| 1 | Is the neutrophil to lymphocyte ratio associated with liver fibrosis in patients with chronic hepatitis B?显示文摘AIM: To determine the association between the neutrophil to lymphocyte(N/L) ratio and the degree of liver fibrosis in patients with chronic hepatitis B(CHB) infection. METHODS: Between December 2011 and February 2013, 129 consecutive CHB patients who were admitted to the study hospitals for histological evaluation of chronic hepatitis B-related liver fibrosis were included in this retrospective study. The patients were divided into two groups based on the fibrosis score: individuals with a fibrosis score of F0 or F1 were included in the 'no/minimal liver fibrosis' group, whereas patients with a fibrosis score of F2, F3, or F4 were included in the 'advanced liver fibrosis' group. The Statistical Package for Social Sciences 18.0 for Windows was used to analyze the data. A P value of < 0.05 was accepted as statistically significant.RESULTS: Three experienced and blinded pathologists evaluated the fibrotic status and inflammatory activity of 129 liver biopsy samples from the CHB patients. Following histopathological examination, the 'no/minimal fibrosis' group included 79 individuals, while the 'advanced fibrosis' group included 50 individuals. Mean(N/L) ratio levels were notably lower in patients with advanced fibrosis when compared with patients with no/minimal fibrosis. The mean value of the aspartate aminotransferase-platelet ratio index was markedly higher in cases with advanced fibrosis compared to those with no/minimal fibrosis.CONCLUSION: Reduced levels of the peripheral blood N/L ratio were found to give high sensitivity, specificity and predictive values in CHB patients with significant fibrosis. | 2 | Circulating microRNAs in patients with non-alcoholic fatty liver disease显示文摘AIM: To identify novel non-invasive biomarkers for non-alcoholic fatty liver disease(NAFLD). METHODS: Twenty patients with histologically proven NAFLD and 20 controls were included. All NAFLD cases were scored using the NAFLD activity score. The rela-tive expressions of miR-197, miR-146 b, miR-10 b, miR-181d, miR-34 a, miR-122, miR-99 a and miR-29 a were analyzed using real-time polymerase chain reaction. RESULTS: Serum levels of miR-181 d, miR-99 a, miR-197 and miR-146 b were significantly lower in biopsy-proven NAFLD patients than in the healthy controls. Serum lev-els of miR-197 and miR-10 b were inversely correlated with degree of inflammation and miR-181 d and miR-99 a were inversely correlated with serum gamma glu-tamyl transferase levels in non-alcoholic steatohepatitis patients. CONCLUSION: NAFLD is associated with altered se-rum miRNA expression pattern. | 3 | Investigations into the corrosive environments contributing to premature failure of Australian coal mine rock bolts显示文摘University of New South Wales(UNSW Australia) had been involved in the study of premature failure of rock bolts in Australia coal mines from the initial identification of the problem in 1999. Rock bolt steel changes over the last decade appear to have not reduced the incidence of failures. A broadened UNSW research project funded by the Australian Research Council(ARC) and Industry has targeted finding the environmental causes through extensive field and laboratory experiments. This paper describes the field studies conducted in underground coal mines,in particular attempts to measure the contribution to corrosion from groundwater,mineralogy and microbial activity. Various underground survey techniques were used to determine the extent of broken bolts,with the presence of both stress corrosion cracking(SCC) and localized deep pitting making no single technique suitable on their own.Groundwater found dripping from bolts across various coalfields in Australia were found to be not aggressive and known groundwater corrosivity classification systems did not correlate to where broken bolts were found. In-hole coupon bolts placed in roof strata containing claystone bands confirmed the clay as being a major contributor to corrosion. Microbes capable of contributing to steel corrosion were found to be present in groundwater,and culturing of the microbes taken from in-situ coupon bolts proved that the bacteria was present on the bolt surface. An ‘in-hole bolt corrosion coupon' development by the project may have multiple benefits of (1) helping quantify newly developed corrosivity classification systems,(2) providing an in-situ ground support corrosion monitoring tool,and (3) for testing possible corrosion protection solutions. | Craig P. | 5 | Numerical and analytical simulation of the structural behaviour of fully grouted cable bolts under impulsive loading显示文摘Designing reliable yielding support system to mitigate the effect of the kinetic energy in burst-prone conditions in mining and tunneling excavations is one of the challenges for geotechnical engineers. A combination of the support elements can be used to increase rock strength and minimise the displacement of unstable rock mass. It is important to understand how the support system works to ensure the stability of underground excavations. Cable bolts have been commonly used as an effective underground support system and an