| 1 | Management of recurrent rectal cancer:A population based study in greater Amsterdam显示文摘AIM: To analyze,retrospectively in a population-based study,the management and survival of patients with recurrent rectal cancer initially treated with a macroscopically radical resection obtained with total mesorectal excision (TME). METHODS: All rectal carcinomas diagnosed during 1998 to 2000 and initially treated with a macroscopically radical resection (632 patients) were selected from the Amsterdam Cancer Registry. For patients with recurrent disease,information on treatment of the recurrence was collected from the medical records. RESULTS: Local recurrence with or without clinically apparent distant dissemination occurred in 62 patients (10%). Thirty-two patients had an isolated local recurrence. Ten of these 32 patients (31%) underwent radical re-resection and experienced the highest survival (three quarters survived for at least 3 years). Eight patients (25%) underwent non-radical surgery (median survival 24 mo),seven patients (22%) were treated with radio-and/or chemotherapy without surgery (median survival 15 mo) and seven patients (22%) only received best supportive care (median survival 5 mo). Distant dissemination occurred in 124 patients (20%) of whom 30 patients also had a local recurrence. The majority (54%) of these patients were treated with radio-and/or chemotherapy without surgery (median survival 15 mo). Twenty-seven percent of these patients only received best supportive care (median survival 6 mo),while 16% underwent surgery for their recurrence. Survival was best in the latter group (median survival 32 mo). | 2 | Prediction and diagnosis of colorectal anastomotic leakage: A systematic review of literature显示文摘Although many studies have focused on the preoperative risk factors of anastomotic leakage after colorectal surgery(CAL), postoperative delay in diagnosis is common and harmful. This review provides a systematic overview of all available literature on diagnostic tools used for CAL. A systematic search of literature was undertaken using Medline, Embase, Cochrane and Webof-Science libraries. Articles were selected when a diagnostic or prediction tool for CAL was described and tested. Two reviewers separately assessed the eligibility and level of evidence of the papers. Sixty-nine articles were selected(clinical methods: 11, laboratory tests: 12, drain fluid analysis: 12, intraoperative techniques:22, radiology: 16). Clinical scoring leads to early awareness of probability of CAL and reduces delay of diagnosis. C-reactive protein measurement at postoperative day 3-4 is helpful. CAL patients are characterized by elevated cytokine levels in drain fluid in the very early postoperative phase in CAL patients. Intraoperative testing using the air leak test allows intraoperative repair of the anastomosis. Routine contrast enema is not recommended. If CAL is clinically suspected, rectal contrast-computer tomography is recommended by a few studies. In many studies a 'no-test' control group was lacking, furthermore no golden standard for CAL is available. These two factors contributed to a relatively low level of evidence in the majority of the papers. This paper provides a systematic overview of literature on the available tools for diagnosing CAL. | 3 | Multifractal Simulation of GeochemicalMap Patterns显示文摘Using a simple multifractal model based on the model De Wijs, various geochemical map patterns for element concentration values are being simulated. Each pattern is self-similar on the average in that a similar pattern can be derived by application of the multiplicative cascade model used to any small subarea on the pattern. In other experiments, the original, self-similar pattern is distorted by superimposing a 2-dimensional trend pattern and by mixing it with a constant concentration value model. It is investigated how such distortions change the multifractal spectrum estimated by means of the 3-step method of moments. Discrete and continuous frequency distribution models are derived for patterns that satisfy the model of De Wijs. These simulated patterns satisfy a discrete frequency distribution model that as upper bound has a continuous frequency distribution to which it approaches in form when the subdivisions of the multiplicative cascade model are repeated indefinitely. This limiting distribution is lognormal in the center and has Pareto tails. Although these tumors are usually considered to be more related to hepatocellular carcinoma than to cholangiocarcinoma, they sometimes, in contrast to hepatocellular carcinoma, contain a significant amount of fibrous stroma. This might in some cases explain atypical radiological features. We report a case of a cHCC-CC in a 47-year-old female that resembled focal nodular hyperplasia on Magnetic Resonance Imaging. | 8 | Early prediction of major depression in chronic hepatitis C patients during peg-interferon α-2b treatment by assessment of vegetative-depressive symptoms after four weeks显示文摘AIM: To study the predictive value of the vegetative- depressive symptoms of the Zung Depression Rating Scale for the occurrence of depression during treatment with peg-interferon α-2b of chronic hepatitis C (CHC) patients. METHODS: The predictive value of vegetative- depressive symptoms at 4 wk of treatment for the occurrence of a subsequent diagnosis of major depressive disorder (MDD) was studied in CHC patients infected after substance use in a prospective, multi- center treatment trial in Belgium. The presence of vegetative-depressive symptoms was assessed using the Zung Scale before and 4 wk after the start of antiviral treatment. RESULTS: Out of 49 eligible patients, 19 (39%) developed MDD. The area under the ROC curve of the vegetative Zung subscale was 0.73, P = 0.004. The sensitivity at a cut-point of > 15/35 was 95% (95% CI: 74-100). The positive predictive value equalled 44% (95% CI: 29-60). CONCLUSION: In this group of Belgian CHC patients infected after substance use, antiviral treatment