1Geomorphological and environmental evolution in the upper reaches of the Yellow River during the late Cenozoic显示文摘Studies of the geomorphology and stratigraphy in the upper reaches of the Yellow River during the late Cenozoic demonstrate that the period of 30 - 3.4 Ma is characterized by a relatively stable tectonic setting, a persistently hot climate and a wide distribution of inland basins. This long-term stable environment resulted in a planation surface, the main surface on the Qinghai-Xizang (Qingzang) Plateau, on which red earth crust and karst arc formed. Since 3.4 Ma, the Qingzang Plateau was 'uplifted as a whole massif, accompanied by the dissection and disintegration of the main surface, termination of deposition and deformation of stratigraphy in red basins, increasing relief and commencing accumulation of inland molasse formation mainly of conglomerate. Subsequent strong uplift of the Qingzang Plateau at 2.5 Ma and 1.7-1.66 Ma led to cormation of the basic geomorphological configuration of the Qingzang Plateau and occurrence of the Yellow River. 2Risk factors for complications associated with upper gastrointestinal foreign bodies显示文摘AIM: To investigate predictive risk factors associated with complications in the endoscopic removal of foreign bodies from the upper gastrointestinal tract.METHODS: We retrospectively reviewed the medical records of 194 patients with a diagnosis of foreign body impaction in the upper gastrointestinal tract,confirmed by endoscopy,at two university hospital in South Korea.Patient demographic data,including age,gender,intention to ingestion,symptoms at admission,and comorbidities,were collected.Clinical features of the foreign bodies,such as type,size,sharpness of edges,number,and location,were analyzed.Endoscopic data those were analyzed included duration of foreign body impaction,duration of endoscopic performance,endoscopic device,days of hospitalization,complication rate,30-d mortality rate,and the number of operationsrelated to foreign body removal.RESULTS: The types of upper gastrointestinal foreign bodies included fish bones,drugs,shells,meat,metal,and animal bones.The locations of impacted foreign bodies were the upper esophagus(57.2%),mid esophagus(28.4%),stomach(10.8%),and lower esophagus(3.6%).The median size of the foreign bodies was 26.2 ± 16.7 mm.Among 194 patients,endoscopic removal was achieved in 189,and complications developed in 51 patients(26.9%).Significant complications associated with foreign body impaction and removal included deep lacerations with minor bleeding(n = 31,16%),ulcer(n = 11,5.7%),perforation(n = 3,1.5%),and abscess(n = 1,0.5%).Four patients underwent operations because of incomplete endoscopic foreign body extraction.In multivariate analyses,risk factors for endoscopic complications and failure were sharpness(HR = 2.48,95%CI: 1.07-5.72; P = 0.034) and a greater than 12-h duration of impaction(HR = 2.42,95%CI: 1.12-5.25,P = 0.025).CONCLUSION: In cases of longer than 12 h since foreign body ingestion or sharp-pointed objects,rapid endoscopic intervention should be provided in patients with ingested foreign bodies.Kyong Hee Hong Yoon Jae Kim Jae Hak Kim Song Wook Chun Hee Man Kim Jae Hee Cho 2015World Journal of Gastroenterology2015,21,26:35
3Portal hypertensive gastropathy:A systematic review of thepathophysiology,clinical presentation,natural history andtherapy显示文摘AIM: To describe the pathophysiology, clinical presentation, natural history, and therapy of portal hypertensive gastropathy(PHG) based on a systematic literature review.METHODS: Computerized search of the literature was performed via Pub Med using the following medical subject headings or keywords: 'portal' and 'gastropathy'; or 'portal' and 'hypertensive'; or 'congestive' and 'gastropathy'; or 'congestive' and 'gastroenteropathy'. The following criteria were applied for study inclusion: Publication in peer-reviewed journals, and publication since 1980. Articles were independently evaluated by each author and selected for inclusion by consensus after discussion based on the following criteria: Well-designed, prospective trials; recent studies; large study populations; and study emphasis on PHG. RESULTS: PHG is diagnosed by characteristic endoscopic findings of small polygonal areas of variable erythema surrounded by a pale, reticular border in a mosaic pattern in the gastric fundus/body in a patient with cirrhotic or non-cirrhotic portal hypertension. Histologic findings include capillary and venule dilatation, congestion, and tortuosity, without vascular fibrin thrombi or inflammatory cells in gastric submucosa. PHG is differentiated from gastric antral vascular ectasia by a different endoscopic appearance. The etiology of PHG is inadequately