| 1 | Cryptogenic multifocal ulcerous stenosing enteritis: Radiologic features and clinical behavior显示文摘AIM To investigate the characteristic radiologic findings of cryptogenic multifocal ulcerous stenosing enteritis(CMUSE) which can be differentiated from other similar bowel disease and to assess their clinical behavior.METHODS Twenty pathologically and clinically confirmed CMUSE patients(males:females = 8:12; mean age: 40.4 years) between March 2002 and August 2015 from seven academic centers in South Korea were retrospectively reviewed. We evaluated small bowel series(SBS; n = 25), computed tomography(CT) enterography(n = 21), magnetic resonance(MR) enterography(n = 2), and abdominopelvic CT(n = 18) images, focusing on enteric and perienteric manifestations. Any change in radiologic features during followup period was recorded. We evaluated clinical data including presenting symptoms, laboratory finding and presence of relapse from electronic medical records. Histopathologic findings were also evaluated. RESULTS The main symptoms were abdominal pain(n = 12) and anemia(n = 10). All patients showed small bowel strictures(n = 52, mean: 2.6 per patient) on initial CT/MR, located in the ileum(n = 47) or jejunum(n = 5). Strictures showed short-length(mean: 10.44 mm) and circumferential bowel wall thickening(mean: 5.56 mm) with layered enhancement(n = 48) that were also noted on initial SBS(n = 36) with shallow ulcers(n = 10). Some ulcerative lesions or wall thickening progressed into strictures on follow-up SBS/CT, and some strictures revealed recurrent ulceration on followup SBS. There were no penetrating disease features like fistula or abscess and no gastrointestinal tract involvement except the small bowel. Nine patients experienced disease recurrence(median relapse-free period: 32 mo) even post-operatively. Histopathologic features of surgically resected specimens were characterized as multiple superficial ulcerations confined to mucosa or submucosa and multiple strictures.CONCLUSION Under characteristic radiologic findings with multiple short-segmental strictures and/or shallow ulcers of the small intestine, CMUSE should be considered when assessing patients with recurrent abdominal pain and anemia. | Jiyoung Hwang Jin Sil Kim Ah Young Kim Joon Seok Lim Se Hyung Kim Min Ju Kim Mi Sung Kim Kyoung Doo Song Ji Young Woo | 2017 | World Journal of Gastroenterology2017,23,25: | 4 |

| 9 | Effect of the glyceryl monooleate-based lyotropic phases on skin permeation using in vitro diffusion and skin imaging显示文摘Glyceryl monooleate(GMO)is a polar lipid that can exist in various liquid crystalline phases in the presence of different amounts of water. It is regarded as a permeation enhancer due to its amphiphilic property.Various phases of GMO/solvent system containing sodium fluorescein were prepared to compare permeability using confocal laser scanning microscopy(CLSM).GMO was melted in a vial in a water bath heated to 45℃.Propylene glycol and hexanediol were homogeneously dissolved in the melted GMO.Sodium fluorescein in aqueous solution was diluted to various ratios and thoroughly mixed by an ultrasonic homogenizer. Each GMO/Solvent system with fluorescein was applied onto the epidermal side of excised pig skin and incubated overnight.CLSM was performed to observe how the GMO/solvent system in its different phases affect skin permeability.Cubic and lamellar phase formulations enhanced the fluorescein permeation through the stratum corneum.A solution system had the weakest permeability compared to the other two phases.Due to the amphiphilic nature of GMO,cubic and lamellar phases might reduce the barrier function of stratum corneum which was observed by CLSM as fluorescein accumulated in the dermis.Based on the results, the glyceryl monooleate lyotropic mixtures could be applied to enhance skin permeation in various topical and transdermal formulations. | Dae Gon Lim Won-Wook Jeong Nam Ah Kim Jun Yeul Lim Seol-Hoon Lee Woo Sun Shim Nae-Gyu Kang Seong Hoon Jeong | 2014 | Asian Journal of Pharmaceutical Sciences2014,9,6: | 0 |
| 10 | Pancreatic pseudocystocolonic fistula treated without surgical or endoscopic intervention显示文摘We report here a case of pancreatic pseudocystocolic fistula that was treated without surgical or endoscopic intervention.A 76-year-old woman, presenting with a fever and epigastric pain, was referred to our institution.Three months prior to this admission, the patient had been admitted to the hospital for acute pancreatitis.Abdominal computerized tomography(CT)revealed a 9 cm pseudocyst containing air,and a fistular opening was observed via colonoscopy. After colonoscopy, the abdominal pain was slightly improved, the fever subsided and laboratory results showed decreased C-reactive protein levels. The observed improvement was likely due to the cleansing of the bowel, which induced spontaneous drainage from the pseudocyst into the colon.Antibiotic therapy was administered and daily bowel cleansing was performed using a polyethylene glycol solution. After three weeks,a follow-up CT revealed that the size of the pseudocyst had decreased significantly from 9 to 5.3 cm.In addition, laboratory tests were improved. The patient was able to resume a normal diet and was discharged in good overall health from the hospital, without aggravation of the symptoms.A colonoscopy performed 3 mo later and a follow-up CT performed 6 mo later confirmed that both the fistula and pseudocyst had completely disappeared. | Jae Choon Kwon Bo Yean Kim Ah Lim Kim Tae Hoon Kim Myung Il Park Ho Jin Jung Jang Hwan Lim Jae Kwon Jung Hyun Soo Kim Dong Wook Lee | 2014 | World Journal of Gastroenterology2014,20,7: | 0 |
| 11 | Optic perineuritis as an initial presentation of ocular toxoplasmosis:a case report显示文摘Dear Editor,Ocular toxoplasmosis, caused by Toxoplasma gondii infection,is one of the most common causes of posterior uveitis worldwide[1].It typically manifests as white focal retinitis with overlying vitreous inflammation that results in a chorioretinal scar and associated scotoma[2].However,the diagnosis of ocular toxoplasmosis can be challenging when it presents as an isolated papillitis without other typical signs of retinochoroiditis or vitritis[2].Although optic nerve involvement in ocular toxoplasmosis was present in 5.3%of cases, isolated papillitis was extremely rare(3 eyes out of 926 patients)and was only considered a presumed diagnosis in such cases given the presence of old toxoplasmic retinochoroiditis lesions[3].Few reports have described optic nerve involvement preceding toxoplasmic retinochoroiditis, while perineuritis,an uncommon form of orbital inflammatory disease involving the optic nerve sheath, has not been reported at all[4-5].Herein,we describe a case of ocular toxoplasmosis that initially presented with optic perineuritis followed by typical retinochoroidal inflammation. To our knowledge, this is the first case report of ocular toxoplasmosis with perineuritis in an immunocompetent individual. | Seung Ah Chung Chungwoon Kim Jong Yoon Lim Yoo-Ri Chung | 2022 | International Journal of Ophthalmology(English edition)2022,15,12: | 0 |
