1亚太地区胃食管反流病的处理共识:更新版显示文摘背景与目的:自从2004年亚太地区胃食管反流病(GERD)共识发表以来,更多关于GERD流行病学和处理的文献资料相继出现。有必要对这些资料进行循证综述,对共识作出更新。方法:由多学科专家组应用德尔菲(Delphi)法制定共识条文,提呈相关资料,并对证据质量、推荐力度和共识水平进行分级。结果:亚洲GERD发生率日益增加。其危险因素包括老年、男性、种族、家族史、社会经济地位高、体重指数增加和吸烟。对于有典型症状而无报警症状的患者,对质子泵抑制剂(PPI)试验有症状应答具有诊断意义。如PPI试验失败,停止治疗后pH监测结果阴性可排除GERD。窄带成像、胶囊内镜检查和无线pH监测的作用尚未明确。亚洲诊断策略的制定须考虑到并存的胃癌和消化性溃疡。减轻体质量和抬高床头可改善反流症状。PPIs是最有效的内科治疗手段。对于非糜烂性反流病(NERD)患者,按需治疗较为适宜。有慢性咳嗽、喉炎和典型GERD症状的患者在排除非GERD病因后,应予PPI每天两次治疗。如有经验丰富的外科医师,GERD患者可行胃底折叠术。除临床试验外,GERD不应采用内镜治疗。结论:新的诊断方法和内镜治疗的作用有待进一步研究阐明。亚洲GERD诊断策略的制定须考虑到并存的胃癌和消化性溃疡。PPIs仍为治疗的基石。Kwong Ming Fock Nicholas J Talley Ronnie Fass Khean Lee Goh Peter Katelaris Richard Hunt Michio Hongo Tiing Leong Ang Gerald Holtmann Sanjay Nandurkar San Ren Lin Benjamin CY Wong Francis KL Chan Abdul Aziz Rani Young-Tae Bak Jose Sollano Khek Yu Ho Sathoporn Manatsathit 钱本余 2008胃肠病学2008,13,7:21

4Globally Proper Saddle Point in Ic-Cone-Convexlike Set-Valued Optimization Problems显示文摘The existence conditions of globally proper efficient points and a useful property of iccone-convexlike set-valued maps are obtained. Under the assumption of the ic-cone-convexlikeness,the optimality conditions for globally proper efficient solutions are established in terms of Lagrangemultipliers. The new concept of globally proper saddle-point for an appropriate set-valued Lagrangemap is introduced and used to characterize the globally proper efficient solutions. 5Association of a SLC30A8 Genetic Variant with Monotherapy of Repaglinide and Rosiglitazone Effect in Newly Diagnosed Type 2 Diabetes Patients in China显示文摘Objective To investigate a potential relationship between Solute carrier family 30 (zinc transporter) member 8 (SLC30A8) rs13266634 variant and efficacy of rosiglitazone or repaglinide in treating newly diagnosed Chinese type 2 diabetes patients.Methods A total of 209 diabetic patients without any antihyperglycemic history were recruited and treated with repaglinide or rosiglitazone randomly for 48 weeks (104 and 105 patients,respectively).Anthropometric measurements and clinical laboratory tests were carried out before and after the treatment.An non-synonymous variant rs13266634 was genotyped by matrix-assisted laser desorption ionization-time of flight mass spectroscopy.Results Ninety-one patients in repaglinide group and ninety-three patients in rosiglitazone group completed the study.Δ value of homeostasis model assessment of beta cell function (HOMA-B)and Δ value of fasting proinsulin levels were statistically significant between three genotype groups (P=0.0149 and 0.0246,respectively) after rosiglitazone treatment.However,no genotype association was observed in the repaglinide or rosiglitazone group with other parameters.Conclusion The SLC30A8 variant was associated with the efficacy of insulin sensitizer monotherapy on insulin secretion in patients with newly diagnosed type 2 diabetes mellitus in Shanghai,China.JIANG Feng LI Qing HU Cheng ZHANG Rong WANG Cong Rong YU Wei Hui LU Jing Yi TANG Shan Shan BAO Yu Qian XIANG Kun San JIA Wei Ping 2012Biomedical and Environmental Sciences2012,25,1:8

