In final, the Chinese consensus guidelines for the diagnosis and management of GIST(2017 edition) was formed on the basis of 2013 edition consensus guidelines, which is hereby announced. | 2 | Generalized non-linear strength theory and transformed stress space显示文摘Based on the test data of frictional materials and previous research achievements in this field, a generalized non-linear strength theory (GNST) is proposed. It describes non-linear strength properties on the π-plane and the meridian plane using a unified formula, and it includes almost all the present non-linear strength theories, which can be used in just one material. The shape of failure function of the GNST is a smooth curve between the SMP criterion and the Mises criterion on the π-plane, and an exponential curve on the meridian plane. | 3 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. | 4 | Tom20 senses iron-activated ROS signaling to promote melanoma cell pyroptosis显示文摘Iron has been shown to trigger oxidative stress by elevating reactive oxygen species (ROS) and to participate in different modes of cell death,such as ferroptosis,apoptosis and necroptosis. However,whether iron-elevated ROS is also linked to pyroptosis has not been reported. Here,we demonstrate that iron-activated ROS can induce pyroptosis via a Tom20-Bax-caspase-GSDME pathway. In melanoma cells,iron enhanced ROS signaling initiated by CCCP,causing the oxidation and oligomerization of the mitochondrial outer membrane protein Tom20. Bax is recruited to mitochondria by oxidized Tom20,which facilitates cytochrome c release to cytosol to activate caspase-3;eventually triggering pyroptotic death by inducing GSDME cleavage. Therefore,ROS acts as a causative factor and Tom20 senses ROS signaling for iron-driven pyroptotic death of melanoma cells. Since iron activates ROS for GSDME-dependent pyroptosis induction and melanoma cells specifically express a high level of GSDME,iron may be a potential candidate for melanoma therapy. Based on the functional mechanism of iron shown above,we further demonstrate that iron supplementation at a dosage used in iron-deficient patients is sufficient to maximize the anti-tumor effect of clinical ROS-inducing drugs to inhibit xenograft tumor growth and metastasis of melanoma cells through GSDME-dependent pyroptosis. Moreover,no obvious side effects are observed in the normal tissues and organs of mice during the combined treatment of clinical drugs and iron. | 7 | Chinese deserts and sand fields in Last Glacial Maximum and Holocene Optimum显示文摘The Last Glacial Maximum (LGM, c. 26-16 ka) and the Holocene Optimum (HO, c. 9-5 ka) were characterized by cold-dry and warm-wet climates respectively in the recently geological Earth. How Chinese deserts and sand fields responded to these distinctive climatic changes is still not clear, however. To reconstruct environments of the deserts and sand fields during the LGM and HO is helpful to understand the forcing mechanisms of environment change in this arid region, and to test paleoclimatic modeling results. Through our long-term field and laboratory investigations, 400 optically stimulated luminescence (OSL) ages and more than 100 depositional records in the Chinese deserts and sand fields were obtained; on the basis of these data, we reconstruct spatial distributions of the deserts and sand fields during the LGM and HO. Our results show that the sand fields of Mu Us, Hunshandake, Horqin and Hulun Buir in northern and northeastern China had expanded 25%, 37%, 38% and 270%, respectively, during the LGM; the sand fields of Gonghe in the northeastern Qinghai-Tibetan Plateau had expanded 20%, and the deserts of Badain Jaran, Tengger in central northern China had expanded 39% and 29% separately during the LGM; the deserts of Taklimakan, Gurbantünggüt and Kumtag in northwestern China had expanded 10%-20% respectively, compared to their modern areas. On the other hand, all of the sand fields were nearly completely covered by vegetation during the HO; the deserts in northwestern and central northern China were reduced by around 5%-20% in area during this time. Lakes in this arid region were probably expanded during the HO but this conclusion needs more investigation. Compared with the geological distributions of deserts and sand fields, human activity has clearly changed (expanded) the area of active sand dunes at the present time. | 8 | 42,573 cases of hepatectomy in China: a multicenter retrospective investigation显示文摘Hepatectomy is currently routinely performed in most hospitals in China. China owns the largest population of liver diseases and the biggest number of liver resection cases. A nationwide multicenter retrospective investigation involving 112 hospitals was performed, and focused on liver resection for patients with hepatocellular carcinoma(HCC). 