1Three-dimensional-printed individualized porous implants:A new“implant-bone”interface fusion concept for large bone defect treatment显示文摘Bone defect repairs are based on bone graft fusion or replacement.Current large bone defect treatments are inadequate and lack of reliable technology.Therefore,we aimed to investigate a simple technique using three-dimensional(3D)-printed individualized porous implants without any bone grafts,osteoinductive agents,or surface biofunctionalization to treat large bone defects,and systematically study its long-term therapeutic effects and osseointegration characteristics.Twenty-six patients with large bone defects caused by tumor,infection,or trauma received treatment with individualized porous implants;among them,three typical cases underwent a detailed study.Additionally,a large segmental femur defect sheep model was used to study the osseointegration characteristics.Immediate and long-term biomechanical stability was achieved,and the animal study revealed that the bone grew into the pores with gradual remodeling,resulting in a long-term mechanically stable implant-bone complex.Advantages of 3D-printed microporous implants for the repair of bone defects included 1)that the stabilization devices were immediately designed and constructed to achieve early postoperative mobility,and 2)that osseointegration between the host bone and implants was achieved without bone grafting.Our osseointegration method,in which the“implant-bone”interface fusion concept was used instead of“bone-bone”fusion,subverts the traditional idea of osseointegration.Teng Zhang Qingguang Wei Hua Zhou Zehao Jing Xiaoguang Liu Yufeng Zheng Hong Cai Feng Wei Liang Jiang Miao Yu Yan Cheng Daoyang Fan Wenhao Zhou Xinhong Lin Huijie Leng Jian Li Xinyu Li Caimei Wang Yun Tian Zhongjun Liu 2021Bioactive Materials2021,6,11:9
2Medical expenditures for colorectal cancer diagnosis and treatment: A 10-year high-level-hospital-based multicenter retrospective survey in China, 2002-2011显示文摘Objective: Colorectal cancer(CRC) causes a substantial burden of disease in China and the evidence of economic burden triggered is fundamental for priority setting. The aim of this survey was to quantify medical expenditures and the time trends for CRC diagnosis and treatment in China.Methods: From 2012 to 2014, a hospital-based multicenter retrospective survey was conducted in 13 provinces across China. For each eligible CRC patient diagnosed from 2002 to 2011, clinical information and expenditure data were extracted using a uniform questionnaire. All expenditure data were reported in Chinese Yuan(CNY)using 2011 values.Results: Of the 14,536 CRC patients included, the average age at diagnosis was 58.2 years and 15.8% were stageI cases. The average medical expenditure per patient was estimated at 37,902 CNY [95 % confidence interval(95%CI): 37,282-38,522], and the annual average increase rate was 9.2% from 2002 to 2011(P for trend <0.001), with a cumulative increase of 2.4 times(from 23,275 CNY to 56,010 CNY). The expenditure per patient in stages Ⅰ, Ⅱ, Ⅲ and Ⅳ were 31,698 CNY, 37,067 CNY, 38,918 CNY and 42,614 CNY, respectively(P<0.001). Expenditure significantly differed within various subgroups. Expenses for drugs contributed the largest proportion(52.6%).Conclusions: These conservative estimates illustrated that medical expenditures for CRC diagnosis and treatment in tertiary hospitals in China were substantial and increased rapidly over the 10 years, with drugs continually being the main expense by 2011. Relatively, medical expenditures are lower for CRC in the earlier stages. 3Simultaneous hepatic resection benefits patients with synchronous colorectal cancer liver metastases显示文摘Objective: The safety of the simultaneous resection of synchronous colorectal cancer liver metastases(SCRLM) is still being debated.However, this simultaneous operative approach is more commonly adopted at present than in the past.Therefore, we compared simultaneous hepatic resection with delayed hepatic resection in this study.Methods: All patients with SCRLM diagnosed before initial treatment between January 2009 and September 2013 were retrospectively included in our study.Short-term and long-term outcomes were compared in patients who underwent simultaneous colorectal and hepatic resection and those treated by delayed hepatectomy.Results: Among