Small sphincterotomy combined with endoscopic papillary large balloon dilation versus sphincterotomy显示文摘AIM:To compare small sphincterotomy combined with endoscopic papillary large balloon dilation(SES+ ELBD)and endoscopic sphincterotomy(EST)for large bile duct stones. METHODS:We compared prospectively SES+ELBD (group A,n=27)with conventional EST(group B, n=28)for the treatment of large bile duct stones(≥ 15 mm). When the stone could not be removed with a normal basket, mechanical lithotripsy was performed. We compared the rates of complete stone removal with one session and application of mechanical lithotripsy. RESULTS:No significant differences were observed in the mean largest stone size(A:20.8 mm,B:21.3 mm), bile duct diameter(A:21.4 mm,B:20.5 mm),number of stones(A:2.2,B:2.3),or procedure time(A:18 min, B:19 min)between the two groups. The rates of complete stone removal with one session was 85%in group A and 86%in group B(P=0.473).Mechanical lithotripsy was required for stone removal in nine of 27 patients(33%)in group A and nine of 28 patients (32%,P=0.527)in group B.CONCLUSION:SES+ELBD did not show significant benefits compared to conventional EST,especially for the removal of large(≥15 mm)bile duct stones. | Hyun Gun Kim Young Koog Cheon Young Deok Cho Jong Ho Moon Do Hyun Park Tae Hoon Lee Hyun Jong Choi Sang-Heum Park Joon Seong Lee Moon Sung Lee | 2009 | World Journal of Gastroenterology2009,15,34: | 34
Endoscopic removal of gastric ectopic pancreas:An initial experience with endoscopic submucosal dissection显示文摘AIM:To evaluate the therapeutic usefulness and safety of endoscopic resection in patients with gastric ectopic pancreas.METHODS:A total of eight patients with ectopic pancreas were included. All of them underwent endoscopic ultrasonography before endoscopic resection.Endo-scopic resection was performed by two methods:endo-scopic mucosal resection(EMR)by the injection-and-cut technique or endoscopic mucosal dissection(ESD).RESULTS:We planned to perform EMR in all eight cases but EMR was successful in only four cases. In the other four cases, saline spread into surrounding normal tissues and the lesions becameattened, which made it impossible to remove them by EMR.Inthose four cases,we performed ESD and removed the lesions without any complications.CONCLUSION:If conventional EMR is difficult to remove gastric ectopic pancreas,ESD is a feasible alternative method for successful removal. | Dong Yup Ryu Gwang Ha Kim Do Youn Park Bong Eun Lee Jae Hoon Cheong Dong Uk Kim Hyun Young Woo Jeong Heo Geun Am Song | 2010 | World Journal of Gastroenterology2010,16,36: | 24
Characterization of focal liver masses using acoustic radiation force impulse elastography显示文摘AIM:To investigate the diagnostic performance of acoustic radiation force impulse(ARFI) elastography for characterizing focal liver mass by quantifying their stiffness.METHODS:This prospective study included 62 patients with a focal liver mass that was well visualized on conventional ultrasonography performed in our institution from February 2011 to November 2011.Among them,12 patients were excluded for ARFI measurement failure due to a lesion that was smaller than the region of the interest and at an inaccessible location(deeper than 8 cm)(n = 7) or poor compliance to hold their breath as required(n = 5).Finally,50 patients with valid ARFI measurements were enrolled. If a patient had multiple liver masses, only one mass of interest was chosen. The masses were diagnosed by histological examination or clinical diagnostic criteria.During ultrasonographic evaluation, stiffness, expressed as velocity, was checked 10 times per focal liver mass and the surrounding liver parenchyma.RESULTS:After further excluding three masses that were non-diagnostic on biopsy,a total of 47 focal mass lesions were tested, including 39(83.0%) malignant masses [24 hepatocellular carcinomas(HCC),seven cholangiocellular carcinomas(CCC),and eight liver metastases] and eight(17.0%) benign masses(five hemangiomas and three focal nodular hyperplasias,FNH).Thirty-seven(74.0%) masses were confirmed by histological examination. The mean velocity was 2.48 m/s in HCCs,1.65 m/s in CCCs,2.35 m/s in metastases,1.83 m/s in hemangiomas, and 0.97 m/s in FNHs.Although considerable overlap was still