1Role of liver biopsy in hepatocellular carcinoma显示文摘The role of liver biopsy in the diagnosis of hepatocellular carcinoma(HCC)has been challenged over time by the ability of imaging techniques to characterize liver lesions in patients with known cirrhosis.In fact,in the diagnostic algorithm for this tumor,histology is currently relegated to controversial cases.Furthermore,the risk of complications,such as tumor seeding and bleeding,as well as inadequate sampling have further limited the use of liver biopsy for HCC management.However,there is growing evidence of prognostic and therapeutic information available from microscopic and molecular analysis of HCC and,as the information content of the tissue sample increases,the advantages of liver biopsy might modify the current risk/benefit ratio.We herein review the role and potentiality of liver biopsy in the diagnosis and management of HCC.As the potentiality of precision medicine comes to the management of HCC,it will be crucial to have rapid pathways to define prognosis,and even treatment,by identifying the patients who could most benefit from target-driven therapies.All of the above reasons suggest that the current role of liver biopsy in the management of HCC needs substantial reconsideration.Luca Di Tommaso Marco Spadaccini Matteo Donadon Nicola Personeni Abubaker Elamin Alessio Aghemo Ana Lleo 2019World Journal of Gastroenterology2019,25,40:11
2Effects of deep vertical rotary tillage on the grain yield and resource use efficiency of winter wheat in the Huang-Huai-Hai Plain of China显示文摘Tillage represents an important practice that is used to dynamically regulate soil properties,and affects the grain production process and resource use efficiency of crops.The objectives of this 3-year field study carried out in the Huang-Huai-Hai(HHH) Plain of China were to compare the effects of a new deep vertical rotary tillage (DVRT) with the conventional shallow rotary tillage (CT) on soil properties,winter wheat (Triticum aestivum L.) grain yield and water and nitrogen use efficiency at different productivity levels,and to identify a comprehensive management that optimizes both grain yield and resource use efficiency in the HHH Plain.A split-plot design was adopted in field experiments in the winter wheat growing seasons of 2016–2017 (S1),2017–2018 (S2) and 2018–2019 (S3),with DVRT (conducted once in June 2016) and CT performed in the main plots.Subplots were treated with one of four targeted productivity level treatments (SH,the super high productivity level;HH,the high productivity and high efficiency productivity level;FP,the farmer productivity level;ISP,the inherent soil productivity level).The results showed that the soil bulk density was reduced and the soil water content at the anthesis stage was increased in all three years,which were due to the significant effects of DVRT.Compared with CT,grain yields,partial factor productivity of nitrogen (PFP_(N)),and water use efficiency (WUE) under DVRT were increased by 22.0,14.5 and 19.0%.Path analysis and direct correlation decomposition uncovered that grain yield variation of winter wheat was mostly contributed by the spike numbers per area under different tillage modes.General line model analysis revealed that tillage mode played a significant role on grain yield,PFP_(N) and WUE not only as a single factor,but also along with other factors(year and productivity level) in interaction manners.In addition,PFP_(N) and WUE were the highest in HH under DVRT in all three growth seasons.These results provided a theoretical basis and technical support for coordinating the high yield with high resource use efficiency of winter wheat in the resource-restricted region in the HHH Plain of China.WU Fen ZHAI Li-chao XU Ping ZHANG Zheng-bin Elamin Hafiz BAILLO Lemessa Negasa TOLOSA Roy Njoroge KIMOTHO JIA Xiu-ling GUO Hai-qian 2021Journal of Integrative Agriculture2021,20,2:7
3Carbamazepine solubility enhancement in tandem with swellable polymer osmotic pump tablet: A promising approach for extended delivery of poorly water-soluble drugs显示文摘Elementary osmotic pump(EOP)is a unique extended release(ER)drug delivery system based on the principle of osmosis.It has the ability to minimize the amount of the drug,accumulation and fluctuation in drug level during chronic uses.Carbamazepine(CBZ),a poorly water-soluble antiepileptic drug,has serious side effects on overdoses and chronic uses.The aim of the present study was to design a new EOP tablet of CBZ containing a solubility enhancers and swellable polymer to reduce its side effects and enhance the patient compliance.Firstly,a combination of solubilizing carriers was selected to improve the dissolution of the slightly soluble drug.Then,designing the new EOP tablet and investigating the effect of different variables of core and coat formulations on drug release behavior by single parameter optimization and by Taguchi orthogonal design with analysis of variance(ANOVA),respectively.The results showed that CBZ solubility was successfully enhanced by a minimum amount of combined polyvinyl pyrrolidone(PVP K30)and sodium lauryl sulfate(SLS).The plasticizer amount and molecular weight(MW)together with the osmotic agent amount directly affect the release rate whereas the swellable polymer amount and viscosity together with the semi-permeable membrane(SPM)thickness inversely influence the release rate.In addition,the tendency of following zero order kinetics was mainly affected by the coat components rather than those of the core.Further,orifice size does not have any significant effect on the release behavior within the range of 0.1 mm to 0.8 mm.In this study we report the successful formulation of CBZ-EOP tablets,which were similar to the marketed product Tegretol CR 200 and able to satisfy the USP criterion limits and to deliver about 80%of CBZ at a rate of approximately zero order for up to 12 h.Hadjira Rabti Jumah Masoud Mohammed Salmani Eltayeb Sulimen Elamin Narimane Lammari Jie Zhang Qineng Ping 2014Asian Journal of Pharmaceutical Sciences2014,9,3:4

20老年糖尿病病人应激性高血糖与胸腔镜下肺切除术后肺部并发症的关系显示文摘目的探讨老年糖尿病病人应激性高血糖(stress hyperglycemia,SH)与胸腔镜下肺切除术后肺部并发症(postoperative pulmonary complications,PPCs)的关系。方法回顾性选择2017年1月至2020年7月在我院行择期胸腔镜下肺切除术的老年糖尿病病人367例,收集病人一般资料、合并症、医师组别、术中液体出入量、手术时长及肌酐、Hb、白细胞、HbA1c、血糖水平等。采用应激性高血糖比率(stress hyperglycemia ratio,SHR)评估SH强度。采用多因素Logistic回归分析病人发生PPCs的危险因素。结果162例(44.1%)老年糖尿病病人发生PPCs(并发症组)。与无并发症组相比,并发症组病人入院时FPG、术日动脉FPG水平以及入院时SHR显著升高(P<0.05)。多因素Logistic回归分析显示,术日动脉FPG水平与入院时SHR是老年糖尿病病人胸腔镜下肺切除术后发生PPCs的独立危险因素。结论术日动脉FPG水平以及入院时SHR是老年糖尿病病人胸腔镜下肺切除术后发生PPCs的独立危险因素。陈紫璇 Sara Musa Abdalla Elamin 朱琳佳 查天明 许斯洋 桂波 张扬 2022实用老年医学2022,36,5:1
