1Retinal nerve fiber layer (RNFL) integrity and its relations to retinal microvasculature and microcirculation in myopic eyes显示文摘Background:The aim was to determine retinal nerve fiber layer function and its relations to retinal microvasculature and microcirculation in patients with myopia.Method:Polarization-sensitive optical coherence tomography(PS-OCT)was used to measure phase retardation per unit depth(PR/UD,proportional to the birefringence)of the retinal nerve fiber layer(RNFL).Optical coherence tomography angiography(OCTA)was used to measure macular vessel density analyzed using fractal analysis.In addition,a retinal function imager(RFI)was used to measure macular blood flow velocities in arterioles and venules.Twenty-two patients with moderate myopia(MM,refraction>3 and<6 diopters),seventeen patients with high myopia(HM,≥6 D)and 29 healthy control subjects(HC,≤3.00 D)were recruited.One eye of each patient was imaged.Results:Although the average PR/UD of the RNFL in the HM group did not reach a significant level,the birefringence of the inferior quadrant was significantly lower(P<0.05)in the HM group compared to the HC group.Significant thinning of the average RNFL and focal thinning of RFNL in temporal,superior and inferior quadrants in the HM group were found,compared to the HC group(P<0.05).There were no significant differences of retinal blood flow velocities in arterioles and venules among groups(P>0.05).The macular vessel density in both superficial and deep vascular plexuses was significantly lower in the HM group than in the other two groups(P<0.05)as well as in the MM group than in the HC group(P<0.05).The average PR/UD and PR/UD in the inferior quadrant were not related to refraction,axial length,blood flow velocities and macular vessel densities(r ranged from−0.09 to 0.19,P>0.05).Conclusions:The impairment of the retinal nerve fiber birefringence in the HM group may be one of the independent features in high myopic eyes,which appeared not to relate to macular microvascular density and blood flow velocity.Dongyi Qu Ying Lin Hong Jiang Yi Shao Yingying Shi Shriya Airen Giovanni Gregori Jianhua Wang 2018Eye and Vision2018,5,1:5

5Nutritional and medical food therapies for diabetic retinopathy显示文摘Diabetic retinopathy(DR)is a form of microangiopathy.Reducing oxidative stress in the mitochondria and cell membranes decreases ischemic injury and end-organ damage to the retina.New approaches are needed,which reduce the risk and improve the outcomes of DR while complementing current therapeutic approaches.Homocysteine(Hcy)elevation and oxidative stress are potential therapeutic targets in DR.Common genetic polymorphisms such as those of methylenetetrahydrofolate reductase(MTHFR),increase Hcy and DR risk and severity.Patients with DR have high incidences of deficiencies of crucial vitamins,minerals,and related compounds,which also lead to elevation of Hcy and oxidative stress.Addressing the effects of the MTHFR polymorphism and addressing comorbid deficiencies and insufficiencies reduce the impact and severity of the disease.This approach provides safe and simple strategies that support conventional care and improve outcomes.Suboptimal vitamin co-factor availability also impairs the release of neurotrophic and neuroprotective growth factors.Collectively,this accounts for variability in presentation and response of DR to conventional therapy.Fortunately,there are straightforward recommendations for addressing these issues and supporting traditional treatment plans.We have reviewed the literature for nutritional interventions that support conventional therapies to reduce disease risk and severity.Optimal combinations of vitamins B1,B2,B6,L-methylfolate,methylcobalamin(B12),C,D,natural vitamin E complex,lutein,zeaxanthin,alpha-lipoic acid,and n-acetylcysteine are identified for protecting the retina and choroid.Certain medical foods have been successfully used as therapy for retinopathy.Recommendations based on this review and our clinical experience are developed for clinicians to use to support conventional therapy for DR.DR from both type 1 diabetes mellitus(T1DM)and type 2 diabetes mellitus(T2DM)have similar retinal findings and responses to nutritional therapies.Ce Shi Peng Wang Shriya Airen Craig Brown Zhiping Liu Justin H.Townsend Jianhua Wang Hong Jiang 2020Eye and Vision2020,7,1:1
