2Psychiatric disorders and caregiver burden in children with transfusion dependentβ-thalassaemia and their caregivers显示文摘BACKGROUND Children with thalassemia need care from the first years of life owing to the physical and psychological effects of their disorder.Thalassemia is a concern not only for the children’s physical health but also the mental health of themselves and their caregivers.AIM To screen the psychosocial problems and assessment of psychiatric morbidities among thalassaemic children and their caretakers,along with an assessment of caregiver burden in them.METHODS In this observational cross-sectional study,children with transfusion-dependent thalassemia,were included and were assessed for psychiatric morbidity and global functioning.Their parents were assessed for psychiatric morbidity and the caregiver burden they faced.All the parents completed two different questionnaires to assess their knowledge about the psycho-social functioning[using Pediatric Symptom Checklist-35(PSC-35)]of their children and the level of the burden faced by them by Caregiver Burden Scale(CBS).RESULTS A total of 46 children(28 boys and 18 girls)with transfusion-dependent thalassemia with a mean age of 8.83±2.70 years and 46 parents(12 fathers and 34 mothers)were included in this study.More than 32 children had some psychosocial problems on screening by PSC-35.On assessment by CBS moderate caregiver burden was perceived in domains of general strain,isolation,disappointment,emotional involvement,and environment.A total of 65.3%of children and 62.7%of parents were diagnosed with psychiatric problems.CONCLUSION Thalassemia affects not only the persons with the disorder but also their caregivers in several aspects,including their psychosocial well-being.This study emphasizes the role of a supportive group in the psychological well-being of caregivers,which could be used to prevent the pathological effects of caregiver burden and enhance their psychological well-being through counselling.Samiksha Sahu Amit Agrawal Jyotsna Shrivastava Sudhir Tonk 2023World Journal of Clinical Pediatrics2023,12,3:1

4Effect of aging on stem cells显示文摘Pluripotent stem cells have the remarkable self-renewal ability and are capable of differentiating into multiple diverse cells. There is increasing evidence that the aging process can have adverse effects on stem cells. As stem cells age, their renewal ability deteriorates and their ability to differentiate into the various cell types is altered. Accordingly, it is suggested aging-induced deterioration of stem cell functions may play a key role in the pathophysiology of the various aging-associated disorders. Understanding the role of the aging process in deterioration of stem cell function is crucial, not only in understanding the pathophysiology of agingassociated disorders, but also in future development of novel effective stem cell-based therapies to treat agingassociated diseases. This review article first focuses on the basis of the various aging disease-related stem cell dysfunction. It then addresses the several concepts on the potential mechanism that causes aging-related stem cell dysfunction. 5Fragment emission and critical behavior in light and heavy charged systems显示文摘We study the emission of fragments in central collisions of light and heavily charged systems of^4+Ar+^45 Sc and^s4Kr+^197 Au,respectively,using the Quantum Molecular Dynamics(QMD)model as the primary model.The fragments are identified using an energy based clusterization algorithm,i.e.,the Simulated Annealing Clusterization Algorithm(SACA).The charge distributions of intermediate mass fragments[3≤Zf≤12]are fitted with power-law(∝Zf^-τ)and exp on ential(∝e^-λZf)fits in order to extract the parametersτandλ,whose minimum values are also sometimes linked with the onset of fragmentation or the critical point for a liquid-gas phase transition.Other parameters such as the normalized second moment,<γ2>,average size of the second largest cluster,phase separation parameter(Sp),bimodal parameter(P),information entropy(H),and Zipfs law are also analyzed to find the exact energy of the onset of fragmentation.Our detailed analysis predicts that an energy point exists between 20-23.1 MeV/nucleon,which is very close to the experimentally observed value of 23.9 MeV/nucleon for the^40Ar+^45Sc reaction.We also find that the critical energy deduced using Zipfs law is higher than those predicted from other critical exponents.Moreover,no minimum is found for t values of the highly charged system of^84Kr+^197Au,in agreement with experimental findings and various theoretical calculations.We observe that the QMD+SACA model calculations are in agreement with the experimental observations.This agreement supports our results regarding the energy point of the liquid-gas phase transition and the onset of fragmentation.Samiksha Sood Rohit Kumar Arun Sharma Sakshi Gautam Rajeev K.Puri 2021Chinese Physics C2021,45,1:0
