Metabolic syndrome and colorectal neoplasms:An ominous association显示文摘AIM:To evaluate the association of metabolic syndrome(MS) and colorectal cancer and adenomas in a Western country, where the incidence of MS is over 27%.METHODS:This was a prospective study between March 2013 and March 2014.MS was diagnosed according to the National Cholesterol Education ProgramATP III.Demographic characteristics, anthropometric measurements, metabolic risk factors, and colonoscopic pathologic findings were assessed in patients with MS(group 1) who underwent routine colonoscopy at our department. This data was compared with consecutive patients without metabolic syndrome(group 2),with no differences regarding sex and age.Patients with incomplete colonoscopy, family history,or past history of colorectal neoplasm were excluded.Informed consent was obtained and the ethics committee approved this study.Statistical analysis was performed using Student's t-test and χ2 test, with a P value ≤ 0.05 being considered statistically significant.RESULTS:Of 258 patients,129 had MS;51% males;mean-age 67.1 years(50-87).Among the MS group,94% had high blood pressure,91% had increased waist circumference,60% had diabetes,55% had low high-density lipoprotein cholesterol level,50% had increased triglyceride level, and 54% were obese [body mass index(BMI) 30 kg/m2].51% presented 4 criteria of MS.MS was associated with increased prevalence of adenomas(43% vs 25%,P = 0.004) and colorectal cancer(13% vs 5%,P = 0.027),compared with patients without MS.MS was also positively associated with multiple(≥ 3) adenomas(35% vs 9%,P = 0.024) and sessile adenomas(69% vs 53%,P = 0.05).No difference existed between location(P = 0.086),grade of dysplasia(P = 0.196),or size(P= 0.841) of adenomas. In addition,no difference was found between BMI(P = 0.078),smoking(P = 0.146),alcohol consumption(P = 0.231),and the presence of adenomas.CONCLUSION:MS is positively associated with adenomas and colorectal cancer. However, there is not enough information in western European countries to justify screening in patients with MS.To our knowledge,no previous study has evaluated this association in Portuguese patients. | Daniel Trabulo Suzane Ribeiro Cláudio Martins Cristina Teixeira Cláudia Cardoso Jo?o Mangualde Ricardo Freire élia Gamito Ana L Alves Fátima Augusto Ana P Oliveira Isabelle Cremers | 2015 | World Journal of Gastroenterology2015,21,17: | 5
Chronic myeloid leukemia-from the Philadelphia chromosome to specific target drugs:A literature review显示文摘Chronic myeloid leukemia(CML)is a myeloproliferative neoplasm and was the first neoplastic disease associated with a well-defined genotypic anomaly―the presence of the Philadelphia chromosome. The advances in cytogenetic and molecular assays are of great importance to the diagnosis, prognosis, treatment, and monitoring of CML.The discovery of the breakpoint cluster region(BCR)-Abelson murine leukemia(ABL)1 fusion oncogene has revolutionized the treatment of CML patients by allowing the development of targeted drugs that inhibit the tyrosine kinase activity of the BCR-ABL oncoprotein.Tyrosine kinase inhibitors(known as TKIs)are the standard therapy for CML and greatly increase the survival rates, despite adverse effects and the odds of residual disease after discontinuation of treatment. As therapeutic alternatives, the subsequent TKIs lead to faster and deeper molecular remissions;however, with the emergence of resistance to these drugs, immunotherapy appears as an alternative, which may have a cure potential in these patients.Against this background, this article aims at providing an overview on CML clinical management and a summary on the main targeted drugs available in that context. | Mariana Miranda Sampaio Maria Luísa Cordeiro Santos Hanna Santos Marques Vinícius Lima de Souza Gonçalves Glauber Rocha Lima Araújo Luana Weber Lopes Jonathan Santos Apolonio Camilo Santana Silva Luana Kauany de SáSantos Beatriz Rocha Cuzzuol Quézia Estéfani Silva Guimarães Mariana Novaes Santos Breno Bittencourt de Brito Filipe Antônio França da Silva Márcio Vasconcelos Oliveira Cláudio Lima Souza Fabrício Freire de Melo | 2021 | World Journal of Clinical Oncology2021,12,2: | 3