caused a considerable risk of depression. | 9 | Fractals and Spatial Statistics of Point Patterns显示文摘The relationship between fractal point pattern modeling and statistical methods of parameter estimation in point-process modeling is reviewed. Statistical estimation of the cluster fractal dimension by using Ripley's K-function has advantages in comparison with the more commonly used methods of box-counting and cluster fractal dimension estimation because it corrects for edge effects, not only for rectangular study areas but also for study areas with curved boundaries determined by regional geology. Application of box-counting to estimate the fractal dimension of point patterns has the disadvantage that, in general, it is subject to relatively strong 'roll-off' effects for smaller boxes. Point patterns used for example in this paper are mainly for gold deposits in the Abitibi volcanic belt on the Canadian Shield. Additionally, it is proposed that, worldwide, the local point patterns of podiform Cr, volcanogenic massive sulphide and porphyry copper deposits, which are spatially distributed within irregularly shaped favorable tracts, satisfy the fractal clustering model with similar fractal dimensions. The problem of deposit size (metal tonnage) is also considered. | 11 | Development of a classification system based on corneal biomechanical properties using artificial intelligence predicting keratoconus severity显示文摘Background:To investigate machine-learning(ML)algorithms to differentiate corneal biomechanical properties between different topographical stages of keratoconus(KC)by dynamic Scheimpflug tonometry(CST,Corvis ST,Oculus,Wetzlar,Germany).In the following,ML models were used to predict the severity in a training and validation dataset.Methods:Three hundred and eighteen keratoconic and one hundred sixteen healthy eyes were included in this monocentric and cross-sectional pilot study.Dynamic corneal response(DCR)and corneal thickness related(pachymetric)parameters from CST were chosen by appropriated selection techniques to develop a ML algorithm.The stage of KC was determined by the topographical keratoconus classification system(TKC,Pentacam,Oculus).Patients who were classified as TKC 1,TKC 2 and TKC 3 were assigned to subgroup mild,moderate,and advanced KC.If patients were classified as TKC 1-2,TKC 2-3 or TKC 3-4,they were assigned to subgroups according to the normative range of further corneal indices(index of surface variance,keratoconus index and minimum radius).Patients classified as TKC 4 were not included in this study due to the limited amount of cases.Linear discriminant analysis(LDA)and random forest(RF)algorithms were used to develop the classification models.Data were divided into training(70%of cases)and validation(30%of cases)datasets.Results:LDA model predicted healthy,mild,moderate,and advanced KC eyes with a sensitivity(S_(n))/specificity(S_(p))of 82%/97%,73%/81%,62%/83%and 68%/95%from a validation dataset,respectively.For the RF model,a S_(n)/S_(p) of 91%/94%,80%/90%,63%/87%,72%/95%could be reached for predicting healthy,mild,moderate,and advanced KC eyes,respectively.The overall accuracy of LDA and RF was 71%and 78%,respectively.The accuracy for KC detection including all subgroups of KC severity was 93%in both models.Conclusion:The RF model showed good accuracy in predicting healthy eyes and various stages of KC.The accuracy was superior with respect to the LDA model.The clinical importance of the models is that the standalone dynamic Scheimpflug tonometry is able to predict the severity of KC without having the keratometric data. | Robert Herber Lutz E.Pillunat Frederik Raiskup | 2021 | Eye and Vision2021,8,1: | 3 |
| 12 | Quality of life after liver transplantation:State of the art显示文摘Quality of life(QoL) after deceased donor liver transplantation is increasingly recognized as a major outcome parameter. We reviewed recent publications in this rapidly evolving field in order to summarize recent achievements in the field and to define opportunities and perspectives for research and improvement of patient care. QoL does improve after liver transplantation according to a typical pattern. During the first year, there is a significant improvement in QoL. After one year, the improvement does stabilise and tends to decline slightly. In addition to the physical condition, different psychological parameters(such as depression, anxiety, sexual function) and sociodemographic elements(professional state, sex, marital state) seem to impact QoL. | 14 | Implications of fine water mist environment on the post-detonation processes of a PE4 explosive charge in a semi-confined blast chamber显示文摘The effects of a fine water mist environment in a semi-confined blast chamber on the chemical and thermodynamic processes following detonation of a 20 g PE4 explosive charge have been investigated.The effects were quantified by the analysis of pressure profiles recorded where several parameters including arrival time of the shock at the sensors, peak overpressures, specific impulse of the positive phase, period of the negative phase and the specific impulse of the multiple reflections were quantified.The effect of the fine water mist on the arrival time, peak pressures and the specific impulse of the positive phase agrees with previous findings in literature. In this paper, the focus is on the implications of the fine water mist on the negative phase and the impulse of multiple pressure reflections. The period of the negative phase was found to have increased by 40% and with higher negative peak pressure in the mist condition compared to the atmospheric condition. Hypomagnesemia can be a serious adverse effect to proton pump inhibitor(PPI) therapy, which is worrying due to the widespread use of PPIs. Current evidence suggest that the mechanism of PPI induced hypomagnesemia is impaired intestinal magnesium absorption. In this report, we present the case of a long-term PPI user with persistent hypomagnesemia with severe symptoms at presentation.