element of reinforcement to improve rock stability. Cable bolts are usually considered to be subjected to static loads under relatively low stress environments, however, in burst-prone conditions, they might be subjected to dynamic loads. Cable bolts as well as other support elements are used in burst-prone conditions to absorb the kinetic energy of the removed rock to avoid sudden and violent failures. This paper develops numerical and a novel analytical simulation technique for cable bolts to assess their structural behaviour under static and dynamic loading conditions. | 6 | Evaluation of leukocyte esterase and nitrite strip tests to detect spontaneous bacterial peritonitis in cirrhotic patients显示文摘AIM: To investigate the diagnostic eff icacy of leukocyte esterase and nitrite reagent strips for bedside diagnosis of spontaneous bacterial peritonitis (SBP). METHODS: A total of 63 consecutive patients with cirrhotic ascites (38 male, 25 female) tested between April 2005 and July 2006 were included in the study. Bedside reagent strip testing was performed on ascitic ? uid and the results compared to manual cell counting and ascitic fluid culture. SBP was defined as having a polymorphonuclear ascites count of ≥ 250/mm3. RESULTS: Fifteen samples showed SBP. The sensitivity, specificity, positive and negative predictive values of the leukocyte esterase reagent strips were; 93%, 100%, 100%, and 98%, respectively. The sensitivity, specifi city, positive and negative predictive value of the nitrite reagent strips were 13%, 93%, 40%, and 77%, respectively. The combination of leukocyte esterase and nitrite reagents strips did not yield statistically signif icant effects on diagnostic accuracy. | 7 | Intraoperative acidosis is a new predictor for postoperative pancreatic fistula after pancreaticoduodenectomy显示文摘BACKGROUND:Early diagnosis of postoperative pancreatic fistula(POPF) is important for proper interventions.The preoperative,intraoperative and early postoperative biochemical markers have predictive value of POPF.The present study was to evaluate several simple biochemical parameters in the prediction of POPF.METHODS:Patients who underwent pancreaticoduodenectomy in our center between 2006 and 2015 were reviewed retrospectively.Preoperative and early postoperative biochemical parameters were evaluated.Additionally,the relationship between POPF and p H and lactate level at the end of surgery were analyzed,and neutrophil-to-lymphocyte ratio(NLR),plateletto-lymphocyte ratio(PLR),and red cell distribution widthto-platelet ratio(RPR) were calculated for postoperative days(PODs) 1 and 3.Diagnosis and grading of POPF were performed according to the standards of the International Study Group on Pancreatic Fistula.The patients were divided into two groups:Group 1 with no fistula or grade-A fistula; group 2 with grade-B or-C fistula.These simple biochemical markers were then compared between the two groups.RESULTS:Serum amylase level was significantly higher at POD3,and p H level was significantly lower at the end of operation in group 2 compared with those in group 1.However,the serum amylase was below the upper limit of normal serum level and therefore,the difference was not significant in clinical practice.Receiver operating charecteristic curve analysis showed that p H level was a reliable predictor of POPF(area under the curve:0.713; 95% CI:0.573-0.853).CONCLUSIONS:A low p H level at the end of pancreaticoduodenectomy was a risk factor of POPF.NLR,PLR,and RPR had no predictive value of POPF after pancreaticoduodenectomy. | Erdem Kinaci Mert Mahsuni Sevinc Abdulkerim Ozakay Savas Bayrak Ekrem Cakar Serkan Sari | 2016 | Hepatobiliary & Pancreatic Diseases International2016,15,3: | 7 |

| 9 | Parametric study on the axial performance of a fully grouted cable bolt with a new pull-out test显示文摘Modified cable bolts are commonly used in underground mines due to their superior performance in preventing bed separation when compared with plain strands. To better test the axial performance of a wide range of cable bolts,a new laboratory short encapsulation pull test(LSEPT) facility was developed. The facility simulates the interaction between cable bolts and surrounding rock mass,using artificial rock cylinders with a diameter of 300 mm in which the cable bolt is grouted. Furthermore,the joint where the load is applied is left unconstrained to allow shear slippage at the cable/grout or grout/rock interface.Based on this apparatus,a series of pull tests were undertaken using the MW9 modified bulb cable bolt.Various parameters including embedment length,test material strength and borehole size were evaluated. It was found that within a limited range of 360 mm,there is a linear relationship between the maximum bearing capacity of the cable bolt and embedment length. Beyond 360 mm,the peak capacity continues to rise but with a much lower slope. When the MW9 cable bolt was grouted in a weak test material,failure always took place along the grout/rock interface. Interestingly,increasing the borehole diameter from 42 to 52 m in weak test material altered the failure mode from grout/rock interface to cable/grout interface and improved the performance in terms of both peak and