| 5 | Pediatric functional constipation treatment with Bifi dobacterium-containing yogurt:A crossover,double-blind,controlled trial显示文摘AIM: To evaluate the treatment of pediatric functional chronic intestinal constipation (FCIC) with a probiotic goat yogurt. METHODS: A crossover double-blind formula-controlled trial was carried out on 59 students (age range: 5-15 years) of a public school in Belo Horizonte, MG, Brazil, presenting a FCIC diagnostic, according to Roma Ⅲ criteria. The students were randomized in two groups to receive a goat yogurt supplemented with 109 colony forming unit/mL Bifidobacterium longum (B.longum) (probiotic) daily or only the yogurt for a period of 5 wk (formula). Afterwards, the groups were intercrossed for another 5 wk. Defecation frequency, stool consistency and abdominal and defecation pain were assessed.RESULTS: Both treatment groups demonstrated improvement in defecation frequency compared to baseline. However, the group treated with probiotic showed most signif icant improvement in the f irst phase of the study. An inversion was observed after crossing over, resulting in a reduction in stool frequency when this group was treated by formula. Probiotic and formula improved stool consistency in the f irst phase of treatment, but the improvement obtained with probiotic was significantly higher (P = 0.03). In the second phase of treatment, the group initially treated with probiotic showed worseningstool consistency when using formula. However, the difference was not signif icant. A signif icant improvement in abdominal pain and defecation pain was observed with both probiotic and formula in the first phase of treatment, but again the improvement was more signif icant for the group treated with B. longum during phase I (P < 0.05). | 7 | Evaluation of clinical relevance of examining K-ras, p16 and p53 mutations along with allelic losses at 9p and 18q in EUS-guided fine needle aspiration samples of patients with chronic pancreatitis and pancreatic cancer显示文摘AIM: To establish an optimum combination of molecular markers resulting in best overall diagnostic sensitivity and specificity for evaluation of suspicious pancreatic mass. METHODS: Endoscopic ultrasound (EUS)-guided fine needle aspiration cytology (FNA) was performed on 101 consecutive patients (63 males, 38 females, 60 ± 12 years; 81 with subsequently diagnosed pancreatic cancer, 20 with chronic pancreatitis) with focal pancreatic mass. Samples were evaluated on-site by an experienced cytopathologist. DNA was extracted from Giemsa stained cells selected by laser microdissection and the presence of K-ras, p53 and p16 somatic mutations was tested by cycling-gradient capillary electrophoresis (CGCE) and single-strand conformation polymorphism (SSCP) techniques. In addition, allelic losses of tumor suppressor genes p16 (INK4, CDKN2A) and DPC4 (MADH4, SMAD4) were detected by monitoring the loss of heterozygosity (LOH) at 9p and 18q, respectively. RESULTS: Sensitivity and specificity of EUS-guided FNA were 75% and 85%, positive and negative predictivevalue reached 100%. The remaining 26% samples were assigned as inconclusive. Testing of molecular markers revealed sensitivity and specificity of 70% and 100% for K-ras mutations (P < 0.001), 24% and 90% for p53 mutations (NS), 13% and 100% for p16 mutations (NS), 85% and 64% for allelic losses at 9p (P < 0.001) and 78% and 57% for allelic losses at 18q (P < 0.05). When tests for different molecular markers were combined, the best results were obtained with K-ras + LOH at 9p (92% and 64%, P < 0.001), K-ras + LOH at 18q (92% and 57%, P < 0.001), and K-ras + LOH 9q + LOH 18q (96% and 43%, P < 0.001). When the molecular markers were used as complements to FNA cytology to evaluate inconclusive samples only, the overall sensitivity of cancer detection was 100% in all patients enrolled in the study. CONCLUSION: EUS-guided FNA cytology combined with screening