| 1 | Endoscopic features of gastro-intestinal lymphomas: From diagnosis to follow-up显示文摘Many progresses have been done in the management of gastrointestinal(GI) lymphomas during last decades, especially after the discovery of Helicobacter pylori-dependent lymphoma development. The stepwise implementation of new endoscopic techniques, by means of echoendoscopy or double-balloon enteroscopy, enabled us to more precisely describe the endoscopic features of GI lymphomas with substantial contribution in patient management and in tailoring the treatment strategy with organ preserving approaches. In this review, we describe the recent progresses in GI lymphoma management from disease diagnosis to follow-up with a specific focus on the endoscopic presentation according to the involved site and the lymphoma subtype. Additionally, new or emerging endoscopic technologies that have an impact on the management of gastrointestinal lymphomas are reported. We here discuss the two most common subtypes of GI lymphomas: the mucosaassociated lymphoid tissue and the diffuse large B cell lymphoma. | 2 | An eighteen-month follow-up study on the effects of Intravitreal Dexamethasone Implant in diabetic macular edema refractory to anti-VEGF therapy显示文摘AIM: To evaluate the long-term efficacy and safety of dexamethasone implants in subjects affected by diabetic macular edema(DME) resistant to anti-vascular endothelial growth factor(VEGF) therapy.METHODS: Thirty-two DME patients were enrolled.A700 microgram slow release Intravitreal Dexamethasone Implant(Ozurdex~) was placed in the vitreous cavity.All patients were followed for 18 mo.Best-corrected visual acuity(BCVA) measured with Early Treatment Diabetic Retinopathy Study(ETDRS) and central macular thickness(CMT) exams were carried out at baseline(T0)and after 1(T1),3(T3),4(T4),6(T6),9(T9),12(T12),15(T15),and 18mo(T18) post injection. RESULTS: Repeated measures ANOVA showed an effect of treatment on ETDRS(P<0.0001).Post hoc analyses revealed that ETDRS values were significantly increased at T1,T3,T4,T9,and T15(P <0.001) as compared to baseline value(T0).At T6,T12,and T18,ETDRS values were still statistically higher than baseline(P<0.001 vs T0).However,at these time points,we observed a trend to return to baseline conditions.ANOVA also showed an effect of treatment(P <0.0001).CMT decreased significantly at T1,T3,T4,T9,and T15(P<0.001).At T6(P<0.01),T12 and T18(P<0.001) CMT was also significantly lower than T0 although a trend to return to the baseline conditions was also observed.CONCLUSION: Our findings demonstrate that Intravitreal Dexamethasone Implant is a good option to improveBCVA and CMT in DME patients resistant to anti-VEGF therapy.Our data also show that the use of drugs administered directly into the vitreous allows achieving appropriate and long-lasting concentration at the site of disease without systemic side effects. | Fernanda Pacella Maria Rosaria Romano Paolo Turchetti Giovanna Tarquini Anna Carnovale Antonella Mollicone Alessandra Mastromatteo Elena Pacella | 2016 | International Journal of Ophthalmology(English edition)2016,9,10: | 5 |
| 3 | Rare gastrointestinal lymphomas: the endoscopic investigation显示文摘Gastrointestinal lymphomas represent up to 10%of gastrointestinal malignancies and about one third of nonHodgkin lymphomas.The most prominent histologies are mucosa-associated lymphoid tissue lymphoma and diffuse large B-cell lymphoma.However,the gastrointestinal tract can be the site of rarer lymphoma subtypes as a primary or secondary localization.Due to their rarity and the multifaceted histology,an endoscopic classification has not been validated yet.This review aims to analyze the endoscopic presentation of rare gastrointestinal lymphomas from disease diagnosis to follow-up,according to the involved site and lymphoma subtype.Existing,new and emerging endoscopic technologies have been examined.In particular,we investigated the diagnostic,prognostic and follow-up endoscopic features of T-cell and natural killer lymphomas,lymphomatous polyposis and mantle cell lymphoma,follicular lymphoma,plasma cell related disease,gastrointestinal lymphomas in immunodeficiency and Hodgkin’s lymphoma of the gastrointestinal tract.Contrarily to more frequent gastrointestinal lymphomas,data about rare lymphomas are mostly extracted from case series and case reports.Due to the data paucity,a synergism between gastroenterologists and hematologists is required inorder to better manage the disease.Indeed,clinical and prognostic features are different from nodal and extranodal or the bone marrow(in case of plasma cell disease)counterpart.Therefore,the approach should be based on the knowledge of the peculiar behavior and natural history of disease. | Calogero Vetro Giacomo Bonanno Giorgio Giulietti Alessandra Romano Concetta Conticello Annalisa Chiarenza Paolo Spina Francesco Coppolino Rosario Cunsolo Francesco Di Raimondo | 2015 | World Journal of Gastrointestinal Endoscopy2015,7,10: | 4 |

| 11 | Wing morphology covaries with migration distance in a highly aerial insectivorous songbird显示文摘According to classical prediction of aerodynamic theory,birds and other powered fliers that migrate over long distances should have longer and more pointed wings than those that migrate less.However,the association between wing morphology and migratory behavior can be masked by contrasting selective pressures related to foraging behavior,habitat selection and predator avoidance,possibly at the cost of lower flight energetic efficiency.We studied the handwing morphology of Eurasian barn swallows Hirundo rustica from four populations representing a migration distance gradient.This species is an aerial insectivore,so it flies extensively while foraging,and may migrate during the day using a‘fly-and-forage’migration strategy.Prolonged foraging flights may reinforce the effects of migration distance on flight morphology.We found that two wings’aerodynamic properties—isometric handwing length and pointedness,both favoring energetically efficient flight,were more pronounced in barn swallows from populations undertaking longer seasonal migrations compared to less migratory populations.Our result contrast with two recent interspecific comparative studies that either reported no relationship or reported a negative relationship between pointedness and the degree of migratory behavior in hirundines.Our results may thus contribute to confirming the universality of the rule that longer migrations are associated with more pointed wings. | Piotr Matyjasiak Cosme Lopez-Calderon Roberto Ambrosini Javier Balbontin Alessandra Costanzo Yosef Kiat Andrea Romano Diego Rubolini | 2023 | Current Zoology2023,69,3: | 0 |
| 12 | Bevacizumab maintenance in metastatic colorectal cancer: How long?显示文摘The management of patients with non-progressive metastatic colorectal cancer after six months of treatment has not yet been codified. The most relevant concerns are the effectiveness of maintenance versus discontinuation, and the tolerability of prolonged treatment. Here we report the case of a 72-year-old man affected by colorectal cancer with lung metastases who achieved a complete response after receiving capecitabine, oxaliplatin and bevacizumab for six months, and bevacizumab alone for six months. Bevacizumab was continued as maintenance regimen for more than three years. It was discontinued because of an arthroplasty. Fifty-eight months after beginning first-line treatment, the patientremains free from relapse.