understood. Portal hypertension is necessary but insufficient to develop PHG because many patients have portal hypertension without PHG.PHG increases in frequency with more severe portal hypertension, advanced liver disease, longer liver disease duration, presence of esophageal varices, and endoscopic variceal obliteration. PHG pathogenesis is related to a hyperdynamic circulation, induced by portal hypertension, characterized by increased intrahepatic resistance to flow, increased splanchnic flow, increased total gastric flow, and most likely decreased gastric mucosal flow. Gastric mucosa in PHG shows increased susceptibility to gastrotoxic chemicals and poor wound healing. Nitrous oxide, free radicals, tumor necrosis factor-alpha, and glucagon may contribute to PHG development. Acute and chronic gastrointestinal bleeding are the only clinical complications. Bleeding is typically mild-to-moderate. Endoscopic therapy is rarely useful because the bleeding is typically diffuse. Acute bleeding is primarily treated with octreotide, often with concomitant proton pump inhibitor therapy, or secondarily treated with vasopressin or terlipressin. Nonselective β-adrenergic receptor antagonists, particularly propranolol, are used to prevent bleeding after an acute episode or for chronic bleeding. Iron deficiency anemia from chronic bleeding may require iron replacement therapy. Transjugular-intrahepaticportosystemic-shunt or liver transplantation is highly successful ultimate therapies because they reduce the underlying portal hypertension.CONCLUSION: PHG is important to recognize in patients with cirrhotic or non-cirrhotic portal hypertension because it can cause acute or chronic GI bleeding that often requires pharmacologic therapy.Mihajlo Gjeorgjievski Mitchell S Cappell 2016World Journal of Hepatology2016,8,4:38
4Seismic anisotropy of upper mantle in eastern Tibetan Plateau and related crust-mantle coupling pattern显示文摘By using the polarization analysis of teleseismic SKS waveform data recorded at 116 seismic stations which respectively involved in China National Digital Seismograph Network, and Yunnan, Sichuan, Gansu and Qinghai regional digital networks, and portable broadband seismic networks deployed in Sichuan, Yunnan and Tibet, we obtained the SKS fast-wave direction and the delay time between fast and slow waves of each station by use of the stacking analysis method, and finally acquired the fine image of upper mantle anisotropy in the eastern Tibetan Plateau and its adjacent regions. We analyzed the crust-mantle coupling deformation on the basis of combining the GPS observation results and the upper mantle anisotropy distribution in the study area. The Yunnan region out of the plateau has dif-ferent features of crust-mantle deformation from the inside plateau. There exists a lateral transitional zone of crust-mantle coupling in the eastern edge of the Tibetan Plateau, which is located in the region between 26° and 27°N in the west of Sichuan and Yunnan. To the south of transitional zone, the fast-wave direction is gradually turned from S60°―70°E in southwestern Yunnan to near EW in south-eastern Yunnan. To the north of transitional zone in northwestern Yunnan and the south of western Sichuan, the fast-wave direction is nearly NS. From crust to upper mantle, the geophysical parameters (e.g. the crustal thickness, the Bouguer gravity anomaly, and tectonic stress direction) show the feature of lateral variation in the transitional zone, although the fault trend on the ground surface is inconsis-tent with the fast-wave direction. 5Clinical features of gastroduodenal injury associated with long-term low-dose aspirin therapy显示文摘Low-dose aspirin(LDA) is clinically used for the prevention of cardiovascular and cerebrovascular events with the advent of an aging society.On the other hand,a very low dose of aspirin(10 mg daily) decreases the gastric mucosal prostaglandin levels and causes significant gastric mucosal damage.The incidence of LDAinduced gastrointestinal mucosal injury and bleeding has increased.It has been noticed that the incidence of LDA-induced gastrointestinal hemorrhage has increased more than that of non-aspirin non-steroidal anti-inflammatory drug(NSAID)-induced lesions.The pathogenesis related to inhibition of cyclooxygenase(COX)-1 includes reduced mucosal flow,reduced mucus and bicarbonate secretion,and impaired platelet aggregation.The pathogenesis related to inhibition of COX-2 involves reduced angiogenesis and increased leukocyte adherence.The pathogenic mechanisms related to direct epithelial damage are acid back diffusion and impaired platelet aggregation.The factors associated with an increased risk of upper gastrointestinal(GI) complications in subjects taking LDA are aspirin