noted between malignant and benign masses, significant differences in ARFI values were observed between malignant and benign masses(mean 2.31 m/s vs 1.51 m/s,P = 0.047),as well as between HCCs and benign masses(mean 2.48 m/s vs 1.51 m/s,P = 0.006).The areas under the receiver operating characteristics curves(AUROC) for discriminating the malignant masses from benign masses was 0.724(95%CI,0.566-0.883,P = 0.048),and the AUROC for discriminating HCCs from benign masses was 0.813(95%CI,0.649-0.976,P = 0.008).To maximize the sum of sensitivity and specificity,an ARFI value of 1.82 m/s was selected as the cutoff value to differentiate malignant from benign liver masses.Furthermore, the cutoff value for distinguishing HCCs from benign masses was also determined to be 1.82 m/s.The diagnostic performance of the sum of the ARFI values for focal liver masses and the surrounding liver parenchyma to differentiate liver masses improved(AUROC = 0.853;95%CI,0.745-0.960;P = 0.002 in malignant liver masses vs benign ones and AUROC = 0.948;95%CI,0.896-0.992,P < 0.001 in HCCs vs benign masses).CONCLUSION:ARFI elastography provides additional information for the differential diagnosis of liver masses. However, our results should be interpreted in clinical context, because considerable overlap in ARFI values existed among liver masses. | Hana Park Jun Yong Park Do Young Kim Sang Hoon Ahn Chae Yoon Chon Kwang-Hyub Han Seung Up Kim | 2013 | World Journal of Gastroenterology2013,19,2: | 22
Management of the complications of endoscopic submucosal dissection显示文摘Endoscopic submucosal dissection(ESD) is currently widely accepted as a standard treatment option for early gastrointestinal neoplasms in Korea. However,ESD has technical difficulties and a longer procedure time than conventional endoscopic resection. So it may have a higher risk of complications than conventional endoscopic resection techniques. We,the ESD study group of Korean Society of Gastrointestinal Endoscopy, have experienced many complications, mostly treated by endoscopic or conservative management. Here,we introduce and share our experiences for managementof post ESD complications and review published papers on the topic. | Seong Hwan Kim Jeong Seop Moon Young Hoon Youn Ki Myung Lee Sung Joon Lee | 2011 | World Journal of Gastroenterology2011,17,31: | 19
Management of entecavir-resistant chronic hepatitis B with adefovir-based combination therapies显示文摘AIM: To evaluate the long-term efficacy adefovir(ADV)-based combination therapies in entecavir(ETV)-resistant chronic hepatitis B(CHB) patients. METHODS: F i fty CHB pat ient s wi t h genotypic resistance to ETV at 13 medical centers in South Korea were included for the analysis. All the patients received rescue therapy with the combination of ADV plus ETV(ADV/ETV,n = 23) or ADV plus lamivudine(LMV)(ADV/LMV,n = 27) for more than 12 mo. Patients were monitored at least every 3-4 mo during ADV-based combination therapy by clinical examination as well as biochemical and virological assessments. Hepatitis B virus(HBV) DNA levels were measured by realtime PCR and logarithmically transformed for analysis. Cumulative rates of virologic response(VR; HBV DNA < 20 IU/m L) were calculated using the Kaplan-Meier method, and the difference was determined by a logrank test. Multivariate logistic regression and Cox proportional hazards models were used to identify independent risk factors significantly associated with short-term and long-term VR,respectively.RESULTS: Baseline median HBV DNA levels were 5.53(2.81-7.63) log10 IU/m L. The most commonly observed ETV genotypic mutation sites were rt184 and rt202. Patients were treated for a median of 27(12-45) mo. Overall, cumulative VR rates at 6,12,24,and 36 mo were 26%,36%,45%,and 68%,respectively. Patients treated with the ADV/ETV combination showed higher cumulative VR rates(35%,43%,65%,and 76%,respectively) than those with the ADV/LAM combination(18%,30%,30%,and 62%,respectively; P = 0.048). In the multivariate analysis, low baseline HBV DNA levels(< 5.2 log10 IU/m L) and initial virologic response at 3 mo(IVR-3; HBV DNA < 3.3 log10 IU/m L after 3 mo) were independent predictive factors for VR. Patients with favorable predictors achieved cumulative VR rates up to 90% at 36 mo. During the same period, the cumulative incidence of virologic breakthrough was as low as 6% in patients with the both favorable predictors.CONCLUSION: If tenofovir is not available,ADV/ETV combination could be considered in ETV-resistant patients with low HBV DNA titers, and may becontinued if IVR-3 is achieved. | Hyoung Su Kim Hyung Joon Yim Myoung Kuk Jang Ji Won Park Sang Jun Suh Yeon Seok Seo Ji Hoon Kim Bo Hyun Kim Sang Jong Park Sae Hwan Lee Sang Gyune Kim Young Seok Kim Jung Il Lee Jin-Woo Lee In Hee Kim Tae Yeob Kim Jin-Wook Kim Sook-Hyang Jeong Young Kul Jung Hana Park Seong Gyu Hwang | 2015 | World Journal of Gastroenterology2015,21,38: | 13