the 73 patients diagnosed with SCRLM, simultaneous colorectal and hepatic resection was performed in 60 patients(82.2%), while delayed hepatic resection was performed in 13 patients(17.8%).The mortality rate was zero.The postoperative complication rate after delayed resection was higher than, but not significantly different from, that after simultaneous resection(46% vs.23%, P=0.166).The duration of operating time(240 vs.420 min, P<0.05) and postoperative hospital stay time(11 vs.18 days, P<0.05) were shorter in the simultaneous resection group.After the initial treatments were given, the 1-, 2-, and 3-year survival rates in the simultaneous resection group were 77%, 59%, and 53%, respectively, whereas those in the delayed resection group were 67%, 42%, and 10%, respectively.The 5-year survival rate in the simultaneous resection group was 23%; overall survival differed significantly between the two groups(P=0.037).Median disease-free survival(DFS) times were 19.1 months in the simultaneous resection group and 8.8 months in the delayed resection group.DFS differed significantly between the two groups.Conclusions: Simultaneous colorectal and hepatic resection is safe and exhibits advantages in the longtime survival of patients.Yuan Li Xinyu Bi Jianjun Zhao Zhen Huang Jianguo Zhou Zhiyu Li Yefan Zhang Hong Zhao Jianqiang Cai 2016Chinese Journal of Cancer Research2016,28,5:7
4Scalable microfabrication of three-dimensional porous interconnected graphene scaffolds with carbon spheres for high-performance all carbon-based micro-supercapacitors显示文摘As one of the most important micro energy storage devices(MESDs),graphene-based micro-supercapacitors(G-MSCs)possess the advantages of excellent flexibility,long cycle life,affordability and high reliability.In most cases,constructing three-dimensional(3D)graphene networks is widely utilized to promote the permeation of electrolyte and enhance the utilization of active materials.In this work,conventional freeze-drying process is utilized in the fabrication of G-MSCs to constitute 3D interconnected networks micro-electrodes,and further by regulating the composition of inks,carbon spheres(CSs)at different mass loadings are introduced into the graphene scaffolds to further increase the active sites of the micro-electrodes.The fabricated all carbon-based MSC with the optimal mass loading of CSs(0.406 mg cm^(-2))exhibits a high specific areal capacitance of 17.01mF cm^(-2)at the scan rate of 10mV s^(-1)and a capacitance retention of 93.14%after 10000 cycles at the scan rate of 500 mV s^(-1).The proposed microfabrication process is facile and fully compatible with modern microtechnologies and will be highly suitable for large-scale production and integration.Yiming Chen Minghao Guo Liang He Wei Yang Lin Xu Jiashen Meng Xiaocong Tian Xinyu Ma Qiang Yu Kaichun Yang Xufeng Hong Liqiang Mai 2019Journal of Materiomics2019,5,2:6
5Clinicopathological features and surgical outcomes of four rare subtypes of primary liver carcinoma显示文摘Objective:This study aimed to analyze clinicopathological and prognostic features of four rare pathological subtypes of primary liver malignancies to make better understanding of their clinical features.Methods:The clinicopathological data of 114 patients who were diagnosed with histologically proven four subtypes:clear cell carcinoma(CCC),giant cell carcinoma(GCC),sarcomatoid carcinoma(SC),and combined hepatocellular-cholangiocarcinoma(CHC)between October 1998 and August 2015 were reviewed.Their survival data were compared with those of 908 patients with histologically proven common hepatocellular carcinoma(HCC)(early-and advanced-stage HCC)during the same period.Results:The outcome of the CCC group was better than that of the other three subgroups,and was similar to that of the early-stage HCC group.Also,the smallest tumor size and the highest incidence of pseudocapsule formation were observed in the CCC group.The SC group had the worst outcome among these four subgroups;the prognosis was much poorer than that of any other subgroups,even poorer than that of the advanced-stage common HCC group.No statistical difference was observed between the GCC,CHC and advanced-stage HCC groups on survival analysis.The incidences of tumor vascular emboli,TNM staging and non-radical resection were three risk factors of the prognosis.Conclusions:CCC is a low-degree malignancy and relatively favorably prognostic subtype of HCC.However,GCC,SC,and CHC are three rare high-degree malignancy subtypes of HCC with poor prognosis.Zhiyu Li Xiaolong Wu Xinyu Bi Yefan Zhang Zhen Huang Haizhen Lu Hong Zhao Jianjun Zhao Jianguo Zhou Muxing Li Jianming Ying Jianqiang Cai 2018Chinese Journal of Cancer Research2018,30,3:6