6Imipenem/cilastatin-induced acute eosinophilic pneumonia:A case report显示文摘Rationale:Acute eosinophilic pneumonia(AEP)is an acute pulmonary illness caused by eosinophilic infiltration of the lung parenchyma.It can happen after using drugs such as daptomycin and minocycline.AEP induced by imipenem/cilastatin is a rare condition.Patient’s Concern:A 45-year-old male patient,who previously suffered from a urinary tract infection and treated with imipenem/cilastatin antibiotic,was presented to us with acute respiratory distress,soon after the initiation of the antibiotic.Computed tomography identified pulmonary infiltrates in the upper and middle lung fields and eosinophils were found to account for 36%of differential count of the broncho-alveolar lavage fluid.He also developed peripheral eosinophilia as the disease progressed.Diagnosis:AEP,secondary to imipenem/cilastatin therapy.Interventions:Steroid therapy was administered and imipenem/cilastatin antibiotic was discontinued.Outcomes:The patient improved completely following the therapy and had clear lung fields on follow-up.Lessons:Imipenem/cilastatin is an uncommon cause of AEP and requires close monitoring during therapy.Gautam Jesrani Samiksha Gupta Amtoj Singh Lamba Shreya Arora Monica Gupta 2023Journal of Acute Disease2023,12,6:0
7Delayed post-hypoxic leukoencephalopathy following barbiturate overdose: A case report显示文摘Rationale:Delayed post-hypoxic leukoencephalopathy(DPHL)is usually an overlooked condition,which arises as a result of a multitude of reversible and irreversible conditions.Patient’s Concern:A 50-year-old female with a history of epilepsy,who developed DPHL 12 days after respiratory failure secondary to barbiturate toxicity.Diagnosis:DPHL on magnetic resonance imaging of the brain.Interventions:Mechanical ventilation was initiated for respiratory failure and hemodialysis for barbiturate toxicity.Outcomes:The patient developed akinetic mutism due to infirmity and had a residual disability,which led to permanent dependency.Lessons:The diagnosis of DPHL is often delayed or missed,given the rarity of this condition and its inconsistent clinical symptomatology.Diagnostic delay can be avoided by early recognition of the classical“delayed onset”symptoms.Arshi Syal Samiksha Gupta Monica Gupta Gautam Jesrani Yajur Arya 2022Journal of Acute Disease2022,11,4:0
8Electrocardiographic abnormalities in prevalent infections in tropical regions:A scoping review显示文摘Cardiovascular manifestations and electrocardiographic abnormalities have been reported among some prevalent infections in tropical regions,which lead to a great amount of morbidity and mortality.The major infectious diseases include chikungunya,dengue fever,H1N1 influenza,and coronavirus disease-19(COVID-19)in the viral category,leptospirosis,salmonellosis,scrub typhus and tuberculosis in the bacterial category,and malaria in the protozoan parasite category.All these infirmities constitute a foremost infection burden worldwide and have been linked to the various cardiac rhythm aberrancies.So we aimed to identify and compile different studies on these infections and associated acute electrocardiographic(ECG)changes.The search was made in online international libraries like PubMed,Google Scholar,and EMBASE,and 38 most relevant articles,including original research,systematic reviews,and unique case reports were selected.All of them were evaluated thoroughly and information regarding ECG was collected.Myocarditis is the predominant underlying pathology for rhythm disturbance and can be affected either due to the direct pathogenic effect or the abnormal immune system activation.ECG variabilities in some infections like chikungunya,scrub typhus,and leptospirosis are associated with longer hospital stay and poor outcome.Tropical infective diseases are associated with prominent acute cardiac rhythm abnormalities due to myocarditis,which can be identified preliminarily by ECG changes.Gautam Jesrani Samiksha Gupta Saurabh Gaba Monica Gupta 2022Journal of Acute Disease2022,11,2:0