Hepatocellular carcinoma in Budd-Chiari syndrome:A single center experience with long-term follow-up in South Korea显示文摘AIM:To evaluate long-term clinical course of BuddChiari syndrome(BCS) and predictive factors associated with the development of hepatocellular carcinoma(HCC) and survival.METHODS:We analyzed 67 patients with BCS between June 1988 and May 2008.The diagnosis of BCS was confirmed by hepatic venous outflow obstruction shown on abdominal ultrasound sonography, computed tomography, magnetic resonance imaging,or venography. The median follow-up period was 103 ± 156 [interquartile range(IQR)] mo.RESULTS:The median age of the patients was 47 ± 16(IQR) years. At diagnosis,54 patients had cirrhosis,25(37.3%) Child-Pugh class A,23(34.3%) Child-Pugh class B,and six(9.0%) patients Child-Pugh class C.During the follow-up period,HCC was developed in 17 patients, and the annual incidence of HCC in patients with BCS was 2.8%.Patients in HCC group(n = 17) had higher hepatic venous pressure gradient(HVPG) than those in non-HCC group(n = 50)(21 ± 12 mmHg vs 14 ± 7 mmHg,P = 0.019).The survival rate of BCS patients was 86.2% for 5 years,73.8% for 10 years, and 61.2% for 15 years. In patients with BCS and HCC,survival was 79% for 5 years,43.1% for 10 years, and 21.5% for 15 years.CONCLUSION:The incidence of HCC in patients with BCS was similar to that in patients with other etiologic cirrhosis in South Korea. The HVPG is expected to provide additional information for predicting HCC development in BCS patients. | Hana Park Jin Young Yoon Kyeong Hye Park Do Young Kim Sang Hoon Ahn Kwang-Hyub Han Chae Yoon Chon Jun Yong Park | 2012 | World Journal of Gastroenterology2012,18,16: | 12
Genetically confirmed Wilson disease in a 9-month old boy with elevations of aminotransferases显示文摘Wilson disease (WD) is an autosomal recessive disorder of copper transport caused by alteration of the adenosine triphosphatase 7B gene. It is rare to diagnose WD below the age of three years. Molecular genetic testing is one of the most important diagnostic methods and may confirm the diagnosis in equivocal cases. We report a case of a 9-mo old boy with WD who presented as chronic hepatitis. | 8 | Clinicopathological features and prognosis of combined hepatocellular carcinoma and cholangiocarcinoma after surgery显示文摘BACKGROUND: Combined hepatocellular carcinoma and cholangiocarcinoma (cH CC-CC) is a rare subtype of primary liver cancer consisting of both hepatocellular carcinoma (HCC)and cholangiocarcinoma (CC). Because of the rarity of this tumor its feature is poorly understood. The present study aimed to evaluate the clinicopathological features and long-term prognosis of patients with cH CC-CC after surgery and to compare with those of the patients with stage-matched HCC and CC.METHODS: The clinicopathological features of the patients who underwent surgery for cH CC-CC at our center during the period of 2001-2010 were retrospectively analyzed and compared with those of stage-matched HCC and CC patients Cancer staging was performed according to the AJCC Cancer Staging Manual (6th ed.). Overall survival and disease-free survival were compared among the groups and prognostic factors of cH CC-CC were evaluated.RESULTS: Significant differences were observed in clinico pathological features among 42 patients with cH CC-CC, 90patients with HCC and 45 patients with CC. Similar to HCC patients, cH CC-CC patients had frequent hepatitis B virus antigen positivity, microscopic vessel invasion, cirrhosis and high level of serum alpha-fetoprotein. Similar to CC patients, cH CC-CC patients showed increased bile duct invasion and decreased