42,573 cases of hepatectomy were enrolled, and 18,275 valid cases of liver resection for HCC patients were selected for statistical analysis. The epidemiology of HCC, distribution of hepatectomy, postoperative complications and prognosis were finally analyzed. In the 18,275 HCC patients,81% had hepatitis B virus infection and 10% had hepatitis C virus infection. 38% of the HCC patients had normal Alphafetoprotein(AFP) level, and other 35% had an AFP level lower than 400 ng mL^(-1). In the study period, 97% of the hepatectomy for HCC were treated with open surgery, and 23.81% had vascular exclusion techniques. The operation time was(191.7±105.6) min,the blood loss was(546.0±562.8) m L, and blood transfusion was(543.0±1,035.2) m L. The median survival for HCC patients was 631 days, with 1-, 3-, and 5-year overall survival of 73.2%, 28.8% and 19.6%, respectively. | 10 | A comparison of the Medieval Warm Period, Little Ice Age and 20th century warming simulated by the FGOALS climate system model显示文摘To compare differences among the Medieval Warm Period (MWP), Little Ice Age (LIA), and 20th century global warming (20CW), six sets of transient and equilibrium simulations were generated using the climate system model FGOALS_gl. This model was developed by the State Key Laboratory of Numerical Modeling for Atmospheric Sciences and Geophysical Fluid Dynamics, Institute of Atmospheric Physics, Chinese Academy of Sciences. The results indicate that MWP warming is evident on a global scale, except for at mid-latitudes of the North Pacific. However, the magnitude of the warming is weaker than that in the 20th century. The warming in the high latitudes of the Northern Hemisphere is stronger than that in the Southern Hemisphere. The LIA cooling is also evident on a global scale, with a strong cooling over the high Eurasian continent, while the cooling center is over the Arctic domain. Both the MWP and the 20CW experiments exhibit the strongest warming anomalies in the middle troposphere around 200-300 hPa, but the cooling center of the LIA experiment is seen in the polar surface of the Northern Hemisphere. A comparison of model simulation against the reconstruction indicates that model's performance in simulating the surface air temperature changes during the warm periods is better than that during the cold periods. The consistencies between model and reconstruction in lower latitudes are better than those in high latitudes. Comparison of the inter-annual variability mode of East Asian summer monsoon (EASM) rainfall during the MWP, LIA and 20CW reveals a similar rainfall anomalies pattern. However, the time spectra of the principal component during the three typical periods of the last millennium are different, and the quasi-biannual oscillation is more evident during the two warm periods. At a centennial time scale, the external mode of the EASM variability driven by the changes of effective solar radiation is determined by the changes of large scale land-sea thermal contrast. | 11 | Baicalin prevents LPS-induced activation of TLR4/NF-κB p65 pathway and inflammation in mice via inhibiting the expression of CD14显示文摘Previous studies have shown that baicalin,an active ingredient of the Chinese traditional medicine Huangqin,attenuates LPS-induced inflammation by inhibiting the activation of TLR4/NF-κBp65 pathway,but how it affects this pathway is unknown.It has been shown that CD14 binds directly to LPS and plays an important role in sensitizing the cells to minute quantities of LPS via chaperoning LPS molecules to the TLR4/MD-2 signaling complex.In the present study we investigated the role of CD14 in the anti-inflammatory effects of baicalin in vitro and in vivo.Exposure to LPS(1μg/mL)induced inflammatory responses in RAW264.7 cells,evidenced by marked increases in the expression of MHC II molecules and the secretion of NO and IL-6,and by activation of MyD88/NF-κB p65 signaling pathway,as well as the expression of CD14 and