dose,history of ulcer or upper GI bleeding,age > 70 years,concomitant use of non-aspirin NSAIDs including COX-2-selective NSAIDs,and Helicobacter pylori(H.pylori) infection.Moreover,no significant differences have been found between ulcer and non-ulcer groups in the frequency and severity of symptoms such as nausea,acid regurgitation,heartburn,and bloating.It has been shown that the ratios of ulcers located in the body,fundus and cardia are significantly higher in bleeding patients than the ratio of gastroduodenal ulcers in patients taking LDA.Proton pump inhibitors reduce the risk of developing gastric and duodenal ulcers.In contrast to NSAIDinduced gastrointestinal ulcers,a well-tolerated histamine H2-receptor antagonist is reportedly effective in prevention of LDA-induced gastrointestinal ulcers.The eradication of H.pylori is equivalent to treatment with omeprazole in preventing recurrent bleeding.Continuous aspirin therapy for patients with gastrointestinal bleeding may increase the risk of recurrent bleeding but potentially reduces the mortality rates,as stopping aspirin therapy is associated with higher mortality rates.It is very important to prevent LDA-induced gastroduodenal ulcer complications including bleeding,and every effort should be exercised to prevent the bleeding complications.Junichi Iwamoto Yoshifumi Saito Akira Honda Yasushi Matsuzaki 2013World Journal of Gastroenterology2013,19,11:29
6Recent advances in endovascular techniques for management of acute nonvariceal upper gastrointestinal bleeding显示文摘Over the past two decades,transcatheter arterial embolization has become the first-line therapy for the management of upper gastrointestinal bleeding that is refractory to endoscopic hemostasis.Advances in catheter-based techniques and newer embolic agents, as well as recognition of the effectiveness of minimally invasive treatment options,have expanded the role of interventional radiology in the management of hemorrhage for a variety of indications,such as peptic ulcerbleeding,malignant disease,hemorrhagic Dieulafoy lesions and iatrogenic or trauma bleeding.Transcatheter interventions include the following:selective embolization of the feeding artery,sandwich coil occlusion of the gastroduodenal artery,blind or empiric embolization of the supposed bleeding vessel based on endoscopic findings and coil pseudoaneurysm or aneurysm embolization by three-dimensional sac packing with preservation of the parent artery.Transcatheter embolization is a fast,safe and effective,minimally invasive alternative to surgery when endoscopic treatment fails to control bleeding from the upper gastrointestinal tract.This article reviews the various transcatheter endovascular techniques and devices that are used in a variety of clinical scenarios for the management of hemorrhagic gastrointestinal emergencies.Romaric F Loffroy Basem A Abualsaud Ming D Lin Pramod P Rao 2011World Journal of Gastrointestinal Surgery2011,3,7:28
7Late Triassic sedimentary records in the northern Tethyan Himalaya:Tectonic link with Greater India显示文摘The Upper Triassic flysch sediments(Nieru Formation and Langjiexue Group)exposed in the Eastern Tethyan Himalayan Sequence are crucial for unraveling the controversial paleogeography and paleotectonics of the Himalayan orogen.This work reports new detrital zircon U-Pb ages and whole-rock geochemical data for clastic rocks from flysch strata in the Shannan area.The mineral modal composition data suggest that these units were mainly sourced from recycled orogen provenances.The chemical compositions of the sandstones in the strata are similar to the chemical composition of upper continental crust.These rocks have relatively low Chemical Index of Alteration values(with an average of 62)and Index of Compositional Variability values(0.69),indicating that they experienced weak weathering and were mainly derived from a mature source.The geochemical compositions of the Upper Triassic strata are similar to those of graywackes from continental island arcs and are indicative of an acidicintermediate igneous source.Furthermore,hornblende and feldspar experienced decomposition in the provenance,and the sediment became enriched in zircon and monazite during sediment transport.The detrital zircons in the strata feature two main age peaks at 225-275 Ma and 500-600 Ma,nearly continuous Paleoproterozoic to Neoproterozoic ages,and a broad inconspicuous cluster in the Tonian-Stenian(800-1200 Ma).The detrital zircons from the Upper Triassic sandstones in the study area lack peaks at 300-325 Ma(characteristic of the Lhasa block)and 1150-1200 Ma(characteristic of the Lhasa and West Australia blocks).Therefore,neither the Lhasa block nor the West Australia blocks likely acted as the main