6Current status and future challenges with Coronavirus(COVID-19):an update on vaccines显示文摘SARS-COV-2 coronavirus causes COVID-19,which is a respiratory disease.COVID-19,the common cold,seasonal allergies,and the flu have many similar symptoms.COVID-19 is caused by SARS-CoV-2,while rhinoviruses most often cause the common cold.The World Health Organization issued notifications on December 30,2019,and January 30,2020,classifying this viral disease as a Public Health Emergency of International Concern.SARS-CoV-2 was detected in humans and animals in 2019.Several investigations are underway to cure COVID-19 and develop a vaccine to prevent infection with SARS-CoV-2.Several COVID-19 vaccines have now been approved or licensed for human use(SII/Covishield,AstraZeneca/AZD1222,Janssen/Ad26.COV 2.S,Sinopharm,Moderna,Sinovac-CoronaVac,Covaxin).Four issues arise in the research and manufacture of the COVID-19 vaccine:production,affordability,allocation and deployment.The adenovirus-vectored and mRNA-based vaccines for COVID-19 showed the highest efficacy after the first and second doses,respectively.The mRNA-based vaccines had higher side effects.Remarkably few experienced extreme adverse effects and all stimulated robust immune responses.Kumar Rakesh Shriya Gupta Anant Gupta 2023Life Research2023,6,1:0
7Acclimatization of microalgae Arthrospira platensis for treatment of heavy metals in Yamuna River显示文摘Bioaccumulation and biosorption in microalgae are effective approaches for the removal of heavy metals(HMs)from river water.The objective of this study was to investigate the potential for use of acclimatized microalgae in the removal of HMs from the Yamuna River water as an acclimatizing medium.An active culture of Arthrospira platensis(A.platensis)was acclimatized to HMs up to a concentration of 100 mg/L.It was gradually exposed to increasing concentrations of HMs in five subsequent batches with a step increase of 20 mg/L to acclimatize live cells in the simulated Yamuna River water.The presence of high levels of HMs in the Yamuna River water caused growth inhibition.An empirical growth inhibition model was developed,and it predicted high threshold concentrations of HMs(210.7e424.5 mg/L),producing a positive specific growth rate of A.platensis.A.platensis also showed high average removal efficiencies of HMs,including 74.0%for Cu,77.0%for Cd,50.5%for Ni,76.0%for Cr,76.5%for Pb,and 63.5%for Co,from HMs-enriched Yamuna River water.The findings demonstrated that the maximum specific removal amounts of Cu,Cd,Ni,Cr,Pb,and Co were 54.0,58.0,39.0,62.8,58.9,and 45.3 mg/g,respectively.The maximum yields of the value-added products chlorophyll and phycocyanin were 2.5 mg/g(in a batch of 40 mg/L for Cd)and 1054 mg/g(in a batch of 20 mg/L for Cu),respectively.Therefore,acclimatized A.platensis was proven to be a potential microalga not only for sequestration of HMs but also for production of valuable pigments.Nilesh Kumar Shriya Hans Ritu Verma Aradhana Srivastava 2020Water Science and Engineering2020,13,3:0
8General-motors diet: A quick fix for steatotic live liver donors显示文摘Background and aims:GM(General-Motors)diet is a low-calorie 7-day diet plan using complex carbohydrates used for quick weight loss.Dietary optimisation is known to decrease steatosis and improve outcomes in liver donors.We evaluated the efficacy and feasibility of using GM diet in steatotic live liver donors(LLD).Methods:LLDs who either had BMI>30 kg/m^(2),Computerised tomography liver attenuation index(CT-LAI)<0 HU,Magnetic resonance(MR)fat-fraction of>10%were started on GM diet for 1 week.Records of these LLDs and their recipients were retrieved retrospectively and analysed.Weight,liver function tests(LFT),MR fat-frac-tion,CT-LAI,hospital-stay and complications were analysed in LLDs while early graft dysfunction(EGD)and complications were assessed in recipients.Results:A total of 51 LLDs with mean age of 34.6?9.5 years with baseline mean weight of 80.6±10.8 kg and BMI of 29.1±2.78 kg/m^(2) were started on GM diet.After a week of following the GM diet,there was an average weight loss of 3.46±2.1 kg,a decrease in fat-fraction by 3.8%±2.7,and an improvement in CT-LAI by 6.7?3.7 HU.All patients could successfully undergo donor hepatectomy with an average postoperative ICU stay of 2.86±0.8 days and hospital stay of 6.82?0.81 days.No donor had steatosis>10% on intraoperative Tru-cut biopsy and their post-operative outcomes were similar to normal liver donors.There were intra-abdominal collections in 2 donors requiring drainage and one required re-exploration for intestinal obstruction.There was EGD in 8 recipients(15.6%)and mortality in 1(1.9%)recipients.Conclusion:GM diet is safe and effective regimen to reduce steatosis and increase healthy LLD pool without compromising donor or recipient safety.Anish Gupta Abhideep Chaudhary Gaurav Sood Niteen Kumar Imtiakum Jamir Aditya Shriya Vipin Pal Singh Rekha Subramaniyam Hitesh Soni Milind Mandwar 2023iLIVER2023,2,3:0