TLR4.These changes were dose-dependently attenuated by pretreatment baicalin(12.5–50μM),but not by baicalin post-treatment.In RAW264.7 cells without LPS stimulation,baicalin dose-dependently inhibit the protein and mRNA expression of CD14,but not TLR4.In RAW264.7 cells with CD14 knockdown,baicalin pretreatment did not prevent inflammatory responses and activation of MyD88/NF-κB p65 pathway induced by high concentrations(1000μg/mL)of LPS.Furthermore,baicalin pretreatment also inhibited the expression of CD14 and activation of MyD88/NF-κB p65 pathway in LPS-induced hepatocyte-derived HepG2 cells and intestinal epithelial-derived HT-29 cells.In mice with intraperitoneal injection of LPS and in DSS-induced UC mice,oral administration of baicalin exerted protective effects by inhibition of CD14 expression and inflammation.Taken together,we demonstrate that baicalin pretreatment prevents LPS-induced inflammation in RAW264.7 cells in CD14-dependent manner.This study supports the therapeutic use of baicalin in preventing the progression of LPS-induced inflammatory diseases. | Ya-jun Fu Bo Xu Shao-wei Huang Xia Luo Xiang-liang Deng Shuang Luo Chang Liu Qing Wang Jin-yan Chen Lian Zhou | 2021 | Acta Pharmacologica Sinica2021,42,1: | 31 |
| 12 | The characteristics of atmospheric CO_2 concentration variation of four national background stations in China显示文摘Atmospheric carbon dioxide is an important kind of greenhouse gas which influences global temperature. Its concentration variation could indicate the distribution of human and natural activities in various regions. Through the non-dispersive infrared method, flask sampling of atmospheric CO2 concen- tration was measured weekly at four national background stations including Waliguan, Shangdianzi, Lin’an, and Longfengshan. Based on the data collected from September 2006 to August 2007, along with the Waliguan station’s experience on in situ observational data processing, the selection methods for sampling data through the atmospheric background CO2 concentration analysis were preliminarily discussed. On the basis of this result, the variation features of the four typical regions’ atmospheric background CO2 concentration was analyzed for the first time. The results show that the atmospheric CO2 concentration at Waliguan, Shangdianzi, Lin’an, and Longfengshan is 383.5, 385.9, 387.8, and 384.3 ppm, respectively. During the research period, CO2 concentration at the Waliguan station changed slightly. However, the CO2 concentration changed sharply at the Shangdianzi and the Lin’an stations due to the great influence of human activities in the Jingjinji and the Changjiang Delta economic zones, and changed regularly with seasons at Longfengshan station under dual influences of human activities and plant photosynthesis. | 13 | Three-dimensional finite element analysis of weakened roots restored with different cements in combination with titanium alloy posts显示文摘Background It is very difficult and relatively unpredictable to preserve and restore severely weakened pulpless roots. To provide much needed benefit basis for clinical practice, this study was carried out to analyze the stress distribution in weakened roots restored with different cements in combination with titanium alloy posts. Finite element analysis (FEA) was employed in the study. Methods A pseudo three-dimensional model of a maxillary central incisor with flared root canal, theoretically restored with titanium alloy posts in combination with different cements, was established. The analysis was performed by use of ANSYS software. The tooth was assumed to be isotropic, homogenous and elastic. A load of 100 N at an angle of 45°to the longitudinal axis was applied at the palatal surface of the crown. The distributions of stresses in weakened roots filled with cements of different elastic modulus were analyzed by the three-dimensional FEA model. Results Several stress trends were observed when the stress cloud atlas obtained in the study was analyzed. With the increase of the elastic modulus of cements from 1.8 GPa to 22.4 GPa, the stress values in dentin decreased from 39.58 MPa to 31.43 MPa and from 24.51 MPa to 20.76 MPa (respectively, for maximum principle stress values and Von Mises stress values). When Panavia F and zinc phosphate cement were used, the stress peak values in dentin were very small with no significant difference observed, and