Appropriateness of outpatient gastrointestinal endoscopy in a non-academic hospital显示文摘AIM:To assess the appropriate use and the diagnostic yield of upper gastrointestinal endoscopy and colonoscopy in this subgroup of patients.METHODS:In total,789 consecutive outpatients referred for gastrointestinal(GI) endoscopy [381 for esophagogastroduodenoscopy(EGD) and 408 for colonoscopy] were prospectively enrolled in the study. The American Society for Gastrointestinal Endoscopy(ASGE) guidelines were used to assess the relationship between appropriateness and the presence of relevant endoscopic f indings.RESULTS:The overall inappropriate rate was 13.3%.The indications for EGD and colonoscopy were, respectively, appropriate in 82.7% and 82.6% of the exams, uncertain in 5.8% and 2.4% and inappropriate in 11.5% and 15%.The diagnostic yield was signif icant higher forEGDs and colonoscopies judged appropriate and uncertain when compared with those considered inappropriate(EGD:36.6% vs 36.4% vs 11.4%,P = 0.004;Colonoscopy:24.3% vs 20.0% vs 3.3%,P = 0.001).Of the 25 malignant lesions detected, all but one was detected in exams judged appropriate or uncertain.CONCLUSION:This study shows a good adherence to ASGE guidelines by the referring physicians and a significant increase of the diagnostic yield in appropriate examinations, namely in detecting neoplastic lesions. It underscores the importance that the appropriateness of the indication assumes in assuring high-quality GI endoscopic procedures. | Joo Mangualde Marie I Cremers Ana M Vieira Ricardo Freire lia Gamito Cristina Lobato Ana L Alves Fátima Augusto Ana P Oliveira | 2011 | World Journal of Gastrointestinal Endoscopy2011,3,10: | 2
Helicobacter pylori infection:How does age influence the inflammatory pattern?显示文摘The inflammatory pattern during Helicobacter pylori(H.pylori)infection is changeable and complex.During childhood,it is possible to observe a predominantly regulatory response, evidenced by high concentrations of key cytokines for the maintenance of Treg responses such as TGF-β1 and IL-10,in addition to high expression of the transcription factor FOXP3.On the other hand, there is a predominance of cytokines associated with the Th1 and Th17 responses among H.pylori-positive adults. In the last few years, the participation of the Th17 response in the gastric inflammation against H.pylori infection has been highlighted due to the high levels of TGF-β1 and IL-17 found in this infectious scenario, and growing evidence has supported a close relationship between this immune response profile and unfavorable outcomes related to the infection. Moreover, this cytokine profile might play a pivotal role in the effectiveness of anti-H.pylori vaccines. It is evident that age is one of the main factors influencing the gastric inflammatory pattern during the infection with H.pylori, and understanding the immune response against the bacterium can assist in the development of alternative prophylactic and therapeutic strategies against the infection as well as in the comprehension of the pathogenesis of the outcomes related to that microorganism. | Glauber Rocha Lima Araújo Hanna Santos Marques Maria Luísa Cordeiro Santos Filipe Antônio França daSilva Breno Bittencourt da Brito Gabriel Lima Correa Santos Fabrício Freire de Melo | 2022 | World Journal of Gastroenterology2022,28,4: | 2

Tertiary peritonitis: A disease that should not be ignored显示文摘Intra-abdominal infections can be classified into uncomplicated or complicated(peritonitis). Peritonitis is divided into primary, secondary, and tertiary. Tertiaryperitonitis is the less common but the most severe among peritonitisstratifications, being defined as a recurrent intra-abdominal infection that occurs48 h after a well-succeeded control of a secondary peritonitis. This disease has acomplex pathogenesis that is closely related to the capacity of the peritonealcavity to activate immunological processes. Patients who progress to persistentperitonitis are at an increased risk of developing several infectious complicationssuch as sepsis and multiple organ failure syndrome. Moreover, tertiary peritonitisremains an important cause of hospital death mainly among patients withassociated risk factors. The microbiological profile of organisms causing tertiaryperitonitis is often different from that observed in other types of peritonitis. Inaddition, there is a high prevalence of multidrug-resistant pathogens causing thiscondition, and an appropriate and successful