provenance of the Upper Triassic strata.Newly discovered Permian-Triassic basalt and mafic dikes in the Himalayas could have provided the 225-275 Ma detrital zircons.Therefore,Indian and Himalayan units were the main provenances of the flysch strata.The Tethyan Himalaya was part of the northern passive margin and was not an exotic terrane separated from India during the Permian to Early Cretaceous.This evidence suggests that the Neo-Tethyan ocean opened prior to the Late Triassic and that the Upper Triassic deposits were derived from continental crustal fragments adjacent to the northern passive continental margin of Greater India.Huawen Cao Yong Huang Guangming Li Linkui Zhang Jianyang Wu Lei Dong Zuowen Dai Liu Lu 2018Geoscience Frontiers2018,9,1:27
8Molecular fossils and oil-source rock correlations in Tarim Basin, NW China显示文摘The distribution of 'molecular fossils' (bio-markers) of steroid compounds in extracts from some spe-cific geologic age in the Tarim Basin have been analyzed andare used as the fingerprints for the oil-source rock correla-tion. Having been affected by maturation, migration, phasefractionation and biodegradation, not any molecular fossilsrelated to source and environment can be used as the finger-prints for oil-source rock correlation. Some special bio-markers widely existed in the extracts from Cambrian andOrdovician rocks in the Tarim Basin and showed obviousdifference in each stratum, including dinosteranes (C30),4-methyl-24-ethyl-cholestanes (C30) and their aromatizedsteroids, C24-norcholestanes and C28 steranes originated fromdinoflagellates and diatom. Few oils such as the heavy oildrilled in the Cambrian reservoir from Tadong 2 well (TD2)correlated well with the extracts from the Cambrian. 10Novel treatment options for perforations of the upper gastrointestinal tract:Endoscopic vacuum therapy and over-the-scope clips显示文摘Endoscopic management of leakages and perforations of the upper gastrointestinal tract has gained great importance as it avoids the morbidity and mortality of surgical intervention.In the past years,covered selfexpanding metal stents were the mainstay of endoscopic therapy.However,two new techniques are now available that enlarge the possibilities of defect closure:endoscopic vacuum therapy(EVT),and over-the-scope clip(OTSC).EVT is performed by mounting a polyurethane sponge on a gastric tube and placing it into the leakage.Continuous suction is applied via the tube resulting in effective drainage of the cavity and the induction of wound healing,comparable to the application of vacuum therapy in cutaneous wounds.The system is changed every 3-5 d.The overall success rate of EVT in the literature ranges from 84%to 100%,with a mean of 90%;only few complications have been reported.OTSCs are loaded on a transparent cap which is mounted on the tip of a standard endoscope.By bringing the edges of the perforation into the cap,by suction or by dedicated devices,such as anchor or twin grasper,the OTSC can be placed to close the perforation.For acute endoscopy associated perforations,the mean success rate is 90%(range:70%-100%).For other types of perforations(postoperative,other chronic leaks and fistulas)success rates are somewhat lower(68%,and59%,respectively).Only few complications have been reported.Although first reports are promising,further studies are needed to define the exact role of EVT and OTSC in treatment algorithms of upper gastrointestinal perforations.Rudolf Mennigen Norbert Senninger Mike G Laukoetter 2014World Journal of Gastroenterology2014,20,24:25
11S-wave crustal and upper mantle's velocity structure in the eastern Tibetan Plateau——Deep environment of lower crustal flow显示文摘A teleseismic profile consisting of 26 stations was deployed along 30°N latitude in the eastern Tibetan Plateau. By use of the inversion of P-wave receiver function, the S-wave velocity structures at depth from surface to 80 km beneath the profile have been determined. The inversion results reveal that there is significant lateral variation of the crustal structure between the tectonic blocks on the profile. From Linzhi north of the eastern Himalayan Syntaxis, the crust is gradually thickened in NE direction; the crustal thickness reaches to the maximum value (~72 km) at the Bangong-Nujiang suture, and then decreased to 65 km in the Qiangtang block, to 57―64 km in the Bayan Har block, and to 40―45 km in the Sichuan Basin. The eastern segment of the teleseismic profile (to the east of Batang) coincides geographically with the Zhubalong-Zizhong deep seismic sounding profile carried out in 2000, and the S-wave velocity structure determined from receiver functions is consistent with the P-wave velocity structure obtained by deep seismic sounding in respect of the