the Von Mises stress values were 20.87 MPa and 20.76 MPa respectively. On the other hand, maximum principle stress value and Von Mises stress value in cement layer increased with the increase of the elastic modulus of cements. Conclusions The result of this study demonstrated that elastic modulus was indeed one of the important parameters to evaluate property of the cements. Our three-dimensional FEA model study also found that the cement with elastic modulus similar to that of dentin could reinforce weakened root and reduce the stress in dentin. | 15 | Theories on Formation of an Anomalous Anticyclone in Western North Pacific during El Nino:A Review显示文摘The western North Pacific anomalous anticyclone(WNPAC) is an important atmospheric circulation system that conveys El Ni?o impact on East Asian climate. In this review paper, various theories on the formation and maintenance of the WNPAC, including warm pool atmosphere–ocean interaction, Indian Ocean capacitor, a combination mode that emphasizes nonlinear interaction between ENSO and annual cycle, moist enthalpy advection/Rossby wave modulation, and central Pacific SST forcing, are discussed. It is concluded that local atmosphere–ocean interaction and moist enthalpy advection/Rossby wave modulation mechanisms are essential for the initial development and maintenance of the WNPAC during El Ni?o mature winter and subsequent spring. The Indian Ocean capacitor mechanism does not contribute to the earlier development but helps maintain the WNPAC in El Ni?o decaying summer.The cold SST anomaly in the western North Pacific, although damped in the summer, also plays a role. An interbasin atmosphere–ocean interaction across the Indo-Pacific warm pool emerges as a new mechanism in summer. In addition, the central Pacific cold SST anomaly may induce the WNPAC during rapid El Ni?o decaying/La Ni?a developing or La Ni?a persisting summer. The near-annual periods predicted by the combination mode theory are hardly detected from observations and thus do not contribute to the formation of the WNPAC. | 18 | Intracoronary arterial retrograde thrombolysis with percutaneous coronary intervention: a novel use of thrombolytic to treat acute ST-segment elevation myocardial infarction显示文摘Background Clearance of coronary arterial thrombosis is necessary in patients with acute ST-segment elevation myocardial infarction (STEMI) undergoing urgent percutaneous coronary intervention (PCI). There is currently no highly-recommended method of thrombus removal during interventional procedures. We describe a new method for opening culprit vessels to treat STEMI: intracoronary arterial retrograde thrombolysis (ICART) with PCI. Methods & Results Eight patients underwent ICART. The guidewire was advanced to the distal coronary artery through the occlusion lesion. Then, we inserted a microcatheter into the distal end of the occluded coronary artery over the guidewire. Urokinase (5–10 wu) mixed with contrast agents was slowly injected into the occluded section of the coronary artery through the microcatheter. The intracoronary thrombus gradually dissolved in 3–17 min, and the effect of thrombolysis was visible in real time. Stents were then implanted according to the characteristics of the recanalized culprit lesion to achieve full revascularization. One patient experienced premature ventricular contraction during vascular revascularization, and no malignant arrhythmias were seen in any patient. No reflow or slow flow was not observed post PCI. Thrombolysis in myocardial infarction flow grade and myocardial blush grade post-primary PCI was 3 in all eight patients. No patients experienced bleeding or stroke. Conclusions ICART was accurate and effective for treating intracoronary thrombi in patients with STEMI in this preliminary study. ICART was an effective, feasible, and simple approach to the management of STEMI, and no intraprocedural complications occurred in any of the patients. | 19 | Acute cerebrovascular disease following COVID-19: a single center, retrospective, observational study显示文摘Background and purpose COVID-19 is an infectious disease caused by Severe Acute Respiratory Syndrome Coronavirus 2(SARS-CoV-2).Apart from respiratory complications,acute cerebrovascular disease(CVD)has been observed in some patients with COVID-19.Therefore,we described the clinical characteristics,laboratory features,treatment and