of K-ras mutations and allelic losses of tumor suppressors p16 and DPC4 represents a very sensitive approach in screening for pancreatic malignancy. | 11 | Direct-fed microbes: A tool for improving the utilization of low quality roughages in ruminants显示文摘For many years, ruminant nutritionists and microbiologists have been interested in manipulating the microbial ecosystem of the rumen to improve production efficiency of different ruminant species. Removal and restriction of antibiotics subtherapeutic uses from ruminant diets has amplified interest in improving nutrient utilization and animal performance and search for more safe alternatives. Some bacterial and fungal microorganisms as a direct-fed microbial(DFM) can be the most suitable solutions. Microorganisms that are commonly used in DFM for ruminants may be classified mainly as lactic acid producing bacteria(LAB), lactic acid utilizing bacteria(LUB), or other microorganism's species like Lactobacillus, Bifidobacterium, Enterococcus, Streptococcus, Bacillus, Propionibacterium, Megasphaera elsdenii and Prevotellabryantii, in addition to some fungal species of yeast such as Saccharomyces and Aspergillus. A definitive mode of action for bacterial or fungal DFM has not been established; although a variety of mechanisms have been suggested. Bacterial DFM potentially moderate rumen conditions, and improve weight gain and feed efficiency. Fungal DFM may reduce harmful oxygen from the rumen, prevent excess lactate production, increase feed digestibility, and alter rumen fermentation patterns. DFM may also compete with and inhibit the growth of pathogens, immune system modulation, and modulate microbial balance in the gastrointestinal tract. Improved dry matter intake, milk yield, fat corrected milk yield and milk fat content were obtained with DFM administration. However, the response to DFM is not constant; depending on dosages, feeding times and frequencies, and strains of DFM. | 12 | Efficacy and safety of tenofovir in chronic hepatitis B: Australian real world experience显示文摘AIM To evaluate the long-term treatment outcomes of tenofovir therapy in patients in a real world Australian tertiary care setting.METHODS We performed a retrospective analysis of treatment outcomes among treatment-na?ve and treatment-experienced patients receiving a minimum 3 mo tenofovir therapy through St Vincent's Hospital Melbourne, Australia. We included patients receiving tenofovir [tenofovir disoproxil fumarate(TDF)] monotherapy, as well as patients treated with TDF in combination with a second antiviral agent. Patients were excluded if they demonstrated human immune-deficiency virus/hepatitis C virus/hepatitis delta virus coinfection or were less than 18 years of age. We considered virological and biochemicalresponse, as well as safety outcomes. Virological response was determined by measurement of hepatitis B virus(HBV) DNA using sensitive assays; biochemical response was determined via serum liver function tests; histological response was determined from liver biopsy and fibroscan; safety analysis focused on glomerular renal function and bone mineral density. The primary efficacy endpoint was complete virological suppression over time, defined by HBV DNA < 20 IU/m L. Secondary efficacy endpoints included rates of biochemical response, and HB e antigen(HBe Ag)/HB surface antigen loss and seroconversion over time.RESULTS Ninety-two patients were identified who fulfilled the enrolment criteria. Median follow-up was 26 mo(range 3-114). Mean age was 46(24-78) years, 64(70%) were male and 77(84%) were of Asian origin. 