capsule. The 1-, 3-, and 5-year overall survival and disease free survival of patients with cH CC-CC were not significantly different from those with stage-matched patients with CC;but significantly poorer than those with HCC. In subanalysis of patients with stage II, the overall survival in patients with cH CC-CC or CC was significantly poorer than that in patients , with HCC. We did not find the difference in patients with other stages. Univariate analysis of overall and disease-free survival of patients with cH CC-CC showed that the vascular invasion and intrahepatic metastasis were the significant predictive factors.CONCLUSION: Patients with cH CC-CC showed similar clinico- pathological features as those with HCC or CC, and patients with cH CC-CC or CC had a poorer prognosis compared with those with HCC, especially at matched stage II.. | Seung Duk Lee Sang-Jae Park Sung-Sik Han Seong Hoon Kim Young-Kyu Kim Soon-Ae Lee Young Hwan Ko Eun Kyung Hong | 2014 | Hepatobiliary & Pancreatic Diseases International2014,13,6: | 11
Tailoring strength-ductility balance of caliber-rolled AZ31 Mg alloy through subsequent annealing显示文摘Recently,multi-pass caliber rolling has been shown to be effective for Mg alloys. This study investigated the effect of subsequent annealing on the mechanical properties of a caliber-rolled AZ31 Mg alloy to modulate the strength-ductility relationship. This annealing gave rise to different trends in mechanical properties depending on the temperature regime.Low-temperature annealing(T≤473 K)exhibited a typical trade-off relationship, where an increase in annealing temperature resulted in increased ductility but decreased strength and hardness.Such a heat treatment did not degrade the high strength-ductility balance of the caliber-rolled alloy, suggesting that the mechanical properties could be tailored for different potential applications. In contrast,high-temperature annealing(T>473 K)caused a simultaneous deterioration in strength, hardness, and ductility with increasing annealing temperature. These differences are discussed in terms of the varying microstructural features under the different investigated annealing regimes. | Taein Kong Byung Je Kwak Jonghyun Kim Jeong Hun Lee Sung Hyuk Park Ji Hoon Kim Young Hoon Moon Hyun Sik Yoon Taekyung Lee | 2020 | Journal of Magnesium and Alloys2020,8,1: | 10

Efficacy of Helicobacter pylori eradication for the prevention of metachronous gastric cancer after endoscopic resection for early gastric cancer显示文摘Helicobacter pylori(H.pylori)plays an important role in gastric carcinogenesis,as the majority of gastric cancers develop from H.pylori-infected gastric mucosa. The rate of early gastric cancer diagnosis has increased in Japan and Korea, where H.pylori infection and gastric cancer are highly prevalent.Early intestinal-type gastric cancer without concomitant lymph node metastasis is usually treated by endoscopic resection.Secondary metachronous gastric cancers often develop because atrophic mucosa left untreated after endoscopic treatment confers a high risk of gastric cancer. The efficacy of H.pylori eradication for the prevention of metachronous gastric cancer remains controversial. However,in patients who undergo endoscopic resection of early gastric cancer,H.pylori eradication is recommended to suppress or delay metachronous gastric cancer.Careful and regularly scheduled endoscopy should be performed to detect minute metachronous gastric cancer after endoscopic resection. | Jae Young Jang Hoon Jai Chun | 2014 | World Journal of Gastroenterology2014,20,11: | 10

Optic radiation injury in patients with aneurismal subarachnoid hemorrhage: a preliminary diffusion tensor imaging report显示文摘Visual field defect is one of the various clinical manifestations in patients with subarachnoid hemorrhage(SAH). Little is known about the pathogenic mechanism of visual field defect in SAH. In the current study, we investigated the diffusion tensor imaging(DTI) finding of the optic radiation in patients with SAH following rupture of a cerebral artery aneurysm. We recruited 21 patients with aneurismal SAH(12 males, 9 females, mean age, 52.67 years; range, 41–68 years) who showed no definite lesion along the visual pathway. Twenty-one age-and sex-matched normal control