depths of Moho and major crustal interfaces. In the Qiangtang and the Bayan Har blocks, the lower velocity layer is widespread in the lower crust (at depth of 30―60 km) along the profile, while there is a normal velocity distribution in lower crust in the Sichuan Basin. On an average, the crustal velocity ratio (Poisson ratio) in tectonic blocks on the profile is 1.73 (σ = 0.247) in the Lhasa block, 1.78 (σ = 0.269) in the Banggong-Nujiang suture, 1.80 (σ = 0.275) in the Qiangtang block, 1.86 (σ = 0.294) in the Bayan Har blocks, and 1.77 (σ = 0.265) in the Yangtze block, respectively. The Qiangtang and the Bayan Har blocks are characterized by lower S-wave velocity anomaly in lower crust, complicated Moho transition, and higher crustal Poisson ratio, indicating that there is a hot and weak medium in lower crust. These are considered as the deep environment of lower crustal flow in the eastern Tibetan Plateau. 12Seismic anisotropy of upper mantle in eastern China显示文摘Based on the polarization analysis of teleseismic SKS waveform data recorded at 65 seismic stations which respectively involved in the permanent and temporary broadband seismograph networks deployed in eastern China, the SKS fast-wave direction and the delay time between the fast and slow shear waves at each station were determined by use of SC method and the stacking analysis method, and then the image of upper mantle anisotropy in eastern China was acquired. In the study region, from south to north, the fast-wave polarization directions are basically EW in South China, gradually clockwise rotate to NWW-SEE in North China, then to NW-SE in Northeast China. The delay time falls into the interval [0.41 s, 1.52 s]. Anisotropic characteristics in eastern China indicate that the upper mantle anisotropy is possibly caused by both the collision between the Indian and Eurasian Plates and the subduction from the Pacific and Philippine Sea Plates to the Eurasian Plate. The collision between two plates made the crust of western China thickening and uplifting and the material eastwards extruding, and then caused the upper mantle flow eastwards and southeastwards. The subduction of Pacific Plate and Philippine Sea Plate has resulted in the lithosphere and the asthenosphere deformation in eastern China, and made the alignment of upper mantle peridotite lattice parallel to the deformation direction. 13Three-dimensional thermal structure of the Chinese continental crust and upper mantle显示文摘We invert S-wave velocities for the 3D upper-mantle temperatures, in which the position with a temperature crossing the 1300℃ adiabat is corresponding to the top of the seismic low velocity zone. The temperatures down to the depth of 80 km are then calculated by solving steady-state thermal conduction equation with the constraints of the inverted upper-mantle temperatures and the surface temperatures, and then surface heat flows are calculated from the crustal temperatures. The misfit between the calculated and observed surface heat flow is smaller than 20% for most regions. The result shows that, at a depth of 25 km, the crustal temperature of eastern China (500―600℃) is higher than that of western China (<500℃). At a depth of 100 km, temperatures beneath eastern and southeastern China are higher than the adiabatic temperature of 1300℃, while that beneath west China is lower. The Tarim craton and the Sichuan basin show generally low temperature. At a depth of 150 km, temperatures beneath south China, eastern Yangtze craton, North China craton and around the Qiangtang terrane are higher than the adiabatic temperature of 1300℃, but is the lowest beneath the Sichuan basin and the regions near the Indian-Eurasian collision zone. 14Rockall score in predicting outcomes of elderly patients with acute upper gastrointestinal bleeding显示文摘AIM: To validate the clinical Rockall score in predicting outcomes (rebleeding, surgery and mortality) in elderly patients with acute upper gastrointestinal bleeding (AUGIB). METHODS: A retrospective analysis was undertaken in 341 patients admitted to the emergency room and Intensive Care Unit of Xuanwu Hospital of Capital Medical University with non-variceal upper gastrointestinal bleeding. The Rockall scores were calculated, and the association between clinical Rockall scores and patient outcomes (rebleeding, surgery and mortality) was assessed. Based on the Rockall scores, patients were divided into three risk categories: low risk ≤ 3, moderate risk 3-4, high risk ≥ 4, and the percentages of rebleeding/death/surgery in each risk category were compared. The area under the receiver operating characteristic (ROC) curve was calculated to assess the validity of the Rockall system in predicting rebleeding, surgery and mortality of