11Transient decrease in sound tolerance levels following hearing deprivation in normal-hearing subjects显示文摘Objective:To determine the circadian influence on sound sensitivity produced by temporal hearing deprivation in healthy normal human subjects.Design:Participants underwent bilateral earplugging before completion of anthropometry,the author's developed questionnaire,the Hamilton Anxiety and Depression Inventory,pure tone audiometry(PTA),stapedial reflex thresholds(SRT),distortion products otoacoustic emissions input/output(DPOAE-I/O),and uncomfortable loudness levels(ULLs).Afterward,the participants were randomly divided into group A,starting at 8:00 a.m.and finishing at 8:00 p.m.,and group B,starting at 4:00 p.m.and ending at 4:00 a.m.Serum cortisol levels and audiological test results were obtained at the beginning and end of the session and 24-h free urinary cortisol levels were measured.Study sample:Thirty healthy volunteers.Results:PTA was 2.68 and 3.33 dB HL in groups A and B,respectively,with no statistical difference between them.ULLs were significantly lower in group A compared to group B,with an average of 8.1 dB SPL in group A and 3.3 dB SPL in group B(p<0.0001).A SRT shift was observed in group A,with no difference in group B,and a night shift in DPOAE-I/O in group B.Conclusions:Reduced loudness tolerance is demonstrated during daytime hearing deprivation in contrast to nighttime;this may be due to increased central gain in the awake cortex.Eleazar Grateron Tricia Scaglione Shriya Airen Stefania Goncalves Sinay A.Ceballos David Baguley Juan A.Chiossone 2022Journal of Otology2022,17,4:0
12Cerebrospinal fluid liver pseudocyst:A bizarre long-term complication of ventriculoperitoneal shunt:A case report显示文摘BACKGROUND Ventriculoperitoneal(VP)shunt placement has become a standard of care procedure in managing hydrocephalus for drainage and absorption of cerebrospinal fluid(CSF)into the peritoneum.Abdominal pseudocysts containing CSF are the common long-term complication of this frequently performed procedure,mainly because VP shunts have significantly prolonged survival.Of these,liver CSF pseudocysts are rare entities that may cause shunt dysfunction,affect normal organ function,and therefore pose therapeutic challenges.CASE SUMMARY A 49-year-old man with history of congenital hydrocephalus status post bilateral VP shunt placement presented with progressively worsening dyspnea on exertion,abdominal discomfort/distention.Abdominal computed tomography(CT)scan revealed a large CSF pseudocyst in the right hepatic lobe with the tip of VP shunt catheter into the hepatic cyst cavity.Patient underwent robotic laparoscopic cyst fenestration with a partial hepatectomy,and repositioning of VP shunt catheter to the right lower quadrant of the abdomen.Follow-up CT demonstrated a significant reduction in hepatic CSF pseudocyst.CONCLUSION A high index of clinical suspicion is required for early detection of liver CSF pseudocysts since their presentation is often asymptomatic and cunning early in the course.Late-stage liver CSF pseudocysts could have adverse outcomes on the treatment course of hydrocephalus as well as on hepatobiliary dysfunction.There is paucity of data to define the management of liver CSF pseudocyst in current guidelines due to rare nature of this entity.The reported occurrences have been managed by laparotomy with debridement,paracentesis,radiological imaging guided fluid aspiration and laparoscopic-associated cyst fenestration.Robotic surgery is an additional minimally invasive option in the management of hepatic CSF pseudocyst;however,its use is limited by lack of widespread availability and cost of surgery.Muhammad Nadeem Yousaf Haider A Naqvi Shriya Kane Fizah S Chaudhary Jason Hawksworth Vikram V Nayar Thomas W Faust 2023World Journal of Hepatology2023,15,5:0
