| 2 | Semistructured interviewing in primary care research:a balance of relationship and rigour显示文摘Semistructured in-depth interviews are commonly used in qualitative research and are the most frequent qualitative data source in health services research.This method typically consists of a dialogue between researcher and participant,guided by a flexible interview protocol and supplemented by follow-up questions,probes and comments.The method allows the researcher to collect open-ended data,to explore participant thoughts,feelings and beliefs about a particular topic and to delve deeply into personal and sometimes sensitive issues.The purpose of this article was to identify and describe the essential skills to designing and conducting semistructured interviews in family medicine and primary care research settings.We reviewed the literature on semistructured interviewing to identify key skills and components for using this method in family medicine and primary care research settings.Overall,semistructured interviewing requires both a relational focus and practice in the skills of facilitation.Skills include:(1)determining the purpose and scope of the study;(2)identifying participants;(3)considering ethical issues;(4)planning logistical aspects;(5)developing the interview guide;(6)establishing trust and rapport;(7)conducting the interview;(8)memoing and reflection;(9)analysing the data;(10)demonstrating the trustworthiness of the research;and(11)presenting findings in a paper or report.Semistructured interviews provide an effective and feasible research method for family physicians to conduct in primary care research settings.Researchers using semistructured interviews for data collection should take on a relational focus and consider the skills of interviewing to ensure quality.Semistructured interviewing can be a powerful tool for family physicians,primary care providers and other health services researchers to use to understand the thoughts,beliefs and experiences of individuals.Despite the utility,semistructured interviews can be intimidating and challenging for researchers not familiar with qualitative approaches.In order to elucidate this method,we provide practical guidance for researchers,including novice researchers and those with few resources,to use semistructured interviewing as a data collection strategy.We provide recommendations for the essential steps to follow in order to best implement semistructured interviews in family medicine and primary care research settings. | Melissa DeJonckheere Lisa M Vaughn | 2019 | Family Medicine and Community Health2019，7，2： | 8 |
| 3 | Risk for emerging bipolar disorder, variants, and symptoms in children with attention deficit hyperactivity disorder, now grown up显示文摘AIM: To determine the prevalence of bipolar disorder(BD) and sub-threshold symptoms in children with attention deficit hyperactivity disorder(ADHD) through 14 years' follow-up, when participants were between 21-24 years old.METHODS: First, we examined rates of BD type Ⅰ?and Ⅱ diagnoses in youth participating in the NIMH-funded Multimodal Treatment Study of ADHD(MTA). We used the diagnostic interview schedule for children(DISC), administered to both parents(DISC-P) and youth(DISCY). We compared the MTA study subjects with ADHD(n = 579) to a local normative comparison group(LNCG, n = 289) at 4 different assessment points: 6, 8, 12, and 14 years of follow-ups. To evaluate the bipolar variants, we compared total symptom counts(TSC) of DSM manic and hypomanic symptoms that were generated by DISC in ADHD and LNCG subjects. Then we sub-divided the TSC into pathognomonic manic(PM) and non-specific manic(NSM) symptoms. We compared the PM and NSM in ADHD and LNCG at each assessment point and over time. We also evaluated the irritability as category A2 manic symptom in both groups and over time. Finally, we studied the irritability symptom in correlation with PM and NSM in ADHD and LNCG subjects.RESULTS: DISC-generated BD diagnosis did not differ significantly in rates between ADHD(1.89%) and LNCG 1.38%). Interestingly, no participant met BD diagnosis more than once in the 4 assessment points in 14 years. However, on the symptom level, ADHD subjects reported significantly higher mean TSC scores: ADHD 3.0; LNCG 1.7; P < 0.001. ADHD status was associated with higher mean NSM: ADHD 2.0 vs LNCG 1.1; P < 0.0001. Also, ADHD subjects had higher PM symptoms than LNCG, with PM means over all time points of 1.3 ADHD; 0.9 LNCG; P = 0.0001. Examining both NSM and PM, ADHD status associated with greater NSM than PM. However, Over 14 years, the NSM symptoms declined and changed to PM over time(df 3, 2523; F = 20.1; P < 0.0001). Finally, Irritability(BD DSM criterion-A2) rates were significantly higher in ADHD than LNCG(χ2 = 122.2, P < 0.0001), but irritability was associated more strongly with NSM than PM(df 3, 2538; F = 43.2; P < 0.0001).CONCLUSION: Individuals with ADHD do not appear to be at significantly greater risk for developing BD, but do show higher rates of BD symptoms, especially NSM. | 15 | Psychiatric disorders associated with intimate partner violence and sexual violence in Thai women:Aresult from the Thai National Mental Health Survey显示文摘Background Intimate partner violence(IPV) and sexual violence(SV) occur commonly and are a violation of basic human rights. There are limited studies to date that examine the impact of IPV, SV and mental health outcomes in Thailand.Aims The objective of the present study was to estimate the prevalence of intimate partner physical violence and IPV in Thai women and the association between these forms of violence and psychiatric disorders.Methods The present study used data from a national cross-sectional, population-based, household design survey. This study analysed data from 3009 female respondents above the age of 18 who were interviewed in person using the World Mental Health-Composite International Diagnostic Interview V.3.0(WMH-CIDI 3.0). We estimated the lifetime and the 12-month period prevalence of IPV and SV, the lifetime and the past 12-month period correlation of IPV/SV with psychiatric disorders and the OR for psychiatric disorders associated with these types of violence.Results There was only 5.2 % of the weighted sample that reported experiencing some form of violence, including reported rates of intimate partner physical violence of 3.5% and IPV of 2.0%. Women who had experienced IPV have a lifetime prevalence for common psychiatric disorders of 28.9%, for suicidal behaviours of 12.2% and for substance use disorders of 8.8%. Women who had experienced SV have a lifetime diagnoses for common psychiatric disorders of 21.4%, for suicidal behaviours of 16.5% and for substance use disorders of 19.4%.
