| 1 | Chemical composition,antioxidant and antibacterial activities of essential oil and methanol extract of Artemisia vulgaris and Gaultheria fragrantissima collected from Nepal显示文摘Objective: To identify the chemical constituents and biological activities of essential oil and crude methanol extract of Artemisia vulgaris(A.vulgaris) and Gaultheria fragrantissima(G.fragrantissima).Methods: Phytochemical screening, total phenolic and flavonoid content, antibacterial activities, anti-oxidant assay of the crude extract were carried out to identify the biological activities and phytonutrients present in the extract.Furthermore, the chemical constituents present in the essential oil and crude methanol extract were analyzed using gas chromatography mass spectroscopy and high performance liquid chromatography(HPLC) analysis.Results: Gas chromatography mass spectroscopy analysis of essential oil from the aerial part of A.vulgaris revealed 24 different compounds in it.Sabinene(11.29%), b-thujone(19.19%), chrysanthenone(4.48%), camphor(11.89%), borneol(4.44%) and germacrene D(8.42%) were the major compounds.Similarly, leaves of G.fragrantissima contained methyl salicylate(95%) and asarone(4.64%).Furthermore, methanol extract of leaves of A.vulgaris and G.fragrantissima were found rich in the total flavonoids and phenolic content.HPLC analysis of the methanol extract of leaves A.vulgaris revealed the presence of morin and luteolin, whereas rutin was found as a major flavonoids compound in the leaves of G.fragrantissima.Further, methanol extract of the A.vulgaris and G.fragrantissima showed the highest antioxidant and antibacterial properties compared to the essential oil.Conclusions: The HPLC analysis of the methanol extract of A.vulgaris shows the presence of luteolin and morin, whereas G.fragrantissima reveals the presence of rutin and a glycosylated flavonoids.Results reveal that A.vulgaris oil is the rich source of monoterpene and sesquiterpene compounds.Furthermore, A.vulgaris and G.fragrantissima are the rich source of the phenolic and flavonoids compounds and show good antioxidant and antibacterial activity. | Bishnu Prasad Pandey Rupak Thapa Anil Upreti | 2017 | Asian Pacific Journal of Tropical Medicine2017,10,10: | 8 |
| 2 | Combined functional and anatomical diagnostic endpoints for assessing arteriovenous fistula dysfunction显示文摘Failure of arteriovenous fistulas(AVF) to mature and thrombosis in matured fistulas have been the major causes of morbidity and mortality in hemodialysis patients. Stenosis, which occurs due to adverse remodeling in AVFs, is one of the major underlying factors under both scenarios. Early diagnosis of a stenosis in an AVF can provide an opportunity to intervene in a timelymanner for either assisting the maturation process or avoiding the thrombosis. The goal of surveillance strategies was to supplement the clinical evaluation(i.e., physical examination) of the AVF for better and earlier diagnosis of a developing stenosis. Surveillance strategies were mainly based on measurement of functional hemodynamic endpoints, including blood flow(Q a) to the vascular access and venous access pressure(VAP). As the changes in arterial pressure(MAP) affects the level of VAP, the ratio of VAP to MAP(VAPR = VAP/MAP) was used for diagnosis. A Q a < 400-500 m L/min or a VAPR > 0.55 is considered sign of significant stenosis, which requires immediate intervention. However, due to the complex nature of AVFs, the surveillance strategies have failed to consistently detect stenosis under different scenarios. VAPR has been primarily developed to detect outflow stenosis in arteriovenous grafts, and it hasn't been successful in accurate diagnosis of outflow lesions in AVFs. Similarly, AVFs can maintain relatively high blood flow despite the presence of a significant outflow stenosis and thus, Q a has been found to be a better predictor of only inflow lesions. Similar shortcomings have been reported in the detection of functional severity of coronary stenosis using diagnostic endpoints that were based on either flow or pressure. This limitation has been associated with the fact that both pressure and flow change in the presence of a