7Analysis of PM2.5 concentrations in Heilongjiang Province associated with forest cover and other factors显示文摘Atmospheric particulate matter(PM2.5) seriously influences air quality. It is considered one of the main environmental triggers for lung and heart diseases. Air pollutants can be adsorbed by forest. In this study we investigated the effect of forest cover on urban PM2.5 concentrations in 12 cities in Heilongjiang Province,China. The forest cover in each city was constant throughout the study period. The average daily concentration of PM2.5 in 12 cities was below 75 lg/m^3 during the non-heating period but exceeded this level during heating period. Furthermore, there were more moderate pollution days in six cities. This indicated that forests had the ability to reduce the concentration of PM2.5 but the main cause of air pollution was excessive human interference and artificial heating in winter. We classified the 12 cities according to the average PM2.5 concentrations. The relationship between PM2.5 concentrations and forest cover was obtained by integrating forest cover, land area,heated areas and number of vehicles in cities. Finally,considering the complexity of PM2.5 formation and based on the theory of random forestry, we selected six cities and analyzed their meteorological and air pollutant data. 8Human adenovirus(HAdV)infection in children with acute respiratory tract infections in Guangzhou,China,2010–2021:a molecular epidemiology study显示文摘Background Human adenovirus(HAdV)infection can cause a variety of diseases.It is a major pathogen of pediatric acute respiratory tract infections(ARIs)and can be life-threatening in younger children.We described the epidemiology and subtypes shifting of HAdV among children with ARI in Guangzhou,China.Methods We conducted a retrospective study of 161,079 children diagnosed with acute respiratory illness at the Guangzhou Women and Children’s Medical Center between 2010 and 2021.HAdV specimens were detected by real-time PCR and the hexon gene was used for phylogenetic analysis.Results Before the COVID-19 outbreak in Guangzhou,the annual frequency of adenovirus infection detected during this period ranged from 3.92%to 13.58%,with an epidemic peak every four to fve years.HAdV demonstrated a clear seasonal distribution,with the lowest positivity in March and peaking during summer(July or August)every year.A signifcant increase in HAdV cases was recorded for 2018 and 2019,which coincided with a shift in the dominant HAdV subtype from HAdV-3 to HAdV-7.The latter was associated with a more severe disease compared to HAdV-3.The average mortality proportion for children infected with HAdV from 2016 to 2019 was 0.38%but increased to 20%in severe cases.After COVID-19 emerged,HAdV cases dropped to 2.68%,suggesting that non-pharmaceutical interventions probably reduced the transmission of HAdV in the community.Conclusion Our study provides the foundation for the understanding of the epidemiology of HAdV and its associated risks in children in Southern China.Yi Chen Tao Lin Chang‑Bing Wang Wan‑Li Liang Guang‑Wan Lian Mark Zanin Sook‑San Wong Xin‑Gui Tian Jia‑Yu Zhong Ying‑Ying Zhang Jia‑Hui Xie Ling‑Ling Zheng Fei‑Yan Chen Run Dang Ming‑Qi Zhao Yi‑Yu Yang Rong Zhou Bing Zhu 2022World Journal of Pediatrics2022,18,8:5

13Functional difference between Sinorhizobium meliloti NifA and Enterobacter cloacae NifA显示文摘The nifA gene is an important regulatory gene and its product, NifA protein, regulates the expression of many nif genes involved in the nitrogen fixation process. We introduced multiple copies of the constitutively expressed Sinorhizobium meliloti (Sm) or Enterobacter cloacae (Ec) nifA gene into both the nifA mutant strain SmY and the wild-type strain Sm1021. Root nodules produced by SmY containing a constitutively expressed Sm nifA gene were capable of fixing nitrogen, while nodules produced by SmY containing the Ec nifA gene remained unable to fix nitrogen, as is the case for SmY itself. However, transfer of an additional Sm nifA gene into Sm1021 improved the nitrogen-fixing efficiency of root nodules to a greater extent than that observed upon transfer of the Ec nifA gene into Sm1021. Comparative analysis of amino acid sequences between Sm NifA and Ec NifA showed that the N-terminal domain was the least similar, but this domain is indispensable for complementation of the Fix? phenotype of SmY by Sm NifA. 14Regional brain structural abnormality in ischemic stroke patients:a voxel-based morphometry study显示文摘Our previous study used regional homogeneity analysis and found that activity in some brain areas of patients with ischemic stroke changed significantly.In the current study,we examined structural changes in these brain regions by taking structural magnetic resonance imaging scans of 11 ischemic stroke patients and 15 healthy participants,and analyzing the data using voxel-based morphometry.Compared with healthy participants,patients exhibited higher gray matter density in the left inferior occipital gyrus and right anterior white matter tract.In contrast,gray matter density in the right cerebellum,left precentral gyrus,right middle frontal gyrus,and left middle temporal gyrus was less in ischemic stroke patients.The changes of gray matter density in the middle frontal gyrus were negatively associated with the clinical rating scales of the Fugl-Meyer Motor Assessment(r=–0.609,P=0.047) and the left middle temporal gyrus was negatively correlated with the clinical rating scales of the nervous functional deficiency scale(r=–0.737,P=0.010).Our findings can objectively identify the functional abnormality in some brain regions of ischemic stroke patients.Ping Wu Yu-mei Zhou Fang Zeng Zheng-jie Li Lu Luo Yong-xin Li Wei Fan Li-hua Qiu Wei Qin Lin Chen Lin Bai Juan Nie San Zhang Yan Xiong Yu Bai Can-xin Yin Fan-rong Liang 2016Neural Regeneration Research2016,11,9:2