6Biogenic carbonate formation and sedimentation in the Xisha Islands: evidences from living Halimeda显示文摘A recent island survey reveals that the Xuande Atoll and the Yongle Atoll in the Xisha Islands can be classified into one of two systems: the depleted atoll system and growth atoll system; the survey also indicates that the decreased area of several shoals is an unbearable burden for the Xisha Islands, of which the largest island area is 2.13 km2 and the minimum elevation is 1.4 m. According to a survey on the ecological characteristics of Halimeda in the Laolongtou breaker zone of Shidao Island in the Xisha Islands, the green and white living Halimeda are collected,the isotopic ages of 14 C contained in the Halimeda are shown to be 27 years and 55 years, respectively, and carbonate mainly occurs in five types, i.e., luster, segment, sand, sand grain, and marl in the formation. The Halimeda segments mainly provide the carbonate sediments of long-term biogenic deposits in the reef environment and the annual productivity per area is 60–100 g/m2; the characteristics of the microstructure of the Halimeda are analyzed, the aragonite raphide carbonate is deposited and enriched in the cortexes, medullas and cysts, and the Halimeda generally contain major elements such as C, O, Ca, Cl, Mg, K, Na, S and Al, and are rich in trace elements such as tellurium(Te), rhodium(Rh) and strontium. It is believed that the Halimeda grow slowly,including the biotic community of reef corals in the reef areas, thus they possess an environmental remediation capacity, but it takes much time to remedy the environment, and it is necessary to make the law to protect the diversity and vulnerability of the Xisha marine ecology, the ecology of the reef community and the island environment in a scientific way. As indicated in the survey, under the background of global warming and sea-level rise, the discovery of large amounts of Halimeda in the Laolongtou sea area is significant for the natural increase of the depleted atoll system of the Xuande Atoll, while the Halimeda segments represent the primary form of the fossil Halimeda, of which the species can be identified and preserved in great numbers under geological conditions. The Miocene was discovered in large amounts in the Xichen-1 well, therefore the study on the characteristics and mechanism of Halimeda carbonate sediments plays a pivotal role in the formation and construction of organic reefs in the South China Sea as well as oil and gas exploration.XU Hong ZHAO Xinwei EBERLI G.P. 7Impact of National Centralized Drug Procurement Policy on Antiviral Utilization and Expenditure for Hepatitis B in China显示文摘Background and Aims:The National Centralized Drug Procurement(NCDP)policy was launched in China's Mainland in April 2019,with entecavir(ETV)and tenofovir disoproxil fumarate(TDF)being included in the procurement list.We conducted the current study to investigate the impact of the NCDP policy on the utilization and expenditures of antiviral therapy for chronic hepatitis B(CHB)in China.Methods:Procurement records,including monthly purchase volume,expenditure,and price of nucleos(t)ide analogs(NAs),were derived from the National Healthcare Security Administration from April 2018 to March 2021.The changes in volumes and expenditures of the first-line NAs and bid-winning products were calculated.The effects of price,volume,and structure related to drug expenditure were calculated by the Addis and Magrini(AM)Index System Analysis.Results:The purchase volume of NAs significantly increased from 134.3 to 318.3 million DDDs,whereas the expenditure sharply decreased from 1,623.41 to 490.43 million renminbi(RMB)or 241.94 to 73.09 million US dollars(USD).The proportions of firstline NAs rose from 72.51%(ETV:69.00%,TDF:3.51%)to 94.97%(ETV:77.42%,TDF:17.55%).AM analysis showed that the NCDP policy decreased the expenditure of all NAs(S=0.91)but increased that of the first-line NAs in the bidwinning list(S=1.13).Assuming the population size of CHB patients remains stable and a compliance rate of≥75%,the proportion of CHB patients receiving first-line antiviral therapy would increase from 6.36–8.48%to 11.56–15.41%.Conclusions:The implementation of the NCDP policy significantly increased the utilization of first-line NAs for CHB patients at a lower expenditure.The findings provided evidence for optimizing antiviral therapy strategy and allocating medical resources in China.Xinyu Zhao Min Li Hao Wang Xiaoqian Xu Xiaoning Wu Yameng Sun Canjian Ning Bingqiong Wang Shuyan Chen Hong You Jidong Jia Yuanyuan Kong 2022Journal of Clinical and Translational Hepatology2022,10,3:4