stenosis and thus, hemodynamic diagnostic endpoints that employ only one of these parameters are inherently prone to inaccuracies. Recent attempts have resulted in development of new diagnostic endpoints that can combine the effects of pressure and flow. These new hemodynamic diagnostic endpoints have shown to be better predictors of functional severity of lesions as compared to either flow or pressure based counterparts. | 3 | Observed changes in surface air temperature and precipitation in the Hindu Kush Himalayan region over the last 100-plus years显示文摘In this paper, we analyzed the long-term changes in temperature and precipitation in the Hindu Kush Himalayan (HKH) region based on climate datasets LSAT-V1.1 and CGP1.0 recently developed by the China Meteorological Administration. The analysis results show that during 1901e2014 the annual mean surface air temperature over the whole HKH has undergone a significant increasing trend. We determined the change rates in the mean temperature, mean maximum temperature, and mean minimum temperature to be 0.104 C per decade, 0.077 C per decade, and 0.176 C per decade, respectively. Most parts of the HKH have experienced a warming trend, with the largest increase occurring on the Tibetan Plateau (TP) and south of Pakistan. The trend of precipitation for the whole HKH is characterized by a slight decrease during 1901e2014. During 1961e2013, however, the trend of the annual precipitation shows a statistically significant increase, with a rate of 5.28% per decade and has a more rapid increase since the mid-1980s. Most parts of northern India and the northern TP have experienced a strong increase in the number of precipitation days (daily rainfall 1 mm), whereas Southwest China and Myanmar have experienced a declining trend in precipitation days. | 4 | Changes in extreme temperature events over the Hindu Kush Himalaya during 1961e2015显示文摘This study uses the CMA (China Meteorological Administration) global land-surface daily air temperature dataset V1.0 (GLSATD V1.0) to analyze long-term changes in extreme temperature events over the Hindu Kush Himalaya (HKH) during 1961e2015. Results show there was a significant decrease in the number of extreme cold events (cold nights, cold days, and frost days) but a significant increase in the number of extreme warm events (warm nights, warm days, and summer days) over the entire HKH during 1961e2015. For percentile-based indices, trends of extreme events related to minimum temperature (Tmin) were greater in magnitude than those related to maximum temperature (Tmax). For absolute-value based indices, maximum Tmax, minimum Tmin, and summer days all show increasing trends, while frost days and the diurnal temperature range (DTR) show significant decreasing trends. In addition, there was a decrease in extreme cold events in most parts of east HKH, particularly in Southwest China and the Tibetan Plateau, while there was a general increase in extreme warm events over the entire HKH. Finally, the change in extreme cold events in the HKH appears to be more sensitive to elevation (with cold nights and cold days decreasing with elevation), whereas the change in warm extremes (warm nights, warm days, and maximum Tmax) shows no detectable relationship with elevation. | 6 | Impact of gout on in-hospital outcomes of acute coronary syndrome-related hospitalizations and revascularizations: Insights from the national inpatient sample显示文摘BACKGROUND Previous studies have established a role of gout in predicting risk and prognosis of cardiovascular diseases. However, large-scale data on the impact of gout on inpatient outcomes of acute coronary syndrome (ACS)-related hospitalizations and post-revascularization is inadequate. AIM To evaluate the impact of gout on in-hospital outcomes of ACS hospitalizations, subsequent healthcare burden and predictors of post-revascularization inpatient mortality. METHODS We used the national inpatient sample (2010-2014) to identify the ACS and goutrelated hospitalizations, relevant comorbidities, revascularization and postrevascularization outcomes using the ICD-9 CM codes. A multivariable analysis was performed to evaluate the predictors