55(60%) patients were treatment-na?ve and 62 patients(67%) were HBe Ag-negative. Complete virological suppression was achieved by 45/65(71%) patients at 12 mo, 37/46(80%) at 24 mo and 25/28(89%) at 36 mo. Partial virological response(HBV DNA 20-2000 IU/m L) was achieved by 89/92(96.7%) of patients. Multivariate analysis showed a significant relationship between virological suppression at end of follow-up and baseline HBV DNA level(OR = 0.897, 95%CI: 0.833-0.967, P = 0.0046) and HBe Ag positive status(OR = 0.373, 95%CI: 0.183-0.762, P = 0.0069). There was no difference in response comparing treatment-na?ve and treatment-experienced patients. Three episodes of virological breakthrough occurred in the setting of noncompliance. Tenofovir therapy was well tolerated.CONCLUSION Tenofovir is an efficacious, safe and well-tolerated treatment in an Australian real-world tertiary care setting. | 17 | Effects of cellulase and xylanase enzymes mixed with increasing doses of Salix babylonica extract on in vitro rumen gas production kinetics of a mixture of corn silage with concentrate显示文摘An in vitro gas production(GP) technique was used to investigate the effects of combining different doses of Salix babylonica extract(SB) with exogenous fibrolytic enzymes(EZ) based on xylanase(X) and cellulase(C), or their mixture(XC; 1:1 v/v) on in vitro fermentation characteristics of a total mixed ration of corn silage and concentrate mixture(50:50, w/w) as substrate. Four levels of SB(0, 0.6, 1.2 and 1.8 m L g–1 dry matter(DM)) and four supplemental styles of EZ(1 μL g–1 DM; control(no enzymes), X, C and XC(1:1, v/v) were used in a 4×4 factorial arrangement. In vitro GP(m L g–1 DM) were recorded at 2, 4, 6, 8, 10, 12, 24, 36, 48 and 72 h of incubation. After 72 h, the incubation process was stopped and supernatant p H was determined, and then filtered to determine dry matter degradability(DMD). Fermentation parameters, such as the 24 h gas yield(GY24), in vitro organic matter digestibility(OMD), metabolizable energy(ME), short chain fatty acid concentrations(SCFA), and microbial crude protein production(MCP) were also estimated. Results indicated that there was a SB?EZ interaction(P<0.0001) for the asymptotic gas production(b), the rate of gas production(c), GP from 6 to 72 h, GP2(P=0.0095), and GP4(P=0.02). The SB and different combination of enzymes supplementation influenced(P<0.001) in vitro GP parameters after 12 h of incubation; the highest doses of SB(i.e., 1.8 m L g–1 DM), in the absence of any EZ, quadratically increased(P<0.05) the initial delay before GP begins(L) and GP at different incubation times, with lowering b(quadratic effect, P<0.0001) and c(quadratic effect, P<0.0001; linear effect, P=0.0018). The GP was the lowest(P<0.05) when the highest SB level was combined with cellulose. There were SB?EZ interactions(P<0.001) for OMD, ME, the partitioning factor at 72 h of incubation(PF72), GY24, SCFA, MCP(P=0.0143), and p H(P=0.0008). The OMD, ME, GY24 and SCFA with supp lementation of SB extract at 1.8 m L g–1 DM were higher(P<0.001) than the other treatments, however,PF72 was lower(quadratic effect, P=0.0194) than the other levels. Both C and X had no effect(P>0.05) on OMD, p H, ME, GY24, SCFA and MP. The combination of SB with EZ increased(P<0.001) OMD, ME, SCFA, PF72 and GP24, whereas there was no impact on p H. Each mode has a pair of cyclonic-anticyclonic gyres. The difference is the location of the cyclonic gyre. The cyclonic gyre stretches from southeast to northwest in the first mode and from the southwest to the northeast in the second mode. The observed modes strongly affect the heat and salt transport in the North Atlantic. | 19 | P2X4受体对活化的小胶质细胞的调控作用显示文摘小胶质细胞是中枢神经系统内的免疫细胞,当脑组织出现损伤时,小胶质细胞被激活,对脑组织起到保护作用。在自身免疫性脑脊髓炎模型大鼠和视神经脊髓炎患者体内,均观察到在脊髓炎性病灶及小胶质细胞内,P2X4受体表达上调。本课题组进行在体和离体实验,用LPS活化小胶质细胞,观察P2X4受体在小胶质细胞炎性反应中的作用。膜片钳检测显示,在LPS激活的小胶质细胞内,P2X4受体活性增加。P2X4受体阻断剂可显著降低小胶质细胞的膜皱缩、TNFα的分泌、细胞形态的改变及LPS导致的小胶质细胞死亡。在体研究显示,LPS髓内注射后会诱发炎性反应,迅速导致小胶质细胞丢失;给予P2X4受体阻断剂可显著减少小胶质细胞的丢失,而P2X4受体激活剂则可显著增加小胶质细胞的丢失。海马齿状回的小胶质细胞特别容易被LPS诱导的炎症反应激活。注射LPS后2 h,位于海马齿状回的小胶质细胞即被激活,大约24 h后死亡,P2X4受体阻断剂可减少LPS诱导的小胶质细胞活化和死亡。上述数据提示,P2X4受体对于小胶质细胞的激活和存活有重要的调控作用。 | Vázquez-Villoldo N Domercq M Martín A Llop J Gómez-Vallejo V Matute C | 2014 | 神经损伤与功能重建2014,9,1: | 4 |