patients with AUGIB. RESULTS: A positive linear correlation between clinical Rockall scores and patient outcomes in terms of rebleeding, surgery and mortality was observed (r =0.962, 0.955 and 0.946, respectively, P = 0.001). High clinical Rockall scores > 3 were associated with adverse outcomes (rebleeding, surgery and death). There was a significant correlation between high Rockall scores and the occurrence of rebleeding, surgery and mortality in the entire patient population (χ 2 = 49.29, 23.10 and 27.64, respectively, P = 0.001). For rebleeding, the area under the ROC curve was 0.788 (95%CI: 0.726-0.849, P = 0.001); For surgery, the area under the ROC curve was 0.752 (95%CI: 0.679-0.825, P = 0.001) and for mortality, the area under the ROC curve was 0.787 (95%CI: 0.716-0.859, P = 0.001). 15Effect of climatic change on snowmelt runoffs in mountainous regions of inland rivers in Northwestern China显示文摘Climatic change has significant impacts on snow cover in mid-latitude mountainous re- gions, in the meantime, spatial and temporal changes of snow cover and snowmelt runoffs are con- sidered as sensitive indicators for climatic change. In this study, the upper Heihe Watershed in the Qilian Mountains was selected as a typical area affected by snow cover and snowmelt runoffs in northwestern China. The changes in air temperatures, precipitation, snowfall and spring snowmelt runoffs were analyzed for the period from 1956 to 2001. The results indicate that climatic warming was apparent, particularly in January and February, but precipitation just fluctuated without a clear trend. The possible changes of snowmelt runoffs in the upper Heihe watershed in response to a warming of 4℃ were simulated using Snowmelt Runoff Model (SRM) based on the degree-day factor algorithm. The results of the simulation indicate that a forward shifting of snow melting season, an increase in water flows in earlier melting season, and a decline in flows in later melting season would occur under a 4℃ warming scenario.WANG Jian1 & LI Shuo2 1. 16Crust and upper mantle structure of the Ailao Shan-Red River fault zone and adjacent regions显示文摘Using arrival data of the body waves recorded by seismic stations, we reconstructed the velocity structure of the crust and upper mantle beneath the southeastern edge of the Tibetan Plateau and the northwestern continental margin of the South China Sea through a travel time tomography technique. The result revealed the apparent tectonic variation along the Ailao Shan-Red River fault zone and its adjacent regions. High velocities are observed in the upper and middle crust beneath the Ailao Shan-Red River fault zone and they reflect the character of the fast uplifting and cooling of the metamorphic belt after the ductile shearing of the fault zone, while low velocities in the lower crust and near the Moho imply a relatively active crust-mantle boundary beneath the fault zone. On the west of the fault zone, the large-scale low velocities in the uppermost mantle beneath western Yunnan prove the influence of the mantle heat flow on volcano, hot spring and magma activities, however, the upper mantle on the east of the fault zone shows a relatively stable structure similar to the Yangtze block. 17Endoscopy dissection of small stromal tumors emerged from the muscularis propria in the upper gastrointestinal tract:Preliminary study显示文摘AIM:To investigate the feasibility and safety of the treatment of an upper gastrointestinal(GI) submucosal tumor with endoscopic submucosal dissection(ESD).METHODS:A total of 20 patients with esophageal and gastric submucosal tumors emerged from the muscular layer identified by endoscopic ultrasonography were collected from January 2009 to June 2010.Extramural or dumbbell-like lesions were excluded by an enhanced computerized tomography(CT) scan.All patients had intravenous anesthesia with propofol and then underwent the ESD procedure to resect these submucosal tumors.The incision was closed by clips as much as possible to decrease complications,such as bleeding or perforation,after resection of the tumor.All the specimens were collected and evaluated by hematoxylin,eosin and immunohistochemical staining,with antibodies against CD117,CD34,desmin,α-smooth muscle actin and vimentin to identify the characteristics of the tumors.Fletch's criteria was used to evaluate the risk of gastrointestinal stromal tumors(GISTs).All patients underwent a follow-up endoscopy at 3,6 and 12 mo and CT scan at 6 and 12 mo.RESULTS:The study group consisted of 5 men and 15 women aged 45-73 years,with a mean age of 60.2 years.Three tumors were located in the esophagus,9 in the gastric corpus,4 in the gastric fundus,3 lesions in the gastric antrum and 1 in the gastric angulus.Apart from the