| 2 | Autoimmune gastritis:Pathologist's viewpoint显示文摘Western countries are seeing a constant decline in the incidence of Helicobacter pylori-associated gastritis, coupled with a rising epidemiological and clinical impact of autoimmune gastritis. This latter gastropathy is due to autoimmune aggression targeting parietal cells through a complex interaction of auto-antibodies against the parietal cell proton pump and intrinsic factor, and sensitized T cells. Given the specific target of this aggression, autoimmune gastritis is typically restricted to the gastric corpus-fundus mucosa. In advanced cases, the oxyntic epithelia are replaced by atrophic(and metaplastic) mucosa, creating the phenotypic background in which both gastric neuroendocrine tumors and(intestinal-type) adenocarcinomas may develop. Despite improvements in our understanding of the phenotypic changes or cascades occurring in this autoimmune setting, no reliable biomarkers are available for identifying patients at higher risk of developing a gastric neoplasm. | 3 | The acidic pathway of bile acid synthesis: Not just an alternative pathway显示文摘Over the last two decades,the prevalence of obesity,and metabolic syndromes(MS)such as nonalcoholic fatty liver disease(NAFLD)and type 2 diabetes mellitus(T2DM),have dramatically increased.Bile acids play a major role in the digestion,absorption of nutrients,and the body's redistribution of absorbed lipids as a function of their chemistry and signaling properties.As a result,a renewed interest has developed in the bile acid metabolic pathways with the challenge of gaining insight into novel treatment approaches for this rapidly growing healthcare problem.Of the two major pathways of bile acid synthesis in the liver,the foremost role of the acidic(alternative)pathway is to generate and control the levels of regulatory oxysterols that help control cellular cholesterol and lipid homeostasis.Cholesterol transport to mitochondrial sterol 27-hydroxylase(CYP27A1)by steroidogenic acute regulatory protein(StarD1),and the subsequent 7a-hydroxylation of oxysterols by oxysterol 7a-hydroxylase(CYP7B1)are the key regulatory steps of the pathway.Recent observations suggest CYP7B1 to be the ultimate controller of cellular oxysterol levels.This review discusses the acidic pathway and its contribution to lipid,cholesterol,carbohydrate,and energy homeostasis.Additionally,discussed is how the acidic pathway's dysregulation not only leads to a loss in its ability to control cellular cholesterol and lipid homeostasis,but leads to inflammatory conditions. | William M.Pandak Genta Kakiyama | 2019 | Liver Research2019,3,2: | 4 |
| 4 | Bangkok to Sendai and Beyond: Implications for Disaster Risk Reduction in Asia显示文摘The recently concluded World Conference on Disaster Risk Reduction(WCDRR) in Sendai, Japan and the Sendai Framework for Disaster Risk Reduction2015–2030(SFDRR) have set renewed priorities for disaster risk reduction(DRR) for the next 15 years. Due to Asia’s high exposure to natural hazards, the implications of the new SFDRR have major significance for the future development of the region. The 6th Asian Ministerial Conference on Disaster Risk Reduction(AMCDRR), held in Bangkok in 2014, was a regional preparatory meeting for the WCDRR, and proposed various targets and indicators for DRR in Asia. The AMCDRR recommended inclusion of these goals in the SFDRR. This study focuses on the WCDRR negotiations, particularly outcomes that affect four major groups: local authorities; children and youth;science and technology; and business and industry. An analysis is undertaken of the overlaps and gaps in the outcomes of the 6th AMCDRR and other preceding conferences that fed into the WCDRR. A set of recommendations has evolved from this examination for consideration at the upcoming 7th AMCDRR in 2016. | 12 | Assessing risks for gastric cancer: New tools for pathologists显示文摘Although the Sydney Systems (original and updated) for the classification of gastritis have contributed substantially to the uniformity of the reporting of gastric conditions, they lack immediacy in conveying to the user information about gastric cancer risk. In this review, we summarize the current understanding of the gastric lesions associated with an increased risk for cancer, and present the rationale for a proposal for new ways of reporting gastritis. In addition to the traditional histopathological data gathered and evaluated according to the Sydney System rules, pathologists could add an assessment expressed as grading and staging of the gastric inflammatory and atrophic lesions and integrate these findings with pertinent laboratory information on pepsinogens and gastrin levels.
