1Fecal immunochemical test accuracy in average-risk colorectal cancer screening显示文摘AIM:To assess the fecal immunochemical test(FIT)accuracy for colorectal cancer(CRC)and advanced neoplasia(AN)detection in CRC screening.METHODS:We performed a multicentric,prospective,double blind study of diagnostic tests on asymptomatic average-risk individuals submitted to screening colonoscopy.Two stool samples were collected and the fecal hemoglobin concentration was determined in the first sample(FIT1)and the highest level of both samples(FITmax)using the OC-sensor.Areas under the curve(AUC)for CRC and AN were calculated.The best FIT1and FITmax cut-off values for CRC were determined.At this threshold,number needed to scope(NNS)to detect a CRC and an AN and the cost per lesion detected were calculated.RESULTS:About 779 individuals were included.An AN was found in 97(12.5%)individuals:a CRC in 5(0.6%)and an advanced adenoma(≥10 mm,villous histology or high grade dysplasia)in 92(11.9%)subjects.For CRC diagnosis,FIT1 AUC was 0.96(95%CI:0.95-0.98)and FITmax AUC was 0.95(95%CI:0.93-0.97).For AN,FIT1 and FITmax AUC were similar(0.72,95%CI:0.66-0.78 vs 0.73,95%CI:0.68-0.79,respectively,P=0.34).Depending on the number of determinations and the positivity threshold cut-off used sensitivity for AN detection ranged between 28%and 42%and specificity between 91%and 97%.At the best cut-off point for CRC detection(115 ng/mL),the NNS to detect a CRC were 10.2 and 15.8;and the cost per CRC was 1814€and 2985€on FIT1 and FITmax strategies respectively.At this threshold the sensitivity,NNS and cost per AN detected were 30%,1.76,and 306€,in FIT1 strategy,and 36%,2.26€and 426€,in FITmax strategy,respectively.CONCLUSION:Performing two tests does not improve diagnostic accuracy,but increases cost and NNS to detect a lesion.Vicent Hernandez Joaquin Cubiella M Carmen Gonzalez-Mao Felipe Iglesias Concepción Rivera M Begoa Iglesias Lucía Cid Ines Castro Luisa de Castro Pablo Vega Jose Antonio Hermo Ramiro Macenlle Alfonso Martínez-Turnes David Martínez-Ares Pamela Estevez Estela Cid M Carmen Vidal Angeles López-Martínez Elisabeth Hijona Marta Herreros-Villanueva Luis Bujanda Jose Ignacio Rodriguez-Prada the COLONPREV study investigators 2014World Journal of Gastroenterology2014,20,4:4
2Celecoxib enhances the detoxification of diethylnitrosamine in rat liver cancer显示文摘AIM:To study the effect of celecoxib(CXB) on diethylnitrosamine activation through the regulation of cytochrome P450 in a hepatocarcinogenesis model.METHODS:Six-week-old male Sprague-Dawley rats were randomly divided into fi ve groups,a non-treated group(NT) ,a diethylnitrosamine-treated group(DEN) ,a DEN+CXB-treated group(DEN+CXB) ,and CXB 8 d-treated and CXB 32 d-treated groups.The effects of celecoxib on the enzymatic activities of CYP1A1,2A,2B1/2,and 2E1 were assessed in hepatic microsomes 24 h after DEN administration.Changes in CYP1A1 and CYP2B1/2 protein expression were also evaluated.The rate of DEN metabolism was measured by the production of the deethylation metabolite acetaldehyde,and the denitrosation metabolite nitrite.RESULTS:DEN+CXB administration produced a significant increase in the enzymatic activities of CYP2B1/2 and 1A1,whereas it did not change the activities of CYP2A and 2E1,compared to that of the DEN group.CXB treatment for eight days did not produce a signif icant effect on enzymatic activity when compared to the NT group;however,when it was administered for prolonged times(CXB 32 d group) ,the enzymatic activities were increased in a similar pattern to those in the DEN+CXB group.The observed increase in the enzymatic activities in the DEN+CXB group was accompanied by an increase in the CYP2B1/2 protein levels;no changes were observed in the levels of CYP1A1.In vitro,CXB increased the denitrosation of DEN,a pathway of metabolic detoxification.The addition of SKF-525A,a preferential inhibitor of CYP2B,abrogated the denitrosation of DEN.CONCLUSION:These results suggest that the mechanism of action of CXB involves enhancement of the detoxif ication of DEN by an increasing denitrosation via CYP2B1/2.Martha Estela Salcido-Neyoy Adolfo Sierra-Santoyo Olga Beltrán-Ramírez José Roberto Macías-Pérez Saúl Villa-Trevio 2009World Journal of Gastroenterology2009,15,19:3

14Liver histological changes and lipid peroxidation in the amphibian Ambystoma mexicanum induced by sediment elutriates from the Lake Xochimilco显示文摘Lacustrine sediments accumulate pollutants that input from the lake watershed and can be released to the water column by sediment resuspension; thus, pollutants can change their bioavailability and exert adverse effects to aquatic biota. Shallow-urban lakes are particularly susceptible to receive pollutants from urban discharges and sediment resuspension. Lake Xochimilco, in Mexico City, an urban-shallow lake, faces multiple problems: urban sprawl, overexploitation of aquifers, drying of springs, discharge of wastewater from treatment plants, and sediment resuspension. The aquatic biota living in this ecosystem is continuously exposed to the release of pollutants from the sediments. We assessed the risk that pollutants released from sediments from Lake Xochimilco, Touristic(TZ) and Agriculture zone(AZ), can exert on a native amphibian species of the lake(Ambystoma mexicanum) through exposure bioassays to sediment elutriates. We evaluate alterations in the amphibian by three approaches: biochemical(level of lipid peroxidation,LPO), cellular(ultrastructure) and the liver histology of A. mexicanum and we compare them with a batch control. Additionally, we assessed heavy metals(Pb, Cd and Hg) in elutriates.Elutriates from TZ showed the highest concentrations of the metals assessed. Organisms exposed to sediment elutriates from either study sites showed higher LPO values than control organisms(p < 0.05). Organisms exposed to elutriates from the TZ showed the most conspicuous damages: hepatic vasodilation of sinusoids, capillaries with erythrocytes,leukocyte infiltration and cytoplasmic vacuolation in hepatocytes.