one case in the gastric angulus which was abandoned due to being deeply located in the serosa,94.7%(18/19) achieved complete gross dissection by ESD with operation duration of 60.52±30.32 min.The average maximum diameter of tumor was 14.8±7.6 mm,with a range of 6 to 30 mm,and another lesion was ligated by an endoscopic ligator after most of the lesion was dissected.After pathological and immunohistochemical analysis,12 tumors were identified as a GI stromal tumor and 6 were leiomyoma.Mitotic count of all 12 GIST lesions was fewer than 5 per 50 HPF and all lesions were at very low(9/12,75.0%) or low risk(3/12,25.0%) according to Fletch's criteria.Procedure complications mainly included perforation and GI bleeding;perforation occurred in 1 patient and conservative treatment succeeded by a suturing clip and no post-operative GI bleeding occurred.All patients were followed up for 6.5±1.8 mo(range,3-12 mo) by endoscopy and abdominal CT.Local recurrence and metastasis did not occur in any patient.CONCLUSION:ESD shows promise as a safe and feasible technique to resect esophageal and gastric submucosal tumors and the incidence of complications was very low.Clinical studies with more subjects and longer follow-up are needed to confirm its treatment value.Zhi-Gang Huang Xue-Song Zhang Shi-Liang Huang XiaoGang Yuan 2012World Journal of Gastrointestinal Endoscopy2012,4,12:18

19Shear wave velocity structure of the crust and upper mantle underneath the Tianshan orogenic belt显示文摘From April, 2003 to September, 2004, a passive broadband seismic array consisting of 60 stations was deployed over the Tianshan orogenic belt by State Key Laboratory of Earthquake Dynamics, Institute of Geology, China Earthquake Administration. Among them, 51 stations make up an about 500-km-long profile across the Tianshan Mountains from Kuytun to Kuqa. The receiver function profile and S-wave velocity structure of the crust and upper mantle down to 100 km deep are obtained by using the re-ceiver function method (Liu et al. 1996, 2000). The main results can be summarized as follows: (1) A clear mountain root does not exist beneath the Tianshan Mountains, and the crust-mantle boundaries underneath the stations mostly have transitional structures. This implies that the material differentia-tion between the crust and mantle is not yet accomplished and the orogenic process is still going on. (2) The crust beneath the Tianshan Mountains has laterally blocked structures in direction perpendicular to the mountain strike, and the crust-mantle boundary has a clear dislocation structure. Both of them correspond to each other. (3) The offsets of the Moho discontinuity are highly correlated to the tectonic borders on the surface and that corresponding to the frontal southern Tianshan fault reaches to 14 km. This manifests that large vertical divergent movement took place between different blocks. This sup-ports the discontinuous model of the Tianshan orogeny, and the Tarim block subduction is restricted only to the southern side of the South Tianshan. (4) Inside the upper and middle crust of the Tianshan Mountains exist several low-velocity bodies correlated with high seismicity located on the moun-tain-basin jointures on both sides of the mountain and between different blocks, and the low-velocity bodies on the mountain-basin jointures are inclined obviously to the mountain. 20Seismic anisotropy of upper mantle in Sichuan and adjacent regions显示文摘Based on the polarization analysis of teleseismic SKS waveform data recorded at 94 broadband seis-mic stations in Sichuan and adjacent regions, the SKS fast-wave direction and the delay time between the fast and slow shear waves were determined at each station using the grid searching method of minimum transverse energy and the stacking analysis method, and the image of upper mantle anisot-ropy was acquired. The fast-wave polarization directions are mainly NW-SE in the study area, NWW-SEE to its northeast and NS to its west. The delay time falls into the interval [0.47 s, 1.68 s]. The spatial variation of the fast-wave directions is similar to the variation of GPS velocity directions. The anisotropic image indicates that the regional tectonic stress field has resulted in deformation and flow of upper mantle material, and made the alignment of upper mantle peridotite lattice parallel to the di-rection of material deformation. The crust-upper mantle deformation in Sichuan and adjacent regions accords with the mode of vertically coherent deformation. In the eastern Tibetan Plateau, the crustal material was extruded to east or southeast due to SE traction force of the upper mantle material. The extrusion might be obstructed by a rigid block under the Sichuan Basin and the crust has been de-formed.
