| 1 | Evolving concepts in bone infection: redefining “biofilm”,“acute vs. chronic osteomyelitis”, “the immune proteome” and “local antibiotic therapy”显示文摘Osteomyelitis is a devastating disease caused by microbial infection of bone. While the frequency of infection following elective orthopedic surgery is low, rates of reinfection are disturbingly high. Staphylococcus aureus is responsible for the majority of chronic osteomyelitis cases and is often considered to be incurable due to bacterial persistence deep within bone. Unfortunately, there is no consensus on clinical classifications of osteomyelitis and the ensuing treatment algorithm. Given the high patient morbidity,mortality, and economic burden caused by osteomyelitis, it is important to elucidate mechanisms of bone infection to inform novel strategies for prevention and curative treatment. Recent discoveries in this field have identified three distinct reservoirs of bacterial biofilm including: Staphylococcal abscess communities in the local soft tissue and bone marrow, glycocalyx formation on implant hardware and necrotic tissue, and colonization of the osteocyte-lacuno canalicular network(OLCN) of cortical bone. In contrast, S.aureus intracellular persistence in bone cells has not been substantiated in vivo, which challenges this mode of chronic osteomyelitis. There have also been major advances in our understanding of the immune proteome against S. aureus, from clinical studies of serum antibodies and media enriched for newly synthesized antibodies(MENSA), which may provide new opportunities for osteomyelitis diagnosis, prognosis, and vaccine development. Finally, novel therapies such as antimicrobial implant coatings and antibiotic impregnated 3D-printed scaffolds represent promising strategies for preventing and managing this devastating disease. | 7 | Assessment of multi-modality evaluations of obscure gastrointestinal bleeding显示文摘AIM To determine the frequency of bleeding source detection in patients with obscure gastrointestinal bleeding(OGIB) who underwent double balloon enteroscopy(DBE) after pre-procedure imaging [multiphase computed tomography enterography(MPCTE), video capsule endoscopy(VCE), or both] and assess the impact of imaging on DBE diagnostic yield.METHODS Retrospective cohort study using a prospectively maintained database of all adult patients presenting with OGIB who underwent DBE from September 1^(st), 2002 to June 30^(th), 2013 at a single tertiary center.RESULTS Four hundred and ninety five patients(52% females; median age 68 years) underwent DBE for OGIB. AVCE and/or MPCTE performed within 1 year prior to DBE(in 441 patients) increased the diagnostic yield of DBE(67.1% with preceding imaging vs 59.5% without). Using DBE as the gold standard, VCE and MPCTE had a diagnostic yield of 72.7% and 32.5% respectively. There were no increased odds of finding a bleeding site at DBE compared to VCE(OR = 1.3, P = 0.150). There were increased odds of finding a bleeding site at DBE compared to MPCTE(OR = 5.9, P < 0.001). In inpatients with overt OGIB, diagnostic yield of DBE was not affected by preceding imaging.CONCLUSION DBE is a safe and well-tolerated procedure for the diagnosis and treatment of OGIB, with a diagnostic yield that may be increased after obtaining a preceding VCE or MPCTE. | 13 | Diagnostic role of fractional exhaled nitric oxide in pediatric eosinophilic esophagitis, relationship with gastric and duodenal eosinophils显示文摘BACKGROUND Eosinophilic esophagitis(EoE)is an eosinophilic-predominant inflammation of the esophagus diagnosed by upper endoscopy and biopsies.A non-invasive and cost-effective alternative for management of EoE is being researched.Previous studies assessing utility of fractional exhaled nitric oxide(FeNO)in EoE were low powered.None investigated the contribution of eosinophilic inflammation of the stomach and duodenum to FeNO.AIM To assess the utility of FeNO as a non-invasive biomarker of esophageal eosinophilic inflammation for monitoring disease activity.METHODS Patients aged 6-21 years undergoing scheduled upper endoscopy with biopsy for suspected EoE were recruited in our observational study.Patients on steroids and with persistent