| 1 | Differential diagnosis of benign and malignant branch duct intraductal papillary mucinous neoplasm using contrastenhanced endoscopic ultrasonography显示文摘AIM: To elucidate the role of contrast-enhanced endoscopic ultrasonography(CE-EUS) in the diagnosis of branch duct intraductal papillary mucinous neoplasm(BD-IPMN).METHODS: A total of 50 patients diagnosed with BDIPMN by computed tomography(CT) and endoscopic ultrasonography(EUS) at our institute were included in this study. CE-EUS was performed when mural lesions were detected by EUS. The diagnostic accuracy for identifying mural nodules(MNs) was evaluated by CT, EUS, and EUS combined with CE-EUS. In the patients who underwent resection, the accuracy of measuring MN height with each imaging modality was compared. The cut-off values to diagnose malignant BD-IPMNs based on MN height for each imaging modality were determined using receiver operating characteristic curve analysis.RESULTS: Fifteen patients were diagnosed with BD-IPMN with MNs and underwent resection. The remaining 35 patients were diagnosed with BD-IPMN without MNs and underwent follow-up monitoring. The pathological findings revealed 14 cases with MNs and one case without. The accuracy for diagnosing MNs was 92% using CT and 72% using EUS; the diagnostic accuracy increased to 98% when EUS and CE-EUS were combined. The accuracy for measuring MN height significantly improved when using CE-EUS compared with using CT or EUS(median measurement error value, CT: 3.3 mm vs CE-EUS: 0.6 mm, P < 0.05; EUS: 2.1 mm vs CE-EUS: 0.6 mm, P < 0.01). A cut-off value of 8.8 mm for MN height as measured by CE-EUS improved the accuracy of diagnosing malignant BDIPMN to 93%. | 3 | Colon perforation due to antigenemia-negative cytomegalovirus gastroenteritis after liver transplantation: A case report and review of literature显示文摘BACKGROUND Cytomegalovirus(CMV) remains a critical complication after solid-organ transplantation. The CMV antigenemia(AG) test is useful for monitoring CMV infection. Although the AG-positivity rate in CMV gastroenteritis is known to be low at onset, almost all cases become positive during the disease course. We treated a patient with transverse colon perforation due to AG-negative CMV gastroenteritis, following a living donor liver transplantation(LDLT).CASE SUMMARY The patient was a 52-year-old woman with decompensated liver cirrhosis as a result of autoimmune hepatitis who underwent a blood-type compatible LDLT with her second son as the donor. On day 20 after surgery, upper and lower gastrointestinal endoscopy(GE) revealed multiple gastric ulcers and transverse colon ulcers. The biopsy tissue immunostaining confirmed a diagnosis of CMV gastroenteritis. On day 28 after surgery, an abdominal computed tomography revealed transverse colon perforation, and simple lavage and drainage were performed along with an urgent ileostomy. Although the repeated remission and aggravation of CMV gastroenteritis and acute cellular rejection made the control of immunosuppression difficult, the upper GE eventually revealed an improvement in the gastric ulcers, and the biopsy samples were negative for CMV. | 4 | Pre-hepatectomy type Ⅳ collagen 7S predicts post-hepatectomy liver failure and recovery显示文摘BACKGROUND Liver resection is an effective treatment for benign and malignant liver tumors.However,a method for preoperative evaluation of hepatic reserve has not yet been established.Previously reported assessments of preoperative hepatic reserve focused only on liver failure in the early postoperative period and did not consider the long-term recovery of hepatic reserve.When determining eligibility for hepatectomy,the underlying pathophysiology needs to be considered to determine if the functional hepatic reserve can withstand both surgery and any postoperative therapy.AIM To identify pre-hepatectomy factors associated with both early postoperative liver failure and long-term postoperative liver function recovery.METHODS This study was a retrospective cohort study.We retrospectively investigated 215 patients who underwent hepatectomy at our hospital between May 2013 and December 2016.Early post-hepatectomy liver failure(PHLF)was defined using the International Study Group of Liver Surgery’s definition of PHLF.Long-term postoperative recovery of liver function was defined as the time taken for serum total bilirubin and albumin levels to return to levels of<2 mg/dL and>2.8 g/dL,respectively,and the time taken for Child-Pugh score to return to Child-Pugh class A.RESULTS Preoperative type IV collagen 7S was identified as a significant independent factor associated with both PHLF and postoperative long-term recovery of liver function.Further analysis revealed that the time taken for the recovery of Child-Pugh scores and serum total bilirubin and albumin levels was significantly shorter in patients with type IV collagen 7S≤6 ng/mL than in those with type IV collagen 7S>6 ng/mL.In additional analyses,similar results were observed in patients without chronic viral hepatitis associated with fibrosis.CONCLUSION Preoperative type IV collagen 7S is a preoperative predictor of PHLF and longterm postoperative liver function recovery.It can also be used in patients without chronic hepatitis virus. | Masatsugu Ishii Osamu Itano Masahiro Shinoda Minoru Kitago Yuta Abe Taizo Hibi Hiroshi Yagi Ayano Takeuchi Hanako Tsujikawa Tokiya Abe Yuko Kitagawa | 2020 | World Journal of Gastroenterology2020,26,7: | 3 |

| 9 | Effects of grazing and precipitation variability on vegetation dynamics in a Mongolian dry steppe显示文摘Aims Grazing and water availability are the primary drivers of vegetation dynamics in grazing-dominated regions of Mongolia with a semi-arid climate and frequent droughts.Nomadic animal husbandry still plays a large part in the economy of Mongolia,but more variable precipitation regime and increase in livestock number have severely affected grassland ecosystems through overgrazing,leading to pasture degradation.This study aimed to examine the effects of grazing exclusion,interannual variation of plant-available precipitation(PAP)and their interaction on the aboveground biomass(AGB)of each dominant species,the AGB of annual species and the total AGB in a Mongolian dry steppe,using long-term field data.Methods To detect the effect of grazing on vegetation dynamics,vegetation surveys were conducted in a non-grazed exclosure zone and a fully grazed area outside the exclosure.We assessed the effects of grazing,PAP and their interaction on AGB parameters using a generalized linear model.A detrended correspondence analysis(DCA)was used to visualize the effects of grazing and PAP on the AGB of each species.Important Findings Grazing,PAP and their interaction had significant effects on AGB.The effect of grazing on AGB was larger with higher precipitation and higher amounts of AGB(i.e.forage)while AGB was strongly limited in drought years,which resulted in a smaller grazing effect.The current year PAP had the highest impact(r=0.88,P<0.01)on AGB.The dominance of annual species was characterized by the amount of PAP in the current and preceding years:annuals dominated in wet years that followed consecutive dry years.The DCA Axis 1 reflected the variation of AGB with interannual variation of PAP while the DCA Axis 2 differentiated the grazing effect.The DCA scatter diagram based on species score illustrated that Artemisia adamsii(an unpalatable herb)was clearly linked to grazing disturbance whereas palatable perennials such as Agropyron cristatum,Stipa krylovii and Cleistogenes squarrosa were related to grazing abandonment and wetter conditions.In brief,number of livestock,hence the grazing impacts on vegetation dynamic in this region could have driven by forage availability,which is mainly controlled by current-year PAP. | Tserenpurev Bat-Oyun Masato Shinoda Yunxiang Cheng Yadamjav Purevdorj | 2016 | Journal of Plant Ecology2016,9,5: | 2 |