8Neoadjuvant radiotherapy for resectable hepatocellular carcinoma with portal vein tumor thrombus:a systematic review显示文摘Background:The prognosis of hepatocellular carcinoma(HCC)with portal vein tumor thrombus(PVTT)is extremely poor.The clinical outcome of preoperative radiotherapy(RT)is still controversial.This study aimed to compare the clinical outcomes of combined neoadjuvant RT and hepatectomy with hepatectomy alone for HCC with PVTT.Methods:Comprehensive database searches were performed in PubMed,the Cochrane Library,EMBASE,and the Web of Science to retrieve studies published from the database creation to July 1,2020.Only comparative studies that measured survival between neoadjuvant RT followed by hepatectomy and hepatectomy alone were included.The characteristics of the included studies and patients were extracted,and the included data are presented as relative ratio(RR)estimates with 95%confidence intervals(CIs)for all outcomes.The RRs of each study were pooled using a fixed or random effects model with Review Manager(the Cochrane Collaboration,Oxford,UK)version 5.3.The response rate to RT and the overall survival(OS)rate in neoadjuvant RT followed by hepatectomy and hepatectomy alone were measured.Results:One randomized and two non-randomized controlled trials with 302 patients were included.Most patients were classified as Child-Pugh A,and Type II and III PVTT were the most common types.After RT,29(22.8%)patients were evaluated as partial response(PR)and had a positive RT response,but nine(7.1%)had progressive disease(PD).Neoadjuvant RT followed by hepatectomy was received by 127(42.1%)patients after excluding 15(5.0%)patients with severe complications or PD after RT,and 160(53.0%)patients received hepatectomy alone.In the randomized controlled trial(RCT),the 1-year OS rate in the neoadjuvant RT group and the surgery alone group was 75.2%and 43.1%,respectively(P<0.001).In the two non-randomized studies,a meta-analysis with a fixed effects model showed a longer OS in patients undergoing neoadjuvant RT followed by hepatectomy compared with hepatectomy alone at 1-year follow-up(RR=2.02;95%CI:1.45-2.80;P<0.0001).Conclusions:This systematic review showed that neoadjuvant RT followed by hepatectomy in patients with resectable HCC and PVTT was associated with a longer OS than patients who received hepatectomy alone.Zhewen Wei Jianjun Zhao Xinyu Bi Yefan Zhang Jianguo Zhou Zhiyu Li Zhen Huang Hong Zhao Jianqiang Cai 2022Hepatobiliary Surgery and Nutrition2022,11,5:4
9An initial discussion on major controlling factors on formation of coal-formed large-medium sas fields显示文摘The so-called 'large-medium gas field' means a gas field with proven reserves equal to or larger than 100×108 m3. Up to 1997, 23 coal-formed great-medium fields are discovered in China, excluding two in Taiwan Province. Their proven reserves take 50.9% in total natural gas reserves. Therefore, the coal-formed great-medium gas field plays a decisive role in natural gas reserves and the study of their controlling factors is very important both in theory and in practice. 11A male-ABCD algorithm for hepatocellular carcinoma risk prediction in HBs Ag carriers显示文摘Objective: Hepatocellular carcinoma(HCC) development among hepatitis B surface antigen(HBs Ag) carriers shows gender disparity, influenced by underlying liver diseases that display variations in laboratory tests. We aimed to construct a risk-stratified HCC prediction model for HBs Ag-positive male adults.Methods: HBs Ag-positive males of 35-69 years old(N=6,153) were included from a multi-center populationbased liver cancer screening study. Randomly, three centers were set as training, the other three centers as validation. Within 2 years since initiation, we