| 1 | Predictive factors for early aspiration in liver abscess显示文摘AIM: To determine the predictive factors for early aspiration in liver abscess. METHODS: A retrospective analysis of all patients with liver abscess from 1995 to 2004 was performed. Abscess was diagnosed as amebic in 661 (68%) patients, pyogenic in 200 (21%), indeterminate in 73 (8%) and mixed in 32 (3%). Multiple logistic regression analysis was performed to determine predictive factors for aspiration of liver abscess. RESULTS: A total of 966 patients, 738 (76%) male, mean age 43 ± 17 years, were evaluated: 540 patients responded to medical therapy while adjunctive percutaneous aspiration was performed in 426 patients. Predictive factors for aspiration of liver abscess were: age ≥ 55 years, size of abscess ≥ 5 cm, involvement of both lobes of the liver and duration of symptoms ≥ 7 d. Hospital stay in the aspiration group was relatively longer than in the non aspiration group. Twelve patients died in the aspiration group and this mortality was not statistically significant when compared to the non aspiration group. | 2 | Prevalence of non-Helicobacter pylori duodenal ulcer in Karachi,Pakistan显示文摘AIM: To determine the prevalence of non-Helicobacter pylori (H pylori)-related duodenal ulcer in patients with acid-peptic diseases.METHODS: Medical records of patients who attended the Gastroenterology Department at Aga Khan University Hospital from 1999 to 2001 and had endoscopic diagnosis of duodenal ulcers were reviewed. Duodenal ulcer associated with H pylori was diagnosed on the basis of endoscopy,rapid urease test and histopathology whereas histories of aspirin or other non-steroidal anti-inflammatory drugs (NSAIDs) related duodenal ulcers. Non-H pylori, non-NSAID duodenal ulcers were those without H pylori infection and history of NSAID intake. Co-morbid conditions associated were noted.RESULTS: Of 2 260 patients, 10% (217/2 260) had duodenal ulcer. Duodenal ulcer related to H pylori infectionaccounted for 53% (116/217), NSAID-related 10% (22/217),non-H pylori non-NSAID 29% (62/217), and 8% (17/217) had both Hpyloriinfection and histories of NSAID intake. Fifteen percent (18/116)_patients had past histories of peptic ulcer disease in Hpyloriinfection, while 8% (5/62) in non-Hpylorinon-NSAID ulcer. Co-morbid conditions in H pylori infection were seen in 23% (27/116) and 34%(21/62) in non-H pylori non-NSAID ulcer.CONCLUSION: Incidence of H pylori infection related with duodenal ulcer is common. | 4 | Candida esophagitis:Risk factors in non-HIV population in Pakistan显示文摘AIM:Candida esophagitis is a frequent infection in immunocompromised patients. This study was designed to determine its characteristics in non- human immune deficiency virus (HIV) infected patients attending a teaching hospital.METHODS: Clinical records of all patients coded by international classification of diseases 9th revision with clinical modifications'(ICD-9-CM), with candida esophagitis diagnosed by esophagogastroduodenoscopy (EGD) and histopathology over a period of 5 years were studied.RESULTS: Fifty-one patients (27 males, 24 females, range 21-77 years old and mean age 52.9 years) fulfilled the criteria (0.34% of the EGD). The common predisposing factors were carcinoma (OR 3.87, CI 1.00-14.99) and diabetes mellitus (OR 4.39, CI 1.34-14.42). The frequent clinical symptoms were retrosternal discomfort, dysphagia and epigastric abdominal pain with endoscopic appearance of scattered mucosal plaques. Another endoscopic lesion was associated with candida esophagitis in 15% patients.CONCLUSION: Carcinomas, diabetes mellitus, corticosteroid and antibiotic therapy are major risk factors for candida esophagitis in Pakistan. | 7 | Synthesis,Crystal Structure and Spectroscopic Properties of 1,2-Benzothiazine Derivatives:An Experimental and DFT Study显示文摘1,2-Benzothiazine derivatives methyl 3-methoxy-4-oxo-3,4-dihydro-2H-benzo[e] [1,2]thiazine-3-carboxylate 1,1-dioxide(1) and methyl 2-ethyl-3-hydroxy-4-oxo-3,4-dihydro-2Hbenzo[e][1,2]thiazine-3-carboxylate 1,1-dioxide(2) were synthesized, and characterized by spectroscopic techniques; 1H-NMR and infrared(IR) spectroscopy. Crystals of 1 and 2 were grown by slow evaporation of methanol and ethyl acetate, respectively and their crystal structures were investigated by single-crystal X-ray diffraction analysis. Geometric properties were calculated by the B3 LYP method of density functional theory(DFT) at the 6-31G+(d) basis set to compare with the experimental data. Simulated properties were found in strong agreement with the experimental ones. Intermolecular forces have also been modeled in order to investigate the strength of packing and strong hydrogen bonding was observed in both compounds 1 and 2. | 10 | Modifications of the Optimal Auxiliary Function Method to Fractional Order Fornberg-Whitham Equations显示文摘In this paper,we present a new modification of the newly developed semi-analytical method named the Optimal Auxilary Function Method(OAFM)for fractional-order equations using the Caputo operator,which is named FOAFM.The mathematical theory of FOAFM is presented and the effectiveness of this method is proven by using it with well-known Fornberg-Whitham Equations(FWE).The FOAFM results are compared with other method results along with their exact solutions with the help of tables and plots to prove the validity of FOAFM.A rapidly convergent series solution is obtained from FOAFMand is validated by comparison with other results.The analysis proves that ourmethod is simply applicable,contains less computationalwork,and is rapidly convergent to the exact solution at the first iteration.A series solution to the problem is obtained with the help of FOAFM.The validity of FOAFM results is validated by comparing its results with the results available in the literature.It is observed that FOAFM is simply applicable,contains less computational work,and is fastly convergent.The convergence and stability are obtained with the help of optimal constants.FOAFM is very easy in applicability and provides excellent results at the first iteration for complex nonlinear initial/boundary value problems.FOAFM contains the optimal auxiliary constants through which we can control the convergence as FOAFM contains the auxiliary functions D_(1),D_(2),D_(3)...in which the optimal constants G_(1),G_(2),...and the control convergence parameters exist to play an important role in getting the convergent solution which is obtained rigorously.The computational work in FOAFM is less when compared to other methods and even a low-specification computer can do the computational work easily. | Hakeem Ullah Mehreen Fiza Ilyas Khan Abd Allah A.Mosa Saeed Islam Abdullah Mohammed | 2023 | Computer Modeling in Engineering & Sciences2023,,7: | 0 |

| 12 | Giardiasis in patients with dyspeptic symptoms显示文摘AIM: To investigate the prevalence of giardiasis in patients with dyspeptic symptoms.METHODS: Clinical records of consecutive patients who attended Gastroenterology Department at Aga Khan University Hospital from January 2000 to June 2003 and had esophagogastroduodenoscopy (EGD) with duodenal biopsies and international classification of diseases 9th revision with clinical modifications (ICD-9-CM) coded with giardiasis were studied.RESULTS: Two hundred and twenty patients fulfilled the above criteria. There were 44% (96/220) patients who were giardiasis positive, 72% (69/96) of them were males and 28% (27/96) of them were females.There were 65% (81/124) males and 35% (43/124)females who were giardiasis negative. This class consists of flows for which the vorticity distribution is given by ▽2ψ=ψ+f(x,y). The solutions are obtained by applying the inverse method, which makes certain hypotheses regarding the form of the velocity field and pressure but without making any regarding the boundaries of the domain occupied by the fluid.