clinical management depends on anearly diagnosis, which can be made easier with the use of clinical scorespresenting a good prediction value during the intensive care unit admission.Complementarily, immediate therapy should be performed to control theinfectious focus and to prevent new recurrences. In this sense, the treatment isbased on initial antimicrobial therapy and well-performed peritoneal drainage. | Hanna Santos Marques Glauber Rocha Lima Araújo Filipe Antônio França da Silva Breno Bittencourt de Brito Paulo Victor Dias Versiani Jaqueline Silva Caires Thiago de Carvalho Milet Fabrício Freire de Melo | 2021 | World Journal of Clinical Cases2021,9,10: | 0
Syndemic aspects between COVID-19 pandemic and social inequalities显示文摘Although the coronavirus disease 2019(COVID-19)pandemic has reached all over the world population,it has demonstrated a heterogeneous impact on different populations. The most vulnerable communities which coexist daily with the social inequalities like low access to hygiene and personal protection products, crowded residences, and higher levels of chronic diseases have a higher risk of contact and the spread of infection, beyond unfavorable clinical outcomes. The elevation of the risk of infection exposure can be related to gender due to the presence of a larger contingent of women in essential services,as well as frontline and cleaning professionals who regardless of gender have the greatest exposure to the virus.Such exposures can contribute to the development of fear of contaminating themselves or their family members associated also with the work stress, both of which are related to the emergence of mental disturbances in these populations.Furthermore, conditions of unsanitary living and low socioeconomic status, populations at war,pre-existing social barriers, and ethnicity have contributed to more impact of the pandemic both in the exposure to the virus and access to health services,COVID-19 management, and management of other pathologies. At the same time, factors such as the closing of non-essential services, the loss of jobs, and the increase in household spending aggravated the social vulnerabilities and impacted the family economy.Lastly, the COVID-19 pandemic contributed still more to the impact on women's health since it propitiated a favorable environment for increasing domestic violence rates, through the segregation of women from social life, and increasing the time of the victims with their aggressors. | Jonathan Santos Apolonio Ronaldo Teixeira da Silva Júnior Beatriz Rocha Cuzzuol Glauber Rocha Lima Araújo Hanna Santos Marques Isadora de Souza Barcelos Luana Kauany de SáSantos Luciano Hasimoto Malheiro Vinícius Lima de Souza Gonçalves Fabrício Freire de Melo | 2022 | World Journal of Methodology2022,12,5: | 0
COVID-19 neuropsychiatric repercussions:Current evidence on the subject显示文摘Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection has affected the entire world, causing the coronavirus disease 2019(COVID-19)pandemic since it was first discovered in Wuhan,China in December 2019.Among the clinical presentation of the disease,in addition to fever, fatigue, cough, dyspnea, diarrhea, nausea, vomiting, and abdominal pain, infected patients may also experience neurological and psychiatric repercussions during the course of the disease and as a post-COVID-19 sequelae. Thus, headache, dizziness, olfactory and gustatory dysfunction, cerebrovascular disorders, neuromuscular abnormalities, anxiety, depression, and post-traumatic stress disorder can occur both from the infection itself and from social distancing and quarantine.According to current evidence about this infection, the virus has the ability to infect the central nervous system(CNS)via angiotensin-converting enzyme 2(ACE2)receptors on host cells.Several studies have shown the presence of ACE2 in nerve cells and nasal mucosa,as well as transmembrane serine protease 2,key points for interaction with the viral Spike glycoprotein and entry into the CNS,being olfactory tract and blood-brain barrier, through hematogenous dissemination, potential pathways. Thus, the presence of SARS-CoV-2 in the CNS supports the development of neuropsychiatric symptoms. The management of these manifestations seems more complex, given that the dense parenchyma and impermeability of brain tissue, despite