outcomes of CVD complicating SARS-CoV-2 infection.Materials and methods Demographic and clinical characteristics,laboratory findings,treatments and clinical outcomes were collected and analysed.Clinical characteristics and laboratory findings of patients with COVID-19 with or without new-onset CVD were compared.Results Of 219 patients with COVID-19,10(4.6%)developed acute ischaemic stroke and 1(0.5%)had intracerebral haemorrhage.COVID-19 with new onset of CVD were significantly older(75.7±10.8 years vs 52.1±15.3 years,p<0.001),more likely to present with severe COVID-19(81.8%vs 39.9%,p<0.01)and were more likely to have cardiovascular risk factors,including hypertension,diabetes and medical history of CVD(all p<0.05).In addition,they were more likely to have increased inflammatory response and hypercoagulable state as reflected in C reactive protein(51.1(1.3-127.9)vs 12.1(0.1-212.0)mg/L,p<0.05)and D-dimer(6.9(0.3-20.0)vs 0.5(0.1-20.0)mg/L,p<0.001).Of 10 patients with ischemic stroke;6 received antiplatelet treatment with aspirin or clopidogrel;and 3 of them died.The other four patients received anticoagulant treatment with enoxaparin and 2 of them died.As of 24 March 2020,six patients with CVD died(54.5%).Conclusion Acute CVD is not uncommon in COVID-19.Our findings suggest that older patients with risk factors are more likely to develop CVD.The development of CVD is an important negative prognostic factor which requires further study to identify optimal management strategy to combat the COVID-19 outbreak. | Yanan Li Man Li Mengdie Wang Yifan Zhou Jiang Chang Ying Xian David Wang Ling Mao Huijuan Jin Bo Hu | 2020 | Stroke & Vascular Neurology2020,5,3: | 22 |
| 20 | Cardiotoxicity of 5-fluorouracil and capecitabine in Chinese patients: a prospective study显示文摘Background:5-Fluorouracil(5-FU)and capecitabine-associated cardiotoxicity ranging from asymptomatic electro-cardiography(ECG)abnormalities to severe myocardial infarction has been reported in a number of studies,but such cardiotoxicity in Chinese patients with malignant diseases has not been investigated to date.In the present study,we aimed to prospectively evaluate the incidence rates and clinical manifestations of 5-FU-and capecitabine-associated cardiotoxicity in cancer patients recruited from multiple centers in China.Methods:Among the 527 patients who completed the study,196 received 5-FU-based chemotherapy and 331 received capecitabine-based chemotherapy as either first-line or adjuvant therapy.Adverse events were reported during the treatment and up to 28 days of follow-up.Outcome measures included ECG,myocardial enzymes,cardiac troponin,brain natriuretic peptide and echocardiography.Univariate analysis and logistic regression were performed for subgroup analysis and identification of significant independent variables that are associated with cardiotoxicity of both agents.Results:In total,161 of 527 patients(30.6%)experienced cardiotoxicity.The incidence rate of cardiotoxicity was 33.8%(112/331)in the capecitabine group,which was significantly higher than the rate of 25%(49/196)in the 5-FU group(P=0.0042).110/527 patients(20.9%)suffered arrhythmia,105/527(19.9%)developed ischemic changes,while only 20/527 patients(3.8%)presented heart failure and 6/527 patients(1.1%)had myocardial infarction.Pre-existing cardiac disease,hypertension,capecitabine-based chemotherapy and duration of treatment were identified as sig-nificant risk factors associated with cardiotoxicity.The odds ratio were 15.7(prior history of cardiac disease versus no history),1.86(capecitabine versus 5-FU),1.06(5-8 versus 1-4 chemotherapy cycles)and 1.58(hypertension versus no hypertension),respectively.Conclusions:Cardiotoxicity induced by fluoropyrimidines in the Chinese population may be underestimated in clini-cal practice.Close monitoring of patients is recommended,especially for those patients at high risk for cardiotoxicity.Possible risk factors are duration of treatment,capecitabine-based chemotherapy,pre-existing cardiac diseases and hypertension.Trial registration This study was initiated on January 22,2014 and has been retrospectively registered with the registra-tion number ChiCTR1800015434 . | Jianjun Peng Chao Dong Chang Wang Weihua Li Hao Yu Min Zhang Qun Zhao Bo Zhu Jun Zhang Wenliang Li Fenghua Wang Qiong Wu Wenhao Zhou Ying Yuan Meng Qiu Gong Chen | 2018 | Cancer Communications2018,38,1: | 23 |