subjects were also recruited. DTI data were acquired at an average of 5.9 weeks(range: 3–12 weeks) after onset and reconstruction of the optic radiation was performed using DTI-Studio software. The fractional anisotropy value, apparent diffusion coefficient value, and fiber number of the optic radiation were measured. The fractional anisotropy value of the optic radiation was significantly decreased, and the apparent diffusion coefficient value was significantly increased, in patients with aneurismal SAH than in normal control subjects. However, there was no significant difference in the fiber number of the optic radiation between patients with aneurismal SAH and normal control subjects. The decrement of fractional anisotropy value and increment of apparent diffusion coefficient value of the optic radiation in patients with aneurismal SAH suggest optic radiation injury. Therefore, we recommend a thorough evaluation for optic radiation injury in patient with aneurismal SAH. | Sung Ho Jang Chul Hoon Chang Young Jin Jung Seong Ho Kim Jeong Pyo Seo | 2018 | Neural Regeneration Research2018,13,3: | 9
Wetting Characteristics of Insect Wing Surfaces显示文摘Biological tiny structures have been observed on many kinds of surfaces such as lotus leaves, which have an effect on thecoloration of Morpho butterflies and enhance the hydrophobicity of natural surfaces. We investigated the micro-scale andnano-scale structures on the wing surfaces of insects and found that the hierarchical multiple roughness structures help in enhancingthe hydrophobicity. After examining 10 orders and 24 species of flying Pterygotan insects,we found that micro-scaleand nano-scale structures typically exist on both the upper and lower wing surfaces of flying insects. The tiny structures such asdenticle or setae on the insect wings enhance the hydrophobicity, thereby enabling the wings to be cleaned more easily. And thehydrophobic insect wings undergo a transition from Cassie to Wenzel states at pitch/size ratio of about 20.In order to examinethe wetting characteristics on a rough surface,a biomimetic surface with micro-scale pillars is fabricated on a silicon wafer, which exhibits the same behavior as the insect wing, with the Cassie-Wenzel transition occurring consistently around apitch/width value of 20. | Doyoung Byun Jongin Hong Saputra Jin Hwan Ko Young Jong Lee Hoon Cheol Park Bong-Kyu Byun Jennifer R.Lukes | 2009 | Journal of Bionic Engineering2009,6,1: | 9
Bowel preparations as quality indicators for colonoscopy显示文摘Colonoscopy is the principal investigative procedure for colorectal neoplasms because it can detect and remove most precancerous lesions. The effectiveness of colonoscopy depends on the quality of the examination.Bowel preparation is an essential part of high-quality colonoscopies because only an optimal colonic cleansing allows the colonoscopist to clearly view the entire colonic mucosa and to identify any polyps or other lesions.Suboptimal bowel preparation not only prolongs the overall procedure time, decreases the cecal intubation rate, and increases the costs associated with colonoscopy but also increases the risk of missing polyps or adenomas during the colonoscopy. Therefore,a repeat examination or a shorter colonoscopy follow-up interval may be suitable strategies for a patient with suboptimal bowel preparation. | Jae Young Jang Hoon Jai Chun | 2014 | World Journal of Gastroenterology2014,20,11: | 8
| 19 | Comments to young surgeons concerning laparoscopic spleenpreserving D2 lymph node dissection for advanced gastric cancer on the upper body显示文摘Qualified radical gastrectomy with lymph node dissection is very important to the prognosis of patients with gastric cancer. Now D2 lymph node dissection is standard procedure for gastric cancer surgery, and spleen hilar lymph node dissection is mandatory for gastric cancer in upper body. Because the anatomy of vessels in this area is very complicated, D2 lymph node dissection is technical challenging not only for open gastrectomy but also for laparoscopic one. Adapting a new technique is important to all surgeons, but we surgeons should always consider a patient's safety as the most important factor during surgery and that efforts should be based on scientific rationale with oncologic principles.