of post-revascularization in-hospital mortality. RESULTS We identified 3144744 ACS-related hospitalizations, of which 105198 (3.35%) also had gout. The ACS-gout cohort were more often older white males with a higher prevalence of comorbidities. Coronary artery bypass grafting was required more often in the ACS-gout cohort. Post-revascularization complications including cardiac (3.2% vs 2.9%), respiratory (3.5% vs 2.9%), and hemorrhage (3.1% vs 2.7%) were higher whereas all-cause mortality was lower (2.2% vs 3.0%) in the ACSgout cohort (P < 0.001). An older age (OR 15.63, CI: 5.51-44.39), non-elective admissions (OR 2.00, CI: 1.44-2.79), lower household income (OR 1.44, CI: 1.17- 1.78), and comorbid conditions predicted higher mortality in ACS-gout cohort undergoing revascularization (P < 0.001). Odds of post-revascularization inhospital mortality were lower in Hispanics (OR 0.45, CI: 0.31-0.67) and Asians (OR 0.65, CI: 0.45-0.94) as compared to white (P < 0.001). However, postoperative complications significantly raised mortality odds. Mean length of stay, transfer to other facilities, and hospital charges were higher in the ACS-gout cohort. | 7 | Changes in extreme precipitation events over the Hindu Kush Himalayan region during 1961e2012显示文摘Based on a new multi-source dataset (GLDP-V1.0) recently developed in China Meteorological Administration, we employed precipitation indices including percentile-based indices of light (below the 50th percentile), moderate (between the 50th and 90th percentile), and intense (above the 90th percentile) precipitation, maximum 1-day, 3-day, and 5-day precipitation amounts (RX1DAY, RX3DAY, and RX5DAY, respectively), and consecutive wet and dry days (CWDs and CDDs) to analyze variations in extreme precipitation events in the Hindu Kush Himalayan (HKH) during 1961e2012. The main results are presented as follows. Firstly, there was a significant increase in the amount of light and moderate precipitation and number of associated days over various parts of India and northern Tibetan Plateau during 1961e2012; but the intensity of light precipitation decreased significantly in the Hindu Kush and central India, and the regional average intensity also decreased. Secondly, the amount and frequency of intense precipitation mostly increased significantly on the Tibetan Plateau, but there was a heterogeneous change over the remainder of the HKH, and regional average annual intense precipitation amount and frequency significantly increased over the HKH during 1961e2012. | 8 | Downscaled climate change projections for the Hindu Kush Himalayan region using CORDEX South Asia regional climate models显示文摘This study assessed the regional climate models (RCMs) employed in the Coordinated Regional climate Downscaling Experiment (CORDEX) South Asia framework to investigate the qualitative aspects of future change in seasonal mean near surface air temperature and precipitation over the Hindu Kush Himalayan (HKH) region. These RCMs downscaled a subset of atmosphere ocean coupled global climate models (AOGCMs) in the Coupled Model Intercomparison Project phase 5 (CMIP5) to higher 50 km spatial resolution over a large domain covering South Asia for two representation concentration pathways (RCP4.5 and RCP8.5) future scenarios. The analysis specifically examined and evaluated multi-model and multi-scenario climate change projections over the hilly sub-regions within HKH for the near-future (2036e2065) and far-future (2066e2095) periods. The downscaled multi-RCMs provide relatively better confidence than their driving AOGCMs in projecting the magnitude of seasonal warming for the hilly sub-region within the Karakoram and northwestern Himalaya, with higher projected change of 5.4 C during winter than of 4.9 C during summer monsoon season by the end of 21st century under the high-end emissions (RCP8.5) scenario. There is less agreement among these RCMs on the magnitude of the projected warming over the other sub-regions within HKH for both seasons, particularly associated with higher RCM uncertainty for the hilly sub-region within the central Himalaya. The downscaled multi-RCMs show good consensus and low RCM uncertainty in