protecting the brain from the infectious process, may hinder virus elimination.Still, some alternatives used in non-COVID-19 situations may lead to worse prognosis of acute respiratory syndrome, requiring caution. Therefore, the aim of this review is to bring more current points related to this infection in the CNS,as well as the repercussions of the isolation involved by the pandemic and to present perspectives on interventions in this scenario. | Ronaldo Teixeira da Silva Júnior Jonathan Santos Apolonio Beatriz Rocha Cuzzuol Bruna Teixeira da Costa Camilo Santana Silva Glauber Rocha Lima Araújo Marcel Silva Luz Hanna Santos Marques Luana Kauany de SáSantos Samuel Luca Rocha Pinheiro Vinícius Lima de Souza Gonçalves Mariana Santos Calmon Fabrício Freire de Melo | 2022 | World Journal of Methodology2022,12,5: | 0
Influence of the COVID-19 pandemic in the gastrointestinal oncology setting:An overview显示文摘Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection has been impacting healthcare in various ways worldwide and cancer patients are greatly affected by the coronavirus disease 2019(COVID-19)pandemic.The reorganization of the health facilities in order to supply the high demand resulting from the aforementioned infection as well as the social isolation measures led to impairments for the diagnosis and follow-up of patients with gastrointestinal cancers, which has had an impact on the prognosis of the oncologic patients. In that context, health authorities and organizations have elaborated new guidelines with specific recommendations for the management of individuals with gastrointestinal neoplasms during the pandemic.Of note, oncologic populations seem to be more susceptible to unfavorable outcomes when exposed to SARS-CoV-2 infection and some interactions involving virus, tumor, host immune system and anticancer therapies are probably related to the poorer prognosis observed in those COVID-19 patients. Moreover, vaccination stands out as the main prevention method against severe SARS-CoV-2 infection and some particularities have been observed regarding the seroconversion of vaccinated oncologic patients including those with gastrointestinal malignancies. In this minireview,we gather updated information regarding the influence of the pandemic in the diagnosis of gastrointestinal neoplasms, new recommendations for the management of gastrointestinal cancer patients, the occurrence of SARSCoV-2 infection in those individuals and the scenario of the vaccination against the virus in that population. | Breno Bittencourt de Brito Hanna Santos Marques Filipe Antônio França da Silva Maria Luísa Cordeiro Santos Glauber Rocha Lima Araújo Lara de Araujo Valente Fabrício Freire de Melo | 2022 | World Journal of Gastrointestinal Pathophysiology2022,13,5: | 0
Effects of nutrients on immunomodulation in patients with severe COVID-19:Current knowledge显示文摘Recent research has demonstrated that critically ill patients with coronavirus disease 2019(COVID-19)show significant immune system dysregulation.Due to that, some nutrients that influence immunomodulation have been suggested as a form of treatment against the infection. This review collected the information on the impact of vitamins on the prognosis of COVID-19,with the intention of facilitating treatment and prevention of the disease risk status in patients. The collected information was obtained using the PubMed electronic database by searching for articles that relate COVID-19 and the mechanisms/effects of the nutrients:Proteins, glucose, lipids, vitamin B12,vitamin D,calcium, iron, copper, zinc, and magnesium, including prospective, retrospective, and support articles. The findings reveal an optimal response related mainly to omega-3,eicosapentaenoic acid, docosahexaenoic acid, calcium, and iron that might represent benefits in the treatment of critically ill patients. However, nutrient supplementation should be done with caution due to the limited availability of randomized controlled studies. | Bruna Teixeira da Costa Glauber Rocha Lima Araújo Ronaldo Teixeira da Silva Júnior Luana Kauany de SáSantos Vinícius Lima de Souza Gonçalves Daniel Bastos Alves Lima Beatriz Rocha Cuzzuol Jonathan Santos Apolonio Lorena Sousa de Carvalho Hanna Santos Marques Camilo Santana Silva Isadora de Souza Barcelos Márcio Vasconcelos Oliveira Fabrício Freire de Melo | 2022 | World Journal of Critical Care Medicine2022,11,4: | 0