16Development of nickel based cermet anode materials in solid oxide fuel cells–Now and future显示文摘High temperature solid oxide fuel cell(SOFC)is the most efficient and clean energy conversion technology to electrochemically convert the chemical energy of fuels such as hydrogen,natural gas and hydrocarbons to electricity,and also the most viable alternative to the traditional thermal power plants.However,the power output of a SOFC critically depends on the characteristics and performance of its key components:anode,electrolyte and cathode.Due to the highly reducing environment and strict requirements in electrical conductivity and catalytic activity,there are limited choices in the anode materials of SOFCs,particularly for operation in the intermediate temperature range of 500–800C.Among them,Ni-based cermets are the most common and popular anode materials of SOFCs.The objective of this paper is to review the development of Ni-based anode materials in SOFC from the viewpoints of materials microstructure,performance and industrial scalability associated with the fabrication and optimization processes.The latest advancement in nano-structure architecture,contaminant tolerance and interface optimization of Ni-based cermet anodes is presented.And at the end of this paper,we propose and appeal for the collaborative work of scientists from different disciplines that enable the inter-fusion research of fabrication,microanalysis and modelling,aiming at the challenges in the development of Ni-based cermet anodes for commercially viable intermediate temperature SOFC or IT-SOFC technologies.Yu Liu Zongping Shao Toshiyuki Mori San Ping Jiang 2021Materials Reports(Energy)2021,1,1:2
17Clinical Predictors for Diagnosing Pandemic (H1N1) 2009 and Seasonal Influenza (H3N2) in Fever Clinics in Beijing,China显示文摘Objective Symptomatic predictors of influenza could assess risks and improve decisions about isolation and outpatient treatment.To develop such predictors,we undertook a prospective analysis of pandemic (H1N1) 2009 and seasonal influenza (H3N2) in patients attending fever clinics.Methods From 1 May 2009 to 1 January 2010,all adult patients admitted to fever clinics for suspected influenza,confirmed by real time RT-PCR,were enrolled.Predictors of influenza virus infection were selected with logistic regression models.Measures of sensitivity,specificity,positive and negative likelihood ratios (LRs) were calculated to identify the best predictors.Results The clinical features and routine blood test results of influenza (H1N1) 2009 and seasonal influenza were similar.The positive and negative LRs of current US CDC influenza-like illness (ILI) criteria were modest in predicting influenza infection.Our modified clinic predictors improved the ability of the positive and negative LRs to recognize pandemic (H1N1) 2009 and seasonal influenza.The revised criteria are:fever ≥38℃ accompanied by at least one of the following—cough,arthralgia or relative lymphopenia.Conclusion Patients with symptoms and signs that meet the new criteria are likely to have influenza and timely antiviral therapy may be appropriate.In addition,physicians should ascertain if influenza is circulating within the community or if there is a contact history of influenza and combine this information with the newly developed criteria to clinically diagnose influenza.DAI Xiao Qiu LIU Min ZHANG Tuo Hong YANG Xue Song LI Song Lin LI Xiao Guang LI Yu Ling KADEERBAI Hai San WU Huang 2012Biomedical and Environmental Sciences2012,25,1:2

19Kinetic Characteristics of Neutralizing Antibody Responses Vary among Patients with COVID-19显示文摘Objective The coronavirus disease 2019(COVID-19) pandemic continues to present a major challenge to public health. Vaccine development requires an understanding of the kinetics of neutralizing antibody(NAb) responses to severe acute respiratory syndrome coronavirus 2(SARS-CoV-2).Methods In total, 605 serum samples from 125 COVID-19 patients(from January 1 to March 14, 2020)varying in age, sex, severity of symptoms, and presence of underlying diseases were collected, and antibody titers were measured using a micro-neutralization assay with wild-type SARS-CoV-2.Results NAbs were detectable approximately 10 days post-onset(dpo) of symptoms and peaked at approximately 20 dpo. The NAb levels were slightly higher in young males and severe cases, while no significant difference was observed for the other classifications. In follow-up cases, the NAb titer had increased or stabilized in 18 cases, whereas it had decreased in 26 cases, and in one case NAbs were undetectable at the end of our observation. Although a decreasing trend in NAb titer was observed in many cases, the NAb level was generally still protective.Conclusion We demonstrated that NAb levels vary among all categories of COVID-19 patients.
