| 1 | Tooth regeneration: a revolution in stomatology and evolution in regenerative medicine显示文摘A tooth is a complex biological organ and consists of multiple tissues including the enamel, dentin, cementum and pulp. Tooth loss is the most common organ failure. Can a tooth be regenerated? Can adult stem cells be orchestrated to regenerate tooth structures such as the enamel, dentin, cementum and dental pulp, or even an entire tooth? If not, what are the therapeutically viable sources of stem cells for tooth regeneration? Do stem cells necessarily need to be taken out of the body, and manipulated ex vivo before they are transplanted for tooth regeneration? How can regenerated teeth be economically competitive with dental implants? Would it be possible to make regenerated teeth affordable by a large segment of the population worldwide? This review article explores existing and visionary approaches that address some of the above-mentioned questions. Tooth regeneration represents a revolution in stomatology as a shift in the paradigm from repair to regeneration: repair is by metal or artificial materials whereas regeneration is by biological restoration. Tooth regeneration is an extension of the concepts in the broad field of regenerative medicine to restore a tissue defect to its original form and function by biological substitutes. | Sibel Yildirim Susan Y. | 2 | 3-Dimensional liver volume assessment in patients with hepatitis B virus-related liver cirrhosis during long-term oral nucleos(t)ide analogues therapy显示文摘AIM To assess the effect of long-term oral nucleos(t)ide analogues(NUCs) therapy on liver volume change in patients with suppress hepatitis B virus(HBV)-related liver cirrhosis.METHODS We reviewed the data of na?ve patients with HBVrelated liver cirrhosis, who had taken oral NUCs therapy, between 2003 and 2007 at Chonbuk University Hospital. We analyzed two consecutive sets of abdominal computerized tomography scans-one at the time oftreatment initiation and another at the second-year follow-up. Liver volume was calculated by 3-dimensional liver extraction volumetry program.RESULTS A total of 55 patients(34 males) were included. There was 114.3 m L ± 167.8 m L(12.9% ± 17.9%) of increase in liver volume during the two years of NUCs therapy(993.8 m L ± 242.8 m L at baseline vs 1108.1 m L ± 263.3 m L at two-year follow-up, P < 0.001). The ratio of the measured baseline liver volume to the estimated standard liver volume was improved from 70.8% to 78.0%. An increase in liver volume was shown not only in patients with compensated cirrhosis(P = 0.046) but also in those with decompensated cirrhosis(P < 0.001). Significant factors for volume increases were Child-Turcotte-Pugh grade and model for end-stage liver disease score improvement without virological breakthrough. | 3 | Assessment of scoring systems for acute-on-chronic liver failure at predicting short-term mortality in patients with alcoholic hepatitis显示文摘AIM To assess the performance of proposed scores specific for acute-on-chronic liver failure in predicting shortterm mortality among patients with alcoholic hepatitis.METHODS We retrospectively collected data from 264 patients with clinically diagnosed alcoholic hepatitis from January to December 2013 at 21 academic hospitals in Korea. The performance for predicting short-term mortality was calculated for Chronic Liver FailureSequential Organ Failure Assessment(CLIF-SOFA), CLIF Consortium Organ Failure score(CLIF-C OFs), Maddrey'sdiscriminant function(DF), age, bilirubin, international normalized ratio and creatinine score(ABIC), Glasgow Alcoholic Hepatitis Score(GAHS), model for end-stage liver disease(MELD), and MELD-Na.RESULTS Of 264 patients, 32(12%) patients died within 28 d. The area under receiver operating characteristic curve of CLIF-SOFA, CLIF-C OFs, DF, ABIC, GAHS, MELD, and MELD-Na was 0.86(0.81-0.90), 0.89(0.84-0.92), 0.79(0.74-0.84), 0.78(0.72-0.83), 0.81(0.76-0.86), 0.83(0.78-0.88), and 0.83(0.78-0.88), respectively, for 28-d mortality. The performance of CLIF-SOFA had no statistically significant differences for 28-d mortality. The performance of CLIF-C OFs was superior to that of DF, ABIC, and GAHS, while comparable to that of MELD and MELD-Na in predicting 28-d mortality. | 19 | Newly developed autoimmune cholangitis without relapse of autoimmune pancreatitis after discontinuing prednisolone显示文摘A 57-year-old man presented with a 2-wk history of painless jaundice and weight loss.He had a large illdefined enhancing mass-like lesion in the uncinate process of the pancreas with stricture of the distal common bile duct.Aspiration cytology of the pancreatic mass demonstrated inflammatory cells without evidence of malignancy.Total serum immunoglobulin G level was slightly elevated,but IgG4 level was normal.After the 2-wk 40 mg prednisolone trial,the patient's symptoms and bilirubin level improved significantly.A follow-up computed tomography(CT) scan showed a dramatic resolution of the pancreatic lesion.A low dose steroid was continued.After six months he self-discontinued prednisolone for 3 wk,and was presented with jaundice again.A CT scan showed newly developed intrahepatic biliary dilatation and marked concentric wall thickening of the common hepatic duct and the proximal common bile duct without pancreatic aggravation.The patient' s IgG4 level was elevated to 2.51 g/L.Prednisolone was started again,after which his serum bilirubin level became normal and the thickening of the bile duct was resolved.This case suggests that autoimmune pancreatitis can progress to other organs that are not involved at the initial diagnosis,even with sustained pancreatic remission. | Ji Hun Kim Jae Hyuck Chang Sung Min Nam Mi Jeong Lee Il Ho Maeng Jin Young Park Yun Sun Im Tae Ho Kim Chang Whan Kim Sok Won Han | 2012 | World Journal of Gastroenterology2012，18，41： | 1 |