Epidemiological and clinical characteristics of COVID-19 in a Brazilian public hospital显示文摘BACKGROUND The coronavirus disease 2019(COVID-19)pandemic has become a major health concern worldwide. In that context, the understanding of epidemiological and clinical features associated with the disease and its severity is crucial for the establishment of strategies aimed at disease control and remedy.AIM To describe epidemiological features, signs, symptoms, and laboratory findings among severely ill COVID-19 patients from an inten-sive care unit in northeastern Brazil as well as to evaluate predictor factors for disease outcomes.METHODS This is a prospective single-center study that evaluated 115 patients admitted to the intensive care unit in a northeastern Brazilian hospital.RESULTS The patients had a median age of 65.60±15.78 years.Dyspnea was the most frequent symptom, affecting 73.9%of the patients, followed by cough(54.7%).Fever was reported in approximately one-third of patients and myalgia in 20.8%of the patients. At least two comorbidities were found in 41.7%of the patients, and hypertension was the most prevalent(57.3%).In addition, having two or more comorbidities was a predictor of mortality, and lower platelet count was positively associated with death.Nausea and vomiting were two symptoms that were predictors of death, and the presence of a cough was a protective factor.CONCLUSION This is the first report of a negative correlation between cough and death in severely ill severe acute respiratory syndrome coronavirus 2-infected individuals. The associations between comorbidities, advanced age,and low platelet count and the outcomes of the infection were similar to the results of previous studies, highlighting the relevance of these features. | Fillipe Dantas Pinheiro Luana Weber Lopes Rafael Santos Dantas Miranda Dórea Glauber Rocha Lima Araújo Filipe Antônio França da Silva Breno Bittencourt de Brito Maria Luísa Cordeiro Santos Geovani Moreno Santos Júnior Maria Teresa Araújo de Lorenzo Barcia Renata de Amorim Marques AndréBezerra Botelho Anna Carolina Saúde Dantas Davi Tanajura Costa Adriano Fernandes Teixeira Cláudio Lima Souza Lucas Miranda Marques Guilherme Barretos Campos Márcio Vasconcelos Oliveira Dulciene Maria de Magalhães Queiroz Fabrício Freire de Melo | 2023 | World Journal of Clinical Cases2023,11,8: | 0
Case report of a criminal dismemberment in Northeast Brazil显示文摘Several decomposed body parts were received for examination by the Forensic Anthropology section of the Medico-legal Institute of João Pessoa,Paraíba,Brazil. The portions of the lower and upper limbs, ribs, vertebrae, and a skull were thoroughly examined. The biological profile indicated a male individual with an estimated age range between 23 and 57 years and a mean age of 35.2 years(SD=9.4;phase IV,Suchey-Brooks).The skeleton showed injuries caused by sharp force and sharp-blunt force trauma that affected all body segments.Macroscopically, the lesions are mainly in the diaphyseal segments of the long bones, sacrum, pelvis, mandible, maxilla, scapulae, sternum, vertebrae, the distal epiphysis of the left fibula, and the distal epiphysis of the left tibia displayed characteristics compatible with injuries produced perimortem. It was not possible to determine the cause of death.DNAanalysis resulted in a positive identification.Because of common difficulties faced in forensic practice,it is often not possible for forensic anthropologists to go to the crime scene,X-ray or body scanners are frequently unavailable, and the victim’s medical and/or dental records are sometimes absent. These difficulties make identification ultimately depend on genetic analysis, which is more time-consuming than other identification methods.Despite this, bone fragment examination in dismemberment cases is a complex task.Forensic Anthropology can shed light on cases involving the identification of dismembered remains, which are challenging because of the number of traumatic injuries,as well as different injury patterns,on bones. | Evelyne Pessoa Soriano Maria do Socorro Dantas de Araújo Francisca de Assis Nascimento Pereira Francisca Divina Silveira de Melo Cristiane Helena da Silva Barbosa Freire Marcus Vitor Diniz de Carvalho | 2022 | Forensic Sciences Research2022,7,4: | 0
