| 1 | Prevalence of gastroesophageal reflux symptoms in a large unselected general population in Japan显示文摘AIM:To examine the prevalence of gastroesophageal reflux disease (GERD) symptoms in a large unselected general population in Japan. METHODS: In Japan, mature adults are offered regular check-ups for the prevention of gastric cancer. A notice was sent by mail to all inhabitants aged > 40 years. A total of 160 983 Japanese (60 774 male, 100 209 female; mean age 61.9 years) who underwent a stomach check up were enrolled in this study. In addition, from these 160 983 subjects, we randomly selected a total of 82 894 (34 275 male, 48 619 female; mean age 62.4 years) to evaluate the prevalence of abdominal pain. The respective subjects were prospectively asked to complete questionnaires concerning the symptoms of heartburn, dysphagia, and abdominal pain for a 1 mo period. RESULTS: The respective prevalences of the symptoms in males and females were: heartburn, 15.8% vs 20.7%; dysphagia, 5.4% vs 7.8%; and abdominal pain, 6.6% vs 9.6%. Among these symptoms, heartburn was significantly high compared with the other symptoms, and the prevalence of heartburn was significantly more frequent in females than in males in the 60-89-year agegroup. Dysphagia was also significantly more frequent in female patients. | 2 | Sessile serrated adenoma detection rate is correlated with adenoma detection rate显示文摘AIM To investigated the association between adenoma detection rate(ADR) and sessile serrated ADR(SSADR) and significant predictors for sessile serrated adenomas(SSA) detection.METHODS This study is a retrospective, single-center analysis. Total colonoscopies performed by the gastroenterologists at the University of Tokyo Hospital between January and December 2014 were retrospectively identified. Polyps were classified as low-grade or high-grade adenoma, cancer, SSA, or SSA with cytological dysplasia, and the prevalence of each type of polyp was investigated. Predictors of adenoma and SSA detection were examined using logistic generalized estimating equation models. The association between ADR and SSADR for each gastroenterologist was investigated by calculating a correlation coefficient weighted by the number of each gastroenterologist's examination.RESULTS A total of 3691 colonoscopies performed by 35 gastroenterologists were assessed. Overall, 978 (26.5%) low-and 84 (2.2%) high-grade adenomas, 81 (2.2%) cancers, 66 (1.8%) SSAs, and 2 (0.1%) SSAs with cytological dysplasia were detected. Overall ADR was 29.5%(men 33.2%, women 23.8%) and overall SSADR was 1.8%(men 1.7%, women 2.1%). In addition, 672 low-grade adenomas (68.8% of all the detected lowgrade adenomas), 58 (69.9%) high-grade adenomas, 29 (34.5%) cancers, 52 (78.8%) SSAs, and 2 (100%) SSAs with cytological dysplasia were found in the proximal colon. Adenoma detection was the only significant predictor of SSA detection (adjusted OR: 2.53, 95%CI: 1.53-4.20; P < 0.001). The correlation coefficient between ADR and SSADR weighted by the number of each gastroenterologist's examinations was 0.606(P < 0.001).CONCLUSION Our results demonstrated that ADR is correlated to SSADR. | 10 | Novel diagnostics for aggravating pancreatic fistulas at the acute phase after pancreatectomy显示文摘AIM:To identify sensitive predictors of clinically relevant postoperative pancreatic fistula(POPF)at the acute phase after pancreatectomy.METHODS:This study included 153 patients diagnosed as having POPFs at postoperative day(POD)3after either open pancreatoduodenectomy or distal pancreatectomy between January 2008 and March 2013.The POPFs were categorized into three grades based on the International Study Group on Pancreatic Fistula Definition,and POPFs of grades B or C were considered to be clinically relevant.The predictive performance for the clinically relevant POPF formation of values at PODs 1,3 and 5 as well as time-dependent changes in levels of inflammatory markers,including white