administrated at least two rounds of HCC screening using Bultrasonography and α-fetoprotein(AFP). We used logistic regression models to determine potential risk factors,built and examined the operating characteristics of a point-based algorithm for HCC risk prediction.Results: With 2 years of follow-up, 302 HCC cases were diagnosed. A male-ABCD algorithm was constructed including participant's age, blood levels of GGT(γ-glutamyl-transpeptidase), counts of platelets, white cells,concentration of DCP(des-γ-carboxy-prothrombin) and AFP, with scores ranging from 0 to 18.3. The area under receiver operating characteristic was 0.91(0.90-0.93), larger than existing models. At 1.5 points of risk score,26.10% of the participants in training cohort and 14.94% in validation cohort were recognized at low risk, with sensitivity of identifying HCC remained 100%. At 2.5 points, 46.51% of the participants in training cohort and 33.68% in validation cohort were recognized at low risk with 99.06% and 97.78% of sensitivity, respectively. 12Prediction of Hot Ductility of Low-Carbon Steels Based on BP Network显示文摘The purpose of the research is to obtain an effective method to predict the hot ductility of low-carbon steels, which will be a reference to evaluate the crack sensitivity of steels. Several sub-networks modeled from BP network were constructed for different temperature use, and the measured reduction of area (AR) of 12 kinds of low-carbon steels under the temperature of 600 to 1000 ℃ were processed as training samples. 13Abnormal overpressure distribution and natural gas accumulation in foreland basins, Western China显示文摘Abnormal overpressure occurs in the forelandbasins of Kuqa, South Junggar and West Sichuan in China.The pressure coefficients are high. Overpressure exists inwide areas and various strata. The layers of overpressurehave a very close relationship with lithology, and the area ofoverpressure is controlled by the piedmont depression. Themechanisms of overpressure formation in the Kuqa andSouth Junggar Depression include disequilibrium compac-tion and tectonic compression; the importance of these twofactors varies in different basins and in different stages of thesame basin. Different models of gas accumulation are estab-lished to explain the relationship between overpressure dis-tribution and gas pool formation, and the influence of over-pressure on the gas pools. 16Role ofβ(FeA1)nanoparticles in abnormal grain growth in the annealing of cast Cu-Al-Mn-Fe shape memory alloys显示文摘In this study,the low-cost production of Cu-AI-Mn-Fe shape memory alloy single crystals exceeding 46 mm by abnormal grain growth was realized only through annealing their cast alloys.The results show that the mis-orientation formed during annealing may be responsible for such abnormal grain growth process.It was con-firmed that this misorientation resulted from the dissolution of bec B(FeAI)nanoparticles during heat treatment at a sufficiently temperature of approximately 1173 K.The rate of migration of the abnormal grain boundary was experimentally measured to be approximately9.3 x 10^-m s^-1within 2 min of the commencement of abnormal grain growth.Additionally,the range of composition of the alloys that can lead to abnormal grain growth was determined.When the Cu-13.0AI-6.5Mn-3.2Fe single crystal close to the[100]direction was deformed to a pre-strain of 12%,full shape recovery happened without any residual strain.At that time,the superelastic strain was approximately 9%.Such a superelastic characteristic remained nearly constant over 50 cycles,showing excellent fatigue resistance.The superelastic properties of the present Cu-13.0AI-6.5Mn-3.2Fe single crystal are compared to those of commercial Ni-Ti-based shape memory alloys.Therefore,it can be considered as a new kind of superelastic material having practical applications.The obtained results should be of great significance in the development of Cu based shape memory alloys.Furthermore,it is expected that a similar microstructure can be designed for the production of more metallic single crystals.Shuiyuan Yang Xinyu Qing Jixun Zhang Lipeng Guo Shen Hong Mingpei Li Jinbin Zhang Cuiping Wang Xingjun Liu 2020Progress in Natural Science:Materials International2020,30,4:2

