| 1 | Minimally invasive surgical treatment of intrahepatic cholangiocarcinoma:A systematic review显示文摘BACKGROUND Intrahepatic cholangiocarcinoma(ICC)is the second most common primary liver cancer and is characterized by an aggressive behavior and a dismal prognosis.Radical surgical resection represents the only potentially curative treatment.Despite the increasing acceptance of laparoscopic liver resection for surgical treatment of malignant liver diseases,its use for ICC is not commonly performed.In fact,to achieve surgical free margins a major resection and/or vascular and/or biliary reconstructions is often needed,as well as an associated lymph node dissection.AIM To review and summarize the current evidences on the minimally invasive resection of ICC.METHODS A systematic review of the literature based on the criteria predetermined by the investigators was performed from the 1st of January 2009 up to the 1st of January2021 in 4 databases(PubMed,Scopus,Google Scholar,and Cochrane databases).All retrospective and prospective studies reporting on the comparative outcomes of open vs minimally invasive treatment of ICC were included.An evaluation of manuscripts quality was achieved using Methodological Index for Non-Randomized Studies criteria and Newcastle-Ottawa Scale.RESULTS After a systematic search 9 studies fulfilled the inclusion criteria.Among the all 3012 included patients,2450 were operated by an open approach and 562 by a minimally invasive(laparoscopic)approach.Baseline characteristics,tumor characteristics,surgical outcomes and oncological outcomes were collected and analyzed,highlighting values with a statistical significant difference between patients treated with open or laparoscopic approach.Shorter hospital stay and lower intraoperative blood losses were reported by some Authors in minimally invasive surgery,on the contrary,in the open group there was a higher number of lymphadenectomies and a higher percentage of major hepatectomies.CONCLUSION Minimally invasive resection of ICC has some short-term benefits and it is safe and feasible only in selected centers with a high experience in laparoscopic approach for liver surgery.Minimally invasive surgery,actually,was considered mainly in patients with a tumor with a diameter<5 cm,without invasion of main biliary duct or main vessel and no vascular or biliary reconstructions were planned.Further studies are needed to elucidate its impact on long term oncologic outcomes. | Renato Patrone Francesco Izzo Raffaele Palaia Vincenza Granata Guglielmo Nasti Alessandro Ottaiano Gilda Pasta Andrea Belli | 2021 | World Journal of Gastrointestinal Oncology2021,13,12: | 3 |

| 4 | Unilateral post-chemotherapy robot-assisted retroperitoneal lymph node dissection in Stage II non-seminomatous germ cell tumor:A tertiary care experience显示文摘Objective Post-chemotherapy retroperitoneal lymph node dissection(PC-RPLND)represents an integral component of the management of patients with non-seminomatous germ cell tumor(NSGCT).Modified templates have been proposed to minimize the surgical morbidity of the procedure.Moreover,the implementation of robotic surgery in this setting has been explored.We report our experience with unilateral post-chemotherapy robot-assisted retroperitoneal lymph node dissection(PC-rRPLND)for clinical Stages IIA and IIB NSGCTs.Methods A retrospective single institution review was performed including 33 patients undergoing PC-rRPLND for Stages IIA and IIB NSGCTs between January 2015 and February 2019.Following orchiectomy,patients were scheduled for chemotherapy with three cycles of bleomycin-etoposide-cisplatin.Patients with a residual tumor of<5 cm and an ipsilateral metastatic disease on pre-and post-chemotherapy CT scans were eligible for a unilateral template in absence of rising tumor markers.Descriptive statistics were provided for demographics,clinical characteristics,intraoperative and postoperative parameters.Perioperative,oncological,and functional outcomes were recorded.Results Overall,7(21.2%)patients exhibited necrosis or fibrosis;14(42.4%)had mature teratoma;and 12(36.4%)had viable tumor at final histology.The median lymph node size at surgery was 25(interquartile range[IQR]21-36)mm.Median operative time was 180(IQR 165-215)min and no major postoperative complications were observed.Anterograde ejaculation was preserved in 75.8%of patients.Median follow-up was 26(IQR 19-30)months and a total of three recurrences were recorded.Conclusion PC-rRPLND is a reliable and technically reproducible procedure with safe oncological outcomes and acceptable postoperative ejaculatory function in well selected patients with NSGCTs. | Dario Franzese Antonio Tufano Alessandro Izzo Raffaele Muscariello Giovanni Grimaldi Giuseppe Quarto Luigi Castaldo Sabrina Rossetti Savio Domenico Pandolfo Sonia Desicato Paola Del Prete Matteo Ferro Sandro Pignata Sisto Perdonà | 2023 | Asian Journal of Urology2023,10,4: | 0 |
| 5 | Biomarkers for glioblastoma multiforme: status quo显示文摘Background:Glioblastoma(GBM)is the most frequent and most malignant central nervous system(CNS)tumor.GBM shows poor prognosis with a median overall survival of 14.6 months,despite current surgical and adjuvant therapies.O(6)-methylguanine-DNA methyltransferase(MGMT)methylation is the strongest molecular prognosticator for GBM with therapeutic implications in adjuvant treatment.Isocitrate dehydrogenase(IDH)mutation is the most recently introduced molecular marker and is important for the GBM classification because distinguishes primary(de novo)from secondary GBM.In the last two decades huge advances in the understanding of biopathological bases of gliomagenesis have been made but,to date,there is a lack of biopathological markers endowed of some prognostic and predictive value for GBM.Aim:In the present review we analyzed the role,as possible prognosticators,of epidermal growth factor receptor(EGFR)variant III(EGFRvIII),phosphatase and tensin homolog(PTEN)deletion and other alteration of the receptor tyrosine kinase(RTK)pathway,and vascular endothelial growth factor(VEGF)expression.We included in the review studies considering both the prognostic value and the predictive value for response to therapy of the above-mentioned biomarkers.Relevance for patients:These factors have a paramount importance in gliomagenesis and are potential targets for individualized therapies.EGFR can be targeted by tyrosine kinase inhibitors(TKIs).mTOR,whose activation is triggered by PTEN loss,is the target of rapalogs and VEGF is the target of the molecular antibody bevacizumab.Unfortunately,current evidence is insufficient to draw a definite prognostic/predictive role for these biomarkers in GBM.Further understanding the gliomagenesis pathways and looking for biomarkers endowed with translational relevance are necessary efforts in order to find the appropriate,tailored therapy for each specific GBM patient. | Nicola Montano Quintino Giorgio D’Alessandris Alessandro Izzo Eduardo Fernandez Roberto Pallini | 2016 | Journal of Clinical & Translational Research2016,2,1: | 0 |
| 6 | Observatory science with eXTP显示文摘In this White Paper we present the potential of the enhanced X-ray Timing and Polarimetry(eXTP) mission for studies related to Observatory Science targets. These include flaring stars, supernova remnants, accreting white dwarfs, low and high mass X-ray binaries, radio quiet and radio loud active galactic nuclei, tidal disruption events, and gamma-ray bursts. eXTP will be excellently suited to study one common aspect of these objects: their often transient nature.