residual capacity. | Chen Jianhang Hagan Paul C. | 11 | Noninvasive assessment of liver damage in chronic hepatitis B显示文摘AIM:To evaluate the efficacy of the aspartate aminotransferase/platelet ratio index(APRI)and neutrophillymphocyte(N/L)ratio to predict liver damage in chronic hepatitis B(CHB).METHODS:We analyzed 89 patients diagnosed with CHB by percutaneous liver biopsy and 43 healthy subjects.Liver biopsy materials were stained with hematoxylin-eosin and Masson’s trichrome.Patients’fibrosis scores and histological activity index(HAI)were calculated according to the Ishak scoring system.Fibrosis score was recognized as follows:F0-1 No/early-stage fibrosis,F2-6 significant fibrosis,F0-4 non-cirrhotic and F5-6 cirrhotic.Significant liver fibrosis was defined as an Ishak score of≥2.APRI and N/L ratio calculation was made by blood test results.RESULTS:The hepatitis B and control group showed no difference in N/L ratios while there was a significant difference in terms of APRI scores(P<0.001).Multiple logistic regression analysis revealed that the only independent predictive factor for liver fibrosis in CHB was platelet count.APRI score was significantly higher in cirrhotic patients than in non-cirrhotic patients.However,this significance was not confirmed by multiple logistic regression analysis.The optimum APRI score cut-off point to identify patients with cirrhosis was 1.01with sensitivity,specificity,positive predictive value and negative predictive value of 62%(36%-86%),74%(62%-83%),29%(13%-49%)and 92%(82%-97%),respectively.In addition,correlation analyses revealed that N/L ratio has a negative and significant relationship with HAI(r=-0.218,P=0.041).CONCLUSION:N/L ratio was negatively correlated with HAI.APRI score may be useful to exclude cirrhosis in CHB patients. | Mehmet Celikbilek Serkan Dogan Sebnem Gursoy Gokmen Zararsιz Alper Yurci Omer Ozbakιr Kadri Guven Mehmet Yucesoy | 2013 | World Journal of Hepatology2013,5,8: | 5 |
| 12 | Interesting rendezvous location in a liver transplantation patient with anastomosis stricture显示文摘An endoscopic or radiologic percutaneous approach may be an initial minimally invasive method for treating biliary strictures after living donor liver transplantation; however, cannulation of biliary strictures is sometimes difficult due to the presence of a sharp or twisted angle within the stricture or a complete stricture. When an angulated or twisted biliary stricture interrupts passage of a guidewire over the stricture, it is difficult to replace the percutaneous biliary drainage catheter with inside stents by endoscopic retrograde cholangiopancreatography. The rendezvous technique can be used to overcome this difficulty. In addition to the classical rendezvous method, in cases with complete transection of the common bile duct a modified technique involving the insertion of a snare into the subhepatic space has been successfully performed. | 18 | Scintigraphic evaluation of gallbladder motor functions in H pylori positive and negative patients in the stomach with dyspepsia显示文摘AIM:To evaluate the relationship between gallbladder (GB) motor function and H pylori infection in the stomach. METHODS: All cases (86) underwent the 14C urea breath test (UBT). 14C-UBT was found as positive in 58 and negative in 28 dyspeptic patients. 14C- UBT was accepted as a gold standard test. Clo test and histopathologic examination were compared with the results of 14C-UBT in cases who tolerated upper gastrointestinal endoscopy procedure. Cholescintigraphy with 99mTc-mebrofenin was used to determine the parameters of GB motor function (GB filling and emptying time, half of the emptying time, ejection fraction at 30th and 60th min) in all patients. RESULTS: We found the sensitivity and specificity as 88% and 86% for Clo test and as 89% and 80% for histologic evaluation, respectively. The parameters of GB function were not significantly different in H pylori positive and negative patients. The GB emptying was normal in both groups. Minimum GB filling time was 30 min in 34 of 86 cases (39.5%), filling was not observed in 2 cases. The GB ultrasonography (USG) results were normal for all cases and bile composition abnormality was not determined.CONCLUSION: Our study showed that 14C-UBT is highly reliable method to detect the presence of H pylori. | 19 | Management of floor heave at Bulga Underground Operations – A case study显示文摘This paper presents an overview of the floor heave management at the Glencore Bulga Underground Operations and investigates the contributing factors to the behaviour of the floor. The mine experienced a number of major floor heave events in gateroads on development. As the longwall face approached the roadways, the magnitude of floor heave frequently increased, while new floor heave also developed.Furthermore, severe floor heave events took place along the longwall face. The most observed failure mode was buckling. While regular floor measurements were conducted to better understand the nature of the phenomenon, and various measures were considered to control the deformation of floor, the mining height was increased for the predicted floor heave domains, which facilitated effective management of the floor issues.