18Chemotherapy initiation with single-course methotrexate alone or combined with dactinomycin versus multi-course methotrexate for low-risk gestational trophoblastic neoplasia: a multi-centric randomized clinical trial显示文摘We aimed to evaluate the effectiveness and safety of single-course initial regimens in patients with low-risk gestational trophoblastic neoplasia(GTN).In this trial (NCT01823315),276 patients were analyzed.Patients were allocated to three initiated regimens:single-course methotrexate(MTX),single-course MTX+dactinomycin(ACTD),and multi-course MTX(control arm).The primary endpoint was the complete remission(CR)rate by initial drug(s).The primary CR rate was 64.4%with multi-course MTX in the control arm.For the single-course MTX arm,the CR rate was 35.8%by one course;it increased to 59.3%after subsequent multi-course MTX,with non-inferiority to the control(difference-5.1%,95%confidence interval(CI)-19.4%to 9.2%,P=0.014).After further treatment with multi-course ACTD,the CR rate(93.3%)was similar to that of the control(95.2%,P=0.577).For the single-course MTX+ACTD arm,the CR rate was 46.7%by one course,which increased to 89.1%after subsequent multi-course,with non-inferiority(difference 24.7%,95%CI 12.8%-36.6%,P<0.001)to the control.It was similar to the CR rate by MTX and further ACTD in the control arm(89.1%vs.95.2%,P=0.135).Four patients experienced recurrence,with no death,during the 2-year follow-up.We demonstrated that chemotherapy initiation with single-course MTX may be an alternative regimen for patients with low-risk GTN.Lili Chen Ling Xi Jie Jiang Rutie Yin Pengpeng Qu Xiuqin Li Xiaoyun Wan Yaxia Chen Dongxiao Hu Yuyan Mao Zimin Pan Xiaodong Cheng Xinyu Wang Qingli Li Danhui Weng Xi Zhang Hong Zhang Quanhong Ping Xiaomei Liu Xing Xie Beihua Kong Ding Ma Weiguo Lu 2022Frontiers of Medicine2022,16,2:2
19Characteristics of porosity and permeability layer of fossil Halimeda reef mineral rock of Miocene in the Xisha Islands and its genetic model显示文摘Halimeda is one of the major reef-building algas in the middle Miocene of Xisha, and one of the significant reefbuilding algas in the algal reef oil and gas field of the South China Sea. However, there have been few reports regarding the characteristics of mineral rocks, reservoir porosity and permeability layers, and sedimentationdiagenetic-evolution of fossil Halimeda systems. The present paper briefly introduces the relevant studies on chlorophyta Halimeda and the research status of oil and gas exploration. Through the 1 043 m core of the Xichen-1 well, we studied the characteristics of the mineral rocks and porosity and permeability of the middle Miocene Halimeda of the Yongle Atoll, identified and described the segments of fossil Halimeda, and pointed out that most of the segment slides are vertical sections in ovular, irregular or long strips. The overwhelming majority of these fossil Halimeda found and studied are vertical sections instead of cross sections. In this paper, knowledge regarding the cross sections of fossil Halimeda is reported and proven to be similar with the microscopic characteristics of modern living Halimeda; fossil Halimeda are buried in superposition; it is shown that there are different structures present, including typical bio-segment structure, and due to its feature of coexisting with red alga, tying structure, twining structure and encrusting structure are all present; and finally, it is suggested to classify the fossil Halimeda into segment algal reef dolomites. In addition, all of the studied intervals are moderately dolomitized. Secondary microcrystalline-dolosparite dominates the original aragonite raphide zones,and aphanitic-micrite dolomite plays the leading role in the cortexes and medullas; in the aragonite raphide zones between medulla and cysts, secondary dissolved pores and intercrystalline pores are formed inside the segments, and algal frame holes are formed between segments; therefore, a pore space network system(dissolved pores + intragranular dissolved pores—intercrystalline pores + algal frame holes) is established. Segment Halimeda dolomite has a porosity of 16.2%–46.1%, a permeability of 0.203×10–3–2 641×10–3 μm2, and a throatradius of 23.42–90.43 μm, therefore it is shown to be a good oil and gas reservoir. For the reasons mentioned above, we suggest building the neogene organic reef-modern reef sedimentation-diagenetic-evolution models for the Xisha Islands.XU Hong ZHU Yurui EBERLI G.P.