asthma requiring daily controller medication were excluded.FeNO measurements were obtained in duplicate using a chemiluminescence nitric oxide analyzer(NIOX MINO,Aerocrine,Inc.;Stockholm,Sweden)prior to endoscopy.Based on the esophageal peak eosinophil count(PEC)/high power field on biopsy,patients were classified as EoE(PEC≥15)or control(PEC≤14).Mean FeNO levels were correlated with presence or absence of EoE,eosinophil counts on esophageal biopsy,and abnormal downstream eosinophilia in the stomach(PEC≥10)and duodenum(PEC≥20).Wilcoxon rank-sum test,Spearman correlation,and logistic regression were used for analysis.P value<0.05 was considered significant.RESULTS We recruited a total of 134 patients,of which 45 were diagnosed with EoE by histopathology.The median interquartile range FeNO level was 17 parts per billion(11-37,range:7-81)in the EoE group and 12 parts per billion(8-19,range:5-71)in the control group.After adjusting for atopic diseases,EoE patients had significantly higher FeNO levels as compared to patients without EoE(Z=3.33,P<0.001).A weak yet statistically significant positive association was found between the number of esophageal eosinophils and FeNO levels(r=0.30,P<0.005).On subgroup analysis within the EoE cohort,higher FeNO levels were noted in patients with abnormal gastric(n=23,18 vs 15)and duodenal eosinophilia(n=28,21 vs 14);however,the difference was not statistically significant.CONCLUSION After ruling out atopy as possible confounder,we found significantly higher FeNO levels in the EoE cohort than in the control group. | Panamdeep Kaur Rachel Chevalier Craig Friesen Jamie Ryan Ashley Sherman Stephanie Page | 2023 | World Journal of Gastrointestinal Endoscopy2023,15,5: | 0 |

| 17 | Enhancing pedestrian mobility in Smart Cities using Big Data显示文摘Smart City is an emerging concept in global urban development.A Smart City applies ICT technologies to provide greater efficiencies for its urban areas and civilian population.One of the key requirements for a Smart City is to exploit data from its ICT infrastructure(such as Internet of Things connected sensors)to improve city services and features such as accessibility and sustainability.To address this requirement,the City of Melbourne(COM)Smart City office maintains several hundred data sets relating to urban activity and development.These datasets address parking,mobility,land use,3D data,statistics,environment,and major city developments such as rail projects.One promising dataset relates to pedestrian traffic.Data are obtained from sensors and updated on the COM website(City of Melbourne Open Data Platform:http://gffzzd1269692889842a8scux6bqv9fb956cqp.ffgz.tsg.suse.edu.cn/.)at regular intervals.These data include the number of pedestrians passing 53 specific locations in the central business district and also their times and directions of travel.In a 24 h period,over 650,000 pedestrians were counted passing all locations.Peak rates of several thousand pedestrians per minute are regularly recorded during city rush hours at hotspots making the data amenable to Big Data analysis techniques.Results are obtained in graphical format as heatmaps and charts of city pedestrian traffic using both Microsoft Excel^(■)for static analysis and PowerBI^(■)for more advanced interactive visualisation and analysis.These findings can identify pedestrian hotspots and inform future locations of traffic lights and street configurations to make the city more pedestrian friendly.Further,the experience gained can be used to examine other data sets such as bicycle traffic that can be analysed to inform city infrastructure projects.Future work is suggested that could link these pedestrian flow data with social media data from smartphones and potentially wearable devices such as fitness monitors to correlate pedestrian satisfaction with traffic flow.The‘happiness’effect of pedestrians passing through green areas such as city parks can also be quantified.This research was undertaken with the assistance of Swinburne University under its Capstone Project scheme. | Ebony Carter Patrick Adam Deon Tsakis Stephanie Shaw Richard Watson Peter Ryan | 2020 | Journal of Management Analytics2020,7,2: | 0 |
