the top 10 causes of death in the world.Since Directly Observed Therapy(DOT)as a core strategy for the global TB control are not applicable to all types of TB patients,and self-management of TB patients(SMTP)as a patient-centered supervision type is a supplement to DOT and can improve TB case management.However,the factors related to SMTP are complex and need more study.This study aimed at identifying the determinants of SMTP and examining the direct/indirect effects of these determinants.Methods The purposive sampling technique was used to select study sites and participants were recruited from the study sites by the consecutive sampling method.The PRECEDE model was used as the framework to analyze the determinants of SMTP.The responses of TB patients were acquired via a questionnaire survey for data collection.A Pearson correlation analysis was used to define the relationship between the predisposing,enabling,reinforcing factors with SMTP behaviors.A regression-based path analysis was used to determine the action paths of the predisposing,enabling,and reinforcing factors on SMTP behaviors.Results The predisposing(TB knowledge),enabling[health education and healthcare workers(HCWs)support],reinforcing factors(family support)had significant positive correlations with SMTP behaviors(P<0.05).The predisposing,enabling,reinforcing factors were positively correlated with each other(r=0.123‒0.918,P<0.05),except for family support and HCWs support.The predisposing factors(TB knowledge,β=0.330)and the enabling factors(HCWs support,β=0.437)had direct effects on SMTP behaviors.The enabling factors(health education and HCWs support)and the reinforcing factors(family support)had indirect effects on SMTP behaviors.Conclusions This study revealed the effects and action path of TB knowledge,health education,HCWs support,and family support on SMTP behaviors via a path analysis.Assessing patient’s needs for SMTP along with promoting effective TB health education and providing firm support from HCWs and family members are potential strategies to promote SMTP behaviors. | Jin Li Jie Pu Jiaqing Liu Qingya Wang Rui Zhang Ting Zhang Jiani Zhou Wei Xing Shengxiang Liang Daiyu Hu Ying Li | 2021 | Infectious Diseases of Poverty2021,10,4: | 2 |