projecting that the summer monsoon precipitation will intensify by about 22% in the hilly subregion within the southeastern Himalaya and Tibetan Plateau for the far-future period under the RCP8.5 scenario. | 9 | Clinical outcomes of combined flow-pressure drop measurements using newly developed diagnostic endpoint:Pressure drop coefficient in patients with coronary artery dysfunction显示文摘AIM:To combine pressure and flow parameter, pressure drop coefficient(CDP) will result in better clinical outcomes in comparison to the fractional flow reserve(FFR) group. METHODS:To test this hypothesis, a comparison was made between the FFR < 0.75 and CDP > 27.9 groups in this study, for the major adverse cardiac events [major adverse cardiac events(MACE): Primary outcome] and patients’ quality of life(secondary outcome). Further, a comparison was also made between the survival curves for the FFR < 0.75 and CDP > 27.9 groups. Two-tailed χ~2 test proportions were performed for the comparison of primary and secondary outcomes. Kaplan-Meier survival analysis was performed to compare the survival curves of FFR < 0.75 and CDP > 27.9 groups(MedcalcV10.2, Mariakerke, Belgium). Results were considered statistically significant for P < 0.05. RESULTS: The primary outcomes(%MACE) in the FFR < 0.75 group(20%, 4 out of 20) was not statistically different(P = 0.24) from the %MACE occurring in CDP > 27.9 group(8.57%, 2 out of 35). Noteworthy is the reduction in the %MACE in the CDP > 27.9 group, in comparison to the FFR < 0.75 group. Further, the secondary outcomes were not statistically significant between the FFR < 0.75 and CDP > 27.9 groups. Survival analysis results suggest that the survival time for the CDP > 27.9 group(n = 35) is significantly higher(P = 0.048) in comparison to the survival time for the FFR < 0.75 group(n = 20). The results remained similar for a FFR = 0.80 cut-off. | 11 | Delineation of epicardial stenosis in patients with microvascular disease using pressure drop coefficient:A pilot outcome study显示文摘AIM To investigate the patient-outcomes of newly developed pressure drop coefficient(CDP) in diagnosing epicardial stenosis(ES) in the presence of concomitant microvascular disease(MVD).METHODS Patients from our clinical trial were divided into two subgroups with:(1) cut-off of coronary flow reserve(CFR) < 2.0;and(2) diabetes.First,correlations were performed for both subgroups between CDP and hyperemic microvascular resistance(HMR),a diagnostic parameter for assessing the severity of MVD.Linear regression analysis was used for these correlations.Further,in each of the subgroups,comparisons were made between fractional flow reserve(FFR) < 0.75 and CDP > 27.9 groups for assessing major adverse cardiac events(MACE:Primary outcome).Comparisons were also made between the survival curves for FFR < 0.75 and CDP > 27.9 groups.Two tailed chi-squared and Fischer's exact tests were performed for comparison of the primary outcomes,and the log-rank test was used to compare the Kaplan-Meier survival curves.P < 0.05 for all tests was considered statistically significant.RESULTS Significant linear correlations were observed between CDP and HMR for both CFR < 2.0(r = 0.58,P < 0.001) and diabetic(r = 0.61,P < 0.001) patients.In the CFR < 2.0 subgroup,the %MACE(primary outcomes) for CDP > 27.9 group(7.7%,2/26) was lower than FFR < 0.75 group(3/14,21.4%);P = 0.21.Similarly,in the diabetic subgroup,the %MACE for CDP > 27.9 group(12.5%,2/16) was lower than FFR < 0.75 group(18.2%,2/11);P = 0.69.Survival analysis for CFR < 2.0 subgroup indicated better event-free survival for CDP > 27.9 group(n = 26) when compared with FFR < 0.75 group(n = 14);P = 0.10.Similarly,for the diabetic subgroup,CDP > 27.9 group(n = 16) showed higher survival times compared to FFR group(n = 11);P = 0.58.CONCLUSION CDP correlated significantly with HMR and resulted in better %MACE as well as survival rates in comparison to FFR.These positive trends demonstrate that CDP could be a potential diagnostic endpoint for delineating MVD with or without ES. | Ullhas Udaya Hebbar Mohamed A Effat Srikara V Peelukhana Imran Arif Rupak K Banerjee | 2017 | World Journal of Cardiology2017,9,12: | 2 |