blood cell count,neutrophil count,total lymphocyte count,C-reactive protein(CRP),procalcitonin level,plateletto-lymphocyte ratio and neutrophil-to-lymphocyte ratio,and amylase content in the drain fluid were compared using the receiver operating characteristic(ROC)curve and multivariable analyses.A scoring system for the prediction of clinically relevant POPFs was created using five risk factors identified in this study,and its diagnostic performance was also evaluated.RESULTS:Over time,77(50%)of 153 enrolled patients followed a protracted course and were categorized as having clinically relevant POPFs.ROC curve analyses revealed that changes in CRP levels from POD1 to POD 3 had the greatest area under the curve value(0.767)and that an elevated CRP level of 28.4 mg/L yielded the most optimal predictive value for clinically relevant POPFs.Multivariable analyses for the risk factors of clinically relevant POPFs identified invasive carcinomas of the pancreas and elevation of the CRP level(≥28.4 mg/L,from POD 1 to POD 3)as independent diagnostic factors for clinically relevant POPFs(OR 2.94,95%CI:1.08-8.55,P=0.035 and OR 4.82,95%CI:1.25-20.2,P=0.022,respectively).A gradual increase in the prevalence of clinically relevant POPFs in proportion to the risk classification score was confirmed.A highly elevated CRP level and a risk score≥8 were significantly associated with a prolonged duration of drain placement and postoperative hospitalization.CONCLUSION:A steep rise in the serum CRP level from POD 1 to POD 3 was a highly predictive factor for subsequent clinically relevant POPFs. | Mitsuro Ka Tsutomu Fujii Hideki Takami Masaya Suenaga Yoshikuni Inokawa Suguru Yamada Daisuke Kobayashi Chie Tanaka Hiroyuki Sugimoto Masahiko Koike Shuji Nomoto Michitaka Fujiwara Yasuhiro Kodera | 2014 | World Journal of Gastroenterology2014,20,26: | 1 |
| 11 | Laparoscopic surgery for small-bowel obstruction caused by Meckel's diverticulum显示文摘A 26-year-old woman was referred to our hospital because of abdominal distention and vomiting. Contrastenhanced computed tomography showed a blind loop of the bowel extending to near the uterus and a fibrotic band connecting the mesentery to the top of the bowel,suggestive of Meckel's diverticulum(MD) and a mesodiverticular band(MDB). After intestinal decompression,elective laparoscopic surgery was carried out. Using three 5-mm ports,MD was dissected from the surrounding adhesion and MDB was divided intracorporeally. And subsequent Meckel's diverticulectomy was performed. The presence of heterotopic gastric mucosa was confirmed histologically. The patient had an uneventful postoperative course and was discharged 5 d after the operation. She has remained healthy and symptom-free during 4 years of follow-up. | 14 | Tongue-like Barrett's esophagus is associated with gastroesophageal reflux disease显示文摘AIM:To test this hypothesis of barrett esophagus (BE) classified into two types and to further determine if there was any correlation between the shape of endoscopically suspected esophageal metaplasia (ESEM), prevalence of reflux esophagitis (RE) and heartburn. METHODS: A total of 6504 Japanese who underwent endoscopy for their annual stomach check-up were enrolled in this study. BE was detected without histological confirmation that is ESEM. We originally classified cases of ESEM into 3 types based on its shape: Tongue-like (T type), Dome-like (D type) and Wave-like (W type) ESEM. The respective subjects were prospectively asked to complete questionnaires concerning the symptoms of heartburn, dysphagia, and abdominal pain for a one-month period. RESULTS: ESEM was observed in 10.3% of 6504 subjects (ESEM < 1 cm, 9.4%; 1 cm ≤ ESEM < 3 cm, 1.7%; ESEM ≥ 3 cm, 0.5%). The frequency of ESEM was significantly higher in males compared with female subjects. Statistical analysis showed that the prevalence of heartburn and RE were significantly higher in the T type ESEM than in the W type ESEM (P < 0.05).
